PURPOSE: Current treatments for osteosarcoma (OS) have a poor prognosis, particularly for patients with metastasis and recurrence, underscoring an urgent need for new targeted therapies to improve survival. Targeted alpha-particle therapy selectively delivers cytotoxic payloads to tumors with radiolabeled molecules that recognize tumor-associated antigens. We have recently demonstrated the potential of an FDA approved, humanized anti-GD2 antibody, hu3F8, as a targeted delivery vector for radiopharmaceutical imaging of OS. The current study aims to advance this system for alpha-particle therapy

Prostate-specific membrane antigen (PSMA) is a validated theranostic target with clinical relevance extending beyond prostate cancer to PSMA-expressing neovasculature in solid tumors. Despite the success of 177Lu-PSMA therapy, a significant proportion of patients experience disease recurrence or partial responses, motivating the development of targeted alpha therapy (TAT) approaches. In this study, novel monomeric and dimeric 225Ac-labeled PSMA inhibitors, [225Ac]Ac-DOTA-iPSMA ([225Ac]Ac-D-iPSMA) and [225Ac]Ac-MACROPA-iPSMA ([225Ac]Ac-M-iPSMA), were designed through receptor-based computationa

Targeted radionuclide therapy represents a promising therapeutic method for treating cancer. However, currently approved radioligands still require improvement in terms of tumor uptake and retention. This study employed molecular simulation to design a novel radioligand, designated PSMA-MAL-5, which covalently binds to the prostate-specific membrane antigen (PSMA). This ligand can be efficiently radiolabeled with the beta emitter (177Lu) and the alpha emitter (225Ac) and exhibits a high level of radiostability. Autoradiography confirmed that 177Lu-PSMA-MAL-5 binds covalently to PSMA. In vivo e

Prostate-specific membrane antigen (PSMA), overexpressed in prostate cancer, has become a popular target for radionuclide-based theranostic applications in the advanced stages of prostate cancer. We conducted a meta-analysis of the therapeutic effects of PSMA-targeting α-therapy (225Ac-PSMA radioligand therapy [RLT]) in patients with metastatic castration-resistant prostate cancer (mCRPC). Methods: A systematic search was performed using the keywords "mCRPC," "225Ac-PSMA," and "alpha therapy." Therapeutic responses were analyzed as the pooled proportions of patients with more than a 50% prosta

Myeloid cells are key mediators of immunosuppression and treatment resistance in primary brain tumors, including glioblastoma (GBM). This study aims to eradicate CD11b+ immunosuppressive cells at the tumor site to enhance overall survival in a model of GBM using an α-emitting radiopharmaceutical therapy targeted to tumor-associated myeloid cells as a monotherapy or in combination with immune checkpoint inhibitors. An anti-CD11b (αCD11b) antibody was modified for radiolabeling with diagnostic (zirconium-89) or therapeutic (actinium-225) radioisotopes. Initial PET imaging and biodistribution stu

Despite the increasing availability of systemic therapies to treat advanced hepatocellular carcinoma (HCC), the prognosis remains poor. There is therefore an unmet need for targeted therapies with a novel mode of action that are more tolerable and effective than current treatment options. Targeted α-therapies selectively deliver high-energy α-particle-emitting radionuclides to tumor-associated cell surface antigens, inducing difficult-to-repair DNA double-stranded breaks, while limiting damage to surrounding tissue. Elevated expression of the oncofetal cell surface heparan sulfate proteoglycan

PURPOSE: 225Ac-Targeted alpha therapy is a potent and promising option for patients with metastatic castration-resistant prostate cancer (mCRPC) and failure of guideline-based therapies and 177Lu-PSMA-radioligand therapy. Unfortunately, side effects associated with TAT can significantly affect quality of life. A combination treatment regimen adding 177Lu and reducing 225Ac activities may mitigate side effects while preserving sufficient anti-tumour efficacy. We therefore evaluated different combinations [225Ac]Ac-/[177Lu]Lu-PSMA-I&T (ALCT) with regard to response and adverse events.

RATIONALE: Combining targeted alpha and beta therapy may address challenges such as toxicity, treatment resistance, and tumor heterogeneity. We evaluated the feasibility and therapeutic effectiveness of a DOTA-PRIT approach using a 177Lu/225Ac radioisotope cocktail, directly compared with monotherapies targeting GPA33 in human colorectal cancer (CRC) xenografts in mice.
METHODS: A three-step pretargeting regimen was employed: an anti-GPA33/anti-DOTA bispecific antibody (BsAb), a dendrimeric clearing agent, and radioligands labeled with 177Lu, 225Ac, alone or in combination. Serial biodistribu

Objective.Targeted Alpha Therapy (TAT) is a promising approach for treating metastatic cancers, utilizing alpha-emitting radionuclides conjugated to tumor-targeting molecules. Actinium-225 (225Ac) has emerged as a clinically relevant candidate due to its decay chain, which produces four successive alpha emissions, effectively damaging cancer cells. However, the nuclear recoil effect can lead to off-target redistribution of decay daughters, complicating dosimetry and increasing potential toxicity. This study aims to address these challenges by developing a direct alpha spectroscopy method forin

225Ac-labeled (S)-ibuprofen-diaminobutyric acid-PSMA (SibuDAB) is a novel, albumin-binding compound in prostate-specific membrane antigen (PSMA)-targeted α-therapy of prostate cancer. It showed superior preclinical efficacy results to [225Ac]Ac-PSMA-617. 225Ac decays via multiple α-particle emissions. The full therapeutic potential requires fast tumor uptake and retention of parent and daughter nuclides, such as 213Bi, in the tumor. High decay energies lead to daughter nuclide recoil of the chelator and, therefore, altered pharmacokinetics and potential irradiation of off-target organs. We her

BACKGROUND: Metastatic castration resistance prostate cancer (mCRPC) is a challenging disease with a significant burden of mortality and morbidity. Most of the patients attain resistance to the available treatments, necessitating further novel therapies in this clinical setting. Actinium 225 (225Ac) prostate-specific membrane antigen (PSMA) radioligand therapy has emerged as a promising option and has been utilized for the last decade. Although a few meta-analyses were performed on the efficacy and safety of 225Ac-PSMA RLT in mCRPC patients, several current studies have been added to the liter

UNLABELLED: The recently developed metabolically more stable minigastrin derivative, DOTA-CCK-66, displayed promising preclinical data when labeled either with 68Ga or 177Lu. First positron emission tomography/computed tomography (PET/CT) imaging using [68Ga]Ga-DOTA-CCK-66 in two patients suffering from medullary thyroid carcinoma (MTC) displayed a favorable biodistribution profile. Here, we aim to investigate the therapeutic potential of [225Ac]Ac-DOTA-CCK-66 as a targeted α-therapy (TAT) agent in a comparative treatment study of [177Lu]Lu- versus [225Ac]Ac-DOTA-CCK-66.

Background: Pancreatic ductal adenocarcinoma (PDAC) presents a formidable challenge in oncology due to its aggressive nature and resistance to therapy. Current treatments, including surgery, chemotherapy, and radiotherapy, have limited success in improving patient outcomes. This study addresses the urgent need for novel radiotheranostic strategies for PDAC by investigating EphA2 as a potential target. Methods and Results: Analysis of genomic data from the Cancer Cell Line Encyclopedia (CCLE) and The Cancer Genome Atlas (TCGA) revealed elevated EphA2 expression in PDAC, confirmed by immunohisto

PURPOSE: Metastatic castration-resistant prostate cancer has limited treatment options and a poor prognosis. Recently, prostate-specific membrane antigen (PSMA)-targeted alpha-particle therapy agents using actinium-225 (225Ac) have shown promising results in prostate cancer treatment, but a significant fraction of patients with advanced mCRPC demonstrate loss of PSMA expression. We have previously reported that PSMA-null and PSMA-positive tumors can be detected and treated effectively with CD46-targeted radiopharmaceuticals. This study evaluates the CD46-targeting PET imaging agent [89Zr]DFO-Y

BACKGROUND: Peritoneal metastasis with micrometastatic cell clusters is a common feature of advanced ovarian cancer. Targeted alpha therapy (TAT) is an attractive approach for treating micrometastatic diseases as alpha particles release enormous amounts of energy within a short distance. A pretargeting approach - leveraging the inverse-electron-demand Diels-Alder reaction between tetrazines (Tz) and trans-cyclooctene (TCO) - can minimize off-target toxicity related to TAT, often associated with full-length antibodies. We hypothesized that a pretargeting strategy could effectively treat high-gr

Botulinum Toxin injections into salivary glands (SG) up to a total dose of 100 units IncobotulinumtoxinA (IncoA) represent the treatment of choice for sialorrhea. However, BTX might also protect SG against sialotoxic radioligand cancer therapies. The radioligand Actinium-225-PSMA effectively targets Prostate Cancer (PCa) metastases but inevitably destroys SG due to unintended gland uptake. A preliminary case series with regular-dose IncoA failed to reduce SG PSMA-radioligand uptake. We therefore increased IncoA dosage in combination with transdermal scopolamine until a clinically relevant SG P

PURPOSE: Trophoblast cell surface antigen-2 (Trop-2) is a membrane-bound antigen associated with cancer invasiveness and poorer patient outcomes. Delivery of therapeutics via Trop-2 targeting antibody-drug conjugates (ADC) has proven to be a viable and FDA-approved model. However, in the clinic, ADC dosing is not based on Trop-2 positivity or expression, leading to potential adverse effects if the antigen membrane level on cancer cells is insufficient for targeting. The challenge of adequate patient and therapy selection can be overcome by leveraging the theranostic approach. We previously est

PURPOSE: The spatial distribution of radiopharmaceuticals within multicellular clusters is known to have a significant effect on their biological response. Most therapeutic radiopharmaceuticals distribute nonuniformly in tissues which makes predicting responses of micrometastases challenging. The work presented here analyzes published temporally dependent nonuniform activity distributions within tumor spheroids treated with actinium-225-DOTA encapsulating liposomes (225Ac-liposomes) and uses these data in MIRDcell V3.11 to calculate absorbed dose distributions and predict biological response.

Around 150 million people are at risk from arsenic-contaminated groundwater in India and Bangladesh. Multiple metal analysis in Bangladesh has found other toxic elements above the World Health Organization (WHO) health-based drinking water guidelines which significantly increases the number of people at risk due to drinking groundwater. In this study, drinking water samples from the Bongaon area (North 24 Parganas district, West Bengal, India) were analyzed for multiple metal contamination in order to evaluate groundwater quality on the neighbourhood scale. Each sample was analyzed for arsenic
