OBJECTIVE: To assess the efficacy and safety of sulfasalazine (SSZ) compared to placebo and other disease modifying drugs.
METHODS: A metaanalysis was performed on 15 randomized clinical trials of rheumatoid arthritis (RA) that included SSZ (2 g/day average dose, 36 weeks average followup) as a treatment. Eight trials included a placebo group (PL), 2 hydroxychloroquine (HCQ) (350 mg/day average dose), 3 D-penicillamine (D-Pen) (667 mg/day average dose), and 4 gold sodium thiomalate or aurothioglucose (GST) (25 mg, 1 g/wk).

Renal biopsy specimens from two patients with gold nephropathy showed two distinctly different types of alterations. The first demonstrated lesions suggestive of immunologic injury primarily to the glomerulus and identical with membranous glomerulonephritis. We report about the case of a 74-year-old woman who suffered diffuse alveolar damage and consecutive lethal pulmonary failure after gold therapy for rheumatoid arthritis. This is the fourth documented case of fatal pulmonary failure following gold therapy. The clinical findings were dominated by severe dyspnoea that warranted respirator therapy shortly after admission. Nonspecific therapy of asthma with Solganal and insulin showed excellent results in more than 80% of the patients, in which 50% or more recovered from asthma completely. Over all, recovered condition lasted for a long period of time. Solganal-treated patients showed resistance to acetylcholine provocation tests, good results in Tromp's water bath tests, and decreases in intensity of itching and erythema of P. K. reaction and of those caused by intracutaneous histamine injection. Animal experiments investigating damage to the liver, kidney and heart with Solganal showed no serious findings.

Two young black male patients with seronegative rheumatoid arthritis and treated with nonsteroidal antiinflammatory agents developed fulminant hepatic necrosis following the institution of parenteral gold therapy. These cases, reported from different institutions, may represent a severe form of idiosyncratic gold hepatonecrosis. Awareness of the possible association between gold therapy and severe hepatic injury may become increasingly important as oral gold preparations become widely available.

The molecular mechanisms of action of gold complexes in the treatment of rheumatoid arthritis are partially known, as are the mechanisms of action and potential utility of gold complexes in the treatment of neoplastic disease. In this paper, data relative to the mechanism of cytotoxicity and structure activity relationships are presented. Concepts of future research are also discussed.

BACKGROUND: Sodium aurothiomalate has been reported to have in vitro activity against Helicobacter pylori. Intramuscular gold, as given to patients with rheumatoid arthritis (RA), may therefore influence the colonisation of the gastric mucosa with H pylori.
METHODS: Two groups were compared. One group of 42 patients was treated with intramuscular gold; the other group of 58 patients was treated with antimalarial drugs. Auranofin is the first oral gold therapy known to be effective at relatively low toxicities. Extensive clinical trials in rheumatoid arthritis have recently been complemented by first experiences in juvenile chronic polyarthritis. Twelve patients with adverse hematologic reactions to chrysotherapy received intravenous N-acetylcysteine (IV NAC) to achieve removal and/or redistribution of gold. Urine gold excretion while on IV NAC doubled that predicted from control measurements. Treatment within 20 days of the last gold injection was associated with better recovery of hematologic parameters. No untoward reactions from IV NAC were encountered. The pathogenesis of gold induced hematologic reactions is examined from both toxic as well as immunological perspectives.

Gold, silver and lead thioglucose were evaluated in rodents to determine the value of organo-metallic compounds in the study of metabolic abnormalities. Increased radioactivity concentration was noted in the hypothalamus with all compounds. Two hundred and eight patients entered a prospective, controlled, double-blind multicenter trial comparing auranofin, gold sodium thiomalate (GST), and placebo. One hundred and sixty-one patients completed at least 20 weeks of therapy. No remissions were seen in the trial.
