[Neuromuscular disorders in critically ill patients].

INTRODUCTION AND OBJECTIVE: Critically ill patients admitted to the Intensive Care Unit (ICU) often develop neuromuscular disorders. The objective of this study was to diagnose these and determine the causes. MATERIAL AND METHODS: We present a series of 13 critically ill patients who developed weakness or paresia, reduced or absent ROT and normal brain stem reflexes, in whom ENG and EMG studies were done in EESS and II which were considered together with data from general laboratory analysis, radiological and microbiological examinations, medication given and posterior clinical course of the

Bilateral vestibular neurectomy for treatment of vertigo.

The effects of bilateral vestibular neurectomy on equilibrium and vestibular function were clinically evaluated in two patients more than 15 years after surgery. Both patients had bilateral Menière's disease and their vertiginous spells were permanently resolved after the second vestibular neurectomy. Symptoms of disequilibrium were absent in one patient and mild in the other. Reflexive horizontal eye movements on whole body rotation in darkness were absent on low angular accelerations (2 degrees/s2), but could be elicited with angular accelerations of 20 degrees/s2 or higher. Extravestibular

Recovery of the vestibular function after vestibular neuronitis.

We performed steroid therapy on 34 cases with vestibular neuronitis and compared them with 77 patients not subjected to this therapy to examine the role of recovery of their vestibular function. Since no relation was noted between use of steroid and changes in subjective symptom of dizziness, the use of steroid is likely to facilitate the disappearance of spontaneous nystagmus in the early recovery stage. Canal paralysis recovered significantly by steroid and in cases of slight and moderate paralysis at the onset, recovery was more significant. Steroid therapy is argued to be effective for the

Acute intracranial vascular accidents.

Cerebellar hemorrhage, cerebellar infarction, pontine hemorrhage, and pontine infarction are four clinical conditions representative of intracranial posterior fossa accidents, which can mimic acute vestibular disturbances. It is exceedingly important to recognize these clinical entities early and to differentiate them from peripheral vestibulopathies so that appropriate diagnostic tests and therapeutic measures can be instituted early to prevent the high mortality rate seen without appropriate treatment. It is appreciated that without computed axial tomography, rapid diagnosis of these clinica

Do central vestibular findings predict abnormal findings on magnetic resonance imaging?

OBJECTIVE: To determine whether central findings from vestibular tests predict abnormal findings on magnetic resonance imaging. METHOD: This study was a retrospective case series at a tertiary referral centre. The main outcome measure of this diagnostic intervention study was the positive predictive value of central vestibular findings in relation to magnetic resonance imaging abnormalities. RESULTS: Central vestibular findings had a 50.9 per cent positive predictive value for magnetic resonance imaging abnormalities across all age groups although they varied according to age group. Optokine

Cerebral plasticity in acute vestibular deficit.

The aim of this study was to analyze the effect of acute vestibular deficit on the cerebral cortex and its correlation with clinical signs and symptoms. Eight right-handed patients affected by vestibular neuritis, a purely peripheral vestibular lesion, underwent two brain single photon emission computed tomography (SPECT) in 1 month. The first SPECT analysis revealed reduced blood flow in the temporal frontal area of the right hemisphere in seven of eight patients, independent of the right/left location of the lesion. The alteration was present always in the right, non-dominant hemisphere and

Vestibular dysfunction in Gulf War syndrome.

METHODS: Vestibular complaints of Gulf War veterans were characterized by a nested case-control study of 23 veterans with 3 different Gulf War syndromes and 20 matched control subjects. All subjects completed a standardized symptom questionnaire and underwent standard audiovestibular tests administered by audiologists blinded to group identities. RESULTS: The prevalence of reported dizzy spells was higher in veterans with Gulf War syndromes 1 (100%), 2 (85%), and 3 (100%) than in controls (25%, P < 0.0001). Dizzy spells were more frequent, lasted longer, and involved a wider variety of accomp

Neurotic, neuromuscular and autonomic nervous form of magnesium imbalance.

The nervous form of magnesium imbalance represents the best documented experimental and clinical aspects of magnesium disorders. The nervous form of primary magnesium deficit (MD) in the adult appears as the best descriptive model for analysis of the symptomatology, aetiology, physiopathology, diagnosis and therapy of the most frequent form of MD. Nervous hyperexcitability due to chronic MD in the adult results in a non-specific clinical pattern with associated central and peripheral neuromuscular symptoms, analogous to the symptomatology previously described in medical literature as latent te

Ocular flutter associated with a lesion of the right upper pontine tegmentum.

Ocular flutter is a relatively rare eye movement disorder characterized by intermittent bursts of conjugate horizontal saccades without an intersaccadic interval. Its anatomical and neurophysiological basis remains controversial. Here we present a case of ocular flutter in which the flutter declined as the clinical stage of the associated ailment declined, until it eventually disappeared. Radiological examination revealed a tiny lesion in the right upper pontine tegmentum that included areas of the reticular formation, superior cerebellar peduncle and locus ceruleus. Because the lesion occupie

Immunosuppressive treatment in bilateral vestibulopathy with inner ear antibodies.

CONCLUSIONS: Although vestibular recovery was observed after steroid treatment, it remains uncertain whether this improvement was spontaneous or due to medication. These data do not allow us to generally recommend corticosteroid treatment in patients with BVF and inner ear antibodies. OBJECTIVE: A retrospective study was performed based on the observation of two patients with suspected autoimmune bilateral vestibular failure (BVF) with normal hearing and antilabyrinthine or nervous tissue-specific serum antibodies who showed vestibular recovery after corticosteroid treatment. Cross-power spectral density analysis of pursuit tracking. Evaluation of central and peripheral pathology.

Twenty-one normal subjects were evaluated for pursuit tracking performance at frequencies of .2, .4, .8, 1.2 and 1.6 Hz. Measures of phase (lead or lag of target), gain (amplitude of response) and spectral purity (distortion of smooth tracking) were obtained by a cross-power spectrum analysis of the stimulus input and the eye movement output. It was determined that normal subjects could quite adequately track a target moving at .4 Hz. After establishing this parameter for normals, 43 patients with central and peripheral pathology related to the vestibulo-oculomotor system were tested at this f

Head-tilting stabilometry in patients with benign paroxysmal positional vertigo.

OBJECTIVE: One of the pathologic conditions underlying benign paroxysmal positional vertigo (BPPV) is degeneration of the otolith organs. In this study, we examined changes in the parameters of stabilometry under an upright condition and head-tilt conditions in patients with BPPV. METHODS: We performed stabilometry on 21 patients with right BPPV, on 21 patients with left BPPV and on 21 controls. First, the subject stood barefoot in an upright position with both feet together on the platform with eyes closed. Next, tilting of the head about 30 degrees to the left was added. Then, tilting about
