Azoospermia represents the most severe manifestation of male infertility and is classified into obstructive azoospermia (OA) and non-obstructive azoospermia (NOA). NOA patients experience a lack of sperm due to testicular dysfunction, posing significant challenges in clinical diagnosis and treatment. Recent advancements in molecular biology and high-throughput technologies have led to the discovery and validation of numerous biomarkers, including proteins, non-coding RNAs, genetic polymorphisms, and imaging indicators, which have greatly enhanced the understanding of the pathophysiological mec

This review focuses on the diagnostic algorithm for nonobstructive azoospermia (NOA), a significant male factor contributing to infertility. NOA, characterized by the absence of sperm in the ejaculate, requires a systematic diagnostic approach to identify reversible conditions, genetic factors, and prognosis for achieving pregnancy. The diagnostic pathway involves semen analysis and a comprehensive evaluation for hormonal deficiencies, anatomical abnormalities, and genetic factors. The importance of medical history, physical examination, endocrine evaluation, imaging, and genetic testing is em

Non-obstructive azoospermia (NOA) is the most common cause of male infertility, accounting for approximately 60 % of azoospermia cases. In recent years, gene mutations have emerged as the primary factor under investigation for the etiology of NOA. Therefore, finding the cause and pathogenesis of NOA at the genetic level has become one of the current research hotspots. Genetic analysis of NOA patients revealed that gene mutations primarily concentrate in protein-coding regions and non-coding RNAs, predominantly occurring in cases of non-obstructive azoospermia. Hence, understanding the relation

BACKGROUND: Azoospermia, characterized by the absence of spermatozoa in the ejaculate, affects approximately 1% of all men and 10 - 15% of infertile males, representing the most severe form of male infertility. It is classified into obstructive azoospermia (OA) and nonobstructive azoospermia (NOA), with the latter often resulting from unexplained failures in spermatogenesis. This study endeavored to clarify the molecular underpinnings of sper-matogenesis in NOA and to identify viable therapeutic targets.
METHODS: We analyzed expression data from NOA and normal spermatogenesis samples obtained

31-year-old male was referred to our hospital due to azoospermia. Physical examination revealed impalpable testes in the scrotum, and operative scar presented at lower abdominal midline. Magnetic resonance imaging (MRI) revealed that small testes were located subcutaneously in the lower abdominal midline. Since luteinizing hormone (LH) and follicle stimulating hormone (FSH) were elevated, we diagnosed non-obstructive azoospermia (NOA) due to abdominal migration of the testes. Microscopic testicular sperm extraction (micro-TESE) was performed, however, no sperm were recovered. Pathological diag

In the past two decades, with the rapid development of assisted reproductive technology and particularly the technological advances in male infertility microsurgery, many obstructive azoospermia-related infertile couples can now acquire the chances of natural pregnancy via reconstruction of the seminal tract. This article highlights the latest advances in surgical reconstruction of the seminal tract for the treatment of obstructive azoospermia, such as the application of laparoscopic and robotic techniques, with a discussion on microsurgical epididymal sperm aspiration and preservation, potent

OBJECTIVE: To determine the sensitivity and specificity of different ultrasound (US) modalities in predicting an obstructive vs a nonobstructive etiology of azoospermia.
MATERIALS AND METHODS: A total of 268 azoospermic men with available histopathologic slides were separated into obstructive (n = 104) and nonobstructive (n = 164) groups. Scrotal US studies, including color Doppler and transrectal US examinations, were performed in all patients and compared with the testicular biopsy results as the reference standard.
RESULTS: We found that a decreased testicular volume and intra- and extrat

OBJECTIVE: To investigate the diagnosis and treatment of epididymal obstructive azoospermia (OA) by microsurgery.
METHODS: We performed surgical scrotal exploration for 57 cases of OA whose obstruction was suspected to be in the epididymis. Those confirmed to be epididymal OA cases and with motile sperm in the epididymis underwent longitudinal-2-suture intussusceptive vasoepididymostomy (LIVES). And for those with sperm in the epididymal head only or with bilateral obstruction or absence of the vas deferens in the distal epididymis, the sperm were collected and subjected to cryopreservation f

The development of intracytoplasmic sperm injection (ICSI) opened a new era in the field of assisted reproduction and revolutionized the assisted reproductive technology protocols for couples with male factor infertility. Fertilisation and pregnancies can be achieved with spermatozoa recovered not only from the ejaculate but also from the seminiferous tubules. The most common methods for retrieving testicular sperm in non-obstructive azoospermia (NOA) are testicular sperm aspiration (TESA: needle/fine needle aspiration) and open testicular biopsy (testicular sperm extraction: TESE). The optima

In order to assess the frequency of chromosomal abnormalities in azoospermic males from western Mexico, we carried out a retrospective study in 227 patients. Forty-three (18.9%) cases with an abnormal karyotype were found. The most frequent chromosomal anomaly was 47,XXY, which was identified in 35 subjects (15.4%). In six cases (2.6%), structural aberrations were detected: two Robertsonian translocations [(45,XY,t(13;22)(p11;p11) and (45,XY,t(13;15)(p11;p11)], a Y;autosome translocation [46,XY,der(15)t(Y;15)(q12;p11)], and three mosaics [mos45,X/46,X,idic(Y)(q11)]. In general, these findings

PURPOSE: To evaluate prospectively the accuracy of scrotal ultrasonography (US) in distinguishing obstructive azoospermia from nonobstructive azoospermia in infertile men by using histologic findings as the reference standard.
MATERIALS AND METHODS: The institutional review board approved the study, and informed consent was obtained from each patient. Twenty infertile men (mean age, 34.7 years; 40 testes) with azoospermia were evaluated at scrotal US, with an emphasis on the course of the proximal genital duct: mediastinum testis, epididymal head, epididymal body, and epididymal tail. Cystic fibrosis (CF) is the most common autosomal recessive disorder in Caucasian population that involves the lungs, pancreas, sweat glands, intestine, liver and reproductive tract. The majority of men with CF are infertile due to a bilateral congenital absence of the vas deferens (CBAVD). The purpose of the research was to evaluate the occurrence frequency of alleles of CFTR gene in patients with azoospermia. Twenty four men from sterile marriages were examined in whom spermatozoa were not found in 2-3 successive examinations of their semen. In every patient the basic concentration in blood

Although minor complications have been reported, hydrocelectomy generally is considered to be a safe procedure. We recently treated 3 patients who became azoospermic after hydrocele repair. Of the patients 2 underwent bilateral and 1 unilateral hydrocelectomy. All 3 patients had iatrogenic injury to the vas deferens or epididymis after hydrocele repair.

A case of a typhoid fever patient with azoospermia is reported. Before typhoid fever, the semen analyses indicated an oligozoospermia (6.4 million/ml). Degree of fever up to 41.2 degrees C during 10 days was enough to suppress spermatogenesis, resulting in an azoospermic state. After the fever ceased, the spermatogenesis recovered and sperm production functioned normally after +/- 3 months. The situation corresponds with the time needed for spermatogenesis as previously reported.

It is estimated that approximately 25% of nonobstructive azoospermia (NOA) cases are caused by single genetic anomalies, including chromosomal aberrations and gene mutations. The identification of these mutations in NOA patients has always been a research hot spot in the area of human infertility. However, compared with more than 600 genes reported to be essential for fertility in mice, mutations in approximately 75 genes have been confirmed to be pathogenic in patients with male infertility, in which only 14 were identified from NOA patients. The small proportion suggested that there is much
