Atenolol. A reappraisal of its pharmacological properties and therapeutic use in cardiovascular disorders.

Atenolol is a selective beta 1-adrenoceptor antagonist with a duration of activity of at least 24 hours. The scope of therapeutic use of the drug has been expanded and become better defined since it was first reviewed in the Journal in 1979. Atenolol is effective and generally well tolerated in patients with all grades of hypertension. Data from comparative studies show that when administered orally, atenolol reduces blood pressure to a similar extent, and in a similar proportion of patients, as usual therapeutic doses of other beta-adrenoceptor antagonists (such as acebutolol, celiprolol, bet

Ischemic pain relief in patients with acute myocardial infarction by intravenous atenolol.

Pain relief in acute myocardial infarction (AMI) by the beta-adrenoceptor antagonist, atenolol, was demonstrated by three separate studies. First, 18 patients were randomized to double-blind intravenous atenolol (5 mg) or saline immediately after admission, followed by oral atenolol (50 mg) or placebo 10 minutes later. In patients receiving atenolol, pain relief coincided with reduction in heart rate (HR), systolic blood pressure (SBP), and HR X SBP product (p less than 0.05); however, pain and these parameters were unchanged by placebo. The degree of pain relief was related to the reduction i

Influence of the beta-blocking atenolol and other medications on visual reaction time.

Visual reaction time as a measure of vigilance and of the psychophysiological condition of subjects, was determined after combined physical and mental stress to examine the influence of beta blockade. Using the technique of electro-oculography, 40 subjects aged 25.7 +/- 6 years, with a mean blood pressure of 126/79 torr, were studied in a double-blind crossover design after application of placebo or 50 mg atenolol for 3 d. Visual reaction time was defined as the time between display of a peripheral light signal and the start of the eye movement that shifts the direction of gaze from the refere

Sustainable Synthesis of the Active Pharmaceutical Ingredient Atenolol in Deep Eutectic Solvents.

Atenolol, one of the top five best-selling drugs in the world today used to treat angina and hypertension, and to reduce the risk of death after a heart attack, faces challenges in current synthetic methods to address inefficiencies and environmental concerns. The traditional synthesis of this drug involves a process that generates a large amount of waste and other by-products that need disposal. This study presents a one-pot DES-based sustainable protocol for synthesizing atenolol. The use of the DES allowed the entire process to be conducted with no need for additional bases or catalysts, in

Investigation of the short-term effects of a transdermal formulation of atenolol in healthy cats.

OBJECTIVE: To investigate associations between short-term treatment with a previously described compounded transdermal formulation of atenolol and heart rate in cats. ANIMALS: 11 healthy adult cats. PROCEDURES: Cats received the atenolol gel formulation (gradually increased from 12.5 mg/cat, q 24 h to 25 mg/cat, q 12 h) by application to the pinnae at home over a 10-day period in a prospective, experimental study. On day 10, cats were hospitalized for measurement of serum atenolol concentrations 3, 6, and 12 hours after the morning treatment. Mean heart rate measured at the 3- and 6-hour tim

Atenolol vs nonatenolol β-blockers for the treatment of hypertension: a meta-analysis.

BACKGROUND: Previous reviews have shown that β-blocker use for the treatment of hypertension without compelling indications was associated with increased risk of stroke in the elderly. It remains unclear whether this increased risk was driven by the type of β-blocker. We sought to compare the efficacy of atenolol vs nonatenolol β-blockers in clinical trials enrolling young (< 60 years) and older patients with hypertension. METHODS: The Cochrane and MEDLINE databases were searched (January 2006-May 2013) for randomized trials evaluating stroke, myocardial infarction, death, or composite cardio

Mucous membrane pemphigoid in a patient with hypertension treated with atenolol: a case report.

INTRODUCTION: Atenolol is commonly used by patients with hypertension, angina pectoris, or myocardial infarction. There have been reports of various adverse effects associated with the use of atenolol including bullous pemphigoid. To the best of our knowledge we present the first case report of atenolol-induced mucous membrane pemphigoid. CASE PRESENTATION: A 42-year-old Thai man presented to our faculty after developing generalized fiery red gingiva and ulcerations on the buccal and labial mucosa after beginning atenolol treatment. Drug-induced mucous membrane pemphigoid was diagnosed from h

Long-term control of hypertension in dialysis patients by low dose atenolol.

Hypertension in dialysis patients is considered a major factor in cardiovascular mortality. We investigated long-term efficacy of intermittent atenolol (AT) administration in 10 (7M/3F) hypertensive dialysis patients, age 60.5 (38-72), on dialysis for 56.5 months (8-156) thrice per week (10.5-13.5 h/w) (A). A similar group of 11 normotensive patients served as controls (B). Hypertension was defined as BP> 140/90 (day) and >120/80 mmHg (night) by a 44-h ambulatory BP monitoring (ABPM) after the mid-week session. Dialysis ultrafiltration, hematology, biochemistry were similar in A and B. Atenolo

Treatment with atenolol prevents progression of microalbuminuria in type I diabetic patients.

Nine patients with Type I diabetes mellitus, diastolic blood pressure of 90 to 100 mmHg and persistent microalbuminuria of greater than or equal to 30 micrograms/min were treated with 50 to 100 mg atenolol daily for 3 years in an uncontrolled pilot study to assess the effect of long-term reduction of blood pressure on microalbuminuria. Treatment with atenolol prevented progression of microalbuminuria with a median (range) urinary albumin excretion rate before treatment of 74 (33 to 196) micrograms/min and 50 (5 to 123) micrograms/min after 3 years of therapy (p less than 0.05). The effects of propranolol or atenolol on the cardiovascular responses to central hypovolaemia in Europeans and Bengalees.

1. The effects of single oral doses of propranolol (80 mg), or atenolol (100 mg) on resting heart rate, blood pressure, forearm blood flow and forearm vascular resistance and on responses to central hypovolaemia, were compared with those of placebo in nine healthy European and nine healthy Bengalee volunteers, in a double-blind, three-period, cross-over study. 2. Atenolol induced a significant reduction in resting systolic blood pressure (SBP) in Europeans but not in Bengalees, although the bradycardic effects of atenolol were similar in both groups. Atenolol did not have any significant effec

Efficacy and safety of combinations of nitrendipine, atenolol, and hydrochlorothiazide in black hypertensive patients.

Twenty-four black hypertensive patients were studied in a randomized, double-blind trial of combinations of nitrendipine, atenolol, and hydrochlorothiazide. After six weeks, blood pressure was normalized in all three treatment groups, i.e. diastolic blood pressure less than 95 mmHg and a decline in diastolic blood pressure of at least 5 mmHg from baseline. The drug combinations containing hydrochlorothiazide were associated with higher incidence of metabolic abnormalities, especially hypokalaemia and hyperuricaemia.

Naloxone pretreatment blocks the hypotensive effects of atenolol in SHR and WKY rats.

The present study examined the interaction of opioidergic systems in the hypotensive action of atenolol. Anesthetized, adult spontaneously hypertensive (SHR) and Wistar Kyoto (WKY) rats were instrumented to monitor blood pressure and heart rate. Atenolol (100 micrograms/kg, i.v.) produced a decrease in blood pressure of similar magnitude in both SHR and WKY rats. However, the bradycardia was greater in SHR. Pretreatment with naloxone (0.1 mg/kg, i.v.) 15 min prior to the administration of atenolol completely prevented the hypotensive response in SHR and decreased the maximum hypotensive respon

Pharmacokinetic and pharmacodynamic modelling of atenolol in rabbits maintained on continuous peritoneal dialysis.

The pharmacokinetic and pharmacodynamic profiles of atenolol were studied in adult male rabbits on continuous peritoneal dialysis given 3 mg/kg i.v. before and during renal failure. The average terminal elimination half-life for the drug was 2.5 h calculated from blood, dialysate or urinary data. This value increased about nine times in anuric conditions. Although atenolol was eliminated in the peritoneal fluid, the amount excreted was relatively low both in normal conditions and renal failure, respectively 0.6 and 7% of the administered dose. The pharmacokinetic model was extended by an "effe

Effects of atenolol on exercise capacity in patients with mitral stenosis with sinus rhythm.

Exercise capacity is frequently impaired in patients with mitral stenosis (MS) and sinus rhythm (SR). The resulting increased heart rate, which shortens the diastolic filling period, and the increased cardiac output lead to further elevations of left atrial pressure and subsequent pulmonary congestion. The effect of the beta-receptor blocking agent atenolol, 100 mg/day, was assessed in 13 patients with MS and SR. Exercise performance was assessed using a modified multistage Bruce protocol after 2 weeks of placebo and after 2 weeks therapy with atenolol in a single-blind, crossover, placebo-con
