Background: Glucagon-like peptide-1 receptor agonists and dual glucose-dependent insulinotropic polypeptide/glucagon-like peptide-1 (GIP/GLP-1) receptor agonists have substantially improved the management of obesity and cardiometabolic disease, providing significant benefits on weight reduction, glycemic control, and cardiovascular outcomes. Despite these advances, gastrointestinal (GI) adverse events remain a major cause of dose reduction, treatment interruption, and poor long-term adherence. Increasing evidence suggests that interactions between incretin-based therapies and the intestinal mi

The co-occurrence of atrial fibrillation (AF) and dementia is increasingly common in aging populations. Anticoagulants reduce the risk of thromboembolic events associated with AF, but they introduce an increased risk of severe bleeding, adherence concerns, and questionable net benefit in the very elderly. This narrative review evaluates the existing literature on anticoagulation in the subset of patients with AF and dementia and aims to clarify clinical guidance on managing AF alongside bleeding risk concerns. Observational cohort studies, meta-analyses, and registry data were analyzed. Multip

The Spanish Asthma Guideline (GEMA) 5.5 marks a significant conceptual and clinical advance in asthma management across Spanish-speaking healthcare systems. This updated edition incorporates the latest scientific insights into the pathophysiology, diagnosis, phenotyping, and treatment of asthma, while maintaining its practical, evidence-based orientation. A key innovation is the redefinition of therapeutic objectives: treatment is no longer limited to symptom control but is directed toward achieving and sustaining clinical remission, following the principles established by the Spanish REMAS co

Telemonitoring, typically incorporating home blood pressure (BP) monitoring, has become an established component of hypertension management. Conventional telemonitoring has primarily relied on remote transmission of BP measurements to support clinician-directed treatment adjustment. However, recent advances in digital health technologies and the emergence of digital hypertension have substantially broadened the role of telemonitoring in hypertension care. Within the framework of digital hypertension, telemonitoring is evolving beyond simple BP surveillance in three major domains. First, advanc

Metabolic dysfunction-associated steatotic liver disease is the most prevalent chronic liver condition, affecting over one-third of the global population, with cirrhosis present in up to 3.3% of cases. Early detection of advanced liver disease in at-risk populations can enable timely intervention, prevent progression, and reduce complications. This review focuses on the current recommendations for early detection of advanced liver disease, evaluates the evidence for the performance of non-invasive tests in the target population for screening, and examines the multifaceted burden of screening,

Autoimmune diseases such as rheumatoid arthritis represent major clinical challenges due to the complexity of immune system dysfunction and the limitations of current treatment strategies. Combination therapies involving JAK inhibitors, methotrexate, and natural compounds such as quercetin can target multiple disease pathways and improve therapeutic outcomes. Previous studies have shown that the concurrent administration of multiple medications may result in reduced patient adherence, systemic adverse effects, and suboptimal tissue targeting. Hydrogel-Forming Microneedles (HFMNs) enable contro

Advanced heart failure therapies, including heart transplantation and durable mechanical circulatory support, are available to a limited number of patients because of the scarcity of donors, expense, and large burden of care. The importance of psychological and social determinants of health, including cognitive status, health literacy, psychopathology, social support, medical adherence, and substance abuse, are emphasized in advanced heart failure and further amplified in the context of mechanical circulatory support and heart transplantation. The psychosocial assessment of advanced heart fail

Depression is a complex psychiatric disorder marked by persistent emotional disturbances such as sadness, hopelessness, and fatigue, frequently accompanied by psychosocial impairments. Current treatment approaches are hindered by limited efficacy, poor patient adherence, and the inability of many therapeutic agents to effectively penetrate the blood-brain barrier (BBB). The BBB, a selective and protective interface between the bloodstream and brain tissue, restricts drug delivery to the central nervous system (CNS), resulting in suboptimal concentrations of antidepressants at the target site a

Multiple landmark trials have helped to advance the treatment of heart failure with reduced ejection fraction (HFrEF) significantly over the past decade. These trials have led to the introduction of four main drug classes into the 2021 ESC guideline, namely angiotensin-receptor neprilysin inhibitors/angiotensin-converting-enzyme inhibitors, beta-blockers, mineralocorticoid receptor antagonists, and sodium-glucose cotransporter-2 inhibitors. The life-saving effect of these therapies has been shown to be additive and becomes apparent within weeks, which is why maximally tolerated or target doses

OBJECTIVE: Topical retinoids have been a cornerstone in the management of acne vulgaris for over 5 decades, and they remain important even as treatment paradigms evolve. There is an ongoing need to understand the history and context of current retinoid use to support future innovations and improvements in the care of patients with acne.
METHODS: Peer-reviewed literature was searched to identify articles related to topical retinoids, including their development, receptor selectivity, mechanisms of action, efficacy, tolerability, and role in the treatment of acne and management of acne sequelae

BACKGROUND: Adults with cardiovascular disease frequently experience anxiety, sleep disturbance, and challenges with long-term treatment adherence. These factors are associated with poorer quality of life, reduced engagement in guideline-directed therapy, and increased healthcare utilization. Although nurses are well-positioned to address these needs, nurse-led behavioral interventions often encounter barriers to sustained adoption in routine clinical practice.
OBJECTIVE: In this study, we aimed to describe a theory-informed implementation blueprint for integrating a nurse-led coping-sleep-ad

CONTEXT: Many patients with potentially curable cancer do not complete their prescribed treatment regimens because of the toxicity. There is evidence that the common endpoints of many of these toxicities are amenable to quality of life (QOL)-directed interventions.
OBJECTIVES: This study was conducted to determine the effect of a multidisciplinary QOL-directed intervention on patients' adherence to planned chemoradiation (CR) regimens.
METHODS: The results of two randomized controlled trials that used the same QOL intervention were pooled to form a cohort of 61 patients with advanced localiz

About 10 million people in China are infected with hepatitis C virus (HCV), with the seroprevalence of anti-HCV in the general population estimated at 0.6%. Delaying effective treatment of chronic hepatitis C (CHC) is associated with liver disease progression, cirrhosis, hepatocellular carcinoma, and liver-related mortality. The extrahepatic manifestations of CHC further add to the disease burden of patients. Managing CHC-related advanced liver diseases and systemic manifestations are costly for both the healthcare system and society. Loss of work productivity due to reduced well-being and qua

INTRODUCTION: Hepatitis C is a blood-borne virus (HCV) that can seriously damage the liver and is spread mainly through blood-to-blood contact with an infected person. Over 85% of individuals who have HCV in Scotland became infected following injecting drug use. Since people who inject drugs (PWID) are the main source of new infections, theoretical modelling has suggested that treatment of HCV infection in PWID may effectively reduce HCV prevalence and accomplish elimination. This protocol describes a clinical trial delivering HCV treatment within injecting equipment provision sites (IEPS) in

PURPOSE OF REVIEW: HIV management in people who use drugs (PWUD) is typically complex and challenging due to the presence of multiple medical and psychiatric comorbidities as well as social, physical, economic and legal factors that often disrupt the HIV continuum of care. In this review, we describe the individual, health systems and societal barriers to HIV treatment access and care retention for PWUD. In addition, the clinical management of HIV-infected PWUD is often complicated by the presence of multiple infectious and noninfectious comorbidities.
RECENT FINDINGS: Improved HIV treatment

Neurosurgery is a high-risk specialty currently undertaking the pursuit of systematic approaches to reducing risk and to measuring and improving outcomes. The authors performed a review of patterns and frequencies of adverse events in neurosurgery as background for future efforts directed at the improvement of quality and safety in neurosurgery. They found 6 categories of contributory factors in neurosurgical adverse events, categorizing the events as influenced by issues in surgical technique, perioperative medical management, use of and adherence to protocols, preoperative optimization, tech

BACKGROUND: 29 autoimmune diseases, including Rheumatoid Arthritis, gout, Crohn's Disease, and Systematic Lupus Erythematosus affect 7.6-9.4% of the population. While effective therapy is available, many patients do not follow treatment or use medications as directed. Digital health and Web 2.0 interventions have demonstrated much promise in increasing medication and treatment adherence, but to date many Internet tools have proven disappointing. In fact, most digital interventions continue to suffer from high attrition in patient populations, are burdensome for healthcare professionals, and ha

Neovascular age-related macular degeneration (nAMD) is a major cause of irreversible vision loss in the elderly, driven by choroidal neovascularization and dysregulated vascular endothelial growth factor (VEGF) signaling. While anti-VEGF injections have transformed management, their frequent administration imposes a substantial burden on patients and limits adherence. Adeno-associated virus (AAV)-based gene therapy offers sustained intraocular delivery of anti-angiogenic agents with a single treatment, potentially overcoming these limitations. This review summarizes the rationale for AAV use i

Dental caries, a significant global health concern, is intricately linked to the development and persistence of cariogenic biofilms on tooth surfaces. These complex microbial ecosystems orchestrate a multifactorial process of decay, adhering to teeth, producing acids that erode enamel, and exhibiting resistance to traditional therapies. This review delves into the intricate mechanisms of cariogenic biofilm formation and pathogenesis, exploring adhesion, acid production, extracellular polymeric substance (EPS) mediated acid tolerance, and nutrient acquisition within the biofilm. We then explore
