Apomorphine and levodopa challenge in patients with a focal midbrain lesion.

Three patients who presented with parkinsonian signs resulting from a focal midbrain lesion are reported. In all patients parkinsonian features occurred acutely and improved following acute challenge with apomorphine but not with levodopa. Remission of parkinsonian signs occurred spontaneously to a different degree. Inconsistent clinical response following administration of levodopa has been well documented in patients with focal midbrain lesions associated with parkinsonian signs; however, the efficacy of apomorphine has not been tested before. Anatomic or etiologic features do not allow us t

Effects of adenosine drugs on apomorphine-induced licking in rats.

1. In the present work, the effect of adenosine agonists and antagonists on apomorphine-induced licking has been studied. 2. Subcutaneous (s.c.) injection of apomorphine (0.125, 0.25 and 0.5 mg/kg) produced dose-dependent licking in rats. 3. Adenosine agonists 5'-N-ethylcarboxamide-adenosine (NECA) and N6-cyclohexyladenosine (CHA) decreased or increased the apomorphine response respectively. 4. Adenosine antagonists theophylline and 8-phenyltheophylline (8-PT) decreased the response induced by apomorphine. Potentiation of licking induced by CHA was decreased by 8-PT pretreatment. 5. It is conc

[The apomorphine test in the differential diagnosis of Parkinsonism. Initial experience].

BACKGROUND: The reactivity of postsynaptic dopamine receptors in the striatum is preserved in Parkinson's disease (PD) but not in parkinsonian syndromes of various origin (PS). Hence, the effect of dopaminergic stimulation can serve as one of the diagnostic criteria allowing to distinguish between PD and PS. The present study aimed to ascertain the clinical utility of Apomorphine (APO), a powerful short acting direct D1 and D2 receptors agonist drug, in the differential diagnosis of PD and PS. METHODS AND RESULTS: We studied 39 patients presenting parkinsonian signs. Based on clinical examina

Effects of naloxone on d-amphetamine- and apomorphine-induced behavior.

The effects of acute naloxone administration on d-amphetamine- and apomorphine-induced behavior were studied. Naloxone, in doses of 0.3-10 mg/kg (s.c.), antagonized the increase in ambulation and rearing induced by 1 mg/kg of d-amphetamine. When the dose of d-amphetamine was increased to 3 mg/kg, naloxone (3 mg/kg) antagonized only the increase in rearing activity. No dose (0.3-10 mg/kg, s.c.) of naloxone significantly affected d-amphetamine- or apomorphine-induced stereotyped activity. Naloxone (3 mg/kg) significantly augmented the apomorphine (1 mg/kg, s.c.)-induced increase in ambulation bu

Continuous subcutaneous perfusion of apomorphine in Parkinson's disease: Towards monotherapy?

BACKGROUND: Continuous subcutaneous apomorphine infusion (CSAI) is an effective therapy for Parkinson's disease (PD) patients with motor complications. Whether CSAI should be used in association or in monotherapy to prevent any pulsatile stimulation due to oral treatments remains unclear. OBJECTIVE: This study aimed to evaluate the feasibility, efficacy, and tolerance of CSAI monotherapy compared to its use in combination therapy. METHOD: We retrospectively collected the clinical characteristics of PD patients treated with CSAI between 2007 and 2023, before the introduction of CSAI (M0) and

Clinical trial evaluating apomorphine oromucosal solution in Parkinson's disease patients.

Apomorphine, used to treat OFF episodes in patients with Parkinson's disease (PD), is typically administered via subcutaneous injections. Administration of an oromucosal solution could offer a non-invasive and user-friendly alternative. This two-part clinical study evaluated the safety, tolerability, pharmacokinetics (PK), and dose proportionality of a novel apomorphine hydrochloride oromucosal solution, as well as its relative bioavailability to subcutaneous apomorphine injection and apomorphine sublingual film. In part A of the study, 12 patients with PD received 2 mg oromucosal apomorphine

A Pig Model to Assess Skin Lesions after Apomorphine Application.

Owing to their similarities, pigs are often used as experimental models for humans. In particular, the similarity of the skin allows them to be a good dermatological model. The aim of the study was to develop an animal model in conventional domestic pigs to evaluate skin lesions macroscopically and histologically after a continuous subcutaneous apomorphine application. A total of 16 pigs from two different age groups were injected with four different apomorphine formulations for 12 h daily over a period of 28 days into the subcutis, which was then evaluated macroscopically for nodules and eryt

An organic selenium compound attenuates apomorphine-induced stereotypy in mice.

Selenium compounds display neuroprotective activities mediated at least in part by their antioxidant actions. Oxidative damage has been implicated in psychiatric disorders including schizophrenia and bipolar disorder, and an alteration in expression of selenium-binding protein-1 (SELENBP-1) has been recently reported in both the blood and brain of schizophrenic patients. In the present study we examined the effects of the organic selenium compound 3'3-ditrifluoromethyldiphenyl diselenide [(F3CPhSe)2] on apomorphine-induced stereotypy in mice, an animal model of psychosis. Systemic administrati

Intravenous apomorphine therapy in Parkinson's disease: clinical and pharmacokinetic observations.

Six patients with Parkinson's disease and refractory motor fluctuations, with severe subcutaneous (s.c.) nodule formation as a result of long-term s.c. apomorphine infusions, were switched to intravenous (i.v.) therapy via a long-term in-dwelling venous catheter. Five patients were followed-up for a mean of 7 months (range 0.5-18 months). All patients had plasma apomorphine concentrations measured at baseline during s.c. infusions and three had follow-up measurements when stabilized on i.v. therapy, to test the hypothesis that motor fluctuations in these patients are largely due to impaired ab

Paradoxical behavioral response to apomorphine in tenascin-gene knockout mouse.

Tenascin is a large extracellular matrix glycoprotein which is highly expressed in the developing nervous system. To examine the role of tenascin in vivo, we have produced mice in which the tenascin-gene is inactivated. These animals did not easily habituate to unfamiliar circumstances and displayed hyperlocomotion. A dopamine receptor agonist, apomorphine, reduced this hyperlocomotion dose dependently, but this phenomenon was not due to the appearance of apomorphine-induced stereotypic behavior, suggesting that tenascin-gene mutant mice have a paradoxical behavioral response to apomorphine co

Apomorphine-induced motor behavior in rats exposed prenatally to alcohol.

We psychopharmacologically examined dopamine function in rats exposed to ethanol prenatally. Pregnant rats received liquid diets of 35% or 0% ethanol-derived calories (EDC), or ad lib lab chow (LC). Twenty-eight-day-old offspring received systemic doses of apomorphine chosen to stimulate predominantly presynaptic (0.02 or 0.1 mg/kg) or postsynaptic dopamine receptors (2.0 or 5.0 mg/kg). Behavior was scored automatically for 60 min in an "open field." For males, prenatal ethanol exposure resulted in a dose-response shift to the left for locomotor activity. Females exposed to the liquid diet, wi

Assessing Eligibility for Apomorphine Therapy in Parkinson's Disease: The Telemonitoring Potential.

BACKGROUND: Although there are established guidelines for the selection of suitable candidates, objective and timely referral for the use of apomorphine remains challenging. OBJECTIVES: This research examined how telemonitoring may facilitate referrals for continuous subcutaneous apomorphine infusion in Parkinson's disease (PD). METHODS: A Blind Rater and a multi-disciplinary team (MDT) including an expert PD nurse, neurologist, and geriatrician determined apomorphine pump treatment eligibility for 20 patients with PD. The MDT considered telemonitoring data and physical examination to determ

Hyperphosphorylated Tau Inflicts Intracellular Stress Responses that Are Mitigated by Apomorphine.

Abnormal phosphorylation of the microtubule-binding protein tau in the brain is a key pathological marker for Alzheimer's disease and additional neurodegenerative tauopathies. However, how hyperphosphorylated tau causes cellular dysfunction or death that underlies neurodegeneration remains an unsolved question critical for the understanding of disease mechanism and the design of efficacious drugs. Using a recombinant hyperphosphorylated tau protein (p-tau) synthesized by the PIMAX approach, we examined how cells responded to the cytotoxic tau and explored means to enhance cellular resistance t

Depletion of apomorphine induced behavioral sensitization in rats treated with escitalopram.

Apomorphine is a classical psychostimulant and used throughout the world as prescribed medicine. Despite of their therapeutic effects, use of psychostimulants is restricted because some psychosis and impulse control disorders are the consequence of their long term use. Studies suggest that serotonin (5-Hydroxytryptamine; 5-HT) has a critical role in psychosis and drug abuse. Center of the present article is to assess the impacts of serotonin in the easing of misuse potential; the sensitization prompted by recommended psychostimulant apomorphine. We researched that whether Escitalopram can weak

Parkinson's disease, visual hallucinations and apomorphine: A review of the available evidence.

BACKGROUND: Visual hallucinations (VH) occur in the clinical course of Parkinson's disease (PD) and are predictive for PD dementia. The genesis of VH is related to impaired bottom-up and/or top-down visual processing which can be linked to cholinergic dysfunction and mono-amine imbalance. The risk of developing VH with oral dopamine agonists seems to increase with advancing disease, while in contrast some clinical studies suggest that apomorphine does not worsen VH, or might even improve VH. METHODS: The aim of this study is to review the current evidence of apomorphine and its effects on VH

Sustained efficacy of apomorphine in Japanese patients with advanced Parkinson's disease.

BACKGROUND: This report presents data from one of the first trials of apomorphine rescue treatment for advanced Parkinson's disease (PD) conducted in Japan. This 3 month trial aimed to evaluate the sustainability of efficacy of intermittent apomorphine rescue treatment. METHODS: A phase III, double-blind, placebo-controlled trial was conducted in PD patients (n = 31) with motor fluctuations in spite of individually titrated treatment with levodopa and other anti PD. Intermittent treatment was titrated to the maintenance dose with a subsequent unblind 12-week outpatient phase. At the week-12 v

Effects of central dopaminergic stimulation by apomorphine on speech in Parkinson's disease.

OBJECTIVE: To determine the effect of central dopaminergic stimulation with apomorphine on speech in PD. BACKGROUND: Most patients with PD have a speech disorder. Of those, 89% have involvement of laryngeal function, and 45% have additional articulatory dysfunction. The effect of dopaminergic medications on these two dimensions of speech impairment in PD has not been selectively studied. METHODS: In a randomized, double-blind, placebo-controlled crossover design, patients with PD and speech impairment, Hoehn and Yahr stages 2 to 4 "off," and without severe dyskinesias were given placebo or a
