Interpositional bone grafting and Le Fort I osteotomy for reconstruction of the atrophic edentulous maxilla. A two-stage technique.

This study presents the results from ten consecutive patients who, because of insufficient bone volume for conventional implant placement in the maxilla, were treated with an interpositional bone graft and Le Fort I osteotomy. The endosteal implants were placed six months after the osteotomy. A total of 60 screw-shaped titanium implants (Brånemark) were placed, of which three failed to integrate during the six-month healing period. No further implants were lost during the follow-up period, ranging from 15 to 39 months after placement of the implants. All patients received fixed bridges and all

Computer-guided calvarial mono-cortical bone blocks harvest: A novel approach for three-dimensional alveolar reconstruction of atrophic maxilla.

BACKGROUND: Calvarial bone grafts have been one of the grafting options that provides predictable results for three-dimensional reconstruction of severely atrophied ridges owing to their low resorption and rapid revascularization, thus providing adequate volume for implant insertion. The aim of this study was to introduce a computer-guided technique for calvarial graft harvest to minimize the complications known with grafting from this donor site. MATERIALS AND METHODS: Eight patients, suffering from severely atrophied, completely edentulous maxillary ridges, with an age range (21-30 years) w

Tissue Engineering Therapy for Atrophic Maxilla Using Minimally Manipulated CD90 and CD105 Bone Marrow Stem Cells: A Case Report.

Bone deficiencies in the jaws of edentulous patients limit the possibility of successful placement and osseointegration of endosseous implants. Bone resorption is defined as a vertical and horizontal loss of quality and quantity in the residual bone ridges; lack of bone structure requires bone grafting procedures to enable implant placement and to improve the function, predictability, and longevity of implants. The objective of this case study was to demonstrate the feasibility of a novel cell therapy treatment for localized jaw bone defects with tissue repair cells composed of a mixture of bo

A Novel 2-Piece Zygoma Extension System for Optimizing Prosthetic Emergence in the Atrophic Maxilla: A 3-Case Clinical Series.

Zygomatic and pterygoid implants are essential tools in the rehabilitation of the severely atrophic maxilla, yet they often result in prosthetic emergence in unfavorable posterior or palatal locations. This article introduces a novel 2-piece zygoma extension system, available in straight and angled (12°, 20°, 25°) configurations, designed to redirect emergence to the alveolar crest with improved biomechanics and prosthetic accessibility. Three patients with varying degrees of maxillary atrophy underwent treatment using MegaGen BLUEDIAMOND implants combined with precision-milled titanium straig

Bone augmentation of the atrophic anterior maxilla for dental implants using rhBMP-2 and titanium mesh: histological and tomographic analysis.

The loss of multiple teeth or trauma to the anterior maxilla often results in a deficient ridge width for prosthetic tooth rehabilitation. This study evaluated the use of titanium mesh and recombinant human bone morphogenetic protein 2 (rhBMP-2) for the repair of major bone defects in the alveolar bone. Five patients were enrolled in the study; these patients required implant replacements for two contiguous missing teeth in the anterior maxilla, which lacked sufficient bone. Residual ridges were augmented with rhBMP-2 and titanium mesh to direct the geometry of the newly formed bone. Seven mon

Biomechanical Analysis of Grafted and Nongrafted Maxillary Sinus Augmentation in the Atrophic Posterior Maxilla with Three-Dimensional Finite Element Method.

This study is aimed at determining the optimal sinus augmentation approach considering the poor bone condition in the zone of atrophic posterior maxilla. A series of simplified maxillary segment models varying in residual bone height (RBH) and bone quality were established. A 10 mm standard implant combined with two types of maxillary sinus augmentation methods was applied with the RBH, which was less than 10 mm in the maxilla. The maximal equivalent von Mises (EQV) stress in residual bone was evaluated. Bone quality had an enormous impact on the stress magnitude of supporting bone. Applying s

Osteotome sinus floor elevation technique without grafting material and immediate implant placement in atrophic posterior maxilla: report of 2 cases.

PURPOSE: This case report discusses 2 patients who required implant placement in the atrophic posterior maxilla to support a fixed prosthesis with the least invasive and shortest procedure. MATERIALS AND METHODS: The reference standard of care would be to perform sinus augmentation with an autologous bone graft through the lateral approach with delayed implant placement. However, in these cases, the posterior maxillas were treated with an osteotome sinus floor elevation procedure without grafting material and simultaneous placement of short, 8- and 10-mm-long, tapered implants. Transnasal Implants as an Alternative to Quadzygoma in the Rehabilitation of Extremely Atrophic Maxillae: Long-Term Results of the Vanderlim Technique Cases Report.

UNLABELLED: Transnasal Implants® are long implants installed in the residual crest of the maxilla, tangent to the lateral wall of the nasal cavity, and anchored apically to the conchal crest of the maxilla. This procedure is known as the Vanderlim Technique® and provides high-torque anterior maxillary anchorage (in Zone 1), favoring immediate loading. Despite being used clinically since 2016, no publication until now has addressed the long-term results of this technique. Therefore, this multicenter study aimed to investigate the survival rates of Transnasal Implants®. MATERIALS AND METHODS: T

Comparison of posterior support strategies with pterygoid implants for full-arch implant rehabilitation in the atrophic maxilla: a finite element study.

BACKGROUND: Rehabilitation of the atrophic maxilla is challenging due to bone resorption and anatomical limitations. Although the All-on-Four concept offers a predictable treatment strategy, posterior cantilevers may increase biomechanical risk. Pterygoid implants have been proposed as an alternative to enhance posterior support without the need for bone grafting. This study aimed to compare stress distributions in bone and prosthetic components using three different implant-supported treatment strategies for the atrophic maxilla. METHODS: Three three-dimensional finite element models were de

Long-Term Success of Dental Implants in Atrophic Maxillae: A 3-Year Case Series Using Hydroxyapatite and L-PRF.

Dental implants are essential for the prosthetic rehabilitation of edentulous patients, requiring adequate bone volume and density for osseointegration and load support. The posterior region of the maxilla, commonly deficient in bone quality and quantity, represents a clinical challenge. This case series reports an analysis involving 69 dental implants in the atrophic maxilla of nine patients. The procedures adopted combined alloplastic hydroxyapatite grafting and leukocyte platelet-rich fibrin (L-PRF) applied to the alveolar ridge and maxillary sinus lift. With an average follow up of three y

A Minimally Invasive Technique for Short Spiral Implant Insertion with Contextual Crestal Sinus Lifting in the Atrophic Maxilla: A Preliminary Report.

The most recently reported techniques for the rehabilitation of the atrophic posterior maxilla are increasingly less invasive, as they are generally oriented to avoid sinus floor elevation with lateral access. The authors describe a mini-invasive surgical technique for short spiral implant insertion for the prosthetic rehabilitation of the atrophic posterior maxilla, which could be considered a combination of several previously described techniques based on the under-preparation of the implant site to improve fixture primary stability and crestal approach to the sinus floor elevation without h

Atrophic Maxilla Rehabilitation Through Dental Implants Using the V-4 Strategy Associated With Guided Surgery: A Case Report and Technique Description.

Atrophic maxilla rehabilitation through dental implants is always a challenging procedure. However, alternative approaches such as guided surgery and the installation of short implants are progressively supplanting more invasive bone regeneration procedures. A V-4 technique described in 2016 facilitates the installation of dental implants in patients with atrophic maxilla; however, its authors recommend incision, flap opening, and elevation of the anterior area of the maxillary sinuses. This case report describes a less invasive proposal for modifying the technique through the association of g

Reconstruction of the atrophic maxilla combining Le Fort I osteotomy, sinus augmentation and titanium mesh guided bone regeneration: a technical note.

Severe atrophy of the edentulous maxilla poses a significant surgical challenge. This technical note presents a novel approach combining Le Fort I osteotomy, bilateral sinus augmentation, and titanium mesh guided bone regeneration. Preoperative prosthetic planning allows accurate maxillary repositioning to restore a skeletal Class I relationship and enable an implant-supported prosthesis without prosthetic gingiva. The titanium mesh provides stable, three-dimensional support for uniform ridge reconstruction, enabling ideal implant placement. This technique offers a reproducible and less invasi
