Obstructive apnea during sleep elevates the set point for efferent sympathetic outflow during wakefulness. Such resetting is attributed to hypoxia-induced upregulation of peripheral chemoreceptor and brain stem sympathetic function. Whether recurrent arousal from sleep also influences daytime muscle sympathetic nerve activity is unknown. We therefore tested, in a cohort of 48 primarily nonsleepy, middle-aged, male (30) and female (18) volunteers (age: 59±1 years, mean±SE), the hypothesis that the frequency of arousals from sleep (arousal index) would relate to daytime muscle sympathetic burst

INTRODUCTION: Persistent genital arousal disorder (PGAD) is a potentially debilitating disorder of unwanted genital sensation and arousal that is generally spontaneous and unrelenting. Since its first description in 2001, many potential etiologies and management strategies have been suggested.
AIM: To review the literature on PGAD, identify possible causes of the disorder, and provide approaches to the assessment and treatment of the disorder based on the authors' experience and recent literature.
METHODS: PubMed searches through July 2012 were conducted to identify articles relevant to pers

Phallometric assessments of single-victim sexual offenders against children have suggested that only about 50% of these men are more attracted to children than they are to adults. This has raised the question of what motivates the other 50% of men to approach young girls for sex. Freund et al. showed that gynephilic men (i.e., men preferentially attracted to adult women) evidenced greater arousal to images of prepubescent girls than to images of males of any age or to nonerotic images, arguing that gynephilic men may approach prepubescent girls as a "surrogate" for their preferred erotic targe

Over the past years, several authors have used voluntary hyperventilation as a method for inducing physiological arousal and the experience of anxiety. The purpose of this investigation is to analyze the relationship between the process of arousal and the experience of anxiety. In order to induce anxiety, two tasks were assigned: hyperventilating and preparing a speech. General hypothesis tested: hyperventilation will induce a high level of physiological arousal (heart rate and skin conductance) with a limited experience of anxiety (subjective appraisal of valence, intensity, and control), whe

A low arousal threshold is believed to predispose to breathing instability during sleep. The present authors hypothesised that trazodone, a nonmyorelaxant sleep-promoting agent, would increase the effort-related arousal threshold in obstructive sleep apnoea (OSA) patients. In total, nine OSA patients, mean+/-sd age 49+/-9 yrs, apnoea/hypopnoea index 52+/-32 events.h(-1), were studied on 2 nights, one with trazodone at 100 mg and one with a placebo, in a double blind randomised fashion. While receiving continuous positive airway pressure (CPAP), repeated arousals were induced: 1) by increasing

This study examined the role of presleep attributions about physiological events during sleep in nocturnal panic attacks. Patients who regularly experienced nocturnal panic were physiologically monitored as audio signals were presented during sleep. They were randomly assigned to 1 of 3 conditions: expected, in which signals of intense physiological changes were expected; unexpected, in which signals of intense physiological changes were not expected; or control, involving distinctly different signals unrelated to physiological responses. The unexpected condition led to substantially more self

This review describes the role of the pedunculopontine nucleus (PPN) in various functions, including sleep-wake mechanisms, arousal, locomotion and in several pathological conditions. Special emphasis is placed on the auditory input to the PPN and the possible role of this nucleus in the manifestation of the P1 middle latency auditory evoked response. The importance of these considerations is evident because the PPN is part of the cholinergic arm of the reticular activating system. As such, the auditory input to this region may modulate the level of arousal of the CNS and, consequently, abnorm

The endocrine correlates of sexual arousal were explored in male mice. Using indwelling atrial cannulae, temporal changes in the blood levels of six hormones were monitored in sexually rested vs. sexually sated males before and during their exposure to receptive females. The vaginae of some test females were artificially occluded to assess separately the impact of intromission on hormone levels. Sexually rested males responded to receptive females with dramatic increases in their blood levels of corticosterone, epinephrine, and norepinephrine. These changes occurred independently of intromissi

The human brain must support both stable and flexible neural dynamics to adapt to changing contexts. The thalamus has been hypothesized to coordinate these opposing dynamical regimes in the cerebral cortex; however, this has not been directly tested at the systems level. Here, we apply a time-resolved measure of linearity to multimodal recordings in humans and macaques, demonstrating irregular fluctuations in both human functional magnetic resonance imaging and electrophysiological in the awake state. These fluctuations follow a key neuroanatomical axis of the thalamus, are diminished under ph

Sleep’s microstructure emerges from the continual coupling of intrinsic arousal with the homeostatic descent of the night. To ask how this coupling unfolds across disorders, we mapped the nocturnal trajectory of phasic events in idiopathic REM sleep behaviour disorder (iRBD; n = 19), narcolepsy type 1 (NT1; n = 19), NREM parasomnia (n = 18), fibromyalgia (n = 13) and healthy controls (n = 40). Using an automated, cyclic alternating pattern (CAP)- inspired, A‑phase index (API), a B‑phase–agnostic, 60‑s read‑out of phasic burden, we profiled subtype‑specific activity across normalised nights in

Savoring is an effective positive emotion up-regulation strategy that has been shown to increase electrocortical and subjective response to positive and neutral pictures. In the emotion down-regulation literature, engaged emotion regulation strategies, which involve substantial processing of stimulus content, have been found to be better suited to low-arousing stimuli. Further, prior work had suggested that savoring might be less effective for neutral versus positive pictures (using ratings) and for pictures of cute animals compared to people - results that could be construed as evidence of sa

As the primary source of noradrenaline in the brain, the locus coeruleus (LC) regulates arousal, avoidance and stress responses1,2. However, how local neuromodulatory inputs control LC function remains unresolved. Here we identify a population of transcriptionally, spatially and functionally diverse GABAergic (γ-aminobutyric acid-producing) neurons in the LC dendritic field that receive distant inputs and modulate modes of LC firing to control global arousal levels and arousal-related processing and behaviours. We define peri-LC anatomy using viral tracing and combine single-cell RNA sequencin

Prescription stimulants such as methylphenidate are being used by an increasing portion of the population, primarily children. These potent norepinephrine and dopamine reuptake inhibitors promote wakefulness, suppress appetite, enhance physical performance, and are purported to increase attentional abilities. Prior functional magnetic resonance imaging (fMRI) studies have yielded conflicting results about the effects of stimulants on the brain's attention, action/motor, and salience regions that are difficult to reconcile with their proposed attentional effects. Here, we utilized resting-state

Sleep quality declines with age; however, the underlying mechanisms remain elusive. We found that hyperexcitable hypocretin/orexin (Hcrt/OX) neurons drive sleep fragmentation during aging. In aged mice, Hcrt neurons exhibited more frequent neuronal activity epochs driving wake bouts, and optogenetic activation of Hcrt neurons elicited more prolonged wakefulness. Aged Hcrt neurons showed hyperexcitability with lower KCNQ2 expression and impaired M-current, mediated by KCNQ2/3 channels. Single-nucleus RNA-sequencing revealed adaptive changes to Hcrt neuron loss in the aging brain. Disruption of

Arousal from sleep in response to CO2 is a life-preserving reflex that enhances ventilatory drive and facilitates behavioural adaptations to restore eupnoeic breathing. Recurrent activation of the CO2 -arousal reflex is associated with sleep disruption in obstructive sleep apnoea. In this review we examine the role of chemoreceptors in the carotid bodies, the retrotrapezoid nucleus and serotonergic neurons in the dorsal raphe in the CO2 -arousal reflex. We also provide an overview of the supra-medullary structures that mediate CO2 -induced arousal. We propose a framework for the CO2 -arousal r

Ongoing changes in arousal influence sensory processing and behavioral performance. Yet the circuit-level correlates for this influence remain poorly understood. Here, we investigate how functional interaction between posterior parietal cortex (PPC) and lateral posterior (LP)/Pulvinar is influenced by ongoing fluctuations in pupil-linked arousal, which is a non-invasive measure of neuromodulatory tone in the brain. We find that fluctuations in pupil-linked arousal correlate with changes to PPC to LP/Pulvinar oscillatory interaction, with cortical alpha oscillations driving activity during low

Sexual offenses cause significant harm to victims, their families, and society as a whole and thus are an important social concern. While it is commonly assumed that sexual offenses are committed solely by males, research has shown that approximately 5 % of sex crimes in the USA and Canada are committed by females. Penile plethysmography (PPG) is a method to measure male genital arousal, which is commonly used in the assessment and treatment of male sex offenders and men with paraphilic sexual interests. Similarly, vaginal photoplethysmography (VPP) is a test to measure female genital arousal
