Stent-graft implantation for clinically diagnosed syphilitic aortic aneurysm in an HIV-infected patient.

We describe our experience with stent-graft placement in a patient with a clinically diagnosed syphilitic aortic aneurysm. The patient was a 43-year-old man with syphilitic and human immunodeficiency virus (HIV) co-infection. Computed tomography (CT) revealed an aortic aneurysm with 89 mm in maximum size which was located at distal aortic arch and was considered syphilis derived saccular aneurysm. The aneurysm was judged at high risk of rupture from its shape. We decided to perform stent-graft implantation. Before surgery, the patient was given antibacterial and anti-HIV agents. Hand-made fenes

In situ polytetrafluoroethylene graft bypass for primary infected aneurysm of the infrarenal abdominal aorta.

BACKGROUND: Reinfection is a major issue of surgical treatment for patients with infected abdominal aortic aneurysm (AAA). The present report describes outcomes after use of our procedure for treating patients with infected aneurysm of the infrarenal abdominal aorta. The procedure involved an in situ polytetrafluoroethylene (PTFE) graft bypass and omental wrapping of the graft. The procedure was used regardless of the presence of Gram-stain-positive pus or tissue or the type of pathogen identified. METHODS: We retrospectively reviewed nine consecutive patients with primary infected aneurysms

Survival following non-operative management of infected coronary artery aneurysm with infected pericardial effusion complicated by multi-organ failure: a case report.

BACKGROUND: Infected coronary artery aneurysm with infected pericardial effusion is a very rare complication following percutaneous coronary intervention (PCI) and is associated with high mortality. Management options include open cardiothoracic surgery or non-operative management with pericardiocentesis. The best management option is currently unknown. CASE SUMMARY: A 76-year-old man with a background of hypertension, type two diabetes mellitus, chronic kidney disease, and a pacemaker presented with worsening shortness of breath 5 weeks following PCI to the right coronary artery (RCA) for a

Infected aneurysms of the suprarenal abdominal aorta.

BACKGROUND: Infected aneurysm of the suprarenal abdominal aorta is rare and can be fatal without surgery. There have been only sporadic case reports or small case series. We review our experience with 14 patients over 13 years. METHODS: Retrospective chart review. RESULTS: Between 1997 and 2010, 14 cases of infected aneurysms of the suprarenal abdominal aorta were treated at our hospital. There were 11 men with median age of 75.5 years (range, 35-88). Of the 13 pathogens isolated, the most common responsible microorganism was nontyphoid Salmonella in eight (62%) followed by Staphylococcus au

Mitral valve aneurysm with infective endocarditis.

A case of mitral valve aneurysm associated with infective endocarditis is reported. Two-dimensional echocardiography revealed a saccular structure in the anterior leaflet that bulged into the left atrium throughout the cardiac cycle. During operation, the vegetation on the commissure of the right and left aortic leaflet and a 3-mm perforation on the noncoronary leaflet were found. The mitral valve and aortic valve were replaced with mechanical prosthesis. Pathology of the excised valves showed inflammation. For this patient, we considered that the infected aortic regurgitant jet striking the v

Infected aortic aneurysms. A changing entity.

Infected (mycotic) aortic aneurysms are infrequent and, without surgical intervention, usually lead to uncontrolled sepsis or catastrophic hemorrhage. Symptoms are frequently absent or non-specific during the early stages, and a high index of suspicion is essential to make the diagnosis. Surgery performed after rupture carries high morbidity and mortality rates. Bacterial endocarditis with streptococcus pyogenes was the most common cause of infected aortic aneurysm in the pre-antibiotic era. Today, arterial trauma due to iatrogenic manipulation and depressed immunocompetence have become more c

Multiple Intracranial Aneurysms in HIV Infection.

Neurological findings in HIV are common and include cognitive impairment, microcephaly, nonspecific white matter lesions and seizures. Cerebral vasculopathy and stroke are uncommon and may be due to primary HIV vasculopathy or opportunistic infections such as tuberculosis and cryptococcal meningitis. The authors describe a 7-y-old boy who presented with severe headache and was detected to have aneurysmal bleed due to intracranial aneurysm.

Surgical management of infected thoracic aneurysms.

Infected thoracic aortic aneurysm (ITAA) is a relatively rare disease. The diagnosis of ITAA is generally made comprehensively based on symptoms, laboratory data and CT findings. Several series of blood cultures are mandatory to first detect the infecting organism. ITAA is usually suspected as a result of the CT findings. A short-interval CT re-examination is essential to confirm the correct diagnosis. A CT scan commonly demonstrates a rapid enlargement of the aneurismal lumen and soft tissue mass surrounding the aorta. One of the characteristics of ITAA is the presence of several nodular or s

Infective aneurysm of the inferior pancreaticoduodenal artery.

INTRODUCTION: Pancreaticoduodenal (PD) artery aneurysms account for less than 2% of all splanchnic aneurysms. A mycotic aetiology is extremely uncommon. INTRODUCTION: Two weeks following an episode of sepsis related to a prostatic biopsy, a 59-year-old man presented with abdominal pain and anaemia. Ultrasonography and computed tomography revealed an inferior PD artery pseudoaneurysm with an associated mesenteric root haematoma. This was treated successfully by transcatheter embolisation. INTRODUCTION: Infective pseudoaneurysms of the PD artery are rare but can be associated with rupture into

Peripheral mycotic aneurysms in infective endocarditis.

BACKGROUND AND AIM OF THE STUDY: Today, infective endocarditis (IE) remains a severe illness, with high mortality and morbidity. Mycotic aneurysms (MAs) are rare complications of IE. For most patients, surgical intervention represents the only hope for radical cure of extracranial MAs, and survival. METHODS: A total of 238 patients with IE was treated at the authors' center between January 1990 and December 2003. Among these patients, 10 underwent surgical intervention due to peripheral MAs. RESULTS: Concomitant surgery for intracardiac and extracardiac pathology was applied in three patient

Histoplasma infection of abdominal aortic aneurysms.

Fungal endarteritis resulting from progressive disseminated histoplasmosis may cause arterial aneurysms, or lead to infection of pre-existing aneurysms. Three patients with Histoplasma capsulatum infections of abdominal aortic aneurysms are reported. All had previous disseminated histoplasmosis and atherosclerotic peripheral vascular disease. All were considered cured of systemic infection when their aneurysms were discovered. Atherosclerotic vascular lesions may become infected during the course of systemic fungal disease and may serve as a haven for viable organisms in patients whose dissemi

Infected Thoracic Aneurysm with Progressive Exudative Unilateral Pleural Effusion: A Case Report and Literature Review.

An infected aneurysm can be fatal; however, its early diagnosis is challenging because of the non-specificity of its symptoms. We herein report the case of a 90-year-old female patient with an infected aneurysm who presented with progressive, unilateral pleural effusion. A pleural fluid analysis revealed a mononuclear cell-predominant exudate with an elevated adenosine deaminase (ADA) level of 61.4 U/L. While these findings indicate that an infected aneurysm should be considered a possible cause of unilateral pleural effusion, the presence of elevated ADA, suggesting an unrelated condition, ma

А Rare Case of an Infected, Ruptured Popliteal Artery Aneurysm Occurring Following Surgical Treatment for Panaritium.

We present a successful case of treating an infected popliteal aneurysm in a 71-year-old man who arrived at the emergency department in a septic state, reporting a three-week history of fever, lethargy, general malaise, and pain and swelling in the right popliteal fossa. Previously diagnosed with a sizable right popliteal aneurysm, the patient had undergone endovascular treatment using a Viabahn (WL Gore & Associates, Flagstaff, USA) endoprosthesis two months earlier. His fever and malaise emerged a week following minor surgery for a toe infection (panaritium) on the right foot, leading to sub

Candia albicans lumbar spondylodiscitis contiguous to infected abdominal aortic aneurysm in an intravenous drug user.

While the incidence of spondylodiscitis is rising because of longer life expectancy and the increasing use of immunosuppressant drug, indwelling devices and spinal surgeries, the fungal aetiology remains rare, sometimes affecting intravenous drug users. Candida spondylodiscitis is an extremely rare complication post aortic aneurysm repair. It is potentially fatal due to the risk of aneurysm rupture and septic complications. The growing problem of systemic diseases caused by Candida species reflects the enormous increase of patients at risk. The treatment of this complicated entity is challengi

Emergent repair of infected aortic aneurysm with contained rupture using a femoral vein neoaortoiliac system.

Infected aortic aneurysms are rare but are associated with high morbidity and mortality. Management involves surgical resection and debridement of the infected aorta and surrounding tissues, arterial reconstruction or bypass, and flap coverage, followed by long-term antibiotic therapy. Autogenous reconstruction using a neoaortoiliac system (NAIS) is a durable form of repair with a decreased risk of reinfection. However, NAIS reconstruction is generally thought to be contraindicated for emergent, but not impending, rupture settings. We present the successful application of NAIS for a contained
