[Treatment of advanced metastatic breast cancer with exemestane, a multicenter randomized controlled study of 195 cases].

OBJECTIVE: To observe the efficacy of exemestane for postmenopausal advanced metastatic breast cancer, and to assess its side effects. METHODS: A randomized, double-blind, and parallel controlled study was conducted among 195 patients with postmenopausal advanced metastatic breast cancer from December 2001 to June 2002. Except for the 4 cases who were lost to follow-up, the remaining 191 patients were divided into two groups: study group (n = 96, treated with exemestane capsule 25 mg and one model tablet of letrozole orally q.d. for 8 weeks), and control group (n = 95, treated with letrozole

Overview of the pharmacology of the aromatase inactivator exemestane.

One third of all breast cancers and two thirds of postmenopausal breast cancers are estrogen dependent. Antiestrogen strategies, such as inhibition of estrogen-receptor binding and estrogen deprivation, are effective for the management of hormone-dependent breast cancer. Although currently available agents are effective, the development of more potent and selective agents continues. Both steroidal and nonsteroidal inhibitors of aromatase have been developed for clinical uses. A novel class of steroidal irreversible antiaromatase agents demonstrates a high degree of specificity for the aromatas

Other anti-inflammatory uses of intranasal corticosteroids in upper respiratory inflammatory diseases.

Topical corticosteroids are an effective treatment for nasal polyposis and other conditions associated with rhinitis. There is evidence that asthma, otitis media with effusion, and acute sinusitis may all benefit from such therapy. Fluticasone propionate (FP) drops have been shown to reduce polyp size, and to improve nasal inspiratory flow, while avoiding the side-effects of, for example, topical betamethasone therapy. In small children with allergic rhinitis, fluticasone propionate aqueous nasal spray (FPANS) has been shown to be significantly more effective than ketotifen in relieving night

Exemestane induces neurodevelopmental defects and behavioral diorder in zebrafish embryos by p53 signaling pathway.

Exemestane, a third-generation oral aromatase inhibitor used clinically for breast cancer treatment, may exhibit potential developmental neurotoxicity, though the underlying mechanisms remain unclear. This study employed zebrafish embryos to investigate exemestane's impact on embryonic neurodevelopment. Embryos were exposed to exemestane (3, 6, and 9 μM) from 6 to 72 h post-fertilization (hpf). Exemestane exposure significantly increased larval mortality and malformation rates and reduced body length. It also impaired physiological functions, decreasing heart rate and spontaneous movement (tai

Comparison of in vitro lipolysis, sequential lipolysis - ex vivo permeability, and simultaneous lipolysis - in situ perfusion for exemestane-loaded lipid-based formulations.

This study aimed to investigate the in vitro lipolysis of five different self-nanoemulsifying drug delivery system (SNEDDS) formulations and to test the formulations by combining the in vitro lipolysis with ex vivo permeability and in situ perfusion methods as permeation steps, and then to compare the methods. Exemestane (EXE) was selected as the model drug due to its low solubility and nonionic nature. Four SNEDDS formulations, differing from the main formulation (MCFK40) in terms of cosolvent content (MCFCoS), oil type (LCF), digestible surfactant content (MCFT80), and oil amount (MCFI988),

Pharmaceutical transition to non-CFC pressurized metered dose inhalers.

The production of ozone-depleting chlorofluorocarbons (CFCs) was discontinued on 1 January 1996 for all uses deemed non-essential under the Montreal Protocol. However, the use of CFCs as propellants in pressurized metered dose inhalers (pMDIs) was classed as essential, providing an exemption from the agreement. Following extensive research, the hydrofluoroalkanes (HFA) 134a and 227 were identified as the only suitable replacements for CFC propellants in pMDIs. The drug delivery of pMDIs formulated with HFA 134a as a propellant and containing either salbutamol (100 microg per actuation) or flut

[Introductory clinical evaluation of fluticasone propionate efficacy for treatment of allergic rhinitis in children].

The investigations were carried out on 13 children with seasonal rhinitis, and in 10 children with perennial rhinitis. The fluticasone propionate (Flixonase-Glaxo) was introduced in acute state of disease: one dose to each nostril (100 micrograms) for the patient 7-11 years old, and in the patients under 11 years old--two doses to each nostril (200 micrograms) in the morning, during 3 weeks. The clinical effects were established using score-system in 0-3 points scale which included essential symptoms like: itching, sneezing, nasal blockade, rhinorrhoea, mucosal oedema and eyes irritation. Morphometric studies in duodenal biopsies from patients with coeliac disease: the effect of the steroid fluticasone propionate.

Morphometric measurements have been performed on small intestinal biopsy specimens from patients with untreated coeliac disease before and after six weeks oral treatment with a steroid of low systemic bioavailability (fluticasone propionate). Measurements were obtained by point counting and also by a computer-aided measuring system with reference to a constant area of the muscularis mucosa. Fluticasone propionate led to a parallel reduction in the intraepithelial lymphocyte count within the surface (P less than 0.001) and crypt epithelium (P less than 0.01). The intra-epithelial lymphocyte cou

Comparison of Risk of Pneumonia Caused by Fluticasone Propionate versus Budesonide in Chronic Obstructive Pulmonary Disease: A Nationwide Retrospective Cohort Study.

INTRODUCTION: Inhaled corticosteroids (ICSs) play an important role in lowering the risk of acute exacerbation of chronic obstructive pulmonary disease (COPD). However, ICSs are known to increase the risk of pneumonia. Moreover, previous studies have shown that the incidence rate of pneumonia varies depending on the type of ICS. In this study, the risk of pneumonia according to the type of ICS was investigated in a population-based cohort. METHODS: A retrospective cohort study was conducted using claims data of the entire population from the Korean National Health Insurance Service. Patients

Complete response and long-term survival of leptomeningeal carcinomatosis from breast cancer with maintenance endocrine therapy.

Leptomeningeal carcinomatosis carries a poor prognosis in breast cancer. Treatment modalities are geared towards tumour molecular characteristics, as well as symptoms and patient performance status. It has previously been postulated that endocrine treatments used for the treatment of metastatic breast cancer do cross the blood-brain barrier and can achieve antineoplastic effects in the central nervous system. We report a case of metastatic breast cancer in a 65-year-old woman who developed leptomeningeal carcinomatosis. She was initially treated with intrathecal methotrexate, which was stopped

Fluticasone furoate/vilanterol: a review of its use in patients with asthma.

Fluticasone furoate/vilanterol (Relvar(®)) is a once-daily, fixed combination of an inhaled corticosteroid (ICS) and a long-acting β2-adrenoreceptor agonist (LABA), delivered via a dry powder inhaler (Ellipta(®)). It is approved for the treatment of asthma in the EU and Japan, and is the first once-daily ICS/LABA to be available for this indication. Fluticasone furoate is an enhanced-affinity glucocorticoid receptor agonist, with potent anti-inflammatory activity. Vilanterol produces rapid and prolonged bronchodilation. In phase III trials in adolescents and adults with various levels of asthm

Young man presenting with recurrent food bolus impaction.

A young man with recurrent food bolus obstruction presented to the gastroenterology clinic and, after investigation, was found to have eosinophilic oesophagitis. This unusual condition is closely linked to atopy and, in parallel with other atopic conditions, is increasing in incidence. Diagnosis is confirmed on oesophageal biopsy and treatment includes topical steroids and exclusion diet. Eosinophilic oesophagitis is gaining recognition in adult medicine as a cause of dysphagia and is one of the leading causes of recurrent food bolus obstruction. As such, it should be considered as a diagnosis

Incidence and management of arthralgias in breast cancer patients treated with aromatase inhibitors in an outpatient oncology clinic.

PURPOSE: Aromatase inhibitors (AIs) are routinely used as first-line adjuvant treatment of breast cancer in postmenopausal women with hormone receptor positive tumors. The current recommended length of treatment with an AI is 5 years. Arthralgias have been frequently cited as the primary reason for discontinuation of AI therapy. Various treatment strategies are proposed in literature, but a standardized treatment algorithm has not been established. The initial purpose of this study was to describe the incidence and management of AI-induced arthralgias in patients treated at Kellogg Cancer Cent

Letter: Association of targeted intense pulse light system UVA1-UVB and fluticasone in the treatment of vitiligo: Prospective study of 10 patients.

Vitiligo is an acquired pigmentary disorder treated frequently with PUVA and narrowband UVB phototherapy. These total body treatments are associated with side-effects in the lesional or nonlesional skin. A prospective study using a novel multiwavelength targeted Intense Pulse Light System UVA1-UVB in association with fluticasone cream was undertaken. Of the ten patients who participated in the study, only eight completed it. Four patients had grade 1 improvement (1-25%); one had grade 3 (51-75%); two had worsening of the lesions after sun exposure; one had no skin repigmentation.

Fluticasone propionate for treatment of esophageal lichen planus. A case series.

Esophageal lichen planus is a rare condition, and although the majority of cases occur in conjunction with lichen planus at other sites, the endoscopic features are often misinterpreted resulting in a delay in diagnosis. We report a series of five patients presenting to our unit between 2005 and 2009. All five patients were female and presented with dysphagia. Endoscopy demonstrated proximal esophageal stricturing in four patients. Characteristic histological findings were found in four patients. Lichen planus was diagnosed at other sites, and preceded gastrointestinal symptoms, in all patient

Fluticasone furoate nasal spray is effective and well tolerated for perennial allergic rhinitis in adolescents and adults.

BACKGROUND: Fluticasone furoate nasal spray (FFNS), an intranasal corticosteroid, has been shown to be effective in perennial allergic rhinitis in randomized, double-blind, placebo-controlled studies but has been less extensively studied in perennial allergic rhinitis than seasonal allergic rhinitis. This study was designed to evaluate the efficacy and safety of FFNS in perennial allergic rhinitis in adolescents and adults ≥12 years of age. METHODS: In this randomized, double-blind, placebo-controlled, parallel-group study (FFU111439), patients ≥12 years old with perennial allergic rhinitis r
