Addison's disease with adrenal enlargement on computed tomographic scanning. Report on two cases of tuberculosis and review of the literature.

Two patients with Addison's disease related to urogenital tuberculosis had enlargement of one or both adrenal glands detected with computed tomographic scanning. Review of reports of adrenal size on computed tomographic examination suggests that adrenal enlargement in the presence of Addison's disease demands further investigation about the cause of the adrenal insufficiency.

Multiple thoracotomy combined with chemotherapy in metastatic adrenal cortical carcinoma: a case report and review of the literature.

A case is reported in which multiple bilateral pulmonary metastasectomies for metastatic adrenal cortical carcinoma were performed and which resulted in an unusually long survival of over 25 years from the first thoracotomy and over 28 years from removal of the primary adrenal cortical carcinoma. The patient is alive and free of tumor as of the latest follow-up. We believe this is the longest survival after pulmonary metastasectomy for carcinoma published to date.

Primary adrenocortical insufficiency (Addison's disease).

In the United States the etiology of most cases of adrenal gland atrophy or destruction is unknown. Autoimmune processes may be responsible. With rare exceptions, all three zones of the adrenal cortex are destroyed and there is reduced glucocorticoid, mineralocorticoid and androgen secretion. Although the deficiency of glucocorticoid is probably the most severe threat to life, treatment is available to correct all the deficiencies. Patient education and preparedness can prevent serious complications.

A Case of ALL Developing Posterior Reversible Encephalopathy Secondary to Hyponatremia.

Posterior reversible encephalopathy syndrome (PRES), may be due to different causes. It may develop secondary to hypertension, renal decompensation, electrolyte imbalance, and chemotherapeutic drugs. We describe a case of acute lymphoblastic leukemia in which PRES developed secondary to hyponatremia despite being normotensive during receipt of chemotherapy. Magnetic resonance imaging findings were suggestive of PRES. Partial diffusion restriction was observed in lesions in the bilateral occipitoparietal regions and the cerebellum. The patient was treated with appropriate medications with the r

Orthostatic hypotension associated with dorsal medullary cavernous angioma.

BACKGROUND: Orthostatic hypotension (OH) is a rare manifestation of medulla oblongata lesions that may be because of interruption of descending sympathoexcitatory axons. AIMS: To illustrate the location of a medullary lesion that produced OH following resection in relationship to the location of putative sympathoexcitatory pathways. METHOD: A case with dorsal medullary cavernous angioma presenting with OH is described. The possible localization of lesion was compared with distribution of tyrosine hydroxylase (TH)-immunoreactive axons in a comparable section of the medulla of a control brain.

Close association of urinary excretion of aquaporin-2 with appropriate and inappropriate arginine vasopressin-dependent antidiuresis in hyponatremia in elderly subjects.

The present study was undertaken to determine whether urinary excretion of aquaporin-2 (AQP-2) participates in the involvement of arginine vasopressin (AVP) in hyponatremia less than 130 mmol/L in 33 elderly subjects (> or =65 yr old) during the last 5-yr period. Subjects were separated into euvolemic hyponatremia groups: 13 with hypopituitarism, 8 with syndrome of inappropriate secretion of antidiuretic hormone (SIADH), 8 with mineralocorticoid-responsive hyponatremia of the elderly, and 4 with miscellaneous diseases. Approximately 40% of those with hyponatremia was derived from hypopituitari

Diagnostic and therapeutic pitfalls in the management of pediatric patients with 3β-hydroxysteroid dehydrogenase type 2 (3β-HSD2) deficiency - a single center experience.

INTRODUCTION: Congenital adrenal hyperplasia (CAH) due to 3β-hydroxysteroid dehydrogenase type 2 deficiency (3β-HSD2D) is an exceptionally rare disorder affecting adrenal steroidogenesis, leading to variable clinical presentations. This study aims to highlight the phenotypic variability and management challenges associated with 3β-HSD2D through the analysis of three pediatric cases. METHODS: We retrospectively reviewed three patients diagnosed with 3β-HSD2D at the Pediatric Endocrinology Department of the University Children's Hospital in Krakow. Clinical features, laboratory findings, geneti

Metabolic dysfunction-associated steatotic liver disease in individuals with type 2 diabetes mellitus and diabetic foot ulcers: prevalence and associated factors.

BACKGROUND: Classical congenital adrenal hyperplasia (CAH) is characterized by a defect in cortisol and aldosterone secretion, adrenal hyperandrogenism, impaired adrenal medullary function and insulin insensitivity. The latter along with the increased tendency towards obesity raises questions whether other cardiovascular risk factors are altered in CAH. OBJECTIVE: To evaluate 24-h ambulatory blood pressure and obesity in patients with salt-wasting 21-hydroxylase deficiency diagnosed in the neonatal period and treated with hydrocortisone and 9alpha-fludrocortisone thereafter. METHODS: Thirty-

Clinical presentation, predictive factors and management of patients with Nelson syndrome: a retrospective study.

BACKGROUND: Nelson syndrome (NS), or corticotroph tumor progression after bilateral adrenalectomy (CTP-BADX/NS), is a serious complication in patients with Cushing disease (CD) following BADX. Surgical tumor removal is the recommended treatment, though adjuvant therapies may be necessary. AIM OF THE STUDY: To evaluate clinical, radiological, and hormonal features of CD patients after BADX, identify risk factors for CTP-BADX/NS and assessed treatment outcome and cardio-metabolic complications. METHODS: Retrospective study of 30 patients (male/female: 9/21; median age at CD diagnosis: 33 years

Primary aldosteronism in hypertensive patients: clinical implications and target therapy.

BACKGROUND: The prevalence of primary aldosteronism (PA) in hypertensive patients varies according to diagnostic testing and ascertained normal cut-offs. The aim of this case-control study was to confirm the high prevalence of PA in a large hypertensive population and evaluate the antihypertensive effect of mineralocorticoid receptor antagonists (MRA) treatment. MATERIAL AND METHODS: We investigated 327 hypertensive and 90 matched normotensive subjects with normal adrenal imaging. Serum aldosterone (ALD), active renin (REN) levels and aldosterone/active renin (ALD/REN) ratio were measured bef

STAR splicing mutations cause the severe phenotype of lipoid congenital adrenal hyperplasia: insights from a novel splice mutation and review of reported cases.

OBJECTIVE: The steroidogenic acute regulatory protein (StAR) transports cholesterol to the mitochondria for steroidogenesis. Loss of StAR function causes lipoid congenital adrenal hyperplasia (LCAH) which is characterized by impaired synthesis of adrenal and gonadal steroids causing adrenal insufficiency, 46,XY disorder of sex development (DSD) and failure of pubertal development. Partial loss of StAR activity may cause adrenal insufficiency only. PATIENT: A newborn girl was admitted for mild dehydration, hyponatremia, hyperkalemia and hypoglycaemia and had normal external female genitalia wi

Treatment of orthostatic hypotension with erythropoietin.

BACKGROUND AND METHODS: Patients with orthostatic hypotension caused by autonomic neuropathy frequently have a decreased red-cell mass. This would be expected to compromise their effective circulating blood volume and aggravate the orthostatic hypotension. We studied the effect of increasing the red-cell mass with erythropoietin, given subcutaneously in a dose of 50 U per kilogram of body weight three times a week for 6 to 10 weeks to eight patients with orthostatic hypotension--four men, one teenage boy, and three women (age range, 17 to 68 years). Four patients had type I diabetes mellitus a

Cerebral X-Linked Adrenoleukodystrophy Associated with Hemophilia A: A Case Report.

INTRODUCTION: The co-occurrence of hemophilia A and X-linked adrenoleukodystrophy (ALD) represents an exceptionally rare clinical scenario. Both the F8 and ABCD1 genes are situated at the Xq28 locus, a gene-dense region where independent pathogenic variants may co-segregate due to low recombination rates and the presence of mutational hotspots. We report a rare co-occurrence of distinct pathogenic variants in both genes in the same patient. CASE PRESENTATION: A 12-year-old male with FVIII deficiency (hemophilia A), caused by a heterozygous c.3652delG variant in the F8 gene, presented with cut
