Chemical and physical labyrinthectomy for Meniere's disease.

The use of intratympanic gentamicin is currently a popular and easily performed office procedure for the conservative treatment of the Meniere's disease patient who has failed medical therapy or who is not a candidate for surgical therapy. The procedure provides excellent control for the symptom of vertigo. Despite this success, there remains a significant risk of hearing loss irrespective of administered dose. In the future, antioxidant [42,43] or salicylate therapy may prevent aminoglycoside toxicity [44]. These prophylaxis methods have shown promise in the laboratory. Current methods do not

Gene Therapy: Routes of Inner Ear Delivery.

Gene delivery and the various routes of administration used to deliver viral vectors constitute a rapidly evolving field, with recent success in restoring hearing in children with OTOF variants in ongoing clinical otoferlin trials. With the rise of gene therapy for major hereditary hearing disorders, both syndromic and nonsyndromic, there has been parallel progress in the development of new delivery methods. Advances in intratympanic injection of antibiotics and corticosteroids, along with nearly half a century of experience in cochlear implantation surgery, have enabled multiple local and sys

Stronger efferent suppression of cochlear neural potentials by contralateral acoustic stimulation in awake than in anesthetized chinchilla.

There are two types of sensory cells in the mammalian cochlea, inner hair cells, which make synaptic contact with auditory-nerve afferent fibers, and outer hair cells that are innervated by crossed and uncrossed medial olivocochlear (MOC) efferent fibers. Contralateral acoustic stimulation activates the uncrossed efferent MOC fibers reducing cochlear neural responses, thus modifying the input to the central auditory system. The chinchilla, among all studied mammals, displays the lowest percentage of uncrossed MOC fibers raising questions about the strength and frequency distribution of the con

Cochlear microphonics recorded from fetal and newborn sheep.

PURPOSE: Sounds present within the uterus stimulate the fetal inner ear and central auditory pathway. This study was undertaken to determine the efficiency of transmission of exogenous airborne stimuli to the fetal inner ear. In this way, we may quantify the extent to which the fetal auditory system is isolated from sounds produced outside the mother. MATERIALS AND METHODS: Cochlear microphonics were recorded from fetal and newborn sheep to evaluate the extent to which the fetus is isolated from sounds exogenous to the ewe. Electrodes were surgically placed in contact with the round window me

Vibrant Soundbridge for hearing restoration after chronic ear surgery.

INTRODUCTION: Middle ear surgery is primarily concerned with resolving the discharging pathology, in the case of chronic otitis media (COM), or with complete eradication, in case of cholesteatoma. Either of these procedures may require repeated surgeries, often resulting in severe mixed hearing impairment. A middle ear implant may be indicated in these cases instead of a hearing aid because the anatomical conditions in such cases often impede an adequate acoustic coupling. The objective of this study was to evaluate MED-EL Vibrant Soundbridge (VSB) implantation in patients with severe conducti

Feasibility of direct promontory stimulation by bone conduction: A preliminary study of frequency-response characteristics in cats.

BACKGROUND: As an alternative pathway to air conduction, bone conduction is a multipathway process that transmits sound energy to the inner ear through the skull in general. Based on this mechanism, bone conduction devices (BCDs) have been used widely in the rehabilitation of hearing loss. Although great efforts have been devoted to improving BCDs, drawbacks still exist in most categories of BCDs due to the complicated process of bone conduction. We hypothesized that if a bone conduction transducer was placed on the cochlea to stimulate it directly, the attenuation would be minimized, and the

An injectable PEG hydrogel controlling neurotrophin-3 release by affinity peptides.

Neurotrophin-3 growth factor can improve cochlear neuron survival, and localized delivery of this protein to the round window membrane in the middle ear may be able to reverse sensorineural hearing loss. Thus, the goal of this work was to develop an injectable hydrogel delivery system that can allow localized release of neurotrophin-3 in a controlled and sustained manner. We identified a PEG hydrogel formulation that uses thiol-vinyl sulfone Michael addition for crosslinking. This injectable formulation provides elastic hydrogels with higher mechanical rigidity, better bio-adhesion and longer

Two different techniques to facilitate reconstruction of the long incus process with bone cement: a feasibility study in human cadaveric temporal bones.

The purpose of this feasibility study was to evaluate two novel techniques facilitating bone cement repair of ossicular discontinuity between the incus and stapes. An isolated damage of the long incus process can be repaired using bone cement. However, bridging of a large gap between incus remnant and stapes head with bone cement is difficult, since viscous cement is not stable and the wet cement bridge may collapse. Ten fresh-frozen cadaveric human temporal bones were used. The long process of the incus was subtotally resected. A novel instrument and polylactide acid (PLA) scaffolds were appl

Functional and morphological changes in experimental endolymphatic hydrops.

Electrocochleograms (ECochG) in response to tone bursts and electronystagmograms (ENG) during sinusoidal rotational stimulation were monitored in guinea pigs equipped with chronically implanted round-window and periocular electrodes before and for as long as 90 days after uni- or bilateral surgical obliteration of the endolymphatic duct. The presence of cochlear and saccular endolymphatic hydrops was verified during microdissection of the inner ear, and cochlear hair cell counts were performed in some of the animals. The results indicate that in surgically-induced endolymphatic hydrops, cochle

Effects of fenestra size and piston diameter on the outcome of stapes surgery for clinical otosclerosis.

OBJECTIVES: We determined the effect of piston diameter (0.8 or 0.6 mm) and fenestra size (total, three-quarter, or half removal or calibrated stapedotomy) on hearing gains after stapes surgery for clinical otosclerosis. METHODS: We analyzed the mean air conduction hearing gains at various frequencies in a sample of 911 strictly consecutive patients who underwent 1,168 stapes procedures with piston reconstruction carried out by the same surgeon between 1963 and 1979. Assiduous follow-up of patients was attempted for at least 10 years, and in some cases for 20 years. Results over time at the v

Contemporary directions in the therapy of sensory hearing loss.

Introduction: More than 5% of the world's population experience hearing impairment. The most common form is presbycusis (age-related hearing loss; ARHL). It affects almost one in three people over the age of 65. The hair cells of the cochlea play an important role in the process of sound registration. Genetic mutations, aging and environmental factors can cause damage that contributes to the hearing loss. Methods and results: The currently explored research directions include drug treatments, gene therapies, and stem cell therapies. To date, no significant

Cochlear Implant Insertion Trauma Is Associated With Spiral Ganglion Neuron "Dead Zones" in the Human.

HYPOTHESIS SIGNIFICANT: Cochlear implant (CI) insertion trauma, as evidenced by fracture of the osseous spiral lamina (OSL), is associated with localized "dead zones" and focal spiral ganglion neuron (SGN) loss. BACKGROUND: Hearing and structure preservation approaches to CI insertion aim to minimize trauma and preserve residual SGNs. In cases of significant insertion trauma, peripheral axons running through the bony OSL inherently become damaged if the OSL is fractured. The current investigation sought to determine if the relative location of OSL fracture was associated with focal areas of S
