Real-World Multicenter Registry of Patients with Severe Coronary Artery Calcification Undergoing Orbital Atherectomy.

OBJECTIVES: We evaluated the safety and efficacy of orbital atherectomy in real-world patients with severe coronary artery calcification (CAC). BACKGROUND: The presence of severe CAC increases the complexity of percutaneous coronary intervention as it may impede stent delivery and optimal stent expansion. Atherectomy may be an indispensable tool for uncrossable or undilatable lesions by modifying severe CAC. Although the ORBIT I and II trials report that orbital atherectomy was safe and effective for the treatment of severe CAC, patients with kidney disease, recent myocardial infarction, long

Safety and outcomes of upfront large-size rotational atherectomy burrs in left main coronary interventions.

OBJECTIVES: Data on the safety and efficacy of percutaneous coronary intervention (PCI) with adjunctive rotational atherectomy (RA) in left main (LM) disease are limited. Current guidelines recommend using a small-size RA burr initially, with upsizing if required. The safety of upfront use of a large-size RA burr (≥1.75 mm) is unknown. The authors assessed the safety of an upfront large rotablation burr in the treatment of LM calcification and the procedural and long-term outcome of this approach in comparison to a small-size burr. METHODS: Retrospective analysis of all patients undergoing PC

Intravascular lithotripsy versus rotational atherectomy for coronary atherosclerosis calcification: a systematic review and meta-analysis.

BACKGROUND: Intravascular lithotripsy (IVL) is a novel calcium modification strategy with little evidence of efficacy compared to widely used rotational atherectomy (RA). AIMS: We sought to compare the efficacy and safety level between IVL and RA in moderate-to-severe coronary artery calcification (CAC). METHODS: We searched three databases until March 2025 for eligible observational studies and randomized controlled trials. The primary endpoint was major adverse cardiac event (MACE) while secondary endpoints were clinical outcomes, periprocedural complication, and procedural outcomes. Mante

Optical Coherence Tomography Characterization of Coronary Lithotripsy Following Atherectomy for Treatment of Severely Calcified Lesions.

BACKGROUND: A combined strategy of atherectomy followed by intravascular lithotripsy (dual-prep strategy) for severely calcified lesions may optimize stent deployment, but mechanistic evidence from prospective optical coherence tomography (OCT) imaging is limited. This study was designed to comprehensively characterize patterns of calcium modification during a dual-preparation strategy and to explore imaging determinants of calcium fracture and stent expansion using serial and 3-dimensional OCT assessment. METHODS: This was a prespecified, multicenter, single-arm OCT substudy of the DUAL-PREP

Rotational atherectomy for calcified coronary lesions in severe aortic stenosis before transcatheter aortic valve implantation.

OBJECTIVES: Calcified coronary artery disease (CAD) is prevalent in elderly patients with degenerative severe aortic stenosis (AS). Preparation of such calcified CAD using rotational atherectomy (RA) in those patients with severe AS is controversial and may carry a high risk of complications. The authors aimed to compare in-hospital outcomes following RA in patients with severe AS before transcatheter aortic valve implantation (TAVI) vs patients without AS. METHODS: The authors retrieved data from the Prospective Segeberg TAVI Registry from January 2016 to October 2021. All AS patients who un

Tenting Sign on Intravascular Ultrasound: A Red Flag for Coronary Perforation Risk During Rotational Atherectomy.

Coronary perforation (CP) is a rare but one of the most serious complications of rotational atherectomy (RA). The presence of a "tenting sign" on intravascular ultrasound, which reflects outward deformation of the normal vessel wall due to extreme wire bias, may indicate an elevated risk of CP following RA. We report a case of CP in the left circumflex coronary artery that occurred at a site where the tenting sign was observed on IVUS before RA.

High Risk Optical Coherence Tomography Findings in a Case of Coronary Perforation Following Orbital Atherectomy.

Coronary perforation (CP) is an uncommon but serious complication of orbital atherectomy (OA). Optical coherence tomography (OCT) evidence of catheter "tenting" against the healthy vessel wall may indicate unsatisfactory guidewire bias and elevated perforation risk during OA. We describe OCT findings of a case with OA-related CP.

Impact of Periprocedural Myocardial Injury on Long-Term Outcomes After Percutaneous Coronary Intervention Requiring Atherectomy.

The prognostic implications of cardiac troponin elevation after percutaneous coronary intervention (PCI) with atherectomy have not been established. The aim of this study was to investigate the incidence of periprocedural myocardial injury (PMI) and its association with cardiovascular events in patients with severely calcified lesions who underwent PCI with atherectomy. The study analyzed 346 patients (377 lesions) who underwent PCI with atherectomy between January 2018 and December 2021. Peak post-PCI high-sensitivity cardiac troponin (hs-cTn) was measured. The primary outcome was target lesi

ROBITAL Technique: Combination of Sequential ROtational and orBITAL Atherectomy in Management of Complex Coronary Calcium.

BACKGROUND & AIMS: Heavily calcified coronary artery disease remains one of the most formidable challenges in contemporary percutaneous coronary interventions, often leading to suboptimal stent expansion, procedural complications, and poor long-term outcomes. Over the years, a wide range of calcium modification techniques have been developed, including cutting and scoring balloons, rotational atherectomy, orbital atherectomy, high-pressure balloons, intravascular lithotripsy, and laser atherectomy, each with unique mechanisms, strengths, and limitations. This article aims to provide a comprehe

Feasibility of rotational atherectomy in patients with acute coronary syndrome: favorable in-hospital outcomes and clinical importance of complexed coronary atherosclerosis.

The feasibility of rotational atherectomy (RA) during percutaneous coronary intervention (PCI) in patients who present with acute coronary syndrome (ACS) remains fully unsettled. We retrospectively evaluated 198 consecutive patients who underwent RA during PCI from 2009 to 2020. All patients underwent intracoronary imaging (intravascular ultrasound 96.5%, optical coherence tomography 9.1%, both 5.6%) during PCI. Patients who underwent RA during PCI were divided into two groups: ACS (n = 49; unstable angina pectoris, n = 27; non-ST-elevation myocardial infarction, n = 18, and ST-elevation myoca

Outcomes of Patients With a History of Coronary Artery Bypass Grafting Who Underwent Orbital Atherectomy for Severe Coronary Artery Calcification.

OBJECTIVE: We assess the angiographic and clinical outcomes of patients with a history of coronary artery bypass graft (CABG) surgery who underwent orbital atherectomy for the treatment of severely calcified coronary lesions. BACKGROUND: The presence of severe coronary artery calcification (CAC) increases the complexity of percutaneous coronary intervention (PCI) and is associated with worse clinical outcomes. Patients with a history of CABG who undergo PCI often have comorbidities and are at higher risk for ischemic complications. METHODS: Of the 458 patients who underwent orbital atherecto

Combined Rotational excimer lASER coronary atherectomy (RASER) in non-crossable, non-dilatable coronary artery disease: observations from a single center.

BACKGROUND: Balloon non-crossable stenoses represent a challenging subset of coronary artery disease (CAD). They are clinically associated with patients who are older, frailer, and with multi-morbidities, and angiographically with increased tortuosity and coronary artery calcification. Combined rotational (RA) excimer laser coronary atherectomy (ELCA), or RASER, may facilitate stent delivery and deployment in non-crossable, non-dilatable severely calcified lesions. In this study, we assessed preliminary safety and efficacy of the RASER hybrid technique. METHODS: RASER feasible percutaneous co

Stentless percutaneous coronary intervention with directional coronary atherectomy and drug-coated balloon angioplasty in worsening angina patients with metal allergies.

UNLABELLED: Metal allergy is a concern in percutaneous coronary intervention (PCI) with stent implantation because of its potential association with poor cardiovascular outcomes, such as stent thrombosis and recurrent in-stent restenosis requiring revascularization. Although stentless PCI with drug-coated balloon (DCB) angioplasty is theoretically useful for patients with metal allergies, DCB angioplasty alone for huge plaques in large vessels may yield inadequate luminal enlargement and coronary deep dissection, leading to insufficient results. Directional coronary atherectomy (DCA) is effect

Mechanism of Drug-Eluting Stent Thrombosis Diagnosed by Histopathological Evaluation of Directional Coronary Atherectomy Specimen.

An 89-year-old man with a history of percutaneous coronary intervention using a sirolimus-eluting stent presented with recurrent in-stent occlusion. Pathological assessment of the neointima resected via directional coronary atherectomy revealed a double-layered thrombus. Clopidogrel resistance and limited antithrombotic regimen owing to high bleeding risk likely resulted in the in-stent thrombotic occlusion.
