Improved pointer in auditory alarms enhances response accuracy.

BACKGROUND: Auditory alarms are crucial in clinical settings, alerting clinicians to events requiring immediate attention. However, multitasking can lead to missed alarms and disrupt patient care. Enhancing auditory alarms can improve patient safety and clinician satisfaction. METHODS: In a controlled laboratory study, we recruited 26 clinicians (residents, fellows, advanced practice providers) and 19 non-clinicians (undergraduate students) to compare our previously validated alarm with an improved design. The improved alarm incorporates a 'pointer' (a short sound burst indicating acuity leve

Reference ranges and stability of auditory and vestibular measures in a comprehensive assessment battery for traumatic brain injury.

BACKGROUND: Audiology clinics have many tools available to evaluate auditory and vestibular complaints. However, many tools lack established normative ranges across the life span. We conducted this study to establish reference ranges across the life span for audiology/vestibular measures commonly used to evaluate patients with traumatic brain injury. MATERIALS AND METHODS: In this repeated measures study, 75 adults, ages 18-65 years, without a history of traumatic brain injury, underwent robust auditory/vestibular evaluations three times over six months, including rotational chair, videonysta

Altered folate metabolism disrupts auditory function from neonatal vocalizations to adult perceptual precision.

Methylenetetrahydrofolate‐reductase (MTHFR) is a critical enzyme in folate-dependent one-carbon metabolism. While MTHFR polymorphisms have been linked to sensorineural hearing loss and neurodevelopmental disorders in humans, the translation of early-life metabolic stress into long-term sensory processing and perceptual consequences has remained unclear. Here, we combined neonatal ultrasonic vocalization analysis, adult auditory-brainstem recordings, two-photon calcium imaging of the auditory cortex, and high-resolution psychophysics to map the effects of partial Mthfr deficiency across differe

Differences in hearing levels between siblings with hearing loss caused by GJB2 mutations.

OBJECTIVE: Hearing loss caused by GJB2 mutations is inherited in an autosomal recessive manner (DFNB1); thus siblings of an affected child have a 25% chance of also being affected. Hearing loss among subsequent siblings carrying the same GJB2 mutation is a concern for parents and a frequent topic of enquiry during genetic counseling. Evidence exists for genotype-phenotype correlations of GJB2 mutations; however, no analysis of differences in hearing among siblings, in whom the common genetic background may decrease variation, has been reported. The purpose of the present study was to investiga

[Sensorineural hearing loss associated to Kawasaki Disease].

OBJECTIVE: Kawasaki disease is a systemic idiopathic self-limited vasculitis of small and medium-sized vessels. Thirteen cases of sensorineural hearing loss during the evolution of this disease have been described in the literature. We describe a case of an infant with Kawasaki disease who developed sensorineural hearing loss during the acute phase. This case report shows a complication of Kawasaki disease, with few citations in literature, enhancing the importance of careful evaluation of these patients. DESCRIPTION: Nineteen-month-old-boy, formerly healthy, with persistent daily fever seven

Comparison of Auditory Brainstem Responses Recorded from Three Different Clinical Systems.

BACKGROUND: The auditory brainstem response (ABR) test is used to estimate hearing thresholds and for neurodiagnostic purposes. ABRs can be recorded for traditional stimuli such as broadband clicks and frequency-specific tone bursts (0.5, 1, 2, and 4 kHz). Recently, chirp stimuli (broadband and narrowband chirps [0.5, 1, 2, and 4 kHz]) have been recommended to be used to record ABRs. PURPOSE: There is a lack of comparative studies of ABR latency, amplitude, and thresholds obtained from these systems for both traditional and chirp stimuli. RESEARCH DESIGN: Repeated measure design. An Exploratory Study of Auditory Brainstem Responses and Hearing Thresholds in Essential Tremor.

Background and Objectives: Essential tremor (ET) is the most prevalent movement disorder, yet its neurophysiological basis remains incompletely understood. Emerging evidence indicates that ET may involve non-motor manifestations, including auditory dysfunction. Given the anatomical convergence of tremor-related and auditory pathways at the brainstem level, electrophysiological assessment of the auditory system may provide insights into ET pathophysiology. This study aimed to evaluate auditory pathway function in patients with essential tremor using conventional audiometry, brainstem auditory e

Visual selective attention in individuals with age-related hearing loss.

Evidence from epidemiological studies suggests that hearing loss is associated with an accelerated decline in cognitive function, but the underlying pathophysiological mechanism remains poorly understood. Studies using auditory tasks have suggested that degraded auditory input increases the cognitive load for auditory perceptual processing and thereby reduces the resources available for other cognitive tasks. Attention-related networks are among the systems overrecruited to support degraded auditory perception, but it is unclear how they function when no excessive recruitment of cognitive reso

Central auditory processing in children with sickle cell disease.

The complexity of sickle cell disease (SCD) goes beyond hematological manifestations, affecting different organs and systems, including auditory system. We aimed to assess Central Auditory Processing (CAP) abilities in children with SCD as well as to detect risk factors for Auditory Processing Disorders (APD) in children with SCD. A diagnostic observational cross sectional study that included thirty-three patients aged 6-16 years with a confirmed diagnosis of SCD. Demographic, clinical and laboratory characteristics were collected. Audiological testing included tympanometry, pure tone audiomet

Auditory Numerosity Judgement Is Driven by Non-Spatial Factors Across Azimuth, Elevation, and Distance.

Everyday listening often requires the auditory system to move beyond the perception of individual sound sources towards a more global representation of acoustic scenes. While auditory behaviour has been well characterised for one or a few sources, it remains unclear how listeners perceive scenes containing many simultaneous sounds. As multiple sound sources combine into a single acoustic mixture, separating individual sources becomes increasingly difficult, potentially requiring listeners to rely on summary representations rather than discrete object-based parsing. Auditory numerosity judgemen

[Critical analysis of children with suspicion of auditory processing disorders].

BACKGROUND: APDs at infancy has become a common reason of introduction at phoniatric/pedaudiologic departments. The question arises whether this neurocognitive construct can be accounted with the standard clinical diagnostics. METHODS: The children with suspicion of APD referred to the Department of Phoniatrics/Pedaudiology at the University Hospital Goettingen/Germany were clinically examined. PATIENTS: Of 107 children 8 % demonstrated abnormal hearing acuity, 7 % learning and intelligence disorders, 7 % developmental dyslexia, 6 % language impairments, and 5 % attention deficit/hyperactivi

Auditory sensitivity in the middle-aged population in southwestern Finland.

The present study deals with the auditory sensitivity of men and women 40 to 64 years of age living in two municipalities situated in the southwestern part of Finland and the prevalence of defective hearing in this middle-aged population. Hearing acuity was tested in 1043 men (93.2%) and 1221 women (93.1%). It appeared in the investigation that hearing loss increased markedly with age in both sexes. Auditory sensitivity was clearly better in women than in men. Of the entire population 7.1% had evident bilateral hearing loss when an average loss of 25 dB in the speech range was used as the cut-

Symbol Discrimination Speed in Children With Visual Impairments.

PURPOSE: We measured visual acuity and visual discrimination speed simultaneously in children with visual impairments to determine whether they are slower than children with normal vision. METHODS: Five- to twelve-year-old children with visual impairments due to ocular dysfunction (VIo; n = 30) or cerebral visual impairment (CVI; n = 17) performed a speed-acuity test in which they indicated the orientation of Landolt-C symbols as quickly and accurately as possible. The reaction times for symbols ranging between -0.3 and 1.2 logMAR relative to acuity threshold were compared with normative data

Is there a relationship between brain-derived neurotrophic factor for driving neuronal auditory circuits with onset of auditory function and the changes following cochlear injury or during aging?

Brain-derived neurotrophic factor, BDNF, is one of the most important neurotrophic factors acting in the peripheral and central nervous system. In the auditory system its function was initially defined by using constitutive knockout mouse mutants and shown to be essential for survival of neurons and afferent innervation of hair cells in the peripheral auditory system. Further examination of BDNF null mutants also revealed a more complex requirement during re-innervation processes involving the efferent system of the cochlea. Using adult mouse mutants defective in BDNF signaling, it could be sh

Sonar detection of jittering real targets in a free-flying bat.

The auditory system measures time with exceptional precision. Echolocating bats evaluate the time delay between call and echo to measure object range. An extreme and disputed result on ranging acuity was found in the virtual delay jitter experiments. In these studies, echoes with alternating delays were played back to bats, which detected a jitter down to 10 ns, corresponding to a ranging acuity of 1.7 microm. The current study was designed to measure the ranging acuity of the nectarivorous bat Glossophaga soricina under semi-natural conditions. Three free-flying bats were trained to discrimin

Sound stimulus qualities and severe auditory neglect.

We report the case of a patient who developed severe left auditory neglect following a cerebral vascular accident with infarction of areas in the frontal, temporal, and parietal lobes of the right hemisphere. The severity of neglect was documented using pure tones, speech, and environmental stimuli, administered binaurally and dichotically. Acuity examination and unilateral presentation of stimuli allowed exclusion of primary hearing deficit. A dichotic word test was used to evaluate the effects of implicit and explicit cuing. Contrary to our expectations, implicit semantic cues did not attenu

Trial-by-trial auditory brainstem response detection.

The neural response of the brainstem to brief sounds, known as the auditory brainstem response (ABR), is widely employed in the laboratory and the clinic to diagnose hearing loss. Current ABR approaches are limited to analyzing the average ABR to hundreds of stimulus trials. Historically, trial-by-trial ABR analysis has not been possible owing to each trial's small signal-to-noise ratio. Here we overcome this limitation and show how to score individual ABR trials. We use the distribution of individual-trial ABR scores to define thresholds, which we call auditory brainstem detection (ABD) thres

Frequency Following Responses to Electric Cochlear Stimulation in an Animal Model.

PURPOSE: Present-day cochlear-implant (CI) users can achieve high levels of speech reception in quiet surroundings. Nevertheless, sensitivity to the temporal pitch of sounds is limited, which contributes to deficits in speech reception amid multiple talkers and in appreciation of musical melodies. Short-term, invasive neurophysiological studies in animals have demonstrated limitations in neural phase locking in the tonotopic range of the auditory pathway that is activated by CIs. It remains an open question, however, whether those neural limitations can account for perceptual deficits in those

[Elevated ABR thresholds in the presence of otoacoustic emissions: case series].

The article presents three clinical cases of an unusual audiological profile in children, not previously described in the literature. The cases are of interest due to the atypical symptomatology featuring preserved otoacoustic emissions (OAEs) concurrent with elevated thresholds of auditory brainstem responses (ABRs). Notably, wave V exhibited normal morphology and latency; in some instances, the ABR thresholds correlated with the behavioral thresholds. This clinical presentation cannot be classified as either conventional peripheral sensorineural hearing loss or an auditory neuropathy spectru

A case of acute labyrinthitis due to intra-aural tick infestation: a case report.

BACKGROUND: Intra-aural tick infestations, though uncommon, pose a serious clinical challenge owing to the risk of acute labyrinthitis-an inflammatory condition of the inner ear. This inflammation can lead to severe complications such as sensorineural hearing loss, vertigo, and facial nerve palsy. Prompt recognition and management are crucial to prevent these adverse outcomes. CASE PRESENTATION: A 21-year-old female patient of Limbu ethnicity from Dharan presented with sudden-onset right-sided otalgia, accompanied by vertigo, emesis, and auditory impairment. Otoscopic examination revealed a t
