The pupil in stroke.

Pupil sizes were assessed in 100 patients admitted to an intensive care stroke unit over a 1-year period to determine if differences existed and if anisocoria might help in diagnosis. From life-size photographs the pupil diameters were measured accurately, and clinical information compiled by the neurology staff was studied. Of the 19 patients with anisocoria 13 had a final diagnosis of stroke. This incidence of anisocoria, 16%, is similar to that in a normal population (20%). The side of the mydriasis was unrelated to the site of the lesions except when the stroke was due to a brainstem infar

Trigeminal input, pupil size and cognitive performance: From oral to brain matter.

It has been observed that, in patients affected by temporomandibular disorders (TMDs) and edentulism, a left-right asymmetry in electromyographic (EMG) activity of masseter muscles during clenching and in pupil size at rest (anisocoria) is present. Both are greatly reduced by an orthotic-prosthetic correction. In parallel, the correction significantly improves cognitive performance. These effects are possibly due to the recovery of a cortical balance, via Locus Coeruleus (LC) modulation, whose activity is powerfully affected by the sensorimotor trigeminal input. The role of this functional axi

Apraclonidine-An eye opener.

Pharmacological testing with apraclonidine eye drops induces a typical reversal of anisocoria in patients with Horner's syndrome. Moreover, apraclonidine was observed to have an elevating effect on the upper eyelid in Horner's syndrome as well as in healthy subjects, which is thought to be mediated by alpha-1 adrenergic receptors present in the Muller's muscle. We aim to quantitatively investigate the effect of apraclonidine on eyelid position in patients with Horner's syndrome compared to physiological anisocoria based on infrared video recordings from pupillometry. We included 36 patients fo

Pupillotonia in a dog.

A 5-year-old German Shepherd Dog was examined because of anisocoria. Pupillatonia because of parasympathetic denervation of iris sphincter muscle was diagnosed. This finding, most commonly referred to as Adie's tonic pupil in human medical literature, illustrated such a case in a species other than man. Diagnosis consisted of measuring the change in size of the pupillary aperture in response to topical administration of 0.1% pilocarpine. It is hoped that this report will help practitioners to be cognizant of pupillatonia when sorting out anisocorias.

False negative apraclonidine test in two patients with Horner syndrome.

BACKGROUND: Because of denervation supersensitivity, a miotic pupil in a sympathetically-denervated eye dilates in response to a dilute or weak alpha-1-agonist drug. A reversal of anisocoria after topical apraclonidine is considered as a positive test result that diagnoses a unilateral Horner syndrome. HISTORY AND SIGNS: Two women aged 34 and 46 years with a cocaine-confirmed oculosympathetic defect (Horner syndrome) were tested with 1 % topical apraclonidine on separate days. THERAPY AND OUTCOME: In one patient, her miotic Horner pupil dilated marginally but not enough to reverse the baseli

A Rare Case of Mydriasis as an Immediate Complication After Orthognathic Surgery in a Patient with Cleft Lip and Palate.

This study aims to report a rare complication, anisocoria (mydriasis), in a patient with cleft lip and palate in the immediate postoperative period after orthognathic surgery and discuss the literature surrounding this event. Mydriasis after orthognathic surgery may be related to the separation of the pterygoid plate from the sphenoid bone, which can injure the cranial nerves. Therefore, knowledge of possible anatomical variations of the posterior region of the maxilla becomes extremely important to minimize complications in cleft patients. The reported case highlights the management of anisoc

[Characteristics of pupil function in patients with dissociated vertical divergence].

Dissociated vertical divergence (DVD) is frequently associated with the infantile strabismus syndrome. There are different theories on the pathomechanism of this disorder, but none of them is generally accepted. Some authors believe that the slow upward movement of the covered eye is due to a different illumination of both retinae and consequently an unequal activity of the oculomotor nerve's nuclei. In one hypothesis a predominance of crossing pupillary tract fibres from the pretectal area to motor and parasympathetic nuclei of the oculomotor nerve was supposed. The consequence of this abnorm

Positive apraclonidine test 36 hours after acute onset of horner syndrome in dorsolateral pontomedullary stroke.

A 40-year-old man developed a Horner syndrome as part of a dorsolateral medullary brainstem infarction. Thirty-six hours after the onset of the stroke, topical instillation of 0.5% apraclonidine produced reversal of anisocoria. This is the first case in which apraclonidine testing has been applied to a patient with a Horner syndrome caused by a lesion in the first segment of the oculosympathetic pathway and the shortest reported interval between clinical manifestations of the lesion and apraclonidine-induced reversal of anisocoria. A review of all reported cases of apraclonidine testing in Hor

Atypical Horner's syndrome: frequency, features and aetiology in a paediatric population.

BACKGROUND/OBJECTIVES: Paediatric Horner's syndrome (HS) may present atypically with incomplete or intermittent clinical features, yet could represent sinister pathology including neuroblastoma. We aim to report the frequency and features with which atypical HS presents in our population (Northern Ireland) and to propose an investigation algorithm to aid diagnosis in these challenging cases. SUBJECTS/METHODS: Retrospective chart review of all paediatric anisocoria and HS cases presenting to Belfast, Northern Ireland, between 2012 and 2018, identified through searching our paediatric ophthalmo

Apraclonidine for the pharmacologic confirmation of acute Horner syndrome.

Apraclonidine is the most widely used pharmacologic agent to confirm Horner syndrome. It is a strong α-2 and a weak α-1 adrenergic agonist and reversal of anisocoria is considered a positive test. The utility of apraclonidine in acute Horner syndrome remains controversial as the exact timing for denervation sensitivity to develop remains unknown. The goal of this study was to describe the use of apraclonidine in the diagnosis of acute Horner syndrome in patients with an unequivocal onset within 7 days. We identified 3 patients who were referred to ophthalmology/neuro-ophthalmology service and

Positive Apraclonidine Test in Horner Syndrome Caused by Thalamic Hemorrhage.

Reversal of anisocoria following instillation of apraclonidine 0.5% has been reported in Horner syndrome caused by lesions of the central and peripheral nervous system. The shortest documented latency between symptom onset and a positive apraclonidine test is 36 hours, occurring in a patient with a pontomedullary infarct. We present the case of a 69-year-old man with Horner syndrome due to thalamic hemorrhage in whom apraclonidine testing demonstrated reversal of anisocoria 4 days after symptom onset. This is the first reported case of a positive apraclonidine test in a Horner syndrome caused

Pupillary abnormalities in childhood - 2 case presentations.

The midwifery profession varies greatly from one country to another. There are, however, a number of common features such as exposure to biological risks through contact with pregnant women and women in labor, exposure to postural stresses during examinations and medical acts, but also, increasingly, exposure to organizational constraints (work schedules, shift work, etc.). This article aims to give an overview of what is known about the occupational health risks of midwives (MWs). A review of the literature on Medline, from January 1, 2006 to December 31, 2017, was carried out. Articles focus

A novel case of Horner syndrome as the presenting sign of craniosynostosis.

Craniosynostosis, the premature fusion of cranial sutures, can lead to distortion of skull shape and neurological dysfunction. We present a novel case of Horner syndrome as the presenting sign of craniosynostosis associated with elevated intracranial pressure. A 10-year-old boy presenting for strabismus follow-up was noted to have new-onset anisocoria, greater in the dark, and mild right upper eyelid ptosis. Apraclonidine testing was concerning for Horner syndrome. Neuroimaging demonstrated previously undiagnosed sagittal craniosynostosis with tortuous optic nerves and large cerebrospinal flui

False and non-localizing signs in neuro-ophthalmology.

The absolute reliance on abnormal neuro-ophthalmologic findings as signposts for particular lesions of the neuraxis must be tempered by a working knowledge of false and nonlocalizing signs and symptoms. Transient monocular visual loss or complex visual hallucinations may lead to neuroanatomic ambiguity, and elevated intracranial pressure is not the most common cause of swollen optic disks in late life. Sixth nerve palsy associated with increased intracranial pressure is possibly the best-known false localizing sign, but other ocular motility disturbances (divergence paresis, convergence insuff

Stereopsis Simulating Small-Aperture Corneal Inlay and Monovision Conditions.

PURPOSE: To analyze stereoscopic vision under a broad range of experimental conditions simulating small-aperture corneal inlay and monovision conditions. METHODS: A total of 10 patients participated in the study. The anisocoria generated by corneal inlays was simulated by using a small-aperture contact lens fitted on the non-dominant eye. Visual acuity, contrast sensitivity function, visual discrimination capacity (halo perception), and stereoacuity tests were performed under natural conditions or when the non-dominant eye wore the small-aperture contact lens under two conditions: induced ani

[Photographic pupil diagnosis in Horner syndrome].

In cases of miosis without other clinical signs and not contributory medical history it is often hard to differentiate Horner's syndrome. In these cases the pharmacological testing with cocaine eye drops is a well established and validated method. The physiological testing, i.e. the detection and measurement of a dilatation lag in Horner's syndrome, is not so common. For this reason we measured the dilatation of pupils in 22 Horner's syndromes and 16 physiological anisocorias to find limit ("cut off") values. We used flash photography and a commercial available camcorder and took photos in bri
