Attachments: "I Want a Gay Doctor": The Role of Therapeutic Self-disclosure in Child and Adolescent Psychiatry.

A simple, yet pointed question from my teen patients on the inpatient child psychiatry unit made my biggest professional anxiety come true. As a queer psychiatrist-in-training, I spend a lot of time thinking about how I present. Although in my personal life I am more comfortable presenting as queer and pushing the limits of the gender binary, in professional settings, I often mask my authentic self to have passing privilege based on my choice of clothing, tone of voice, and mannerisms. I choose to do this because, in most of medical and psychiatric training, we are taught to rarely-if ever-sel

Justifications for coercive care in child and adolescent psychiatry, a content analysis of medical documentation in Sweden.

BACKGROUND: There has been considerable interest in normative ethics regarding how and when coercive care can be justified. However, only a few empirical studies consider how professionals reason about ethical aspects when assessing the need for coercive care for adults, and even less concerning children and adolescents. The aim of this study was to examine and describe how professionals document their value arguments when considering the need for coercive psychiatric care of young people. METHODS: All 16 clinics that admitted children or adolescents to coercive care during one year in Sweden

[Aesthetic/Plastic Surgery in Children as Seen from the Perspective of Child and Adolescent Psychiatry and Psychotherapy].

Physical deformities may cause psychological stress and lead to psychological disorders in children and adolescents. On the other hand, the correction of non-pathological conditions is a legal issue in patients unable to consent, a group that is partly made up of minors. This article provides an overview on available evidence on the psychological consequences of physical deformities, psychiatric contraindications for plastic surgery due to psychological disorders, and on the issue of minors' ability to consent.

Glutamatergic Agents in the Treatment of Compulsivity and Impulsivity in Child and Adolescent Psychiatry: a Systematic Review of the Literature.

OBJECTIVE: Research has implicated glutamatergic projections between the various frontal subregions in the pathogenesis of compulsivity and impulsivity. Reducing striatal glutamate release, or antagonising the action of glutamate at its receptors, may therefore represent viable treatment strategies. Several glutamatergic agents with regulatory approval for other indications are available and may be of potential benefit in the treatment of compulsivity/impulsivity in psychiatric disorders in paediatric patients. METHOD: This review was performed according to PRISMA guidelines and evaluates ava

Reactive and proactive aggression in children--a review of theory, findings and the relevance for child and adolescent psychiatry.

The clinical population of aggressive children diagnosed as having an oppositional defiant disorder (ODD) or a conduct disorder (CD) is heterogeneous, both with respect to behaviour and aetiology. Recently, the following distinction has been proposed that might further clarify this heterogeneity: reactive aggression is an aggressive response to a perceived threat or provocation, whereas proactive aggression is defined as behaviour that anticipates a reward. In this article we examine various aspects of this distinction. We will [1] examine the evidence that reactive and proactive aggression ar

Review: Identification and Management of Circadian Rhythm Sleep Disorders as a Transdiagnostic Feature in Child and Adolescent Psychiatry.

OBJECTIVE: Sleep disturbances are highly frequent features in a range of child and adolescent psychiatric conditions. However, it is commonly not clear if such sleep problems represent symptomatic features of, comorbidities of, or risk factors for these conditions. It is believed that underlying dysfunction in the daily biological (circadian) clock may play important roles in the etiology of many sleep disorders, and circadian rhythm changes are reported in a number of neuropsychiatric conditions. The aim of this review was to explore the key identifying features of circadian rhythm disorders

ICD-10 classification in Danish child and adolescent psychiatry--have diagnoses changed after the introduction of ICD-10?

The aim was to test this in a nationwide register study of diagnoses used in child and adolescents psychiatry in Denmark. A larger number of different diagnoses were expected to be applied after the introduction of the 10th version of the International Classification of Diseases (ICD-10). Reflecting the time trend, we particularly expected an increase in the number of neuropsychiatric diagnoses. From the Danish Psychiatric Central Register data were drawn on clinical discharge diagnoses. All patients aged 0-15 years examined at psychiatric hospitals from 1995-2002 were included; 22,469 childre

Exploring Diagnostic Classification Prevalence in German and Dutch Child and Adolescent Psychiatry: An International Cross-Sectional Comparative Study.

Classification systems like the DSM-5 and ICD-10 facilitate international comparative research on mental disorders. However, few studies have compared classification distributions in child and adolescent psychiatric settings across countries. This study explored similarities and differences in classification prevalence between German and Dutch children and adolescents referred to psychiatric facilities. Data were retrospectively collected from clinical samples of inpatients and outpatients who underwent diagnostic assessments in Germany (n = 7,089) and the Netherlands (n = 2,574), aged 0-18 ye

Prescribing Patterns in Child and Adolescent Outpatient Psychiatry: A Retrospective Cohort Study of Prescriptions in the First Year.

OBJECTIVE: Recently, excessive and off-label prescribing of psychotropic drugs for children and adolescents have become a problem. Child and adolescent psychiatric patients receive treatment in many departments and department-specific psychotropic drug use has not been analyzed. We surveyed prescriptions for child and adolescent psychiatric outpatients during their first year of treatment. METHODS: The participants were 302 patients, 15 years or younger, who made their first visit to the outpatient psychiatry department of Tokai University Hospital from September 1, 2015 to September 1, 2016.

Predictors of length of stay in inpatient child and adolescent psychiatry: failure to validate an evidence-based model.

OBJECTIVE: To test if predictors of length of stay are based on empirical evidence. METHOD: 1001 regularly terminated treatment episodes from 13 child and adolescent psychiatric hospitals were analysed. In order to cross- validate the results, the sample was randomly divided into a definition sample (n = 500) and a validation sample (n = 501). The variables in the definition sample were screened statistically for their suitability as predictors of logarithmic length of stay (logLOS). Variables shown to be significant and uncorrelated were entered into multifactor analyses of variance in order

Clinical Case Rounds in Child and Adolescent Psychiatry: De Novo Self-Mutilation and Depressive Symptoms in a 17-year-old Adolescent Girl Receiving Depot-Medroxyprogesterone Acetate.

INTRODUCTION: Contraception-induced mood changes have been identified since the 1960s. To our knowledge, there has been no reported case about self-mutilation associated to any form of contraception. We report the case of a 17-year-old adolescent girl who presented with de novo self-mutilation and depressive symptoms three and a half weeks after the administration of 150 mg of Depot-Medroxyprogesterone Acetate (DMPA). METHOD: Clinical case report and literature review. Possible confounding factors are reviewed. RESULTS: The patient had no personal psychiatric history and no significant famil

The implementation of internet-delivered cognitive behaviour therapy for adolescents with insomnia comorbid to psychiatric conditions at two child and adolescent psychiatry clinics.

OBJECTIVE: Insomnia is a risk factor related to the onset and course of other mental disorders and is prevalent among adolescents admitted to psychiatric services. While evidence supports the efficacy of cognitive behavioural therapy for insomnia (CBT-I), uptake of evidence-based interventions in mental health care for children and adolescents is low. Internet delivery can improve access, but research on implementing internet-based CBT (iCBT) in routine care is limited. The aim of this study was to investigate if iCBT for insomnia (iCBT-I) could be successfully implemented at two child and ado

AACAP official action. Summary of the practice parameters for the assessment and treatment of children and adolescents with schizophrenia. American Academy of Child and Adolescent Psychiatry.

These practice parameters review the literature on the assessment and treatment of children and adolescents with schizophrenia. Recommendations are based on the limited research available, the adult literature, and clinical experience. Early-onset schizophrenia is diagnosed using the same criteria as in adults, and it appears to be continuous with the adult form of the disorder. Noted characteristics of youths with schizophrenia include predominance in males, high rates of premorbid abnormalities, and often poor outcome. Differential diagnosis includes psychotic mood disorders, developmental d

Bipolar disorder in children, adolescents and young adults. Part 2. Therapeutic management. Recommendations under the patronage of the Executive Board of the Polish Psychiatric Association, National Consultants in the field of Psychiatry and National Consultants in Child and Adolescent Psychiatry.

Bipolar disorder is characterized by recurrent episodes of depression, mania, hypomania, or mixed states. Early diagnosis and comprehensive treatment, including pharmacotherapy and psychosocial interactions, are associated with a more favorable prognosis. The general principles for the use of drugs in bipolar disorder in children and adolescents remain similar to those in adults. The basic drugs in bipolar disorder are mood stabilizers of the first and second generation; however, their efficacy and safety profile in children and adolescents differ from those in adults. In addition, prophylacti
