The Editorial Board of the Journal of Endodontics has developed a literature-based study guide of topical areas related to endodontics. This study guide is intended to give the reader a focused review of the essential endodontic literature and does not cite all possible articles related to each topic. Although citing all articles would be comprehensive, it would defeat the idea of a study guide. This section presents uses of calcium hydroxide related to apexogenesis, apexification, direct pulp cap, indirect pulp cap, internal resorption that has perforated laterally, intracanal medication, con

The use of mineral trioxide aggregate (MTA) for complete endodontic obturation of an immature maxillary central incisor is reported. The injured tooth of a 9-year-old male was avulsed secondary to a sports-related traumatic event. Early attempts at pulpal revascularization of the replanted tooth proved unsuccessful, as evidenced by radiographic signs of external inflammatory root resorption in the middle to cervical thirds of the root. Calcium hydroxide apexification over a 1-year period promoted cessation of the resorptive process, but was unable to adequately form an apical root canal barrie

Mineral trioxide aggregate (MTA) is a calcium silicate-based cement (CSC) commonly used in endodontic procedures involving pulpal regeneration and hard tissue repair, such as pulp capping, pulpotomy, apexogenesis, apexification, perforation repair, and root-end filling. Despite the superior laboratory and clinical performance of MTA in comparison with previous endodontic repair cements, such as Ca(OH)2 , MTA has poor handling properties and a long setting time. New CSC have been commercially launched and marketed to overcome the limitations of MTA. The aim of the present review was to explore

Regional odontodysplasia is a rare and unique dental developmental anomaly that affects tooth morphogenesis. This case is unusual in that it involved the mandibular dentition. However, in this case the mandible was unilaterally involved. The presence of an abscess in relation to the lower right first permanent molar, in the absence of deep caries or periodontal pathology was an interesting finding. The radiographic appearance of bizarre pulpal morphology with high pulp horns extending to the occlusal surface would provide an open communication between the pulpal tissue and oral cavity. The imp

INTRODUCTION: Regenerative endodontic procedures (REPs) are considered effective treatments for immature necrotic permanent teeth, with favorable outcomes. However, failed cases require subsequent treatment. This study aimed to review and analyze failed cases after REPs and suggest a treatment algorithm to aid clinical decision-making.
METHODS: A total of 111 REP cases were selected that were conducted between 2015 and 2020. Clinical outcomes were assessed based on clinical and radiographic evaluations. The criteria for failure included persistence of clinical signs or symptoms and/or periapi

This umbrella review evaluated systematic reviews in clinical studies on pulp revascularisation in permanent mature necrotic teeth to determine their effectiveness in treating or preventing apical periodontitis. Seven of 294 articles retrieved from PubMed, LILACS, SciELO, Embase, Scopus, Web of Science, Cochrane and DANS Easy databases were included. The most used pulp revascularisation techniques were blood clots associated or not with platelet-poor plasma and umbilical cord mesenchymal stem cells. Meta-analysis and qualitative results from included articles indicated the efficacy of pulp rev

Traumatic dental injuries (TDIs) can significantly compromise oral health-related quality of life. In younger patients, luxation injuries are associated with a higher risk of pulp necrosis. This case report describes the multidisciplinary management of a 9-year-old patient who presented with a rotated and fractured permanent maxillary right central incisor exhibiting pulp necrosis after dental trauma. An initial attempt at regenerative endodontic treatment was unsuccessful, requiring apexification. Orthodontic treatment was performed to achieve maxillary expansion and rotational alignment of t

Managing necrotic immature permanent teeth with an open apex presents a treatment challenge. Regenerative endodontic procedures have emerged as biologically based approaches for these cases, suggesting an alternative to conventional apexification. This report describes successful revascularization of an immature mandibular second premolar with a chronic apical abscess. Following canal disinfection with NaOCl irrigation and applying a calcium hydroxide as an intracanal medicament, a scaffold was created by inducing a blood clot within the canal. During follow-up sessions, the tooth remained asy

This case report presents the surgical endodontic treatment of an Oehlers Class II invaginated immature tooth. In this case, communication between the invagination and the pulp caused pulp necrosis, periapical inflammation, and cessation of root formation. The initial treatment goal was to achieve apexification and then fill both the root canal and the canal of the invagination. After a 6-month treatment with calcium hydroxide dressings, there was no healing radiographically and no sign of a hard tissue barrier in the apex. Periapical surgery with the placement of mineral trioxide aggregate (M

BACKGROUND: Mineral trioxide aggregate, or MTA, is a new material developed for endodontics that appears to be a significant improvement over other materials for procedures in bone. It is the first restorative material that consistently allows for the overgrowth of cementum, and it may facilitate the regeneration of the periodontal ligament.
CASE DESCRIPTION: The authors present five cases in which MTA was used to manage clinical problems. These included vertical root fracture, apexification, perforation repair and repair of a resorptive defect. In each case, MTA allowed bone healing and elim

INTRODUCTION: Dental injuries in immature permanent teeth often result in endodontic complications. Apexification technique using calcium hydroxide is associated with certain flaws, such as long treatment time, the possibility of tooth fracture and incomplete calcification. The use of an apical plug employing mineral trioxide aggregate (MTA) is an alternative treatment option.
CASE REPORT: We reported the successful treatment of 4 maxillary incisors (in a 7-year-old boy and a 10-year-old girl) with open apices and periapical lesions. Apical portions of the canals were filled with MTA plugs in

This article presents a rare case of a young adult with multiple talon cusps on maxillary anterior teeth, dens evaginatus on maxillary second premolars, and hypodontia of the mandibular second premolar. These anomalies can cause occlusal interference, caries, fractures leading to pulp exposure, and esthetic concerns. Management options include gradual cusp reduction, composite resin coverage, pulpotomy, apexification, root canal therapy, or no treatment when asymptomatic. A 20-year-old male patient presented with pain in the left mandibular wisdom tooth. Examination revealed talon cusps on all

Apical fenestration is a defect in the alveolar cortical plate, exposing the root without involving the alveolar bone margin, often linked to trauma, periodontal disease, and orthodontic treatment, leading to symptoms such as pain and abscesses from endodontic infections. This case report describes managing a mucosal fenestration in an endodontically treated tooth with nonsurgical root canal therapy and periodontal surgery. A 44-year-old male presented with mucosal fenestration and pain in the upper front jaw due to trauma and an inadequately treated root canal. Examination revealed an ill-fit

Root fractures often occur in immature maxillary permanent central incisors and can be combined with other injuries, making treating more difficult and prognosis uncertain. A 10-year-old girl sustaining a second dental trauma came to our hospital. Teeth 11 and 21 experienced root fracture following the first injury 15 months earlier. The second injury resulted in lateral luxation of the coronal fragment of Tooth 11 from the original fracture site and subluxation of Tooth 21. Tooth 11 was repositioned, and both Teeth 11 and 21 were splinted for 3 months. An apical plug was applied to Tooth 21 a

Children are uniquely susceptible to craniofacial trauma. Injuries to the teeth occur often as a result of falls and sport activities. The pulp often gets infected after dental trauma resulting in to numerous complications. The authors present a case report of successful restoration of traumatized teeth with open apex which were weakened due to long standing infection and internal resorption. Initially antibiotic combination of 3- mix was used to disinfect the root canals. One tooth is treated with conventional endodontic treatment and the other tooth with open apex and perforation is managed

The aim of this study was to evaluate the adequacy of spirally filled vs injected non-setting calcium hydroxide (NS Ca(OH)(2)) in the management of traumatized permanent teeth undergoing endodontic treatment for apexification, control of infection or the management of root resorption. A random selection of patients with traumatized teeth undergoing endodontic procedures was made. A total of 100 cases were assigned for placement of NS Ca(OH)(2) by spiral filling and 50 cases by injection of the dressing directly into the canal. Post-operative radiographs were assessed for complete canal obturat

The retention of pulpally involved deciduous tooth in a healthy state until the time of normal exfoliation remains to be one of the challenges for Pedodontists. A scientific noise has been generated about several materials some of which have been popular pulpotomy medicaments. Concerns have been raised about the toxicity and potential carcinogenicity of these materials, and alternatives have been proposed to maintain the partial pulp vitality, however to date no material has been accepted as an ideal pulpotomy agent. Mineral trioxide aggregate (MTA) is a biocompatible material which provides a

Endodontic management of the permanent immature tooth continues to be a challenge for both clinicians and researchers. Clinical concerns are primarily related to achieving adequate levels of disinfection as 'aggressive' instrumentation is contraindicated and hence there exists a much greater reliance on endodontic irrigants and medicaments. The open apex has also presented obturation difficulties, notably in controlling length. Long-term apexification procedures with calcium hydroxide have proven to be successful in retaining many of these immature infected teeth but due to their thin dentinal
