INTRODUCTION: An increasing number of patients with metastatic colorectal cancer (mCRC) have achieved no evidence of diseases (NED) status after surgery or other treatments. However, the latest guidelines for colorectal cancer do not recommend an appropriate treatment for patients with mCRC who achieve NED status. Capecitabine metronomic chemotherapy has the advantages of significant efficacy and minimal adverse reactions, it is a potential effective method for maintenance treatment for mCRC, but no RCTs have been reported. Therefore, we designed a randomized controlled trial to evaluate the e

Here we report long-term outcomes of adjuvant metronomic capecitabine in patients with locoregionally advanced nasopharyngeal carcinoma. In this multicenter, open-label, parallel-group, randomized, controlled, phase 3 trial, 406 patients who had completed definitive chemoradiotherapy were randomly assigned (1:1) to receive either adjuvant metronomic capecitabine (650 mg m-2 twice daily for 1 year) or observation. The primary endpoint was failure-free survival; secondary endpoints included overall survival (OS), distant failure-free survival, locoregional failure-free survival, and safety, as r

OBJECTIVE: We describe experimental and theoretical premises of a powerful cancer therapy based on the combination of three approaches. These include (I) in situ vaccination (intratumoral injections of CpG oligonucleotides and anti-OX40 antibody); (II) chronometric or metronomic low-dose cyclophosphamide (CMLD CP)-based chemotherapy; (III) cancer stem cell-eradicating therapy referred to as Karanahan (from the Sanskrit kāraṇa ["source"] + han ["to kill"]).
BACKGROUND: In murine models, the first two approaches are particularly potent in targeting immunogenic tumors for destruction. In situ va

OBJECTIVE: Capecitabine (CAP) is an orally administered prodrug of fluorouracil, predominantly utilized in the treatment of solid tumors. Triggering tumor ferroptosis is an important mechanism for the treatment of hepatocellular carcinoma (HCC). However, the potential of CAP to induce ferroptosis in HCC, along with the underlying mechanisms, remains unknown.
METHODS: In this study, a subcutaneous HCC model was constructed using the syngeneic Hepa1-6 cell line in C57BL/6 mice, followed by treatment with metronomic CAP (mCAP). The anti-tumor effects of mCAP were evaluated by monitoring tumor vo

PURPOSE: Metronomic chemotherapy with oral capecitabine + vinorelbine (Cape + VNL) provides synergistic cytostatic activity and antiangiogenic and immunomodulatory effects, potentially offering prolonged disease control with limited toxicity in HER2-negative metastatic breast cancer (MBC). However, efficacy in the real-world (RW) setting, especially in late lines, and the impact of dihydropyrimidine dehydrogenase (DPYD) polymorphisms on dose reduction and safety remain limited.
METHODS: In this retrospective study, 200 patients with human epidermal growth factor receptor 2 (HER2)-negative MBC

Maintenance treatment after first-line chemotherapy for patients with metastatic colorectal cancer (mCRC) is a priority strategy. However, which medicine is chosen is controversial. This study aimed to determine the efficacy and safety of maintenance treatment with metronomic capecitabine vs observation. In this randomized controlled trial, patients who completed 18 weeks of induction chemotherapy with XELOX and achieved disease control were randomly assigned centrally (1:1) to receive maintenance therapy with metronomic chemotherapy or observation until disease progression. The primary endpoin

BACKGROUND: The standard of care in recurrent/metastatic head and neck squamous cell carcinoma is Pembrolizumab with/without chemotherapy. However, these regimens are minimally affordable and accessible in resource-limited countries. Hence, immunotherapy is combined with oral metronomic chemotherapy in lower doses. Currently, the efficacy and safety of this regimen have been studied in platinum-refractory settings and not beyond them. Thus, we conducted a real-world study on the effectiveness of low-dose immunotherapy and metronomic chemotherapy in recurrent/metastatic head and neck cancer. ME

Metronomic chemotherapy is designed to leverage low-dose scheduling to remodel the tumor microenvironment, yet subtherapeutic exposure may also activate tumor-supportive programs. Here, we investigated whether Gemcitabine (GEM) exhibits a dose-dependent bidirectional phenotype in an immunocompetent 4 T1 breast cancer model and defined accompanying vascular and immune changes. Across a graded GEM dosing range, low-dose exposure promoted tumor progression, increased angiogenesis-associated markers CD31 and laminin, elevated circulating pro-angiogenic cell signatures marked by CD61 and VEGFR2, in

PURPOSE: Advanced head and neck squamous cell carcinoma (HNSCC) carries a poor prognosis, particularly in resource-limited settings with restricted access to targeted or immune therapies. We evaluated whether adding triple oral metronomic chemotherapy (OMCT; erlotinib, celecoxib, methotrexate) to paclitaxel-carboplatin (PC) improves overall survival (OS) in patients with platinum-sensitive, unresectable advanced HNSCC. This was a single-center, investigator-initiated, prospective, randomized, open-label, phase III superiority trial conducted at a tertiary cancer center in North India.

Treatment options for dogs with metastatic (stage III) splenic hemangiosarcoma are limited. A doxorubicin-based chemotherapy regimen is commonly administered; however, there are no published data to support this practice. The aim of this study was to investigate the impact of maximum-tolerated-dose chemotherapy (MTD), metronomic chemotherapy (MC) and no adjuvant treatment on outcome in dogs with stage III splenic hemangiosarcoma undergoing splenectomy. Medical records of dogs with stage III splenic hemangiosarcoma that underwent splenectomy followed by MTD chemotherapy, MC or no adjuvant treat

Medulloblastoma, the most common malignant brain tumor of childhood, is an aggressive embryonal tumor that arises from the posterior fossa. On the molecular level, four clinically relevant subgroups have been established, which have already been integrated into routine diagnostic procedures and treatment stratification. The initial step in treating medulloblastoma typically involves maximal safe surgical resection, followed by craniospinal irradiation in most patients (except very young children) and chemotherapy. Efforts to improve cure rates and reduce long-term detrimental effects led to th

BACKGROUND: HR+/HER2- MBC patients often receive chemotherapy (CHT) following endocrine therapy (ET); however, treatment-related toxicities remain a significant limitation. Metronomic chemotherapy (mCHT) involves the administration of low-dose chemotherapeutic agents at regular intervals without extended breaks. The VICTOR-15 study is a retrospective, matched controlled study designed to compare mCHT with standard chemotherapy (sCHT) in HR+/HER2- metastatic breast cancer (MBC) patients after ET failure. METHODS: We identified consecutive patients treated with mCHT or sCHT between 2015 and 2024
