Challenges in mammography: part 1, artifacts in digital mammography.

OBJECTIVE: Early detection of breast cancer is directly related to the radiologist's ability to detect abnormalities visible only on mammograms. Artifacts on mammograms reduce image quality and may present clinical and technical difficulties for the radiologist, mammography technologist, medical physicist, and equipment service personnel. CONCLUSION: In this article, we will illustrate the appearance of artifacts in full field digital mammography, review the causes of these artifacts, and discuss methods to eliminate artifacts in digital mammography.

Suppressing bladder artifacts in bone SPECT of the pelvis.

OBJECTIVE: Bladder-filling reconstruction artifacts have a detrimental effect on the image quality of pelvic bone single photon emission computed tomography (SPECT). Using a simple protocol consisting of forced diuresis coupled with intravenous (IV) hydration, this study was undertaken to obtain an artifact-free pelvic SPECT after discarding the residual urinary activity. METHODS: Thirty patients were enrolled. In group I, pelvic SPECT was performed directly after normal void, whereas in group II, SPECT was preceded by IV injection of 0.5 mg/kg furosemide (maximum 40 mg) coupled with IV infus

The Haldane malaria hypothesis: facts, artifacts, and a prophecy.

Heterozygous thalassemia and sickle cell disease produce mild hematological symptoms but provide protection against malaria mortality and severe malaria symptoms. Two explanations for resistance are considered in the literature - impaired growth of the parasite or enhanced removal by the host immune cells. A critical overview of studies that connect malaria resistance with impaired intra-erythrocytic growth is presented. All studies are fraught with two kinds of bias. The first one resides in the impossibility of reproducing the in vivo situation in the simplified model in vitro. The second st

Interpretation of tissue artifacts in transbronchial lung biopsy specimens.

Proper interpretation of transbronchial biopsies is critical for appropriate patient management. Artifacts in lung tissue acquired during the biopsy procedure or subsequent processing may mimic "true" disease and potentially lead to incorrect diagnoses. In this study the interpretation of various artifacts in transbronchial biopsies will be correlated with the level of pathologist training and experience. Minced 1 to 2 mm fragments of normal lung tissue were processed to produce various tissue artifacts (atelectasis, sponge artifact, or bubble artifact). Seven hematoxylin-eosin-stained slides

Radiation force imaging of viscoelastic properties with reduced artifacts.

It is well-known that changes in the mechanical properties of tissues are correlated with the presence of disease. In the eye, for example, the vitreous body undergoes dramatic changes in mechanical properties during age-related degradation. These changes may play a significant role in the formation of retinal detachment or other vitreoretinal diseases. We previously presented a noninvasive method called kinetic acoustic vitreoretial examination (KAVE), which may be used to detect these mechanical changes. KAVE uses acoustic radiation force as a means to produce small, localized displacements

Spiral CT of the abdomen: artifacts and potential pitfalls.

Spiral CT has been advocated as a major advancement in CT technology. Spiral CT scanning is rapid, the volume of IV contrast material can potentially be decreased, and high-quality coronal, sagittal, and three-dimensional reconstruction is possible. However, when compared with dynamic CT, because of the shorter acquisition time of spiral CT, vascular and organ enhancement on spiral CT scans is more dependent on factors that affect the delivery of contrast material into the bloodstream and on the time that scanning begins after the start of injection of contrast material. Additionally, organs o

Magnetic susceptibility artifacts in gradient-recalled echo MR imaging.

Artifacts related to magnetic susceptibility differences between bone and soft tissue are prevalent on gradient-recalled echo images, particularly when long echo delay times are used. These susceptibility artifacts spatially distort and artifactually enlarge bone contours. This can alter the apparent shape of the spinal canal and exaggerate the degree of spinal stenosis seen in patients with cervical spondylosis. The effects of magnetic susceptibility artifacts in gradient echo imaging were studied in a phantom model and the results were correlated with MR images obtained in patients with cerv

Acoustic reflex crossover artifacts in infants and young children.

Crossover artifacts occur when the probe microphone of an impedance instrument directly responds to vibratory-acoustic energy that crosses the skull from the stimulus transducer on the contralateral ear. The resulting meter deflections are subjectively similar to and may be interpreted as actual acoustic reflexes (ARs) by unsuspecting examiners. In this study, infants and young children aged 10 days to 6 years 9 months were fit with earplugs to eliminate the AR and were examined for artifact incidence and threshold. Results indicate a high rate of occurrence of the crossover artifact in the yo

Tiger stripe: an innocuous Doppler artifact.

BACKGROUND: Tiger stripes, parallel bands within regurgitant Doppler spectra, are often ascribed to oscillating intracardiac structures. Their frequency and clinical relevance are unclear. METHODS: We retrospectively reviewed 4,567 transthoracic echocardiograms (TTEs) in adults between 18 and 65 years of age for tiger stripes on pulsed-wave (PW)/continuous-wave (CW) Doppler and correlated with valve pathology, presence of oscillating masses, and clinical findings. RESULTS: Tiger stripes occurred in 5.4% of studies (95% CI 4.8-6.1%), predominantly with mitral and aortic regurgitation jets (MR

An Electrocardiographic Artifact Mimicking Pacemaker Malfunction.

CASE SUMMARY: A patient presents with symptomatic bradycardia and high-grade atrioventricular block and receives a dual-chamber pacemaker with left bundle branch area pacing lead. Post procedure, there is concern for dislodgement of the ventricular lead after an electrocardiogram shows a missing ventricular paced beat. After additional device interrogation and review of the device manufacturer algorithms, it was determined that what was thought to be loss-of-capture or loss-of-sensing was coincidental artifact and normal device function. TAKE-HOME MESSAGES: The highest risk for pacemaker lead

In Practice: Maternal Heart Rate Artifact.

Maternal heart rate artifact occurs during fetal heart rate monitoring when the maternal heart rate is mistaken for the fetal heart rate. Unrecognized maternal heart rate artifact poses major patient safety risks due to errors of commission and omission when healthcare teams do not realise that the supposedly "fetal" heart rate tracing that they are observing is misleading. In 2022, a technical update was published by the Society of Obstetricians and Gynaecologists of Canada to mitigate this risk by describing the signs of possible maternal heart rate artifact and the steps to obtain an accura

Schistosome resistance to praziquantel: Fact or artifact?

Praziquantel is the current drug of choice for human schistosomiasis. Recent reports from laboratory and field studies concerning reduced praziquantel efficacy against Schistosoma mansoni have generated some controversy. The prevailing question is whether the emergence of strains of schistosome resistant to praziquantel is a fact, or an artifact resulting from erroneous field or laboratory experimentation. In this article, Padraic Fallon, Liang-feng Tao, Magdi Ismail and James Bennett examine the available evidence for schistosome resistance to praziquantel. Contributory factors to the schisto

Artifacts in the definition of toxicity by cyanides and cyanogens.

Misleading conclusions may be drawn in defining toxicity from administered cyanides or cyanogens if meticulous attention to detail is not given in the design, conduct and interpretation of experimental and analytical procedures. Problems may occur if specimens are not appropriately stored or if interfering factors, such as antidotal agents, are present. Measurement of whole blood cyanide concentrations is valuable for diagnostic purposes, but plasma concentrations may give a better functional index of blood cyanide providing that samples are immediately analyzed. The most appropriate tissues f

Postmortem intravascular bubbling: a decompression artifact?

The relationships between drowning occurring in divers using compressed air, with subsequent recovery of the body to the surface, and the finding of intravascular gas bubbles at autopsy were studied. Guinea pigs were exposed to compressed air in a hyperbaric chamber at various depths [98, 294, and 392 kPa (1,3, and 4 atm)] for various intervals and then drowned at depth. Various decompression schedules were used in returning the drowned animals to the surface. Autopsy examination indicated that large and small intravascular gas bubbles may be found, in the absence of traumatic air embolism or

Pseudohyponatremia: A Potentially Dangerous Laboratory Artifact.

Pseudohyponatremia is a rare laboratory artifact that inaccurately reports a falsely low serum sodium in the presence of excess lipids and/or proteins. Pseudohyponatremia secondary to excess lipids such as lipoprotein X is less common than hypertriglyceridemia or paraproteinemia and occurs in the setting of cholestasis. Here, we present a case of pseudohyponatremia due to suspected elevated serum lipoprotein X levels. Direct ion measurement of serum sodium levels confirmed normal serum sodium levels and the diagnosis of pseudohyponatremia in the presence of vastly elevated cholesterol levels.

GHB. Club drug or confusing artifact?

GHB can be produced either as a pre- or postmortem artifact. The authors describe two cases in which GHB was detected and discuss the problem of determining the role of GHB in each case. In both cases, NaF-preserved blood and urine were analyzed using gas chromatography. The first decedent, a known methamphetamine abuser, had GHB concentrations similar to those observed with subanesthetic doses (femoral blood, 159 microg/ml; urine, 1100 microg/ml). Myocardial fibrosis, in the pattern associated with stimulant abuse, was also evident. The second decedent had a normal heart but higher concentrat

Calculus artifact. A challenge in urinary cytology.

OBJECTIVE: To retrospectively review calculus artifact and compare it with instrument artifact and papillary transitional cell carcinoma (TCC). STUDY DESIGN: Voided urine specimens from patients with calculi (65), TCC (low grade, 10, high grade, 34) and history of prior instrumentation (12) were studied. RESULTS: Nineteen specimens of calculus artifact had unremarkable cytology. Forty-six specimens had abnormal single cells or papillary clusters and cell balls or a mixture of both. The papillary groups had smooth as well as irregular borders, a cytoplasmic collar and cells with occasional cy
