[Progress on the treatment of Achilles tendon rupture].

Achilles tendon rupture is an important research topic in the field of sports medicine. Its imaging diagnosis, treatment methods, rehabilitation strategies, and prognosis assessment have always been the focus of academic attention. MRI examination is widely used in diagnosing Achilles tendon rupture, but it has disadvantages such as high cost, long examination time, and metal contraindications. Ultrasound examination has the advantages of being non-invasive, low cost, comfortable, and portable, and it also has high specificity and accuracy, which has significant application value. The treatmen

Anatomy of the Achilles tendon-A pictorial review.

The Achilles tendon (AT) is the strongest tendon of the human body. The knowledge of AT anatomy is a basic prerequisite for the successful treatment of acute and chronic lesions. The structure of the AT results from a complicated fusion of three parts: the tendons of the medial and lateral gastrocnemius and the soleus muscles. From proximal to distal, the tendon fibers twist in a long spiral into a roughly 90° internal rotation. The tendon is narrowest approximately 5-7 cm above its calcaneal insertion and from there it expands again. The topography of the footprints of the individual AT compo

Bilateral Achilles Tendon Ruptures in the NFL.

Background: Achilles tendon ruptures (ATR) can have devastating results for athletes in the National Football League (NFL). While many studies have examined the effects of an ipsilateral ATR, there are no reports on the impact of bilateral ATRs on NFL athlete performance. Methods: Publicly available online injury data for NFL athletes who sustained bilateral ATRs between the start of the 2007 season and the start of the 2021 season were queried using online news and sports analysis web sources. Results: After applying inclusion and exclusion criteria, 5 NFL athletes were identified. The findin

Biologics in the Treatment of Achilles Tendon Pathologies.

Regenerative medicine is gaining more and more space for the treatment of Achilles pathologic conditions. Biologics could play a role in the management of midportion Achilles tendinopathy as a step between conservative and surgical treatment or as an augmentation. Higher-level studies are needed before determining a level of treatment recommendation for biologic strategies for insertional Achilles tendinopathy. Combining imaging with patient's functional requests could be the way to reach a protocol for the use of biologics for the treatment of midportion Achilles tendinopathy and, for this pe

Synaptic failure: The achilles tendon of sphingolipidoses.

The presence of life-threatening neurological symptoms in more than two-thirds of lysosomal storage diseases (LSDs) underscores how vulnerable the nervous system is to lysosomal failure. Neurological dysfunction in LSDs has historically been attributed to the disruption of neuronal and glial homeostasis resulting from the progressive jamming of the endosomal/lysosomal pathway. In neurons, a dysfunctional endosomal-lysosomal system can elicit dire consequences. Given that neurons are largely postmitotic after birth, one can clearly understand that the inability of these cells to proliferate obl

Is there still a place for Achilles tendon lengthening?

Patients with diabetes and ankle equinus are at particularly high risk for forefoot ulceration because of the development of high forefoot pressures. Stiffness in the triceps surae muscles and tendons are thought to be largely responsible for equinus in patients with diabetes and underpins the surgical rationale for Achilles tendon lengthening (ATL) procedures to alleviate this deformity and reduce ulcer risk. The established/traditional surgical approach is the triple hemisection along the length of the Achilles tendon. Although the percutaneous approach has been successful in achieving incre

Spondylodiscitis and Achilles tendonitis due to gout.

The patient, a 62-year-old man with a 3-year history of hyperuricemia, presented with severe neck pain, Achilles enthesopathy and polyarthralgia. He consumed alcohol heavily. The biochemical profile was normal except for elevated levels of CRP (3.6 mg/dl; normal < 0.3), uric acid (UA) (10.9 mg/dl; normal 2.5-7.5) and creatinine (1.7 mg/dl; normal 0.5-1.0). Bone scintigraphy showed polyarthritis at the right elbow, wrist and bilateral first MTP joints. Notably, bone scintigraphy with computed tomography also revealed spondylodiscitis of C5-C6, which was confirmed by MRI, and left Achilles tendo

Glucocorticoid injections in lesions of the achilles tendon.

BACKGROUND: A number of non-operative interventions are used to manage Achilles tendinopathy. In particular, local glucocorticoid injections have generated controversy. Although a number of case reports indicate symptomatic relief following glucocorticoid injection, one systematic review found little evidence to support their efficacy. Furthermore, local glucocorticoid injections may be associated with rupture of the Achilles tendon. This systematic review considered all available clinical trials measuring the effect of local glucocorticoid injections on symptom relief in patients with Achille

Complications of the treatment of Achilles tendon ruptures.

Since the first reports in the medical literature of treatment of the Achilles tendon, complications have been recognized from both non-operative and operative techniques. These include tendon rerupture, sural nerve morbidity, wound healing problems, changes in tendon morphology, venous thromboembolism, elongation of the tendon, complex regional pain syndrome, and compartment syndrome. This article delineates the incidence for each of these complications, with differing techniques, methods of avoiding these complications and treatment methods if they occur.

Roentgenographic diagnoses of ruptured Achilles tendons.

In a prospective study, the roentgenographic features of the Achilles tendon were investigated in 60 patients with acute total rupture of the Achilles tendon. Sixty ankles without actual injuries, 60 patients with ankle fractures, and 60 patients with ankle sprains were selected as reference groups. At primary examination, 11 patients (18%) with Achilles tendon rupture were clinically estimated to have minor lesions in the Achilles tendon; all had a correct roentgenographic diagnosis. Kager's triangle was positive for rupture of the Achilles tendon in all 60 patients with operative verificatio

[Sonography of the Achilles tendon and its bursa].

Pain and disturbed function in the sole of the foot leads to damage of the tendo Achilles and to abnormalities in its vicinity. They are frequent consequences of some sporting activities (jogging). Differentiation between bursitis, fat necrosis, degenerative changes in tendons and their rupture and from abnormalities of the ankle joint is important for treatment. In the symptomatic patients whom we examined, the bursa was involved in 72.9% and lesions of the tendo Achilles were found in 62.1%.

[Treatment of traumatic sections of the Achilles tendon].

The author maintains that the atrophy of the sural triceps muscle, the retraction of the Achille tendon and the osteoporosis of the calcaneum and of the tarsian bones, occurring after the traumatic sectioning of the Achille tendon, are the results of an inadequate treatment. The cause of these sequellae is the immobilization of the foot in an equinus position, which relaxes the sural triceps and as a result of the lack of mechanical traction factor, leads to local circulatory disturbances followed by a modification in the structure of the bone and of the muscle. On the basis of this pathophysi

Combined Tendon and Bone Allograft Transplantation for Chronic Achilles Tendon Ruptures.

Combined flexor hallucis longus tendon transfer and bone-tendon allograft transplantation is a reasonable option for advanced distal-segment Achilles tendinopathy. This procedure provides anatomic restoration and improved function of the posterior muscle group without sacrificing the regional anatomy. Allograft transplantation is safe and does not require immunosuppressive therapy. The soft tissue envelope should be healthy because wound complications can be an issue. This procedure is especially helpful in patients with significant disability.

The Role of Phagocytic Cells in the Achilles Tendon.

Macrophages and other phagocytic cells are central regulators of tendon immunobiology, orchestrating inflammation, tissue repair, and extracellular matrix (ECM) remodeling in the tendons. They derive from circulating monocytes and resident tendon-specific populations, including tenophages. Macrophage polarization along the M1/M2 axis exerts a decisive influence on tendon healing trajectories. Activated M1 macrophages promote the early healing phase for debris clearance initiating the reparative cascade. However, their sustained activity leads to inflammation, ECM degradation, impaired healing,

Diagnostic Musculoskeletal Ultrasound of the Achilles Tendon.

Musculoskeletal ultrasound (MSKUS) is a pivotal imaging modality for the evaluation and management of Achilles tendon pathologies. Its ability to provide real-time, high-resolution imaging facilitates accurate diagnosis, dynamic assessment, and precise therapeutic interventions. The Achilles tendon, the largest and strongest tendon in the body, is critical for lower limb function and prone to a variety of pathologies, particularly in athletes and active individuals. This paper explores the normal sonographic anatomy of the Achilles tendon, common pathological findings-including tendinopathy, t
