Should all patients with non-valvular atrial fibrillation be anticoagulated?

Atrial fibrillation (AF) is the most common arrhythmia encountered in clinical practice. The prevalence and incidence of AF are rising, as confirmed in several European and American registries. Guidelines published in 2008 from the European Society of Cardiology/American Heart Association and from the American College of Chest Physicians, clarified the strategy of antithrombotic treatment in AF, which is based on the presence of risk factors for thromboembolism. This approach allows physicians to classify patients as at low, moderate or high risk, according to their individual risk characteris

Functional Gaps and Atrial Fibrosis as Predictors of Early Arrhythmia Recurrence After Robotic-Enhanced Hybrid Ablation for Persistent Atrial Fibrillation.

BACKGROUND: Robotic-enhanced hybrid ablation (RE-HA) is a promising strategy for persistent (PsAF) and long-standing persistent atrial fibrillation (LS-PsAF). The impact of post-epicardial conduction gaps and atrial fibrosis on early arrhythmia recurrence (EAR) is unclear. OBJECTIVE: To assess whether conduction gaps and atrial fibrosis predict EAR within 3 months after the epicardial stage of RE-HA. METHODS: Forty-two PsAF/LS-PsAF patients underwent RE-HA with left atrial appendage closure. High-density endocardial mapping at 3 months identified fixed (present in all maps) and functional (p

Triple Percutaneous Therapy for Mitral Regurgitation and Atrial Fibrillation.

BACKGROUND: Patients with severe mitral regurgitation (MR), atrial fibrillation (AF), and contraindications to long-term anticoagulation often require multiple interventions. The feasibility of combining mitral transcatheter edge-to-edge repair, left atrial appendage closure (LAAC), and AF ablation in a single session is unknown. CASE SUMMARY: A 78-year-old woman with dilated cardiomyopathy, severe functional MR, and high-burden paroxysmal AF underwent a single-session procedure including pulsed-field AF ablation, mitral transcatheter edge-to-edge repair, and LAAC. Pulmonary veins were isolat

A Fine Balance: Anticoagulation for Non-Valvular Atrial Fibrillation After Cerebral Amyloid Angiopathy-Related Intracranial Haemorrhage.

The management of anticoagulation in patients with non-valvular atrial fibrillation at high risk of bleeding complications can be challenging. One such group is elderly patients with previous intracranial haemorrhage secondary to cerebral amyloid angiopathy (CAA-ICH). These patients have a high rate of rebleeding, which can be predicted by features on cerebral magnetic resonance imaging. Common bleeding risk calculators, such as the HAS-BLED score, underestimate the bleeding event rate in patients with CAA-ICH due to poor patient representation in validation studies. Observational studies and

Spontaneous Resolution of a Pseudoaneurysm at the Anastomosis Site of Coronary Artery Bypass Graft: A Case Report.

Pseudoaneurysms at the coronary artery bypass graft (CABG) anastomosis site are rare. Here, we report a rare case of spontaneous resolution of a pseudoaneurysm at the anastomosis site developed soon after CABG. A 66-year-old man was referred to our hospital because of chest pain with paroxysmal atrial fibrillation. Pulmonary vein isolation (PVI), atrial appendage closure, and CABG were performed under cardiopulmonary bypass. Contrast computed tomography (CT) on postoperative day (POD) 5 revealed a pseudoaneurysm at the anastomosis of the left internal mammary artery (LIMA) to the circumflex ar

Surgical left atrial appendage occlusion in valvular heart disease without atrial fibrillation: the OPINION trial.

BACKGROUND AND AIMS: While surgical left atrial appendage occlusion (SLAAO) reduces stroke in atrial fibrillation (AF) patients, its efficacy in patients without pre-operative AF but with CHA₂DS₂-VASc ≥2 remains uncertain despite their high post-operative AF risk (15-54%). The aim of this study was to evaluate whether prophylactic SLAAO reduces post-operative thrombo-embolic events in valvular surgery patients. METHODS: The OPINION was a multicentre, open-label, randomized, superiority trial conducted at three cardiac surgery centres in China. Eligible non-AF patients with CHA₂DS₂-VASc ≥2 and

Feasibility, safety, and efficacy of a novel external compression vascular closure device: The LockeT® study.

INTRODUCTION: Hemostasis following large-bore femoral vein access remains a challenge. Manual compression has been the standard of care but requires bedside staff, prolonged bed rest, and longer length of stay. The LockeT is an external compression device that attempts to address these issues while achieving venous hemostasis. OBJECTIVES: We evaluate postprocedural hemostasis and vascular closure outcomes after using LockeT following cardiac electrophysiologic procedures. METHODS: We performed a single-center, observational study of patients who underwent vascular closure for electrophysiolo

Antithrombotic Therapy in Patients Undergoing Transcatheter Interventions for Structural Heart Disease.

Contemporary evidence supports device-based transcatheter interventions for the management of patients with structural heart disease. These procedures, which include aortic valve implantation, mitral or tricuspid valve repair/implantation, left atrial appendage occlusion, and patent foramen ovale closure, profoundly differ with respect to clinical indications and procedural aspects. Yet, patients undergoing transcatheter cardiac interventions require antithrombotic therapy before, during, or after the procedure to prevent thromboembolic events. However, these therapies are associated with an i

An Unusual Intraoperative Transesophageal Echocardiographic Approach to Detect Acute Superior Vena Cava Syndrome in Mitral Valve Repair: A Case Report.

The diagnosis of superior vena cava (SVC) syndrome is always challenging, even more so when it happens intraoperatively. An usual transgastric-transesophageal projection allows for the visualization and quantification of caval flow acceleration, critical in leading to rapid diagnosis and quick reaction in the operative setting. We present the case of a patient who underwent successful mitral valve reconstruction and left atrial appendage closure via median sternotomy on cardiopulmonary bypass and, intraoperatively, developed iatrogenic SVC syndrome with hypotension, facial cyanosis, and conjun

The 2024 ESC guidelines on atrial fibrillation: essential updates for everyday clinical practice.

Atrial fibrillation (AF) is the most prevalent cardiac arrhythmia, and it is associated with substantial morbidity, mortality, and economic burden. Effective management of AF remains a critical focus in contemporary medicine, given its complex and multifaceted nature. In the present paper, we provide the essential updates for everyday clinical practice from the 2024 European Society of Cardiology (ESC) guidelines for AF management. We highlight seven key areas encompassing the adoption of the CHA₂DS₂-VA score for thromboembolic risk stratification, changes in bleeding risk assessment, expanded

[Rapidly evolving techniques for structural heart disease interventions].

Structural heart disease interventions are the transcatheter techniques used for treating non-coronary heart disease. In recent years, these techniques have generated considerable interest even though they still comprise only a small percentage of the total volume of interventions performed in interventional cardiology departments. The level of interest in these techniques is high probably because their application is characterized by a number of special features: a) the need for multidisciplinary teams; b) the need for specialized education and training; c) the requirement for special skills
