Postgastrectomy syndromes.

Anatomic and physiological changes introduced by gastric surgery result in postgastrectomy syndromes in approximately 20% of patients. Most of these disorders are caused by operation-induced abnormalities in the motor functions of the stomach, including disturbances in the gastric reservoir function, the mechanical-digestive function, and the transporting function. Division of the vagal innervation to the stomach and ablation or bypass of the pylorus are the most significant factors contributing to postgastrectomy syndromes. Either rapid or slow emptying may result, depending on the relative i

Etiology of phantom limb syndrome: Insights from a 3D default space consciousness model.

In this article, we examine phantom limb syndrome to gain insights into how the brain functions as the mind and how consciousness arises. We further explore our previously proposed consciousness model in which consciousness and body schema arise when information from throughout the body is processed by corticothalamic feedback loops and integrated by the thalamus. The parietal lobe spatially maps visual and non-visual information and the thalamus integrates and recreates this processed sensory information within a three-dimensional space termed the "3D default space." We propose that phantom l

Afferent loop obstruction after gastrectomy simulating acute pancreatitis.

After gastrectomy and Billroth II anastomosis, if the afferent loop is too long, it may kink and herniate, resulting in obstruction. If it is too short, it may produce acute angulation with obstruction. This condition often simulates acute pancreatitis when the amylase level in urine and serum is increased. Three cases of afferent loop obstruction are reported. The patient who did not undergo laparotomy died; the other two were operated on and survived. The diagnosis, etiology and treatment of these complications are discussed, and similar cases reported in the literature are cited.

Evaluation of recurrent duodenal ulcer after vagotomy-pyloroplasty.

Seventy-five antrectomies were performed for recurrent duodenal ulcer after 1,515 vagotomy and drainage procedures. Eighty-four per cent of proved recurrences at reoperation were associated with positive acid secretory tests. Seventy-eight per cent of recurrences occurred in patients with intact vagal trunks. Incomplete vagotomy is implicated as the cause of recurrence.

Postgastrectomy Syndromes and Nutritional Considerations Following Gastric Surgery.

Postgastrectomy syndromes result from altered form and function of the stomach. Gastrectomy disrupts reservoir capacity, mechanical digestion and gastric emptying. Early recognition of symptoms with prompt evaluation and treatment is essential. Many syndromes resolve with minimal intervention or dietary modifications. Re-operation is not common but often warranted for afferent and efferent loop syndromes and bile reflux gastritis. Preoperative nutritional assessment and treatment of common vitamin and mineral deficiencies after gastrectomy can reduce the incidence of chronic complications. An

A woman with abdominal pain and bilious vomiting. A very late aftermath of Billroth II gastrectomy.

Patients with a history of Billroth II gastrojejunostomy who present with a symptom complex of postprandial nausea, fullness, and bilious vomiting leading to relief should be suspected of having an afferent loop syndrome. Diagnosis depends on barium radiography and upper intestinal endoscopy. Surgical correction is the treatment. The current age of medical therapy has dramatically decreased the frequency and necessity of surgery for peptic ulcer disease. However, we should not forget the lessons of the past and fail to diagnose a patient who has a chronic complication of a previously common op

[Postgastrectomy syndromes. Development of the aspects in past and present (author's transl)].

After gastric operations about 20 to 30% of all patients complain of an abundance of postoperative gastrointestinal and vasomotor complaints. Research into these "postgastrectomy syndromes" is carried out periodically. In the beginning of this century concepts as precipitated evacuation, dumping stomach, too narrow or too wide anastomosis predominate. In the thirties the subdivision into a so-called early and late dumping syndrome follows. The description and treatment of the afferent loop syndrome in the middle of this century suggest the introduction of functional factors into the explanatio

A novel role for surfactant in the lung with implications for the sudden infant death syndrome.

Feedback from mechanoreceptors in the lungs to the brainstem has long been known to be vital for control of rhythmogenesis during normal breathing. Action potentials transmitted via the vagi are shown to display an irreversible relationship with respect to lung area (A), i.e. hysteresis, which closely resembles the hysteresis in surface tension (gamma) versus A for normal surfactant which coats that surface. Since lung recoil is largely determined by surface tension for resting tidal volumes, this will dominate the stretch of mechanoreceptors. Hence, it is postulated that, through the Hering-B

Alkaline reflux gastritis.

Any surgical procedure that ablates the pyloric sphincter mechanism permits increased reflux of duodenal fluid into the stomach or gastric remnant. Although it is reported as most common with Billroth II gastrectomy, our experience indicates that reflux is nearly as frequent after Billroth I gastroduodenostomy and is not at all infrequent after pyloroplasty. The precise constituents of duodenal fluid which damage the gastric mucosa remain controversial. The best present evidence is that the bile acids are probably essential, but that one or more other constituents of duodenal content are also

[The indications for the use and the technical characteristics of gastric resection by Roux's method].

Based on findings of general diagnosis in 62 patients the authors have determined indications for using Roux resection of the stomach which was used as primary operation in 22 patients and repeated operation--in 40 patients. Main indications for Roux resection were chronic disturbance of duodenal patency, chronic pancreatitis, afferent loop syndrome, reflux-gastritis. The work describes technical peculiarities of the Roux resection of the stomach with necessary observation of 6 pathogenetically substantiated rules. Results of Roux resection of the stomach are good, no evacuatory disorders of t

[Clinical studies on the complaints of survivors living more than 3 months after total gastrectomy].

From 1973 to 1983, 802 patients with gastric cancer were operated on. Out of them, 292 (36.4%) received total gastrectomy. Reconstruction was performed mainly by the Billroth II procedure associated with the closure of the afferent loop according to Plenk's method. 90 patients living more than 3 months complained of the following: heartburn, 18 (20%); reflux, 12 (13.3%); retrosternal pain, 3 (3.3%); stenotic sensation, 23 (25.6%); diarrhea, 10 (11.1%); abdominal pain, 14 (15.6%); and dumping syndrome, 6 (6.7%). It seems to indicate that the quality of life after total gastrectomy is satisfacto

Obstruction of defunctionalized small bowel: its occurrence after bypass surgery for morbid obesity.

Complications associated with jejunoileal bypass for morbid obesity are being recognized more frequently. A variety of mechanical obstructions in the defunctionalized small-bowel segment have recently been corrected in seven surgical patients. Volvulus of the defunctional limb was the most frequent cause of obstruction. Intussusception, bypass enteritis, fascial hernia, and adhesive bands were also causes of obstruction. Radiographic contrast studies were valuable in establishing the preoperative diagnosis. The altered small-intestinal anatomy predisposed these patients to a uniquely subtle an

Enterolithiasis.

Enterolithiasis or formation of gastrointestinal concretions is an uncommon medical condition that develops in the setting of intestinal stasis in the presence of the intestinal diverticula, surgical enteroanastomoses, blind pouches, afferent loops, incarcerated hernias, small intestinal tumors, intestinal kinking from intra-abdominal adhesions, and stenosing or stricturing Crohn's disease and intestinal tuberculosis. Enterolithiasis is classified into primary and secondary types. Its prevalence ranges from 0.3% to 10% in selected populations. Proximal primary enteroliths are composed of chole

[Bacterial overgrowth syndrome].

Small bowel bacterial overgrowth is a syndrome caused by an abnormal number of bacteria in the upper part of the small bowel and associated with a complex array of clinical symptoms, i. e., chronic diarrhoea, steatorrhoea, macrocytic anaemia, weight loss, and less commonly, protein-losing enteropathy. The most common underlying factors are small intestinal stagnation or dysmotility, intestinal obstruction, blind or afferent loops, and decreased gastric secretion. The treatment usually consists in the eradication of bacterial overgrowth with repeated courses of antimicrobials, correction of ass

[Cesar Roux and his Roux-en-Y anastomosis].

During the last 100 years, since Cesar Roux (1857-1934) began utilizing his "Loop-en-Y" procedure for gastric outlet obstruction, this technique fell into disfavour but later, with the vagotomy, it was adapted for multiple applications. The greatest factor in its revival has been the treatment of postgastrectomy sequels, including alkaline reflux gastritis, reflux esophagitis, dumping and other syndromes. Additionally Roux-en-Y anastomosis has been used to drain diverse organs as the biliary tract, pancreas and esophagus. The main complication of loop-en-Y is the Roux syndrome, secondary to ga

[Experimental study on the pathogenesis of anastomotic ulcers (author's transl)].

Various forms of operative duodenal exclusion with or without duodenogastric reflux were performed on 54 male Wistar rats. It was demonstrated that after duodenal exclusion with reflux peptic ulcers regularly occur at the site of anastomosis. Continuous reflux of bile and pancreatic secretion is the decisive factor in the development of ulcer at the anastomotic border between stomach and jejunum. Stasis in the excluded duodenum (syndrome of the afferent loop) promotes the ulcerogenic reflux effect. If there is no pylorus, reflux and stasis are potentiated to the highest incidence of ulcer (mor

Jejunal interposition to prevent postgastrectomy syndromes.

BACKGROUND: Postgastrectomy syndromes include reflux gastritis and oesophagitis, dumping syndrome, intractable diarrhoea and afferent loop syndrome. To prevent such syndromes, since January 1994 jejunal interposition has been used following distal gastrectomy. The aim of this study was to evaluate the benefit of this procedure. METHODS: A consecutive series of 42 patients who underwent distal gastrectomy for gastric cancer was studied. Twenty-two patients had a Billroth I procedure before January 1994, and 20 patients had isoperistaltic jejunal interposition using a 10-12-cm segment after Jan
