The association between a lifetime history of low back injury in a motor vehicle collision and future low back pain: a population-based cohort study.

PURPOSE: This population-based cohort study investigated the association between a lifetime history of a low back injury in a motor vehicle collision (MVC) and future troublesome low back pain. Participants with a history of a low back injury in a motor vehicle collision who had recovered (no or mild low back pain) were compared to those without a history of injury. Current evidence from two cross-sectional and one prospective study suggests that individuals with a history of a low back injury in a MVC are more likely to experience future LBP. There is a need to test this association prospecti

Lower extremity bilateral explosive strength asymmetry predicts non-contact low back injuries in mine rescue workers: a prospective cohort study.

OBJECTIVE: The purpose of this study was to investigate the relationship between bilateral lower extremity explosive strength asymmetry and non-contact low back injuries in mine rescue workers, a specialized occupational group. METHODS: A single-leg countermovement jump (SLCMJ) test was performed on 101 rescue members who had participated in frontline rescue missions, using a force platform. The collected force platform data were used to calculate lower limb asymmetry. Injury incidence was measured by recording all non-contact low back injuries occurring during routine training or rescue oper

Ankle dorsiflexion deficit in the back leg is a risk factor for shoulder and elbow injuries in young baseball players.

The relationship between ankle joint function and throwing-related injuries has not been demonstrated. We hypothesized that limited ankle joint range of motion (ROM) was related to risk factors for shoulder and elbow injuries in young baseball players. This 12-month prospective cohort study evaluated the age, height, weight, playing position, shoulder, elbow, and ankle function of 228 enrolled baseball players. Shoulder and elbow injuries were tracked during the season. Univariate and multivariate analyses were performed to identify risk factors for shoulder and elbow injuries among participan

Descriptive analysis of the effect of back protector on the prevention of vertebral and thoracolumbar injuries in serious motorcycle accident.

CONTEXT AND OBJECTIVE: Among the different products and protective gear used by riders of two-wheeled motorized vehicles, back protectors that are designed to prevent damage to the spinal column are widely used today compared other protections. However, few studies measure their effectiveness. Can their effectiveness be measured? How do they help decrease or change the nature of thoracolumbar traumas that occur? To address these questions and remedy the lack of objective data regarding these products, an epidemiological, clinical, and biomechanical analysis of motorcycle riders who were admitt

Confirmatory factor analysis of the Pain Catastrophizing Scale in African American and Caucasian Workers' Compensation claimants with low back injuries.

Pain catastrophizing is an important cognitive construct that has been linked with many aspects of the pain experience, including pain intensity, emotional distress, pain-related disability, and pain behavior. The Pain Catastrophizing Scale (PCS), an instrument often used to assess this construct, reflects three aspects of catastrophizing: Rumination, Magnification, and Helplessness. Its factor structure, however, has never been examined in clinical samples of persons with occupational injury or as a function of race. In this study, exploratory and confirmatory factor analyses were used to exa

The Long Road Back: An Integrative and Interdisciplinary Approach to Care After Traumatic Brain Injury.

Traumatic brain injury (TBI) is frequently associated with significant neuropsychiatric sequelae. Recovery trajectories are highly variable; injury severity is only one influence. Other factors, including preexisting psychiatric or psychological factors and psychosocial contexts also influence recovery. In this clinical challenge, we present the case of a young woman with a history of psychiatric comorbidity who developed complex neuropsychiatric symptoms after sustaining a TBI following a motor vehicle accident. Her clinical presentation included posttraumatic headache, chronic pain, visual d

Functional versus conventional strength and conditioning programs for back injury prevention in emergency responders.

Back pain and back-related injuries are common complaints among emergency responders. The purpose of this study was to compare the effectiveness of two strength and conditioning programs in improving back muscle characteristics and disabilities in emergency responders (firefighters/paramedics). Participants (n = 24) were randomized into two groups to complete 16 weeks of supervised exercise intervention: 1) Functional training used unilateral movements that mimicked the asymmetrical nature of emergency operations, 2) Conventional training performed bilaterally loaded exercises. Outcome measure

Three-dimensional spinal motion and risk of low back injury during sheep shearing.

Sheep shearers are known to work in sustained flexed postures and have a high prevalence of low back pain (LBP). As sustained posture and spinal movement asymmetry under substantial loads are known risk factors for back injury our aim was to describe the 3D spinal movement of shearers while working. We hypothesised that thoraco-lumbar and lumbo-sacral movement would be tri-axial, asymmetric, and task specific. Sufficient retro-reflective markers were placed on the trunk of 12 shearers to define thoraco-lumbar and lumbo-sacral 3D motion during three tasks. Thoraco-lumbar movement consistently i

Effect of instructions to simulate a back injury on torque reproducibility in an isometric lumbar extension task.

The purpose of this study was to investigate the differences in test-retest reliability between maximal and "simulated back injury" efforts in an isometric lumbar extension task and to test the hypothesis that voluntary attempts to "simulate" a back injury would yield less consistent torque production than maximal efforts. Twenty subjects were asked to undergo lumbar extensor testing at seven different positions in a lumbar extension machine. Each subject was tested twice in a maximal effort condition and twice with instructions to "simulate" a back injury. The order of the conditions was coun

Low-back pain in adolescent athletes: detection of stress injury to the pars interarticularis with SPECT.

The authors reviewed 1 year of experience with planar and single photon emission computed tomographic (SPECT) bone scintigraphy in 162 young patients with symptoms of low-back pain possibly related to stress injury to the pars interarticularis or spondylolysis. Planar scintigraphy and SPECT revealed no abnormality in 91 patients (56%). All abnormalities detected on planar images were also detectable with SPECT. SPECT showed an abnormal focus of radiotracer uptake in the lumbar spine in 71 patients (44%). In 32 of these 71 patients, these findings were also evident with planar scintigraphy. In

Under the Radar: Back Pain and Acute Kidney Injury as Harbingers of Type A Aortic Dissection.

Aortic dissection is a catastrophic vascular emergency with a high mortality rate if not diagnosed and managed in a timely manner. The classic presentation of thoracic aortic dissection includes the sudden onset of severe chest pain radiating to the back. Still, it may also present with atypical symptoms and signs of end-organ vascular compromise. We present a case of a 57-year-old female with a medical history of hypertension with poor medication adherence, major depressive disorder, chronic low back pain, and cocaine use disorder, who initially presented to the ED with worsening chronic low

Pain-related fear predicts reduced spinal motion following experimental back injury.

The current study examined the prospective relationship between pain-related fear and altered motor behavior, as well as perceived interference, among 51 healthy participants following induction of delayed-onset muscle soreness (DOMS) to the trunk extensor muscles. Healthy participants without history of back pain completed standardized reaches to high and low targets at self-paced and rapid speeds before and after induction of acute low back pain using a DOMS paradigm. Pain-related fear was assessed prior to DOMS induction. Three-dimensional joint motions of the thoracic spine, lumbar spine,

The role of back injury or trauma in lumbar disc degeneration: an exposure-discordant twin study.

STUDY DESIGN: Exposure discordant twin study. OBJECTIVE: To investigate the effect of injury on lumbar disc degeneration in monozygotic twins with discordant exposures to recalled previous injury/trauma to the lumbar spine. SUMMARY OF BACKGROUND DATA: Disc degeneration is considered a primary source of low back pain. Despite this, important determinants of disc degeneration other than genotype have not been identified. One possible important determinant of disc degeneration that has undergone limited investigation is previous back injury. METHODS: We compared disc degeneration between 37 ma

The value of SPECT in the detection of stress injury to the pars interarticularis in patients with low back pain.

The medical cost associated with back pain in the United States is considerable and growing. Although the differential diagnosis of back pain is broad, epidemiological studies suggest a correlation between adult and adolescent complaints. Injury of the pars interarticularis is one of the most common identifiable causes of ongoing low back pain in adolescent athletes. It constitutes a spectrum of disease ranging from bone stress to spondylolysis and spondylolisthesis. Bone stress may be the earliest sign of disease. Repetitive bone stress causes bone remodeling and may result in spondylolysis,
