Venomous snakebites.

More than 5 million people are bitten by venomous snakes annually and more than 100,000 of them die. In Europe, one person dies due to envenomation every 3 years. There is only one venomous snake species in Lithuania--the common adder (Vipera berus)--which belongs to the Viperidae family; however, there are some exotic poisonous snakes in the zoos and private collections, such as those belonging to the Elapidae family (cobras, mambas, coral snakes, etc.) and the Crotalidae subfamily of the Viperidae family (pit vipers, such as rattlesnakes). Snake venom can be classified into hemotoxic, neurot

Management of venomous snakebite injury to the extremities.

Pit vipers (subfamily Crotalinae) are responsible for most venomous snakebites in the United States. The mixture of proteins with cytotoxic, proteolytic, and/or neurotoxic enzymes in snake venom varies by species. Treatment in the field consists of safe identification of the species of snake and rapid transport of the patient to the nearest health care facility. Swelling, bruising, and systemic symptoms are seen following snakebite. Most patients respond to elevation of the affected extremity and observation. Some require the administration of antivenin. Crotalidae Polyvalent Immune Fab (Ovine

Malayan Pit Viper Envenomation and Treatment in Thailand.

INTRODUCTION: The Malayan pit viper (MPV; Calloselasma rhodostoma) is a hematotoxic snake found in all regions of Thailand and many countries in Southeast Asia. Treatment of MPV envenomation varies among facilities due to their capabilities. MATERIALS AND METHODS: This study was a retrospective review of patients with MPV envenomation who were reported to the Ramathibodi Poison Center from 1 July 2016 to 30 June 2018. RESULTS: Of the 167 patients (median age, 40.5 years; range, 1.3-87.0 years) bitten by an MPV, the most common bite site was the foot (29.3%). Most patients reached the hospita

Venomous bites, stings, and poisoning.

This article discusses the epidemiology, prevention, clinical features, first aid and medical treatment of venomous bites by snakes, lizards, and spiders; stings by fish, jellyfish, echinoderms, and insects; and poisoning by fish and molluscs, in all parts of the world. Of these envenoming and poisonings, snake bite causes the greatest burden of human suffering, killing 46,000 people each year in India alone and more than 100,000 worldwide and resulting in physical handicap in many survivors. Specific antidotes (antivenoms/antivenins) are available to treat envenoming by many of these taxa but

Inhibition of hemorragic snake venom components: old and new approaches.

Snake venoms are complex toxin mixtures. Viperidae and Crotalidae venoms, which are hemotoxic, are responsible for most of the envenomations around the world. Administration of antivenins aimed at the neutralization of toxins in humans is prone to potential risks. Neutralization of snake venom toxins has been achieved through different approaches: plant extracts have been utilized in etnomedicine. Direct electric current from low voltage showed neutralizing properties against venom phospholipase A2 and metalloproteases. This mini-review summarizes new achievements in venom key component inhibi

A bioactive chalcone from the aerial parts of Indigofera conferta Gillet.

Ethnobotanical information indicates that Indigofera conferta is used in northern Nigeria for the management of poisonous snakebites and the methanol extract was previously reported to have antivenin activity. In this study, we report the isolation of an oxyprenylated bioactive secondary metabolite; 2',4'- dihydroxy-4-prenyloxychalcone from the chloroform fraction of the aerial parts of the plant's methanol extract and the in vivo evaluation of the compound against Naja nigricollis venom. The compound has demonstrated significant (p < 0.05) and dose-dependent antivenin activity against LD99 Na

Case report: acute demyelinating encephalomyelitis following viper bite.

The most serious complications of the central nervous system that occur after venomous snake bite are intracranial hemorrhage and ischemic stroke. We present a rarely seen central nervous system complication, acute demyelinating encephalomyelitis, after a treated Deinagkistrodon's viper bite.On April 5, 2015, a 50-year-old male farmer was bitten on his right leg by a Deinagkistrodon's viper. The bite rendered the victim unconscious for 14 days, during which he was treated with tetanus toxoid and polyvalent antisnake venom. Acute demyelinating encephalomyelitis (ADEM) was suspected after magneti

Scorpion anti-venom activity of botanicals: a pharmacological approach.

Scorpion bite is considered as one of the common and dangerous phenomenon throughout the world. The clinical manifestations include pulmonary edema, myocardial damage, intracerebral haemorrhage, brachial plexopathy, renal failure etc. which sometimes leads to mortality. The common antivenin therapy includes anti-scorpion venom serum or prazosin. In the vast rural areas of the third world countries phytotherapy is considered as an alternative system of medicine and scorpion sting is treated with the help of medicinal botanicals. As the safety and efficacy are considered as important aspects of

Presentation and treatment of venomous snakebites at a northern academic medical center.

Poisonous snakebites are relatively rare in the United States. The incidence of venomous snakebites is comparatively high in the southern states compared with the northern states and reports of these accidents from northern states is particularly uncommon. We report the experience with treatment of venomous snakebites at the University of Michigan over a 25-year period from 1976 to 2001. Six cases were identified and are described in detail. All patients were male and all were bitten in the upper extremity by pit vipers. One patient suffered a moderate envenomation and was treated with antiven

Pediatric snakebites: lessons learned from 114 cases.

PURPOSE: Evidence-based guidelines for the treatment of pediatric snakebite injuries are lacking because they occur infrequently in most centers. METHODS: We reviewed our experience treating snakebites from January 1995 through December 2005. Demographic (eg, age, sex, geographic location) and clinical information (eg, location of bite, species of snake, vital signs, laboratories, treatment, hospital length of stay) were obtained. RESULTS: Over the last decade, we have treated 114 children with confirmed snakebites. Mean age was 7.3 +/- 4.2 years (range, 1-17 years), and snakebites were more

[Clinical study of 48 cases of viper (Agkistrodon halys blomhoffii "Mamu shi") bite--Does the swelling at the first medical examination reflect the seriousness of the envenomation?].

We have evaluated 48 cases of viper (Agkistrodon halys blomhoffii "Mamushi") bite treated in our hospital from 2001 to 2005. Of all the patients, 21 were bitten in rice field/farm, 10 were in path, 9 were in yard and 8 were in other places. Most cases occurred between July and September. The most frequently bitten regions were fingers and toes. Because swelling reached its maximum at 1.1+/-0.5 day, it was difficult to judge the severity of Mamushi bite by the degree of swelling at the first examination. There was no significant relation between the grade classification of swelling and CPK valu

Proteins from Mucuna pruriens and enzymes from Echis carinatus venom: characterization and cross-reactions.

Mucuna pruriens seeds have been widely used against snakebite in traditional medicine. The antivenin property of a water extract of seeds was assessed in vivo in mice. The serum of mice treated with extract was tested for its immunological properties. Two proteins of Echis carinatus venom with apparent molecular masses of 25 and 16 kDa were detected by Western blot analysis carried out using IgG of mice immunized with extract or its partially purified protein fractions. By enzymatic in-gel digestion and electrospray ionization-mass spectrometry/mass spectrometry analysis of immunoreactive veno

Protective effects of clinical anticholinergic and anticholinesterase agents against Bungarus multicinctus venom and neurotoxin-rich snake venoms.

Bungarus multicinctus is one of the most venomous and lethal snake species in mainland China, with envenomation resulting in a mortality rate as high as 23%. Currently, antivenin against B. multicinctus remains the gold standard for treating bites from this species. However, in remote mountainous areas or rural regions of China, access to antivenin may be delayed or unavailable following a bite. The lethal components of B. multicinctus venom include α-, β-, γ- and κ-bungarotoxins, which act on either the presynaptic or postsynaptic membrane, thereby altering acetylcholine levels in the synapti

[Acute compartment syndrome following snake bite].

The experience with snake bites, causing local complications is discussed. Whenever systemic envenomation occurs, antivenin is the treatment of choice. Tissue necroses are treated by early debridement and a possible closed compartment syndrome demands the open fasciotomy.

The black widow: is she deadly to children?

Pediatric cases of black widow spider envenomation (BWSE) were reviewed in order to assess the morbidity and mortality in children with BWSE, and to make recommendations for optimal therapy. The methodology includes a retrospective chart review in an urban pediatric tertiary care hospital. Included were all pediatric patients admitted with the BWSE diagnosis in the last 10 years (1984-1994). The results are based on 12 children with ages ranging from 15 months to 18 years. The BWSE syndrome is characterized by several common systemic effects. Our cases revealed abdominal pain (100%), hypertens

Cobras.

Human encounters with cobras in Africa and Asia are common with substantial mortality and primarily respiratory paralysis in untreated cases. First aid includes maintaining calm, placing a lymphatic retardant on the bitten extremity, and transportation to a medical facility. Medical management is antivenin. Physiologic monitoring and the ability to manage the compromised airway are important.
