Traumatic lower-extremity amputations often result in complications and surgical revisions. The authors report the in-hospital morbidity and mortality of traumatic lower-extremity amputations at a metropolitan level I trauma center for a large rural region and compare below-knee (BK) vs higher-level amputation complications. They retrospectively reviewed 168 adult patients during a 10-year period (2005 to 2015) who had a traumatic injury to the lower extremity that required an amputation. Main outcome measurements included amputation level, complication rates, intensive care unit (ICU) admissi

BACKGROUND: The success of replantation following traumatic amputation is determined by the quality of the vascular anastomoses. The purpose of this study was to assess the vascularity of injured arteries from traumatically amputated digits using arteriographic and histopathological analysis.
METHODS: 25 amputated digits were included in the study. Crush and avulsion injuries were evaluated according to the Venkatramani classification. The amputated arteries were dissected under a microscope, and the arterial route determined with a transducer. Arteriography using fluoroscopy was evaluated by

The article deals with one of the currently important problems encountered in modern microsurgery, i. e., a possibility of performing restorative operations in complete and incomplete traumatic amputations of hand fragments. Our experience gained in managing a total of one hundred and fifty-one patients has confirmed the necessity of performing primary reconstructive operations on vessels, which in the majority of cases makes it possible to save the damaged segments. Also given herein are the main indications for carrying out restorative operations, with the appropriate conclusions drawn.

Traumatic ear amputation (TEA) is a complete avulsion of a part or of the total auricular tissue. TEA are rare (only 74 cases have been described in the literature) and their handling is complex. The surgeon's objective is to obtain the best cosmetic result without demolishing the auricular area in order to allow future ear reconstruction in case of replantation failure. Many techniques of ear replantation have been described in the literature during the last 30 years: microsurgical replantation, pocket techniques and reattachment techniques. Microsurgical replantation should be achieved every

BACKGROUND: The demographics of traumatic amputations in children are not well known. The purpose of this review was to identify those demographics for use as a possible guide for prevention strategies.
METHODS: The study was a retrospective review of the data on children with traumatic amputation who had received care at one center in the upper midwestern United States from 1980 to 2000. The child's gender and age at the time of the amputation, the date and etiology of the amputation, and the amputation level were tabulated. Statistical analyses of seasonal variations were performed.

It was shown that of the greatest significance for early results of microsurgical digit and hand segment replantations were the completeness, mechanism and the level of the amputation. The criteria used for the classification of traumas are described in detail. Results of treatment of 154 patients operated upon for replantation of 184 segments of the hand (217 radiuses of the hand) are presented.

Explosive blast causes a pattern of injury including primary blast lung, secondary fragment injury and traumatic amputation of limbs. Major traumatic amputation is rare in survivors of bomb blast but common in those who die. The mechanism of such injury has not been previously determined, but must be established if protective measures are to be developed for members of the armed forces. The nature of 41 traumatic amputations in 29 servicemen who survived to reach medical care after blast injury was investigated to determine the anatomical level of amputation and the pattern of soft tissue dama

Traumatic forequarter amputation is a rare and very devastating injury. The arm, including the scapula, clavicle, and pectoral muscle, is torn from the body by a tremendous traction force, usually combined with a counteracting force from the body. The cases of three patients are presented. One patient was caught by the axle of a harvesting machine, resulting in a forequarter amputation of his right extremity and amputation of his left arm at the level of the upper arm. The other two patients had their arms caught in conveyer belts. All patients survived. Closure of the defect was performed acu

Background Although long-term outcomes and chronic cardiovascular changes following amputation have been studied, there is limited literature on the acute hemodynamic alterations that occur in the immediate postoperative period of patients suffering from a traumatic amputation. Currently, no standardized management algorithms exist to guide care during this critical phase. This retrospective observational study aims to identify factors associated with cardiovascular changes and the relationship to morbidity and mortality in patients with traumatic amputations. Objective To evaluate acute card

The aims of this study were to record patient-reported outcomes of treatment of proximal upper extremity amputation injuries and subsequent return to work. A consecutive cohort of 38 patients with a traumatic amputation at or proximal to the carpus had been treated with a replantation or revision (completion) amputation in Tampere University Hospital between 2009 and 2019, and 31 of them participated in this study. The primary outcome was the Disabilities of the Arm, Shoulder and Hand Outcome Measure (DASH). Patients reported significant disability indicated by DASH score after replantation (m

We report the clinical case of a 9-year-old boy admitted for open trauma to the penis which occurred 3 hours before his admission by domestic accident. The child pounding vegetables, was awkwardly struck on the penis with a pestle. After conditioning, exploration in the operating room found partial amputation of the glans with complete rupture of the urethra. Washing with trimming and urethral anastomosis on a urinary catheter were performed. Healing was obtained without urethral stenosis. Injuries to the penis in children are not common and their management can be tricky.

BACKGROUND: Limb loss has a drastic impact on a patient's life. Severe trauma to the extremities is common in current military conflicts. Among other aspects, "life before limb" damage control surgery hinders immediate replantation within the short post-traumatic timeframe, which is limited in part by the ischemic time for successful replantation. Ex vivo limb perfusion is currently being researched in animal models and shows promising results for its application in human limb replantation and allotransplantation.
PRESENTATION OF THE HYPOTHESIS: The current lack of replantation possibilities

BACKGROUND: Traumatic amputation can result in multiple physical, psychological and socio-economic sequalae. While there has been a significant increase in investment and public profile of the rehabilitation of patients who have experienced traumatic limb amputation, little is known about the prevalence of anxiety and depression, especially in the long term.
OBJECTIVE: To determine the association between traumatic limb amputation and anxiety and depression.
DATA SOURCES: A literature search of available databases including Cochrane, Medline, Embase, and PsycINFO was performed for relevant s

A 34-y-old man presented to Naivasha District Hospital (NDH) in Naivasha Town, Kenya, with near-complete below-knee amputation and hemorrhage after a hippopotamus attack. Residents from the University of Washington (UW), Departments of Surgery, Anesthesia, and Medicine, were rotating at NDH with the Clinical Education Partnership Initiative, a joint venture of UW and University of Nairobi. These providers met the patient in the operating theater. The leg was mangled with severely traumatized soft tissues and tibia-fibula fractures. The visiting UW Surgery resident (R3) and an NDH medical offic

Servicemembers with combat-related limb loss often require substantial rehabilitative care. The prevalence of traumatic brain injury (TBI), which may impair cognitive and functional abilities, among servicemembers has increased. The primary objectives of this study were to determine the frequency of TBI among servicemembers with traumatic amputation and examine whether TBI status was associated with discharge to civilian status and medical and rehabilitative service use postamputation. U.S. servicemembers who had a combat-related amputation while deployed in Iraq or Afghanistan between 2001 an

We present a case of successful total auricular reconstruction after total amputation of the auricle. Owing to the severe damage to the auricular cartilage, replantation was not attempted; instead, for the next step of the reconstruction, severed superficial temporal artery and vein were anastomosed. At the second stage, total auricular reconstruction was planned. A huge temporoparietal fascial flap was used (based on STA/STV), which was used to cover the costal cartilage ear framework. This case suggests that, even for the total loss of the auricle, autogenous reconstruction can still be succ

BACKGROUND: Boating is a common recreational activity that may subject its participants to specific patterns of injury.
OBJECTIVES: We describe two unrelated cases of fifth-finger amputation associated with a specific pontoon boat guardrail design.
CASE REPORT: The individuals in these cases sustained an avulsion-type amputation injury to the fifth finger when their fingers became entrapped in a narrowed portion of the boat railing before jumping into the water.
CONCLUSIONS: Given the widespread use of this type of recreational boat, this apparent design flaw may place additional individual

BACKGROUND: Recent studies have demonstrated that black patients receive substandard care compared with white patients across healthcare settings. The purpose of this study was to evaluate the association of race on the management (salvage vs. amputation) of traumatic lower extremity open fractures.
METHODS: Data analysis was conducted using the American College of Surgeon's National Trauma Data Bank. Open tibial and fibular (OTFF) and open femoral (OFF) fractures among adults above the age of 18 were identified by International Classification of Diseases, 9th Revision codes. Injuries were id
