BACKGROUND AND PURPOSE: Stroke survivors are often described as apathetic. Because apathy may be a barrier to participation in promising therapies, more needs to be learned about apathy symptoms after stroke. The specific objective was to estimate the extent to which apathy changes with time over the first year after stroke and the impact of apathy on recovery.
METHODS: The Apathy Assessed cohort was formed from stroke survivors participating in a longitudinal study of health-related quality of life after stroke. A family caregiver completed an apathy questionnaire by telephone at 1, 3, 6, an

We examined the prevalence and correlates of apathy and irritability in a consecutive series of 101 patients with probable Alzheimer's disease (AD). Based on clinical criteria, 46 (46%) patients had apathy, and 13 (13%) patients had irritability. Apathy was significantly associated with more severe impairments in activities of daily living, significantly more severe extrapyramidal signs, and a significantly higher frequency of both major depression and dysthymia. Patients with irritability had significantly more severe impairments in activities of daily living and significantly higher depressi

BACKGROUND: Multiple sclerosis (MS), the leading cause of non-traumatic neurological disability in young adults, features heterogeneous motor and non-motor symptoms including fatigue, apathy and depression. Although fatigue and depression are well-studied in MS disability progression, to date, the independent and combined contributions of apathy, fatigue and depression to neurological disability remain unexplored.
OBJECTIVE: This cross-sectional study assessed apathy, depression, and fatigue prevalence in 85 Italian MS patients, their associations with Expanded Disability Status Scale (EDSS)

BACKGROUND: Anxiety, fatigue, and excessive daytime sleepiness (EDS) frequently co-occur in Parkinson's disease (PD) and can influence the clinical determination of apathy.
OBJECTIVE: To distinguish patient-reported apathy from other non-motor symptoms.
METHODS: Individuals with PD meeting UK Brain Bank criteria underwent paired OFF- and ON- dopaminergic medication assessments, including the Movement Disorder Society-Unified Parkinson's Disease Rating Scale (MDS-UPDRS), Hamilton Anxiety and Depression Rating Scales (HAM-A, HAM-D), Symbol Digit Modalities Test, Stroop, and Scales for Outcomes

This study was designed to determine whether apathy is associated with neurocognitive symptoms and/or depressive symptoms in HIV/AIDS and also whether apathy is associated with patient expectancies about antiretroviral medication adherence. Seventy-five HIV+ homosexual men and 58 HIV+ women were assessed for depressive disorders and symptoms. Neuropsychological tests measured attention, concentration, learning, memory, executive function, and psychomotor speed. Other measures included Marin's Apathy Evaluation Scale, the Adherence Determinants Questionnaire, CD4 cell count, and HIV RNA viral l

BACKGROUND: We sought to evaluate the characteristics of eye movements in Alzheimer's disease (AD) patients with apathy (AD-A) and their ability to identify AD-A and explore the shared neurostructure of eye movements and apathy.
METHODS: Total 32 normal controls, 36 AD-A, and 72 AD with no apathy (AD-NA) patients were recruited. Parameters of smooth pursuit, fixation, prosaccade, and antisaccade were compared among the three groups. Correlation analyses were conducted among apathy score, eye movement parameters, and gray matter volume (GMV) in AD patients. Receiver operating characteristic cu

Apathy is a prevalent non-motor symptom in Parkinson's disease (PD) that negatively impacts quality of life. Impaired self-awareness of apathy (ISA-a) further impacts patient care by limiting engagement. While apathy has been associated with reduced fronto-striatal functional connectivity (FC), the neural basis of ISA-a remains unclear. We examined ISA-a in 52 individuals and the neural basis of ISA-a in 35 individuals with PD using a dimensional approach (i.e., initiation, executive, and emotional apathy) and resting-state fMRI (3T scanner). Apathetic PD patients (42%) showed poorer self-awar

BACKGROUND: Apathy is highly frequent in behavioral variant frontotemporal dementia (bvFTD). It is presumed to involve different pathophysiological mechanisms and neuroanatomical regions.
OBJECTIVE: We explored the hypothesis that subgroups showing distinct profiles of apathy and distinct patterns of atrophy within frontal lobes could be disentangled in bvFTD.
METHODS: Using data-driven clustering applied to 20 bvFTD patients, we isolated subgroups according to their profiles on the three subscales of the Dimensional Apathy Scale (DAS). We explored their apathy profiles and atrophy patterns.

BACKGROUND: Apathy and depression commonly occur in patients with progressive supranuclear palsy (PSP)-Richardson's syndrome variant; depression often requiring treatment. Little is known, however, about apathy and depression among other PSP variants.
METHODS: We prospectively studied 97 newly diagnosed PSP patients. All were classified into a PSP variant using the 2017 Movement Disorder Society-PSP criteria and administered the Geriatric Depression and Apathy Evaluation Scales. Differences in apathy and depression frequency and severity across six variants, and secondarily across PSP-Richard

Apathy is one of the most common and debilitating nonmotor manifestations of Parkinson's disease (PD) and is characterized by diminished motivation, decreased goal-directed behavior, and flattened affect. Despite its high prevalence, its underlying mechanisms are still poorly understood, having been associated with executive dysfunction, and impaired emotional processing and decision making. Apathy, as a syndrome, has recently been associated with reduced activation in the ventral striatum, suggesting that early- to middle-stage Parkinson's disease patients with this manifestation may have a c

BACKGROUND: Apathy is the most common neuropsychiatric manifestation in Alzheimer disease (AD). Clinical, single-photon emission computed tomography, magnetic resonance imaging, and pathologic studies of apathy in AD have suggested an association with frontal dysfunction, most supportive of anterior cingulate abnormalities, but without a definitive localization.
OBJECTIVE: To examine the association between apathy and cortical metabolic rate on positron emission tomography in AD.
DESIGN: Forty-one subjects with probable AD underwent [(18)F] fluorodeoxyglucose positron emission tomography ima

OBJECTIVE: Apathy is a common negative symptom in schizophrenia. The authors investigated neuropsychological performance and regional brain volumes in schizophrenia patients with high versus low levels of apathy.
METHOD: Schizophrenia patients with low apathy levels (N=18) and high apathy levels (N=20) and 12 healthy comparison subjects completed neuropsychological testing as well as magnetic resonance imaging scanning to obtain lobar volumes after total intracranial volume was controlled.
RESULTS: The high apathy group scored lower than comparison subjects on rapid visuomotor sequencing and

Neuropsychiatric symptoms such as depression and apathy are frequently reported in all subgroups of frontotemporal dementia (FTD) or frontotemporal lobe degeneration (FTLD) and represent a substantial burden for both patients and caregivers. Both symptoms are most common in its behavioral variant (bvFTD) with a prevalence of up to 90%. Their severity is particularly high in bvFTD and the semantic variant of primary progressive aphasia (svPPA). Due to its clinical heterogeneity, bvFTD can mimic depression, especially in its initial stages, making their differentiation a major clinical challenge

BACKGROUND: Parkinson's disease (PD) encompasses motor (e.g., bradykinesia) and non-motor (e.g., apathy) symptoms.
OBJECTIVE: We aimed to use reflexive and voluntary saccades as a proxy for bradykinesia and apathy.
METHODS: Seventeen PD patients and thirteen controls (matched for age and educational level) were recruited. We assessed apathy using the Dimensional Apathy Scale (DAS) and bradykinesia using MDS-UPDRS III. Subjects were asked to fixate successively two green points (cues, 40° apart) alternating at 1 Hz. After 20 s, all stimuli disappeared, and participants were required to contin

A diagnosis of young-onset dementia can pose a significant challenge for the clinician. We present a young patient with a very unusual presentation of Dementia with Lewy Bodies. The lack of motor symptoms and his marked apathy delayed his diagnosis. His symptoms were thought to be due to depression based on normal structural imaging and the psychiatric nature of his presentation. An extensive work-up was performed. Evidence of a structural neurodegenerative process was provided by the HMPAO-SPECT. Cardiac MIBG confirmed the diagnosis.

Although one of the most prevalent and impactful features of Huntington's disease (HD), little is known about the impact of apathy on HD caregivers, although there is evidence it affects perceptions of distress and burden. Given the importance of the caregivers, we aimed to explore the lived experience of people supporting someone with HD and associated apathy. Semi-structured interviews were conducted with 11 caregivers and analysed using reflective thematic analysis, informed by a phenomenological framework. Five overarching themes were produced: (1) What even is apathy? (2) It makes my life

BACKGROUND: Apathy was identified as a feature of HIV early in the epidemic; however, there are no systematic reviews of the diverse literature on the sociodemographic and clinical correlates of apathy in HIV disease.
METHODS: The current study adopted a hybrid systematic-narrative review methodology in which we used PRISMA guidelines to identify, summarize, and critique peer-reviewed, empirical studies of apathy in HIV disease in the era of combination antiretroviral therapy.
RESULTS: A total of 34 studies of apathy in persons living with HIV (PLWH) were identified. Findings across these st

Aim: In Huntington's disease (HD) and Parkinson's disease (PD), apathy is a frequently cited barrier to participation in physical activity. Current diagnostic criteria emphasize dissociable variants of apathy that differentially affect goal-directed behavior. How these dimensions present and affect physical activity in HD and PD is unknown. Methods: Using a qualitative approach, we examined the experience of apathy and its impact on physical activity in 20 people with early-manifest HD or idiopathic PD. Results: Two major themes emerged: the multidimensionality of apathy, including initiation
