Case Report: Venoarterial Extracorporeal Membrane Oxygenation Support for Caowu-Induced Cardiac Arrest.

Introduction: Caowu, the main root of the Aconitum plant, is widely used in China. Aconitine is the main toxic component of Aconitum, which can cause a variety of malignant arrhythmias and lead to death. Four patients who developed malignant arrhythmia after drinking medicinal wine containing Caowu were reported in this study. Cardiac arrest occurred soon after symptom onset. All patients received venoarterial extracorporeal membrane oxygenation (VA-ECMO) support after conservative medical treatment had failed. Patients who were directly transferred to our hospital received VA-ECMO support ear

Pharmacological and electrophysiological characterization of lithium ion flux through the action potential sodium channel in neuroblastoma X glioma hybrid cells.

Interaction of Li+ with the voltage-dependent Na+ channel has been analyzed in neuroblastoma X glioma hybrid cells. The cells were able to generate action potentials in media containing Li+ instead of Na+. The uptake of Li+ into the hybrid cells was investigated for the pharmacological analysis of Li+ permeation through voltage-dependent Na+ channels. Veratridine and aconitine increased the uptake of Li+ to the same degree (EC50 30 microM). This increase was blocked by tetrodotoxin (IC50 20 nM). Veratridine and aconitine did not act synergistically; however, the veratridine-stimulated influx w

Nuciferine analogs block voltage-gated sodium, calcium and potassium channels to regulate the action potential and treat arrhythmia.

Dysfunction of the Nav1.5, Cav1.2, and Kv channels could interfere with the AP and result in arrhythmias and even heart failure. We herein present a novel library of nuciferine analogs that target ion channels for the treatment of arrhythmias. Patch clamp measurements of ventricular myocytes revealed that 6a dramatically blocked both the INa and ICa without altering the currentvoltage relationship (including the activation potential and peak potential), accelerated the inactivation of Nav and Cav channels and delayed the resurrection of these channels after inactivation. Additionally, 6a signi

Sodium Channel-Mediated Ventricular Arrhythmia After Delphinium denudatum Ingestion Managed Conservatively in a Resource-Limited Setting.

BACKGROUND: Delphinium denudatum (jadwār) is a traditional South Asian herbal root containing aconitine-like alkaloids that cause sodium channel-mediated cardiotoxicity. Human toxicity data remain limited, particularly from resource-limited settings. CASE SUMMARY: A man in his late 20s presented 6 hours after accidental ingestion of Ddenudatum root with palpitations and autonomic symptoms. Continuous electrocardiographic monitoring revealed frequent ventricular ectopy with nonsustained polymorphic ventricular tachycardia. He was treated with intravenous lidocaine and magnesium sulfate, result

Effects of ranitidine and cimetidine on experimentally induced ventricular arrhythmias in anaesthetized rats.

The effects of two histamine H2-receptor antagonists, ranitidine and cimetidine, on ventricular arrhythmias induced by acute coronary artery ligation and by aconitine infusion were studied in pentobarbitone-anaesthetized rats. The changes in arterial blood pressure and heart rate were also observed. It was found that both drugs significantly reduced the incidence, and prolonged the time of onset, of ventricular tachycardia and ventricular fibrillation following acute coronary artery ligation; however, they did not significantly alter the incidence or time of onset of ventricular dysrhythmias c

Jesaconitine monitoring in a case of severe aconitum poisoning with torsade de pointes treated via extracorporeal membrane oxygenation.

BACKGROUND: Aconitum poisoning can cause severe arrhythmias. We report, for the first time, the detailed blood and urine concentrations of four aconitine alkaloids in a male patient in his 20s who ingested aconite roots with suicidal intent. CASE PRESENTATION: The patient developed refractory torsade de pointes (TdP) and required veno-arterial extracorporeal membrane oxygenation. His TdP resolved 7 h after arrival, with sinus rhythm returning within 12 h. The patient was discharged 6 days later. Subsequent measurements of the four alkaloids over time showed that jesaconitine had the highest s

Sodium channel neurotoxins as probes of the knockdown resistance mechanism.

A simple paralysis bioassay was used to measure the effects of sodium channel blockers and activators on third instar larvae of three strains of the house fly, Musca domestica. The pyrethroid-resistant strains kdr and super-kdr, which are thought to have rediced neural sensitivity to pyrethroids, displayed 28-fold and greater than 909-fold resistance to deltamethrin when compared to the pyrethroid-susceptible NAIDM strain. The kdr and super-kdr strains were also at least 16-fold resistant to the plant alkaloid aconitine. Previous studies on the interaction of pyrethroids and neurotoxins simila

Pharmacological investigation of a new anti-arrhythmic agent, GYKI-23 107.

GYKI-23 107 is a new antiarrhythmic substance with local anaesthetic activity. Its specific pharmacological and cardiovascular effects were studied in vivo and its efficacy was compared with that of lidocaine and mexiletine. GYKI-23 107 was effective against chemically (aconitine and ouabain) induced arrhythmias after both parenteral and oral administration. In aconitine-induced arrhythmia in mice the new compound was more active than either mexiletine or lidocaine after i.p. treatment. In ouabain-induced arrhythmia in dogs, the ED50 of GYKI-23 107 was approximately half that of mexiletine aft

Inhibition of catecholamine secretion from adrenal medulla cells by neurotoxins and cholinergic antagonists.

The effects of several neurotoxins and cholinergic antagonists on the nicotine-induced secretion of catecholamines by adrenal medulla cells in culture were investigated. Aconitine, veratridine, and batrachotoxin, in the presence of 1 micrometer-tetrodotoxin inhibited the nicotine-stimulated secretion of catecholamines in a dose-dependent manner in Locke's solution. In Na+-free sucrose medium, tetrodotoxin was not required to inhibit the stimulatory effects of aconitine, veratridine, and batrachotoxin, and these agents by themselves inhibited the nicotine-stimulated secretion of catecholamines.

[Short term effects of carbon disulfide on the intracardial irritation development and transmission in the rat].

Studies were conducted into effects of acute and subacute CS2 application on intracardiac irritability and conduction in rat. Aconitine-induced arrhythmia and the coronary occlusion method were used as pathophysiological models. Under physiological conditions, short-time exposure to CS2 caused deceleration of intracardiac impulse conduction, while under pathophysiological conditions, modified arrhythmia or reduced survival rate was the result.

Antiarrhythmic activity of taurhythman.

Antiarrhythmic properties of taurhythman were demonstrated on experimental models of ventricular (early occlusion and calcium chloride-induced) and atrioventricular (aconitine-induced) arrhythmias. The preparation reduced or prevented episodes of paroxysmal tachycardia and ventricular fibrillation, decreased the incidence of arrhythmias, and increased the lethal dose (LD) of arrhythmogenic agents. By its efficiency, taurhythman was superior to procainamide and comparable to lidocaine.

Pharmacokinetics-based chronoefficacy of Fuzi against chronic kidney disease.

OBJECTIVES: Identifying drugs with time-varying efficacy or toxicity, and understanding the underlying mechanisms would help to improve treatment efficacy and reduce adverse effects. In this study, we uncovered that the therapeutic effect of Fuzi (the lateral root of Aconitum carmichaelii Debeaux) depended on the dosing time in mice with adenine-induced chronic kidney disease (CKD). METHODS: The Fuzi efficacy was determined by biomarker measurements [i.e. plasma creatinine (CRE), blood urea nitrogen (BUN) and urinary N-acetyl-β-D-glucosaminidase (NAG)], as well as inflammation, fibrosis and h

Site of anticonvulsant action on sodium channels: autoradiographic and electrophysiological studies in rat brain.

The anticonvulsants phenytoin and carbamazepine interact allosterically with the batrachotoxin binding site of sodium channels. In the present study, we demonstrate an autoradiographic technique to localize the batrachotoxin binding site on sodium channels in rat brain using [3H]batrachotoxinin-A 20-alpha-benzoate (BTX-B). Binding of [3H]BTX-B to brain sections is dependent on potentiating allosteric interactions with scorpion venom and is displaced by BTX-B (Kd approximately 200 nM), aconitine, veratridine, and phenytoin with the same rank order of potencies as described in brain synaptosomes

[Effects of bonnecor on transmembrane potentials of cardiomyocytes of the ventricular contractile myocardium of the intact heart and ventricular fibrillation].

The microelectrode technique has shown that bonnecor given in a dose of as many as 2 mg/kg increases the action potential amplitude, but when its dose is increased up to 3 mg/kg, the agent decreases the action potential amplitude of ventricular cardiomyocytes in the guinea-pig, the resting potential remaining steady. Administration of bonnecor prevents ventricular fibrillation after aconitine application and provokes ventricular fibrillation and flutter when the focus of ectopic automatism is formed.
