Acanthamoeba: a rare primary cause of rhinosinusitis.

Parasitic infections, especially Acanthamoeba, are rarely implicated as a specific cause of rhinosinusitis. It is a fatal disease found in the immunocompromised population, in particular in patients infected with the human immunodeficiency virus (HIV). Less than 10 cases of Acanthamebic rhinosinusitis have been reported in the literature, and only 1 has survived. This case report presents an Acanthamebic infection misdiagnosed as a squamous cell carcinoma of the nasal septum on a presumptive healthy, immunocompetent 35-year-old woman. She was later diagnosed with AIDS (AIDS) along with dissemi

Clinical features and outcome of Acanthamoeba keratitis.

We retrospectively reviewed 11 patients with culture-proven Acanthamoeba keratitis who presented at the National Taiwan University Hospital between 1989 and 1996. We assessed predisposing factors, initial diagnosis, clinical presentation, treatment, and outcome. A history of contact lens-wear, poor contact lens hygiene, intractable eye pain, and ring infiltrates in the cornea were the most prominent characteristics and clinical manifestations. Acanthamoeba keratitis was often misdiagnosed, with herpetic keratitis (7/11) being the most common initial diagnosis from referring hospitals. These pa

Isolation of Acanthamoeba from a cerebral abscess.

A 55-year-old diabetic aboriginal woman presented with a two-week history of fever, altered mental state and convulsions. On the basis of computed tomographic scanning a diagnosis of cerebral abscess was made. The pus that was drained produced no bacterial growth but, on microscopy, amoebic cysts were observed. Special cultures produced a growth of Acanthamoeba. The patient appeared to respond to drainage of the abscess and antiprotozoal therapy. Unfortunately, she developed necrotizing enteritis which led ultimately to her death. Antibiotic sensitivity and pathogenicity testing suggest that t

Probable acanthamoeba meningoencephalitis in a Korean child.

A 5-year-old Korean boy developed multiple subcutaneous, nontuberculous granulomas and died with meningoencephalitis. Autopsy disclosed amebic granulomas in subcutaneous tissue, the left adrenal gland, and the pancreas, with more acute inflammatory lesions in the liver, kidney, and brain. The causative organism is believed to be an Acanthamoeba sp.

Well water sources simultaneous contamination with Cryptosporidium and Acanthamoeba in East-Southeast Asia and Acanthamoeba spp. in biofilms in the Philippines.

Cryptosporidium is the leading agent of waterborne parasitic protozoan outbreaks and is the second leading cause of infant mortality due to diarrhoea worldwide. Acanthamoeba spp. causes Acanthamoeba keratitis (AK) and a life-threatening condition known as granulomatous amoebic encephalitis (GAE). The present study aimed to assess the water quality of an indigenous and a rural community for waterborne parasitic protozoan contamination. Aquatic samples (n = 22) were processed by filtration of 500 mL portion through a 1.2 μm pore size glass microfiber filter and eluted for light microscopy, cultu

Acanthamoeba spp.: Neglected Protists in Veterinary Medicine.

PROPOSE: Acanthamoebaspp. are widespread free-living protists found in various natural and man-made habitats. At times, they can become parasitic, leading to conditions such as Acanthamoeba keratitis (AK), granulomatous amoebic encephalitis (GAE), and systemic infections in humans and animals. In veterinary medicine, infections caused by Acanthamoeba spp. are frequently underdiagnosed, necessitating a high degree of clinical vigilance and laboratory technical skill for accurate diagnosis and treatment. METHODS: This review aimed to provide an overview of documented natural infections in anima

[Acanthamoeba keratitis in Iceland from 1996-2021].

INTRODUCTION: Acanthamoeba keratitis is a rare but serious eye infection primarily associated with contact lens use. Its diagnosis is often delayed due to treatment challenges, frequent co-infections, and the amoeba's high resistance, leading to significant visual impairment. This study assessed the incidence of Acanthamoeba keratitis in Iceland over 25 years, identified risk factors, diagnostic difficulties, and explored potential improvements. MATERIALS AND METHODS: A retrospective descriptive study including all individuals diagnosed with amoebic corneal infection in Iceland from 1996-2021

Acanthamoeba castellanii exhibits intron retention during encystment.

Intron retention (IR) refers to the mechanism of alternative splicing in which an intron is not excised from the mature transcript. IR in the cosmopolitan free-living amoeba Acanthamoeba castellanii has not been studied. We performed an analysis of RNA sequencing data during encystment to identify genes that presented differentially retained introns during this process. We show that IR increases during cyst formation, indicating a potential mechanism of gene regulation that could help downregulate metabolism. We identify 69 introns from 67 genes that are differentially retained comparing the t

Acanthamoeba scleral abscess reoccurring after penetrating keratoplasty.

Two months following penetrating keratoplasty for Acanthamoeba keratitis, a 76-year-old man was referred due to inability to wean high-dose topical steroids. Despite a very healthy graft and minimal pain, a scleral abscess involving three clock hours of the superior conjunctiva was present. The patient underwent conjunctival and scleral excision of the area of apparent infection with adjuvant mitomycin C and double freeze-thaw cryotherapy treatment followed by amniotic membrane graft. Recurrence was confirmed with PCR. Following a multimonth regimen of oral voriconazole and topical polyhexamet

Migration of Acanthamoeba castellanii Through Legionella Biofilms.

The amoeba-resistant bacterium Legionella pneumophila infects humans through aerosols and thereby can cause a life-threatening pneumonia termed Legionnaires' disease. In the environment L. pneumophila forms and colonizes biofilms, which usually comprise complex multispecies communities. In these biofilms L. pneumophila persists and replicates intracellularly in protozoa, such as the amoeba Acanthamoeba castellanii. The interactions between sessile L. pneumophila in biofilms and their natural protozoan hosts are not understood on a molecular level. Here, we describe a method to visualize by con

Acanthamoeba Keratitis: Are Recent Cases More Severe?

PURPOSE: To identify a recent change in disease severity and visual outcomes of patients with Acanthamoeba keratitis (AK) at Wills Eye Hospital (WEH). METHODS: A retrospective chart review was performed of all cases diagnosed with AK at WEH between January 1, 2009, and December 31, 2015. Failure of treatment was defined as having a final best-corrected visual acuity worse than 20/100 and/or requiring keratoplasty. The patients were grouped into 2 outcome categories: successfully treated and those who failed treatment; and prognostic factors associated with failure status were evaluated. Addit

[Acanthamoeba keratitis--a too seldom diagnosed disease?].

Acanthamoeba keratitis has become a difficult diagnostic and management problem in contact lens patients over the last few years. A young female contact lens wearer presented with a long history of painful central corneal ulcer. The suspected diagnosis of Acanthamoeba keratitis could only be confirmed by histology and electron microscopy following penetrating keratoplasty. Special stains of scrapings and culture of material on special media failed to establish the diagnosis. This rare (or underdiagnosed?) infection must be considered in the differential diagnosis of keratitis following contact

Total Anterior Staphyloma Secondary to Acanthamoeba Keratitis.

There are very few published cases of total anterior staphyloma, all of which have been reported as secondary to fungal keratitis. This study reports the clinical and histopathological findings and subsequent management of a 27-year-old healthy female patient who developed total anterior staphyloma after poor compliance with treatment for clinically diagnosed acanthamoeba keratitis. She underwent a successful evisceration with good long-term results. This case highlights that total anterior staphyloma may also result from untreated keratitis which is not fungal in origin. In cases of fungal an

Acanthamoeba keratitis: diagnosis and treatment update 2009.

PURPOSE: To describe the current management of Acanthamoeba keratitis (AK). DESIGN: A perspective based on the literature and author experience. RESULTS: Early diagnosis and appropriate therapy are key to a good prognosis. A provisional diagnosis of AK can be made using the clinical features and confocal microscopy, although a definitive diagnosis requires culture, histology, or identification of Acanthamoeba deoxyribonucleic acid by polymerase chain reaction. Routine use of tissue diagnosis is recommended, particularly for patients unresponsive to treatment for AK. Topical biguanides are th
