DEFORMATION RESPONSE OF THE ILIOTIBIAL BAND-TENSOR FASCIA LATA COMPLEX TO CLINICAL-GRADE LONGITUDINAL TENSION LOADING IN-VITRO.

BACKGROUND: Iliotibial Band (ITB) syndrome is a troublesome condition with prevalence as high as 12% in runners. Stretching has been utilized as a conservative treatment. However, there is limited evidence supporting ITB elongation in response to a stretching force. PURPOSE/HYPOTHESES: The purpose of this study was to describe the iliotibial band tensor fascia lata complex (ITBTFLC) tissue elongation response to a simulated clinical stretch in-vitro. The authors hypothesized that the ITBTFLC would undergo statistically significant elongation when exposed to a clinical-grade stretching regimen

The Endoscopic Endonasal Approach to the Hypoglossal Canal: The Role of the Eustachian Tube as a Landmark for Dissection.

IMPORTANCE: Improvements in endoscopic technology and reconstructive techniques have made the endoscopic endonasal approach (EEA) a viable option to approach ventromedial lesions in the region of the hypoglossal canal. Prior to contemplating this surgical corridor, a thorough understanding of anatomic relationships and landmarks is essential to safely approach this region of the posterior skull base through an EEA. OBJECTIVE: To describe the surgical technique and anatomic landmarks in the EEA to the hypoglossal canal through referencing nasopharyngeal and posterior skull base anatomy. DESIG

Ultrasound-guided greater palatine nerve block: a case series of anatomical descriptions and clinical evaluations.

BACKGROUND: Greater palatine nerve (GPN) block is commonly performed for maxillary and palatal anesthesia by using bony landmarks. Ultrasound (US) can be used to consistently identify greater palatine foramen (GPF) as a defect in the bony palate enabling US-guided injections near the foramen. METHODS: We scanned and injected 16 undissected well-embalmed hemisectioned cadaveric heads after excluding major anatomical malformations. A linear high-frequency hockey stick probe (7-13 MHz) positioned in long axis to the hard palate visualized GPF as a discontinuity in the hard palate. US-guided inje

Evaluation of cervical posterior lateral mass screw placement by oblique radiographs.

STUDY DESIGN: The present study analyzed the two-dimensional representation of cervical lateral mass screws by oblique radiographs compared with cadaveric placement. This was accomplished by posterior and lateral cervical dissection of the lateral masses and intervertebral foramina, keeping the emerging nerve roots intact. The intervertebral foramina were divided into two zones for the study. OBJECTIVES: To identify and describe the value of oblique radiographs in evaluating posterior lateral mass screw placement in the cervical spine. SUMMARY OF BACKGROUND DATA: Posterior plate-screw fixati

Short-term effects of formaldehyde on peak expiratory flow and irritant symptoms.

The authors studied the respiratory effects of formaldehyde exposure among students who dissected cadavers in a gross anatomy laboratory. Peak expiratory flow and respiratory symptoms were measured before and after each weekly laboratory session. Each of 38 students was exposed to formaldehyde for 2.5 hr/wk for 14 wk. Individual, daily formaldehyde measurements averaged 1.1 ppm (standard deviation = 0.56 ppm). Multivariate models demonstrated two different time scales of effect of formaldehyde on peak expiratory flow: (1) exposure during the previous 2.5 hr reduced peak expiratory flow by -1.0

Exploring the Significance of the Obturator Externus Groove as a Marker for Bipedalism: A Cadaveric Study.

Background Bipedalism, defined as locomotion using only two feet, is a distinctive trait that sets humans apart from most other creatures. This study explores the significance of the obturator externus groove (OEG) as a marker for bipedalism. This topic is important due to its implications in evolutionary history. Previous studies have identified various anatomical features indicative of bipedalism, including an S-shaped spine, a centrally positioned foramen magnum, and unique pelvic and femoral structures. Among these, the morphology of the femur, particularly the presence of the OEG, has bee

Mapping the Masseteric Nerve for Facial Reanimation: An Anatomical Study of Two Dissection Strategies.

Background and Objectives: The masseteric nerve (Mn) is increasingly used for facial reanimation because of its reliable location, high axon count, low donor morbidity, and favorable clinical outcomes. Precise topographic knowledge of the Mn relative to reproducible intraoperative landmarks is essential for safe dissection. This study investigated the intramuscular position of the Mn relative to two defined reference lines. Materials and Methods: Seventy-two hemicrania from 36 individuals (aged 54-99 years) embalmed postmortem using the Thiel method were examined. Measurements were referenced

Surgical anatomy of the carotid sympathetic plexus in endoscopic endonasal approaches: strategies for preventing Horner syndrome.

OBJECTIVE: Endoscopic endonasal approaches (EEAs) specifically for procedures involving manipulation of the internal carotid artery (ICA), such as the transcavernous and translacerum approaches, confer a potential risk of carotid sympathetic plexus injury, potentially leading to postganglionic Horner syndrome. The primary aim of this study was to delineate the surgical anatomy of the carotid sympathetic plexus from an endoscopic endonasal perspective, offering insights to facilitate intraoperative anatomical identification and injury prevention. METHODS: A comprehensive dissection was conduct

Cortical and trabecular morphometric properties of the human calvarium.

There is currently a gap in the literature that quantitatively describes the complex bone microarchitecture within the diploë (trabecular bone) and cortical layers of the human calvarium. The purpose of this study was to determine the morphometric properties of the diploë and cortical tables of the human calvarium in which key interacting factors of sex, location on the calvarium, and layers of the sandwich structure were considered. Micro-computed tomography (micro-CT) was utilized to capture images at 18 μm resolution of male (n = 26) and female (n = 24) embalmed calvarium specimens in the f

Relative device stability of anterior versus axillary needle decompression for tension pneumothorax during casualty movement: Preliminary analysis of a human cadaver model.

BACKGROUND: Tension pneumothorax (tPTX) remains a significant cause of potentially preventable death in military and civilian settings. The current prehospital standard of care for tPTX is immediate decompression with a 14-gauge 8-cm angiocatheter; however, failure rates may be as high as 17% to 60%. Alternative devices, such as 10-gauge angiocatheter, modified Veress needle, and laparoscopic trocar, have shown to be potentially more effective in animal models; however, little is known about the relative insertional safety or mechanical stability during casualty movement. METHODS: Seven soft-

Macroscopical and histological characterization of the perineal membrane and deep perineal pouch.

The existence of a striated deep transverse perineal (DTP) muscle remains an ongoing debate. We hypothesize that the DTP is macroscopically and microscopically detectable as a definable striated muscle belly in elderly males, but not in elderly female donors used in the dissection course. Investigations were performed on embalmed human donors of both sexes (n = 10, mean age at death: 82 years). We dissected the ischioanal fossa. After removing all the loose fat and connective tissues, including the arteries and nerves, we exposed the compact transversally running plate-shaped tissue stretched

Biomechanical Comparison of Three Modified Kessler Techniques for Flexor Tendon Repair: Implications in Surgical Practice and Early Active Mobilization.

Objective: Managing flexor tendon injuries surgically remains challenging due to the ongoing debate over the most effective suture technique and materials. An optimal repair must be technically feasible while providing enough strength to allow for early active mobilization during the post-operative phase. This study aimed to assess the biomechanical properties of three modified Kessler repair techniques using two different suture materials: a conventional two-strand and a modified four-strand Kirchmayr-Kessler repair using 3-0 Prolene® (2s-KK-P and 4s-KK-P respectively), and a four-strand Kess

Transorbital approach to the cavernous sinus: an anatomical study of the related cranial nerves.

BACKGROUND: The cavernous sinus (CS) is a demanding surgical territory, given its deep location and the involvement of multiple neurovascular structures. Subjected to recurrent discussion on the optimal surgical access, the endoscopic transorbital approach has been recently proposed as a feasible route for selected lesions in the lateral CS. Still, for this technique to safely evolve and consolidate, a comprehensive anatomical description of involved cranial nerves, dural ligaments, and arterial relations is needed. OBJECTIVE: Detailed anatomical description of the CS, the course of III, IV,

The ultrasound-guided proximal intercostal block: anatomical study and clinical correlation to analgesia for breast surgery.

BACKGROUND: The ultrasound-guided proximal intercostal block (PICB) is performed at the proximal intercostal space (ICS) between the internal intercostal membrane (IIM) and the endothoracic fascia/parietal pleura (EFPP) complex. Injectate spread may follow several routes and allow for multilevel trunk analgesia. The goal of this study was to examine the anatomical spread of large-volume PICB injections and its relevance to breast surgery analgesia. METHODS: Fifteen two-level PICBs were performed in ten soft-embalmed cadavers. Radiographic contrast mixed with methylene blue was injected at the

A safe technique for insertion of heel wires for hindfoot external fixation frames: an anatomical study.

AIMS: Anatomical atlases document classical safe corridors for the placement of transosseous fine wires through the calcaneum during circular frame external fixation. During this process, the posterior tibial neurovascular bundle (PTNVB) is placed at risk, though this has not been previously quantified. We describe a cadaveric study to investigate a safe technique for posterolateral to anteromedial fine wire insertion through the body of the calcaneum. MATERIALS AND METHODS: A total of 20 embalmed cadaveric lower limbs were divided into two groups. Wires were inserted using two possible inser
