Surgical wound complications after major lower limb amputations for chronic limb-threatening ischemia.

OBJECTIVE: To investigate rates, risk factors and the impact of surgical wound complications (SWCs) on health care resources after below-knee amputation (BKA) and above-knee amputation (AKA) for chronic limb-threatening ischemia (CLTI). METHODS: This single-center retrospective study included consecutive patients undergoing major amputation for CLTI between 2011 and 2020. The primary end point was SWC, defined as surgical revision, higher amputation, or a nonhealing wound at 1 year. Risk factors for SWCs were studied in multivariable analyses and expressed as odds ratios (ORs) with 95% confid

Surgical Techniques for Performing a Through-Knee Amputation: A Systematic Review and Development of an Operative Descriptive System.

BACKGROUND: Through-knee amputation (TKA) carries potential biomechanical advantages over above knee amputation (AKA) in patients unsuitable for a below-knee amputation. However, concerns regarding prosthetic fit, cosmesis and wound healing have tempered enthusiasm for the operation. Furthermore, there are many described surgical techniques for performing a TKA. This frustrates attempts to compare past and future comparative data, limiting the opportunity to identify which procedure is associated with the best patient centered outcomes. The aim of this systematic review is to identify all the

Assessment of a Novel Augmented Closure Technique for Surgical Wounds Associated with Transmetatarsal Amputation: A Preliminary Study.

BACKGROUND: Transmetatarsal amputation (TMA) is a viable option to avoid major amputation and limb loss in patients with forefoot sepsis, infection, or tissue loss. However, TMAs are associated with a significant incidence of dehiscence, readmission, and reoperation rates ranging from 26% to 63%. To encourage tissue healing, neovascularization, and durable closure, a nonwoven, resorbable, synthetic hybrid-scale fiber matrix whose architecture is similar to native human extracellular matrix was used in an augmented closure technique. We comparatively evaluated clinical outcomes and complication

Early Diagnosis and Surgical Management of Necrotizing Fasciitis of the Lower Extremities: Risk Factors for Mortality and Amputation.

Objective: In this study, we seek to identify patient characteristics associated with limb loss and mortality while exploring the potential impact a multidisciplinary care team may have. Approach: This was a 10-year retrospective review of patients presenting to our tertiary care center for limb salvage with a diagnosis of lower extremity (LE) necrotizing fasciitis (NF). Patient demographics, clinical history, and outcomes were compared between survivors and nonsurvivors and between those who underwent LE amputation and those who did not. The article adheres to the strengthening the reporting

Turn-up Plasty for Salvage of Transtibial Amputations: An Illustrated Description of the Surgical Technique: A Case Report.

CASE: We present the operative technique for turn-up plasty with a transtibial amputation in three patients with combat-related open tibial fractures who ultimately required amputation. Unaugmented amputation would have resulted in residual limbs of 7 cm; however, with use of the turn-up plasty technique, resultant limbs averaged 14 cm. All three patients achieved union at the osteosynthesis site and regular ambulation with a below-the-knee prosthesis. CONCLUSION: Transtibial turn-up plasty for length and level salvage is a sound option to provide robust, sensate soft-tissue coverage with add

Total hip replacement in an ipsilateral above-the-knee amputation: surgical technique, rehabilitation, and review of the literature.

In this article, we present a case report of a 62-year-old patient who previously underwent an above-the-knee amputation for vascular disease and we performed a total hip replacement on him because of hip osteoarthritis. As the only postoperative complication, the patient developed a methicillin-resistant Staphylococcus aureus surgical site infection, which was successfully treated. The surgical technique and the postoperative rehabilitation program are described in detail in an attempt to detect and face the challenges that patients with major lower limb amputations may present. The literatur

Surgical results of leg amputation according to Ghormley's technique in the treatment of chronic lower limb ischaemia.

The authors present their experience in the treatment of chronic lower limb ischaemia resulting from atherosclerosis by below knee amputation according to Ghormley's technique, with the immediate application of a semi-rigid plaster cast and early rehabilitation. From a group of 664 patients requiring major amputations, 61 patients (52 men) aged 64 +/- 11 fulfilled the criteria for unilateral below-knee amputation for critical limb ischaemia for atherosclerosis consequences. Peri-operative (30-days) mortality was of 3.28%. Stump healing was very good or good (no need for further surgery) in ove

The impact of Syme amputation in surgical treatment of patients with diabetic foot syndrome and Charcot-neuro-osteoarthropathy.

INTRODUCTION: Charcot-neuro-osteoarthropathy with its severe destruction of bones remains a challenge for physicians and surgeons. The aim of the study was to characterise a patient population treated in a specialised foot care centre who underwent surgical treatment for their diabetic foot syndrome. Special attention was paid to patients who suffered from Charcot-neuro-osteoarthropathy and the impact of Syme amputation if amputation of the foot was inevitable. MATERIALS AND METHODS: A total of 121 patients with diabetic foot syndrome and ulcerations underwent an interdisciplinary strategy fo

Surgical morbidity and the risk of amputation due to infected puncture wounds in diabetic versus nondiabetic adults.

We reviewed the hospital course of 77 diabetic and 69 nondiabetic subjects who had incision, drainage, and exploration of infected puncture wounds of the foot. Diabetics were 5 times more likely to have multiple operations and 46 times more likely to have a lower extremity amputation than nondiabetics. The interval from injury to surgery was significantly longer in diabetics than nondiabetics. Total lymphocyte count and hemoglobin, hematocrit, and albumin values were significantly lower in diabetics than in nondiabetics. Diabetic amputees had higher prevalences of nonpalpable pulses, nephropat

A Systematic Review of Interventions Designed to Reduce Surgical Site Infection in Patients Who Undergo Transmetatarsal Amputation.

IntroductionSurgical site infection (SSI) following transmetatarsal amputation (TMA) is thought to be common. In patients who have peripheral arterial disease (PAD) or diabetes related foot complications, little is known regarding the effectiveness of interventions designed to reduce SSI following TMA. Our aim was to perform a systematic review of the literature to inform practice and highlight areas that warrant further research to reduce SSI post TMA.MethodsThis review was performed using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidance and was prospec

Surgical Technique for Osteocutaneous Pedicle Flap Transfer for Salvage of Transtibial Amputation After Severe Lower-Extremity Injury.

INTRODUCTION: Osteocutaneous pedicle flap transfer from the foot can be used to "salvage" a transtibial amputation level when the severity of an injury precludes a transtibial amputation. STEP 1 PREOPERATIVE ASSESSMENT: If the posterior tibial artery is not palpable, obtain an angiogram to determine the vascular supply distal to the traumatic zone and to prepare for conversion to a composite free microvascular transfer. STEP 2 REMOVE PROVISIONAL IMPLANTS: Remove provisional stabilization implants and minimize surgery about the knee. STEP 3 DEVELOP THE FLAP: The dissection about the tarsal t

Influencing factors of outcome after lower-limb amputation: a five-year review in a plastic surgical department.

The crude major lower limb amputation procedure rate is 8.8 per 100,000 of the population per year in Taiwan. From January 2002 to October 2006, patients that received major lower limb amputation in our department were enrolled in this study. Retrospective chart reviews concerning different factors that can affect the eventual postoperative functional status were investigated. Factors that affected the length of hospital stay included duration before amputation (P < 0.001) and renal function (P = 0.045). Phantom limb pain was affected by wound healing time (P = 0.006). Factors that affected th

A reliable surgical approach for the two-staged amputation in unsalvageable limb and life threatening acute progressive diabetic foot infections: tibiotalar disarticulation with vertical crural incisions and secondary transtibial amputation.

BACKGROUND: If all efforts to treat acute progressive diabetic foot infection remain insufficient, the decision of major amputation should be undertaken. For this purpose, guillotine amputation is usually performed first. However, guillotine amputation below the knee level may cause the corresponding infection to spread to preserved anatomical spaces. METHOD: First stage of our procedure consists of tibiotalar disarticulation and vertical incisions performed throughout the lower leg to remove the septic foot and drain the compartments. During the interval period, appropriate antibiotherapy an

Residual muscle activity associated with phantom limb movements after transhumeral amputation without surgical reinnervation: a preliminary ultrasound study.

Transhumeral amputations often result in significant functional limitations. Multiarticulated myoelectric prostheses for individuals with transhumeral amputations are often unreliable because of limitations in signal quality and challenges with accurately detecting motion intent from the available musculature in the residual limb, leading to high prosthesis rejection rates. Surgical approaches like targeted muscle reinnervation have been proposed to mitigate these challenges by biologically amplifying motor signals but are not suitable for all individuals. There is intriguing evidence that dis
