Click-evoked vestibulocollic reflexes in torticollis.

A total of 26 patients with torticollis were studied using a recently developed technique for recording vestibulocollic reflexes from the sternocleidomastoid muscles in addition to conventional caloric tests of vestibular function. Previous reports of abnormalities of vestibulo-ocular reflexes in these patients were confirmed with just fewer than half having significant canal pareses or directional preponderances (nine of 20 tested). In addition, there was a high incidence of abnormal click-evoked vestibulocollic reflexes (17 of 26 tested), which were not simply the result of prior treatment w

The influence of unilateral saccular impairment on functional balance performance and self-report dizziness.

BACKGROUND: Postural stability in humans is largely maintained by vestibular, visual, and somatosensory inputs to the central nervous system. Recent clinical advances in the assessment of otolith function (e.g., cervical and ocular vestibular evoked myogenic potentials [cVEMPs and oVEMPs], subjective visual vertical [SVV] during eccentric rotation) have enabled investigators to identify patients with unilateral otolith impairments. This research has suggested that patients with unilateral otolith impairments perform worse than normal healthy controls on measures of postural stability. It is no

Inner ear damage in children due to noise exposure from toy cap pistols and firecrackers: a retrospective review of 53 cases.

This retrospective study presents the findings of inner ear damage documented in 53 children exposed to impulsive sound emitted by toy weapons and firecrackers. There were 49 boys and four girls aged between four and fourteen years. Thirty-nine children were affected unilaterally while fourteen had bilateral hearing loss (total of 67 ears). Most of the hearing loss (>70%) was sensorineural high frequency hearing loss, while only nine out of the 67 injured ears had sensorineural mid frequency hearing loss. Seven children sustained a traumatic ear drum perforation. Dizziness or tinnitus was repo

[Infrared and video oculography--alternatives to electrooculography?].

With the introduction of each new technique for registering eye movements, the question arises concerning whether these provide a reliable and accurate alternative for the diagnosis of labyrinthine dysfunction when compared to the most commonly used electro-oculographic technique. To answer this question we compared mean slow-phase velocity (SPV) using three different recording techniques:electro-oculography (EOG), video-oculography (VOG) and infrared oculography (IROG) during four different types of examinations. The examinations were the eye target tracking test (ETT), horizontal and vertica

A case of Bickerstaff's encephalitis. With special reference to neurotological findings.

The case of a 60-year old male with prodromal common cold symptoms and progression of brain stem involvement with no cardiac or respiratory complications is described. This conformed to the criteria of Bickerstaff's encephalitis. Neurotological examinations, including the OKN test, the caloric test, and the GBST were performed from onset to recovery of the disease. The results of these tests closely reflected the central nervous system disorders each time, but there was a discrepancy in the results of the two test batteries of equilibrium function, the caloric test and the GBST. The caloric te

Patterns of vestibular function following vestibular nerve section.

Bithermal caloric irrigations, low-frequency rotational chair stimulation, and posturography were performed on 20 patients before and after vestibular nerve section. Twelve patients demonstrated acute postoperative spontaneous nystagmus and rotational vestibulo-ocular reflex (VOR) asymmetry. Eight patients demonstrated minimal acute postoperative spontaneous nystagmus and VOR asymmetry. Four patients had suppression of all vestibular function characterized by an absent contralateral caloric response, low VOR gain, and falls on posturography when required to rely solely on vestibular input to m

Propriospinal myoclonus: a motor phenomenon found in restless legs syndrome different from periodic limb movements during sleep.

Three patients presented with a 25-, 15-, and 5-year history of restless legs syndrome (RLS) and periodic limb movements during sleep (PLMS). For 1, 4, and 5 years, they reported additional involuntary trunk and limbs jerks preceding falling asleep and occasionally during intrasleep wakefulness. Videopolysomnography revealed jerks during relaxed wakefulness arising in axial muscles with a caudal and rostral propagation at a slow conduction velocity, characteristic of propriospinal myoclonus (PSM). Jerk-related EEG-EMG back-averaging did not disclose any preceding cortical potential. During rel

Intratympanic gentamicin in Meniere's disease.

Ninety-three patients with intractable Meniere's disease were treated with gentamicin (Garamycin 40 mg/ml) administered in 1 to 4 transtympanic injections. The patients were tested at frequent intervals and followed up for 2 years. Before treatment all subjects experienced moderate or severe handicap caused by Meniere's disease. Two years after the treatment, rotatory vertigo was abolished in 81% of subjects, Tumarkin attacks were cured in 60%, and work capacity was severely reduced in 10% and moderately reduced in 17% of subjects. The outcome of the caloric responses did not correlate with th

Hearing preservation and improvement of auditory brainstem response findings after acoustic neuroma surgery.

Among 9 patients with acoustic neuroma who underwent tumor removal by the middle cranial fossa approach during the past 5 years, 6 were selected for hearing preservation surgery which was successful in 5 cases (83.3%). For the prediction of success in hearing-preservation, factors related to hearing preservation, including preoperative findings of neurotological examination, were analyzed. Postoperative hearing threshold correlated significantly with wave V latency and I-V interpeak latency of preoperative auditory brainstem response (ABR) and the SP/AP ratio of preoperative electrocochleogram

Vestibular dysfunction in a child with embryonic exposure to accutane.

Children with a history of embryonic exposure to Accutane (isotretinoin) are at great risk for major physical malformations, brain malformations, and decreased intelligence. A case is presented of a 4-year 7-month-old black male with a history of embryonic exposure to Accutane who was born with embryopathy that includes bilateral major ear deformities. The child has a significant bilateral conductive hearing loss, and, in addition, a left sided sensorineural loss. Vestibular function testing revealed evidence of peripheral and central vestibular dysfunction. A course of diphenhydramine hydroch

Evaluation of vestibular function by dynamic posturography and other equilibrium examinations.

We evaluated the significance of dynamic posturography (EquiTest) in vestibular function assessment by comparing it with our routine equilibrium examinations comprised of nystagmographic data and other postural tests. Fifty-two patients with unilateral peripheral vestibular disorders were examined. Differences in test results between pendular rotation test and dynamic posturography were found in 23 out of 52 (44%) patients with unilateral peripheral vestibular disorders. In 5 patients, no abnormality of vestibular function could be detected by means of a three-test battery (spontaneous and pos

Tuberculous meningitis in children during the isoniazid era.

Fifty-nine cases of tuberculous meningitis in children seen at the Charity Hospital at New Orleans since the addition of isoniazid to the therapy in 1952 are reviewed. Fourteen of the children died during hopsitalization. At discharge 21 children had complete or nearly complete clinical recovery. Follow-up of 21 available long-term survivors revealed a significant number with neurologic and social disabilities. The endemicity of tuberculous infections in the households of the children and factors responsible for transmission of tubercle bacilli from an adult source are reported. The importance

Increased Velocity Storage in Subjects with Meniere's Disease.

OBJECTIVE: Velocity storage mechanism is a multisensory rotation estimator; it compensates for errors in the information provided by the peripheral vestibular organs by means of an adjustment in the duration of the vestibular signal. The aim of this study was to determine the activity of the velocity storage mechanism in the presence of a labyrinthine disorder, using galvanic vestibular stimulation to cause direct activation of the vestibular afferent neurons. MATERIALS AND METHODS: Forty-one subjects with definite Meniere's disease (MD) and 36 healthy volunteers were evaluated using a 20-s g

Vestibular function in Lermoyez syndrome at attack.

Lermoyez syndrome (LS) has been regarded as a variant of Ménière's disease (MD), but so far there have been very few cases of LS reported in the literature, so such a conclusion is debatable. Specifying the pattern of auditory and vestibular changes at attack using objective quantitative measures is important for understanding the mechanism responsible for MD and LS. Here we report the first objective measures of dynamic otolith function and dynamic semicircular canal function in an LS patient at the time of the attack as well as at quiescence, documenting the fluctuation in otolith and semici

Comparison of vestibular function between large cerebellopontine angle meningioma and schwannoma.

CONCLUSION: Abnormal caloric and vestibular evoked myogenic potential (VEMP) responses are frequently encountered with a large cerebellopontine angle (CPA) schwannoma, while normal caloric responses and abnormal VEMPs are noted with a large CPA meningioma. This difference may possibly exist because schwannoma causes vestibular deficits via parenchymal involvement, while vestibular deficits in the meningioma are mostly due to compression neuropathy. OBJECTIVES: This study aimed to compare the tumor characteristics in relation to vestibular function, i.e. caloric and VEMP responses, between lar

Vestibular-evoked myogenic potentials in chronic noise-induced hearing loss.

OBJECTIVE: To investigate the effect of chronic noise exposure on vestibular-evoked myogenic potentials. STUDY DESIGN: Prospective study. SUBJECTS AND METHODS: Twenty patients with chronic noise-induced hearing loss, presenting as bilateral notched audiogram at 4 kHz, underwent audiometry, caloric, and vestibular-evoked myogenic potential tests. RESULTS: Caloric and vestibular-evoked myogenic potential tests revealed abnormal responses in nine (45%) and 10 (50%) patients, respectively. However, when both results were considered together, the abnormal rate reached 70% (14 of 20). The hearing
