Sinonasal Packing is Not a Requisite for Successful Cerebrospinal Fluid Leak Repair.

Background  Numerous methods have been described to repair nasal cerebrospinal fluid (CSF) leaks. Most studies have focused on optimizing CSF leak repair success, leading to closure rates of 90 to 95%. Objective  This study aimed to determine if excellent reconstruction rates could be achieved without using sinonasal packing. Methods  A prospective case series of 73 consecutive patients with various CSF leak etiologies and skull base defects was conducted to evaluate reconstruction success without sinonasal packing. The primary outcome measure was postoperative CSF leak. Secondary outcome meas

Nuances of Olfactory Groove Meningioma Surgery: 2-Dimensional Operative Video.

Olfactory groove meningiomas frequently present as large or giant-size tumors associated with marked frontal lobe edema and significant frontal lobe dysfunction. Simpson grade I removal was rare in early reports due to their invasion of the ethmoid sinuses and skull base bone,1 which resulted in high recurrence rates.2,3 Indeed, recurrence occurred in the most celebrated case of olfactory groove meningioma.4,5  To achieve Simpson grade I removal (tumor, dura, bone), protect the frontal lobes from additional injury, and provide the best chance for recovery, we demonstrate a few nuances for olfa

Low aseptic loosening and revision rate in Zweymüller-Plus total hip arthroplasty with ceramic-on-ceramic bearings.

UNLABELLED: A recent comparative study reported lower revision rate and higher survival in ceramic-on-ceramic (CoC) than in metal-on-metal (MoM) coupling in Zweymüller-Plus THA. We retrospectively studied 87 consecutive patients (94 hips), who primarily received Zweymüller-Plus THA (SL-Plus stem, ceramic 28-mm ball head, Bicon cup) with third-generation CoC bearing surfaces for osteoarthritis. Survival, revision and osteolysis rate were studied and compared to those observed in historical series from this institution with the same THA and MoM articulation. The mean ± SD follow-up was 7.4 ± 1.7

Autologous bone grafts and endosseous implants: complementary techniques.

PURPOSE: This article describes predictable techniques to augment contour- or height-deficient edentulous alveolar processes with autologous bone grafts for simultaneous or secondary placement of endosseous implants. METHODS: Augmentation bone grafts harvested from the ilium and mandible were used to reverse alveolar atrophy of the maxilla and mandible. Endosseous implants were either placed simultaneously with the graft or 6 to 9 months after grafting. Implant success was calculated only after an implant-supported prosthesis was in function for a minimum of 12 months. RESULTS: One hundred t

Fascia Lata: Another Workhorse for Complex Skull Base Reconstruction.

BACKGROUND: Multiple methods exist for skull base reconstruction of defects created by expanded endonasal approaches. While the nasoseptal flap (NSF) has been well established as the workhorse of mucosal reconstruction in complex skull base defects in multi-layered closures, a variety of options exist for the inner layer of multilayer reconstruction, including fascia lata (FL). OBJECTIVE: To present our experience and outcomes in utilizing FL in multiple ways to reconstruct a wide variety of complex skull base defects. METHODS: Retrospective review was performed from May 2017 to February 202

Rotational Stability of Scaphoid Waist Nonunion Bone Graft and Fixation Techniques.

PURPOSE: Rotational instability of scaphoid fracture nonunions can lead to persistent nonunion. We hypothesized that a hybrid Russe technique would provide improved rotational stability compared with an instrumented corticocancellous wedge graft in a cadaver model of scaphoid nonunion. METHODS: A volar wedge osteotomy was created at the scaphoid waist in 16 scaphoids from matched-pair specimens. A wedge was inset at the osteotomy site or a 4 × 16-mm strut was inserted in the scaphoid and a screw was placed along the central axis (model 1). The construct was cyclically loaded in torsion until
