The thyroid and autoimmunity.

Autoimmune thyroid diseases (Hashimoto thyroiditis, Graves' disease, postpartum thyroiditis, atrophic thyroiditis and drug induced thyroiditis) are prevalent disorders worldwide, especially in women (related to the millieu of sex steroids and X chromosome effects on the thyroid and the immune system). Disruption of thyroid self tolerance, usually induced by an infection, generates abnormal thyroid--immune interactions, implicating an array of cytokines and their receptors. Thyrocytes achieve antigen presenting cell properties which stimulate effector immune cells (Th1, Th2, Th17), in the conte

Myocarditis: infection versus autoimmunity.

INTRODUCTION: Myocarditis, which is the inflammation of the heart muscle, remains a vexing therapeutic problem. Many cases are associated with viral infection, and appropriate treatment may depend upon whether the disease is primarily infectious, immune-mediated, or both. DISCUSSION: The combination of endomyocardial biopsies with newer molecular and immunologic tools holds a promise of distinguishing the different etiologies of myocarditis, thus, guiding future treatments. Nucleic acid hybridization and polymerase chain reaction have been applied to detect viral genome persisting in the hear

Selenium and thyroid autoimmunity.

The trace element selenium (Se) occurs in the form of the amino acid selenocysteine in selenoproteins. Selenoproteins exerts multiple physiological effects in human health, many of which are related with regulation of reduction-oxidation processes. In fact, the selenoenzyme families of glutathione peroxidase (GPx) and thioredoxin reductase (TRx) display the ability to act as antioxidants, protecting cells from oxidative damage. Furthermore, another class of selenoproteins are the iodothyronine deiodinase enzymes (DIO), which catalyze the conversion of thyroxine (T4) in triiodothyronine (T3), t

Neuronal acetylcholine receptor autoimmunity.

Nicotinic acetylcholine receptors (AChRs) are ligand-gated cation channels that are present throughout the nervous system. The muscle AChR mediates transmission at the neuromuscular junction; antibodies against the muscle AChR are the cause of myasthenia gravis. There are numerous subtypes of AChRs present throughout the nervous system. These neuronal AChRs are closely related to the muscle AChR structurally, but are pharmacologically and immunologically distinct. The ganglionic (alpha3-type) neuronal AChR mediates fast synaptic transmission in sympathetic, parasympathetic, and enteric autonom

Autoimmunity in kidney diseases.

Renal involvement in autoimmunity has many facets. Glomerular, tubular and vascular structures are targeted and damaged as a consequence of autoimmune processes. Most dramatic and life-threatening causes are observed with diseases that result in rapidly progressive glomerulonephritis (GN), frequently accompanied by involvement of additional non-renal organs. Typical diseases with these characteristics are anti-neutrophil cytoplasmic antibody (ANCA)-associated vasculitidis, anti-glomerular basement membrane (anti-GBM) GN and proliferative lupus nephritis. The leading cause of rapidly progressiv

Alopecia areata and autoimmunity.

AIM: The aim of the present study was to estimate the association between alopecia areata (AA) and autoimmunity. To such scope, the authors have estimated the presence of autoantibodies in patients with alopecia areata joints to the authors' observation in the last two years. METHODS: In 63 patients affected with different forms of AA (45 cases with multiple patches, 10 with single patch, 5 with alopecia totalis and 3 with alopecia universalis), has been estimated a autoantibodies panel. In 28 subjects (44%) it has been evidenced the presence of autoantibodies; in particular, 23 subjects were

Stress and thyroid autoimmunity.

While many studies have shown a connection between stress and autoimmune disease, most of the evidence for stress contributing to the onset and course of autoimmune disease is circumstantial and the mechanisms by which stress affects autoimmune disease are not fully understood. The best circumstantial evidence for an effect of stress on autoimmune thyroid disease is the well-known relationship between the onset of Graves' hyperthyroidism and major stress but even this is debated. However, most of the recent case-control studies have supported stress as a factor that affects the onset and clini

Immune deficiency and autoimmunity.

Immunodeficiency and autoimmune phenomena may occur concomitantly in the same individual. Many immune deficiency syndromes, mainly humoral defects, are associated with autoimmune disorders. Hematological manifestations, such as thrombocytopenia and hemolytic anemia, are the most common presentation, but many other autoimmune mediated conditions have also been described. Persistent antigen stimulation, due to an inherently defective immune system ability to eradicate pathogenesis is the primary cause leading to autoimmunity in patients with primary immunodeficiency states. Other factors leading

Autoimmunity in thyroid disease.

The autoimmune thyroid diseases, Graves' disease and autoimmune hypothyroidism, represent the two ends of a disease spectrum where an immune response is directed against the thyroid gland. In Graves' disease, antibodies directed against the thyrotropin receptor (TSH-R) lead to the development of glandular overactivity, while in autoimmune hypothyroidism, cell-mediated and humoral thyroid injury leads to destruction of thyroid tissue and thyroid hormone deficiency. The mechanisms by which these diseases develop are unknown, although it is likely that both diseases occur in genetically susceptib

Antiinfection immunity and autoimmunity.

This review summarizes findings that indicate that immune reactions of T and B cells depend on thresholds of the binding avidity of the receptor, on antigen amounts, on the time period during which antigen is available in secondary lymphatic tissues, as well as on the detection methods used. Usually immunologically ignored, strictly extralymphatic host cells or their antigens may be released by infection or immunopathology to reach lymphatic organs and induce autoimmune diseases. If we know the infection, we call the disease immunopathologically mediated; if we do not recognize or know it, we

Autoimmunity and pregnancy loss.

Clinicians have recognized for several decades that certain autoimmune conditions, such as systemic lupus erythematosus (SLE), are associated with pregnancy loss. During the 1980s, investigations focused attention on fetal wastage in women with antiphospholipid antibodies and the antiphospholipid antibody syndrome (APS) was characterized. Its defining features include fetal wastage in the presence of significant levels of anticardiolipin antibodies. Since that time, interest in other autoimmune diatheses and various specific autoantibodies as possible causes of pregnancy loss has increased. In

Liver transplantation and autoimmunity.

Autoimmune hepatitis (AIH), primary biliary cirrhosis (PBC) and primary sclerosing cholangitis (PSC) represent good indications for orthotopic liver transplantation (OLT). While there is effective treatment for AIH (steroids with or without azathioprine) and PBC (ursodeoxycholic acid) no such treatment is currently established for PSC. The need of transplantation can be delayed for AIH and PBC with appropriate therapies, while treatment options for PSC are still controversially discussed. Although the time point for liver transplantation can be roughly estimated for AIH by failure of immunosup

Bulimia nervosa and autoimmunity.

The autoantibodies that react with dopamine and serotonin are of interest in the study of bulimia nervosa. These neurotransmitters play an important role in appetite control, sexual and social behavior, and stress responses, all of which form a part of the clinical picture of bulimia nervosa. Are these autoantibodies involved in the serotoninergic hypofunctioning present in bulimia nervosa? Are they a part of an immunity regulation system essential for the cerebral system's homeostasis? To address these questions, 31 bulimic females (diagnosed according to DSM-III-R criteria) were compared wit

[Interferon therapy and autoimmunity].

The treatment of viral hepatitis or malignomas with interferon (IFN) can increase the incidence of autoimmune disease. This paper reviews published case and study reports. The incidence of overt autoimmune diseases under IFN treatment is about 3%. Autoantibodies can be detected in 23% of the patients. Autoimmune thyroid diseases are the most frequent ones, but nearly all autoimmune diseases can occur. Beside benign and reversible courses chronic developments and lethal outcomes are possible. Actual concepts concerning the pathogenesis of IFN-associated autoimmunity include induction of MHC and

Thyroid autoimmunity and ageing.

Ageing is associated with the appearance of several serum autoantibodies, including thyroid autoantibodies. The biological and clinical significance of this phenomenon is still unknown, since, with the exception of primary myxedema, the prevalence of clinically overt thyroid autoimmune diseases is not increased in the elderly. The peculiar link between autoimmune thyroid failure and ageing is also underscored by the high prevalence of subclinical hypothyroidism in elderly subjects with positive serum thyroid autoantibodies, and could be the consequence of preferential age-dependent expression

Does Yersinia induce autoimmunity?

Yersinia enterocolitica O:3 and the human thyrotropin receptor share a structural similarity, revealed by their serological cross-reactivity. The exact molecular basis of the similarity is open. In spite of this cross-reactivity, Yersinia seems not to be a major inducer of thyroid autoimmunity. Yersinia infections or avirulent Yersinia strains in the intestinal flora may rather contribute to the development of thyroid autoimmunity arising for other reasons in genetically susceptible individuals. Regarding Yersinia and spondyloarthropathies, it is undetermined whether the chronic sequelae of ac

[Celiac disease and autoimmunity].

Immune mechanisms have been invoked to explain coeliac disease and associations between this and other immunologically mediated diseases have been described. A direct relationship proposed is that coeliac disease may be associated with a range of autoimmune disease through the formation of immune complexes in the small intestine. It's more probable a common origin in an inherited predisposition to hypersensitive immuno responses to extrinsic antigens or auto-antigens. The association between coeliac disease and particular histocompatibility antigens was recognised.

Experimental gold-induced autoimmunity.

The pathogenesis of gold-induced autoimmunity and membranous glomerulopathy is not well understood. HgCl2 and D-penicillamine, other chemicals known to trigger membranous glomerulopathy in humans, induce autoimmune manifestations in Brown-Norway (BN) rats but not in Lewis (LEW) rats. These chemicals trigger T-cell clones which are specific for self class II molecules from the major histocompatibility complex and are probably responsible for the polyclonal B-cell activation observed. The aim of this work was to test the effects of aurothiopropanolsulphonate (ATPS) in BN and LEW rats. In BN rats

Autoimmunity causing thyroid dysfunction.

Considerable evidence for a genetically induced antigen-specific defect in suppressor T lymphocytes as the basis for AITD has been derived from several laboratories and via different types of experimental techniques. This defect may result from abnormal antigen presentation to T lymphocytes via an aberrant antigen-specific HLA-related gene. In addition, there is now evidence for additive effects on reducing generalized suppressor T lymphocyte numbers and function by environmental factors as well as hyperthyroidism itself. These effects would be superimposed on the organ-specific defect. Such e
