Temporomandibular joint disorders in children.

A child's difficulty in verbalizing the precise location and nature of facial pain and jaw dysfunction often results in a nondefinitive history, increasing the importance of the dentist's awareness of the early signs and symptoms of temporomandibular joint disorders (TMD). A focused examination of the masticatory musculature, the temporomandibular joints, and associated capsular and ligamentous structures can reveal if a patient's symptoms are TMD in origin. An accurate differential diagnosis enables timely referral to appropriate health care providers and minimizes the use of diagnostic imagi

Treatment of an adolescent with total ankyloglossia.

This is a report about the orthodontic treatment of a 13-year, 10-month-old boy with total ankyloglossia combined with a Class III occlusion. The patient's tongue was fixed to the floor of his mouth and could not be elevated at all. He had a maxillary deficiency and a mandibular protrusion with a negative overjet. However, he was able to retrude his mandible to an edge-to-edge position. Before orthodontic treatment, the ankyloglossia was surgically rectified. Orthodontic treatment was initiated to improve the patient's occlusion and facial appearance by correcting his retruded maxilla by means

Effective treatment of self-injurious oral trauma in Lesch-Nyhan syndrome: a case report.

Lesch-Nyhan syndrome (LNS) is an X-linked disorder originating from deficiency of the enzyme hypoxanthine guanine phosphoribosyl transferase. It is characterized by neurological manifestations, including the dramatic symptom of compulsive self-mutilation, which results in destruction of oral and perioral tissues. Several drug trials have been administered to improve the severe self-destructive behaviour, with questionable effectiveness. Invasive treatment approaches, such as extraction of teeth and orthognathic surgery, have been suggested with variable success. A conservative treatment with a

[The importance of condyle position in the diagnosis, treatment and prevention of TMD].

When a patient presents with TMJ pain, it could be all too easy for the orthodontists to turn the other way claiming that their occlusion has no connection with their symptoms and that they are suffering from a biopsychosocial disorder. This abundantly illustrated article attempts to demonstrate that patients can appear to display a stable occlusion even though their TMJ situation is unstable. This can give rise to dental abrasion, articular adaptations and pain. Orthodontics allows us to treat patients in pain provided that the treatment plan takes into account the need to restore a stable TM

Recommendations by the EACD for examination, diagnosis, and management of patients with temporomandibular disorders and orofacial pain by the general dental practitioner.

The Council of the European Academy of Craniomandibular Disorders charged the Educational Committee with the task of establishing Guidelines and Recommendations for the examination, diagnosis, and management of patients with temporomandibular disorders and orofacial pain by the general dental practitioner. It was not their purpose to present a thorough and critical review of the vast amount of literature available but to summarize the at-present generally accepted clinical approach. These recommendations are based as much as possible on scientific evidence and on sound clinical judgment in cas

Temporomandibular disorder presenting as chronic migraine; the diagnostic role of MRI and interdisciplinary evaluation: A clinical report.

Temporomandibular disorders can mimic migraines and delay appropriate care. A 40-year-old man with 18 months of refractory temporal headache and facial pain, initially treated as migraine, underwent imaging that revealed bilateral temporalis hypertrophy and temporomandibular joint degenerative changes. More specifically, an interdisciplinary assessment confirmed temporomandibular disorders. Multimodal management included occlusal adjustment with a nocturnal occlusal device, counseling to reduce parafunction, physical therapy, low-level laser therapy, and targeted botulinum toxin A. Symptoms im

Outcomes of management of early temporomandibular joint disorders: How effective is nonsurgical therapy in the long-term?

AIM: The first step in the management of Temporomandibular Disorders (TMD) is usually noninvasive, especially if the disorder is in the early stages. Clinically, pain and clicking are early signs and symptoms of TMD. The management of TMD usually includes "splint therapy" and analgesics. In this study, we report our long-term outcomes in the treatment of patients suffering from early TMD. MATERIALS AND METHODS: We assessed the records of 138 patients who were referred for management of TMD. Selection was based on pain and/or clicking of the Temporomandibular Joint (TMJ), no pathologic lesions

Are Patient-Specific Implants Accurate in Bimaxillary Advancement Surgery Entailing Three-Piece Le Fort I? A Randomized Controlled Trial.

BACKGROUND: The use of patient-specific implants (PSI) in orthognathic surgery was shown to produce accurate results for single-piece Le Fort I. However, the literature lacks evidence on the accuracy of PSI for segmental Le Fort I. PURPOSE: The purpose was to compare the accuracy of maxillary and mandibular bone segment positioning after bimaxillary advancement surgery (BA) using: i) PSI for fixation in a three-piece Le Fort I; ii) conventional intermediate computer-aided design/computer-aided manufacturing splints with mini plates and no final splint. The effectiveness of stabilization appliance therapy among patients with myalgia.

BACKGROUND: The efficacy of stabilization appliance therapy for masticatory muscle pain is debated. Therefore, there are currently no clear usage standards. We analyzed patient factors influencing its efficacy and characterized masticatory muscle pain subtypes to determine appropriate therapy candidates. METHODS: This case series study recruited patients diagnosed with local myalgia or myofascial pain and used variables related to temporomandibular disorders in the analysis. We used temporary appliance to screen patients for sleep bruxism for 2 weeks. Afterwards, we initiated therapy with sta

The Effect Evaluation of Functional Appliance Used for Class II Patients With Temporomandibular Joint Anterior Disc Displacement.

Orthopedic functional appliances are usually used to correct patients with retrognathic mandible. However, a part of cases have a trend of relapse after splint treatment. The aim of this study was to explore the role of temporomandibular joint disc position in functional appliance treatment. This study included 8 patients who had 1 joint anterior disc displacement with reduction (ADDWR) and the other joint anterior disc displacement without reduction (ADDWoR) confirmed by magnetic resonance imaging (MRI). Only the ADDWR joint could return to its normal position after wearing the anterior repos

The Effect of Exercise on Range of Movement and Pain After Temporomandibular Joint Arthrocentesis.

PURPOSE: Arthrocentesis is an effective treatment modality for temporomandibular disorders, especially in patients who have pain and limited mouth opening. Surgeons generally suggest physical exercises after arthrocentesis and arthroscopy procedures; however, there has been no study in the literature evaluating the effects of exercise on clinical outcomes. This study investigated whether physical exercises after arthrocentesis would result in early improvements in clinical symptoms in patients with temporomandibular joint disc displacement without reduction (TMJ DDw/oR). MATERIALS AND METHODS

Oral appliance titration in patients with obstructive sleep apnea induces the appearance of periodic limb movements.

STUDY OBJECTIVES: Oral appliance (OA) therapy is considered a first line choice of therapy for some patients with mild or moderate obstructive sleep apnea (OSA) and an alternative form of treatment in those intolerant of continuous positive airway pressure (CPAP) use. According to several studies, periodic limb movements (PLM) appear during effective treatment of OSA with CPAP, but a similar phenomenon has not been described with the use of oral appliance. Herein, we describe the incidence of PLM in patients with OSA who underwent oral appliance therapy titration. DESIGN: This is a retrospect

Prevention of self-mutilation in patients with Lesch-Nyhan syndrome: review of literature.

There have been no standard methods established to prevent the self-mutilation seen in LNS. Preventive treatment must be developed, therefore, for each patient based on clinical findings. The first method to be utilized should be a nonsurgical splint or mouth guard. If it proves to be unsuccessful then the benefits of surgical procedures should be considered. In the future, the use of medication, such as carbamazepine, may prove to restructure the current approach advocated by the literature today.

Arthroscopic disc repositioning and suturing: a preliminary report.

Eight patients (11 joints) underwent arthroscopic disc repositioning and suturing. Disc displacement was established by physical examination, magnetic resonance imaging (MRI), and diagnostic arthroscopy. Postoperative MRIs were taken at varying intervals between 1 day and 6 months. In all 11 joints, either partial improvement or normal disc position was observed arthroscopically. In 9 of the 11 joints, either partial improvement or normal position was observed on the postoperative MRIs. It is concluded that posterior disc repositioning and suturing is an achievable goal of temporomandibular jo
