[Complete androgen insensitivity syndrome: a novel mutation in a Tunisian family].

INTRODUCTION: Complete androgen insensitivity is a rare syndrome. It is caused by a mutation in the androgen receptor gene. We describe a novel mutation in exon 1. MATERIALS AND METHODS: We report the case of a 29 year-old girl with complete androgen insensitivity syndrome discovered during the exploration of a primary amenorrhoea. The family investigation revealed two other cases. RESULTS: The diagnosis was oriented by the clinical and the biological features and confirmed by the molecular study. A new mutation of the androgen receptor gene, as a deletion in exon 1 not described previously,

[Study of bone mineral density in complete androgen insensitivity syndrome patients].

OBJECTIVE: To explore the characteristics and treatment of bone mineral density (BMD) change in complete androgen insensitivity syndrome (CAIS) of Chinese patients. METHODS: Fourteen cases of CAIS were studied retrospectively through analyzing and comparing BMD of pre- and post-orchidectomy with normal Chinese men and women. BMD at the lumbar spine and the femur were measured by dual energy X-ray absorptiometry (DXA). RESULTS: Ten cases of CAIS had pre-orchidectomy DXA, in which 6 cases had very significantly reduced L(2-4) BMD (0.92 +/- 0.08) g/cm(2) compared with normal men and women (P <

Psychological outcomes and gender-related development in complete androgen insensitivity syndrome.

We evaluated psychological outcomes and gender development in 22 women with complete androgen insensitivity syndrome (CAIS). Participants were recruited through a medical database (n = 10) or through a patient support group (n = 12). Controls included 14 males and 33 females, of whom 22 were matched to women with CAIS for age, race, and sex-of-rearing. Outcome measures included quality of life (self-esteem and psychological general well-being), gender-related psychological characteristics (gender identity, sexual orientation, and gender role behavior in childhood and adulthood), marital status

Corpus albicans-like structures in the gonads in androgen insensitivity syndrome.

The complete androgen insensitivity syndrome is the most common form of male pseudohermaphroditism and is due to end-organ unresponsiveness to androgens. The patients are phenotypically female, but the genotype is 46, XY, and the gonads are testicles. However, the common presence of Sertoli cell adenomalike nodules of immature seminiferous tubules and the frequent presence of stroma resembling ovarian stroma may cause confusion about the nature of the gonad. Herein, we report the occurrence in such a gonad of corpus albicans-like structures, which resulted from progressive hyalinization and ag

Bone mass in androgen-insensitivity syndrome: response to hormonal replacement therapy.

The response of bone mass to long-term treatment with estrogen and progesterone in patients with complete androgen-insensitivity syndrome (AIS) is unknown. We report a 17-year-old female patient (karyotype 46 X, Y) with AIS studied during a 4-year period. Bone mineral density (BMD) measured by dual X-ray absorptiometry in lumbar spine and proximal femur was sharply reduced at the initial visit; and remained unchanged during long-term follow-up on hormone replacement therapy with estrogens and progestin. Bone metabolism markers were all in the normal range. The lack of significant increase in B

[From gene to disease; androgen receptor gene, androgen insensitivity syndrome, and spinal and bulbar muscle atrophy].

Androgen insensitivity is an X-linked disorder of male sexual differentiation resulting from a defective androgen receptor. Spinal and bulbar muscular atrophy (Kennedy's disease) is an X-linked disease, resulting from expansion of the polyglutamine stretch in the N-terminal part of the androgen receptor. Mutation analysis confirms the clinical diagnosis of androgen insensitivity and enables carrier detection and prenatal diagnosis. Kennedy's disease, with its diagnostic problem of clinical variability, is diagnosed or excluded when an expanded CAG-repeat is present or absent in exon 1 of the a

Differences of adrenal-derived androgens in 5α-reductase deficiency versus androgen insensitivity syndrome.

Steroid 5α-reductase type 2 deficiency (5α-RD2) and androgen insensitivity syndrome (AIS) are difficult to distinguish clinically and biochemically, and adrenal-derived androgens have not been investigated in these conditions using modern methods. The objective of the study was to compare Chinese patients with 5α-RD2, AIS, and healthy men. Sixteen patients with 5α-RD2, 10 patients with AIS, and 39 healthy men were included. Serum androgen profiles were compared in these subjects using liquid chromatography/tandem mass spectrometry (LC-MS/MS). Based on clinical features and laboratory tests, 5α

[Trinucleotide repeats polymorphism of the androgenic receptor gene in the detection of carriers of androgen insensitivity syndrome].

Androgen insensitivity syndrome (AIS) is caused by mutation in androgen receptor (AR) gene. This phenomenon leads to the defect of the virilization of the external genitalia and is one of the causes of human male pseudohermaphroditism. Molecular diagnosis of AR gene mutations is very complex because of heterogeneity of AR mutations. AIS is X-linked recessive trait and therefore the simple diagnosis of AIS carriers is a very important problem. Detection of AIS carriers by using the analysis of CAG repeating sequence polymorphism of exon 1 or AR gene is reported.

Minor hypospadias: the "tip of the iceberg" of the partial androgen insensitivity syndrome.

BACKGROUND: Androgens are critical in male external genital development. Alterations in the androgen sensitivity pathway have been identified in severely undermasculinized boys, and mutations of the androgen receptor gene (AR) are usually found in partial or complete androgen insensitivity syndrome (AIS). OBJECTIVE: The aim of this study was to determine whether even the most minor forms of isolated hypospadias are associated with AR mutations and thus whether all types of hypospadias warrant molecular analysis of the AR. MATERIALS AND METHODS: Two hundred and ninety-two Caucasian children p

Androgen insensitivity syndrome: a survey of diagnostic procedures and management in the UK.

OBJECTIVE: A two year survey of androgen insensitivity syndrome (AIS) to assess current diagnostic and management strategies. METHODS: Cases were ascertained by inclusion on the British Paediatric Surveillance Unit monthly report card for 24 months. RESULTS: Fifty one of 139 notifications were confirmed as AIS; 29 cases were complete AIS and 22 cases partial AIS. Seventy six per cent of complete AIS presented with an inguinal hernia, and half the complete AIS patients had an established family history of the disorder. Presentation in the partial AIS group was through ambiguous or undermascul

A case of testicular leiomyoma in androgen insensitivity syndrome: exploring malignancy controversies.

Testicular leiomyoma is an exceptionally rare finding in patients with androgen insensitivity syndrome (AIS). Here, we present a report of a 30-year-old individual diagnosed with complete AIS who presented with an inguinal mass subsequently identified as a right sided testicular leiomyoma. While leiomyoma are generally considered benign, controversies persist regarding the potential for malignancy in inguinal masses among AIS patients. This case underscores the complexity of diagnosing and managing such rare occurrences, particularly when considering the diverse opinions found in the scientifi

Testicular feminization: complete androgen insensitivity syndrome. Discussions based on a case report.

INTRODUCTION AND OBJECTIVES: Testicular feminization is the syndrome when a male, genetically XY, because of various abnormalities of the X chromosome, is resistant to the actions of the androgen hormones, which in turn stops the forming of the male genitalia and gives a female phenotype. The androgen insensitivity syndrome occurs in one out of 20,000 births and can be incomplete (various sexual ambiguities) or complete (the person appears to be a woman). The aim of this paper is to present the diagnosis and treatment of a case of testicular feminization. PATIENT AND METHODS: A 22-year-old pa

Complete androgen insensitivity syndrome: report of a case with solitary pelvic kidney.

Complete androgen insensitivity syndrome, commonly known as the testicular feminization syndrome, is characterized by a 46, XY karyotype, bilateral testes, absent or hypoplastic Wolffian ducts, and female-appearing external genitalia with diminished axillary and pubic hair development. Although initial diagnosis in the child is difficult, the syndrome must be suspected after puberty if primary amenorrhea is present. Coexistence of genital defects with urologic abnormalities is expected in these cases because of close embryologic origin. However, unilateral renal agenesis with pelvic ectopia of

[Complete androgen insensitivity syndrome in 16-year-old girl with female phenotype].

We present a case of a 16-year-old girl who attended Endocrinology Clinic in Kraków. Her main complains were amenorrhea and lack of pubic and axillary hair development. Breast development was normal. Based on those features, male karyotype (46, XY) as well as high levels of blood testosterone and lack of uterus on ultrasound examination allowed for establishing the diagnosis of complete androgen insensitivity syndrome. The authors emphasize the possibility of diagnosing severe disorders of sex differentiation, such as sex reversal, not earlier than in teenage patients with delayed puberty. In

Androgen insensitivity syndrome (testicular feminization): a model for understanding steroid hormone receptors.

Testicular feminization, a syndrome of end-organ androgen resistance, has been characterized at the molecular level. Well-defined mutations in the androgen receptor have been identified in this disorder. This receptor belongs to a family of intracellular hormone receptors, which includes receptors for corticosteroids, mineralocorticoids, sex hormones, vitamin D3, thyroid hormone, and retinoids. An understanding of the structure and molecular defects identified for the androgen receptor serves as a model to understand this entire class of receptors, a group directly involved in the biochemical

Case Report: a Novel Nonsense Mutation in the Androgen Receptor Gene Causing the Complete Androgen Insensitivity Syndrome.

Androgen insensitivity syndrome (AIS) is a rare X-linked genetic disorder caused by mutations in the androgen receptor (AR) gene. AIS can be divided into partial type (PAIS), mild type (MAIS), and complete type (CAIS) based on the degree of androgen insensitivity. CAIS is characterized by a male genotype and a complete female phenotype. A 10-year-old child presented with a bilateral inguinal mass for 9 years. Physical examination revealed a complete feminine genital appearance and a painless mass in bilateral inguinal area. Pelvic magnetic resonance imaging (MRI) revealed long T1 and T2 ellipt

Novel androgen receptor gene variant containing a frameshift mutation in a patient with complete androgen insensitivity syndrome.

A variety of mutations in the androgen receptor (AR) gene are linked to androgen insensitivity syndrome (AIS). AIS is the most common specific cause of 46, XY disorder in sex development. Here, we reported a patient which presented as a female with 46, XY karyotype and normal female external genitalia. The patient was diagnosed with complete AIS caused by a novel mutation (NM_000044, c.2678-2726del, p. Pro893Leufs*35) in the AR gene. Targeted exome sequencing was used to detect the patient's androgen receptor gene mutations. Sanger sequencing was used to validate the mutation. This study showe
