Smoking behaviors across anxiety disorders.

The purpose of this study was to test the theory put forth by Zvolensky et al. [Clin. Psychol. Sci. Pract. 10 (2003) 29] that smoking is specifically associated with panic disorder (PD) and not more generally associated with other anxiety disorders. Smoking behaviors were examined across three anxiety disorders: panic disorder with or without agoraphobia, social phobia (SP), and obsessive-compulsive disorder (OCD). A greater proportion of the PD group (40.4%) reported smoking compared to the SP (20%) and OCD (22.4%) groups. Those in the PD group were also more likely than those in the other gr

Evolutionary aspects of anxiety disorders.

DANGER AND HARM ARE AVOIDED BY STRATEGIC DECISIONS MADE AT ALL THREE LEVELS OF THE TRIUNE FOREBRAIN: rational (neomammalian), emotional (paleomammalian), and instinctive (reptilian). This applies also to potential harm from conspecifics, which leads to a choice between escalating and de-escalating strategies. Anxiety is a component of de-escalating strategies mediated by the paleomammalian and reptilian forebrains. When the neomammalian (rational) brain fails to deal with the threat of conspecific danger, these more primitive de-escalating strategies may be activated and may present as anxiety

Ménière's disease and anxiety disorders.

Diagnosis of Menière's Disease is based upon the wellknown labyrinthic syndrome (hypoacusia, tinnitus and dizziness) which manifests with the typical abscessual, recurrent and unforeseeable course. Many Menière patients also report panic attacks, agoraphobia and anxiety which make evaluation of labyrinthic symptoms difficult, affect the onset and maintenance of vestibular compensation and require specific treatments which are very different from those for the control of endolymphatic hydrops. In order to analyse the nature and the meaning of the association of the vestibular and psychiatric sy

Nonpharmacological treatments for anxiety disorders.

An evidence-based review of nonpharmacological treatments for anxiety disorders is presented. The vast majority of the controlled research is devoted to cognitive behavior therapy (CBT) and shows its efficiency and effectiveness in all the Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition (DSM-IV) anxiety disorders in meta-analyses. Relaxation, psychoanalytic therapies, Rogerian nondirective therapy, hypnotherapy and supportive therapy were examined in a few controlled studies, which preclude any definite conclusion about their effectiveness in specific phobias, agoraphobia

Explicit memory in anxiety disorders.

Two experiments were conducted to study selective memory bias favoring anxiety-relevant materials in patients with anxiety disorders. In the 1st experiment, 32 patients with generalized anxiety disorder (GAD), 30 with social phobia (speaking anxiety), and 31 control participants incidentally learned GAD-relevant words, speech anxiety-relevant words, strongly pleasant words, and words with a neutral valence. Participants did not show any explicit memory bias for threatening materials. Thirty patients suffering from panic disorder (PD) with agoraphobia and 30 controls took part in the 2nd experi

Anxiety disorders in late life.

OBJECTIVE: To review the epidemiology, clinical characteristics, and treatment of anxiety disorders in late life. QUALITY OF EVIDENCE: Epidemiologic and comorbidity data are derived from well designed random-sample community surveys. There are virtually no controlled data specific to treatment of anxiety in the elderly. Guidelines for treating anxiety disorders in late life, therefore, must be extrapolated from results of randomized controlled trials conducted in younger patients. MAIN MESSAGE: Generalized anxiety disorder and agoraphobia account for most cases of anxiety disorder in late li

[Behavior therapy of anxiety disorders].

Behavior therapy is rapidly gaining on importance in the treatment of anxiety disorders. A brief overview of assessment and models of the origins and maintenance of anxiety is presented and demonstrated with a case example of strong avoidance behavior based on feared anxiety attacks in specific situations. The cognitive behavioral intervention comprised 4 sessions of treatment in the office with extensive self-exposure to the feared and previously avoided situation, namely driving the car on the highway and visiting shopping centers, between sessions. The exposure treatment in conjunction with

Anxiety disorders in elderly patients.

BACKGROUND: Late-life anxiety disorders, commonly seen in primary care settings, can coexist with other medical and psychiatric illnesses. A variety of effective treatment options is available for these patients. METHODS: MEDLINE was searched for articles published from 1970 to 1996 using the key words "anxiety," "elderly," "aged," "geriatric," "panic," "obsessive-compulsive," "phobia," and "generalized anxiety disorder." Studies of patients older than 65 years were reviewed. RESULTS: Generalized anxiety disorder, phobias, panic disorder, and obsessive-compulsive disorder are the most common

Anxiety disorders in plastic surgery.

Surgery is a stressful event, with the potential for profound disturbance to the patient's psychological and physiologic homeostasis. Cosmetic surgery is a particularly intense psychological experience because, in addition to the usual concerns about surgical side effects, cosmetic patients bring their hopes and expectations for improved self-image, putting them at risk for the added anxiety of disappointment. High levels of anxiety coupled with the perception of vulnerability or threat to self can cause significant psychological reactions complicating care for the plastic surgical patient. Th

Alcohol, Anxiety, and Depressive Disorders.

Alcoholics frequently experience episodes of intense depression and/or severe anxiety. Depressed or anxious alcohol-dependent people often believe that they drink to relieve symptoms of sadness or nervousness. However, research does not unanimously support the prior existence of severe depressive or anxiety disorders as a usual cause of alcoholism. A review of recent literature (from family studies, prospective investigations, and studies of children of alcoholics) on the complex interaction between alcohol dependence and independent anxiety/depressive disorders reveals that if an association

Anxiety disorders and marital quality.

This study compared perceived marital quality among couples in which neither, one, or both spouses met criteria for an anxiety disorder. Phobic husbands and their wives reported poorer marital quality than did other spouses. Husbands' panic disorders had similar but weaker effects on perceived marital quality, and wives' panic disorders predicted poor perceived marital quality by by husbands. Wives with generalized anxiety disorder perceived their marriages to be less satisfying than did other wives. The effects of husbands' generalized anxiety disorders were strongest in the presence of comor

Cognitive therapy of anxiety disorders.

A review of studies of cognitive-behavioral therapy (CBT) for generalized anxiety disorder, panic disorder with and without agoraphobia, and social phobia indicates that CBT is consistently more effective than waiting-list and placebo control groups. In general, CBT has proved more beneficial than supportive therapy as well. Comparisons with active behavioral treatments provide more variable results. Converging evidence suggests that cognitive change may be a strong predictor of treatment outcome, but that such change may be produced by a number of therapeutic approaches. Pretest-posttest chan

Cognitive therapy for anxiety disorders.

In summary, we conclude that current evidence supports cognitive therapy as a promising approach to the treatment of anxiety. However, definitive conclusions regarding its efficacy must await large-scale, well-conducted, controlled trials that address the methodological shortcomings previously addressed. Furthermore, although there are many components to Beck's version of cognitive therapy, the critical ingredients are not known at this time. Despite the absence of data regarding this issue, we believe that future trials of cognitive therapy should include the following as core components: (a)

An overview of anxiety disorders.

Anxiety disorders are more prevalent than any other mental health problem in the United States. Afflicted persons are often untreated or are treated only by a primary care physician without psychiatric consultation. However, based on current knowledge of the biological and psychological factors involved in these disorders, psychiatry has defined effective treatment that integrates education, psychotherapy, and psychopharmacology. After reviewing current research on possible mechanisms of anxiety, the authors discuss therapeutic interventions and recommend ways to choose appropriate treatment m

Neuroendocrine findings in anxiety disorders.

Anxiety disorders are newly defined syndromes in which inappropriate state anxiety is the sole or primary symptom. Hormonal manifestations of acute stress are usually minimal or absent in these disorders. A number of findings suggest receptor down-regulation or enzyme induction of sorts that would be expected to mute or dampen these responses.

The classification of anxiety disorders.

This paper discusses the relationship between panic and other symptoms of anxiety disorders. The approach to classification of these disorders described in the Diagnostic and Statistical Manual of Mental Disorders (Third Edition) of the American Psychiatric Association is outlined. It is concluded that there are substantial advantages to the adoption of such a system. Not only does the separation of distinct syndromes offer some understanding of aetiology but there is evidence of a differential treatment response.

Antidepressant drugs in anxiety disorders.

The efficacy of tricyclic antidepressants and monoamine oxidase inhibitors (MAOI) in the treatment of panic syndromes (panic disorder and agoraphobia with panic attacks) and obsessive-compulsive disorders is reviewed. Imipramine has been reported as significantly more effective against panic attacks than placebo or other psychoactive drugs in 12 of 16 studies. Clomipramine has been found superior to placebo or other psychoactive drugs in 5 controlled studies. The effectiveness of these drugs is also reflected in a large number of open trials. Anxiety disorders: classification and diagnosis.

Obsessive-Compulsive Disorder and Panic Disorder with or without agoraphobia are emphasized as specific entities that are easily accessible to pharmacological treatment, while more unspecific and heterogeneous syndromes are less predictable in their reaction to treatment. The DSM-III system, not yet official in Sweden, is described and commented on. Agoraphobia has undergone a re-evaluation and should be coupled with Panic Disorder, although the two illness components may have somewhat different genetic and biochemical mechanisms. Obsessive-Compulsive Disorder, on the other hand, should not be
