[Bacterial skin and soft tissue infections].

Skin and soft tissue infections (SSTIs) represent a diverse spectrum of conditions, including erysipelas, cellulitis, cutaneous abscesses, necrotizing fasciitis, and myonecrosis. Erysipelas and cellulitis are the most common community-acquired SSTIs. Erysipelas is typically caused by pyogenic streptococci, while cellulitis often has a staphylococcal etiology. Dia-gnosis of erysipelas and cellulitis is primarily clinical and may be supported by the NEW HAvUN scoring system. Treat-ment is based on the use of narrow-spectrum antibiotics primarily targeting gram-positive bacteria. In the Czech Rep

[Atypical bacterial Infections of the Hand].

Atypical infections of the hand are rare and often misdiagnosed. Delay of treatment can lead to irreversible damage, even life-threatening sepsis. The article provides a survey of some of the most frequent atypical infections and their appearance on the hand (Mykobakteria, Vibriones, Francisella, Actinomycetoma), illustrated by two case reports. Knowing the pathogenesis of the most frequent atypical infections allows considering them in the daily practice for differential diagnosis, initiate specific testing, and administer an adequate therapy at an early stage.

[Superficial skin infections and bacterial dermohypodermitis].

Staphylococcus aureus and Streptococcus pyogenes are the two main bacteria involved in skin infections in children. Mild infections like limited impetigo and furonculosis should preferentially be treated by topical antibiotics (mupirocine or fucidic acid). Empiric antimicrobial therapy of dermohypodermitis consists in amoxicillin-clavulanate through oral route (80 mg/kg/d) or parenteral route (150 mg/kg amoxicillin per d. in 3-4 doses) for complicated features: risk factors of extension of the infection, sepsis or fast evolution. Clindamycin (40 mg/kg/d per d. in 3 doses) should be added to th

Respiratory bacterial infections in cystic fibrosis.

PURPOSE OF REVIEW: Bacterial respiratory infections are the main cause of morbidity and mortality in patients with cystic fibrosis (CF). Pseudomonas aeruginosa remains the main pathogen in adults, but other Gram-negative bacteria such as Achromobacter xylosoxidans and Stenotrophomonas maltophilia as well as nontuberculous mycobacteria have been shown to play an important role in the lung disease. The purpose of this review is to summarize the knowledge on disease and treatment of infection with CF-related pathogens. RECENT FINDINGS: The role of the paranasal sinuses as a source of infection a

Unusual bacterial infections and the pleura.

Rickettsiosis, Q fever, tularemia, and anthrax are all bacterial diseases that can affect the pleura. Rocky Mountain Spotted Fever (RMSF) and Mediterranean Spotted Fever (MSF) are caused by Rickettsia rickettsii and Rickettsia conorii, respectively. Pleural fluid from a patient with MSF had a neutrophil-predominant exudate. Coxiellaburnetii is the causative agent of Q fever. Of the two cases described in the literature, one was an exudate with a marked eosinophilia while the other case was a transudate due to a constrictive pericarditis. Francisella tularensis is the causative agent of tularem

Soil-related bacterial and fungal infections.

A variety of classic and emerging soil-related bacterial and fungal pathogens cause serious human disease that frequently presents in primary care settings. Typically, the growth of these microorganisms is favored by particular soil characteristics and may involve complex life cycles including amoebae or animal hosts. Specific evolved virulence factors or the ability to grow in diverse, sometimes harsh, microenvironments may promote pathogenesis. Infection may occur by direct inoculation or ingestion, ingestion of contaminated food, or inhalation. This narrative review describes the usual pres

[Bacterial infections of the rheumatoid foot].

BACKGROUND: A high incidence of infections has been reported in rheumatoid arthritis (RA) patients, either due to intrinsic factors or as a side effect of immunosuppressive agents used for treatment. The present article provides an overview of incidence and distribution patterns of septic complications in RA. MATERIALS AND METHODS: We prospectively assessed all data from RA patients who underwent in-patient treatment for septic complications in the 3-year period from 01.01.2006 to 31.12.2009. All disease- and infection-specific data were gathered and analysed. RESULTS: Of the 36 cases in tot

Good evidence exists to support the use of CRP measurements in conjunction with other established diagnostic tests (such as a white blood cell (WBC) count with differential and blood culture) to establish or exclude the diagnosis of sepsis in full-term or near-term infants. Sepsis was suspected within the first 3 days after birth in. There were 20% early-onset and 53% late-onset episodes of proven sepsis. CRP had sensitivities o

Bacterial infections: small intestine and colon.

Bacterial enteric infections exact a heavy toll on human populations, particularly among children. Despite the explosion of knowledge on the pathogenesis of enteric diseases experienced during the past decade, the number of diarrheal episodes and human deaths reported worldwide remains of apocalyptic dimensions. However, our better understanding of the pathogenic mechanisms involved in the onset of diarrhea is finally leading to preventive interventions, such as the development of enteric vaccines, that may have a significant impact on the magnitude of this human plague.

Microbiologically proven bacterial infections in AIDS.

We have reviewed the incidence, type and site of microbiologically proven bacterial infection occurring in 52 patients with the acquired immunodeficiency syndrome (AIDS) who presented to Southmead Hospital, Bristol between 1990 and 1994. A total of 30 (58%) patients had significant bacterial isolates. The majority of infections were community acquired. Overall, more infections were caused by Gram-negative organisms but Gram-positive organisms predominated in bacteraemia. Mycobacterium avium intracellulare (MAI) caused infection in the largest number of patients, followed by Staphylococcus aure

Serious bacterial infections after endoscopic procedures.

Transient bacteremia during and after endoscopic procedures is a well- documented phenomenon, but complicated bacteremia such as endocarditis in patients at risk is considered to be extremely rare. The recommendations for prophylaxis before endoscopy in patients with valvular heart disease were recently released. We discuss 16 cases of complicated bacteremia that developed after endoscopy (eight cases previously published in the literature and eight cases we encountered). The endoscopic procedures were gastroscopy (five cases), sclerotherapy (six cases), sigmoidoscopy (three cases), and esopha

Bacterial infections: bone and joint tuberculosis.

In many countries of the world, there is now a dual epidemic of tuberculosis and HIV disease. HIV specifically eliminates the tissue macrophages and CD4 lymphocytes, the very cells that provide immunity against tuberculosis. Tuberculosis is one of the more virulent opportunistic infections and it therefore appears fairly early in HIV disease. For the same reasons, bone and joint tuberculosis is becoming much more common. The disturbances of the lymphocyte count, ESR and antigen skin tests associated with HIV, now often make tuberculosis difficult to distinguish from other inflammatory lesions.

Bacterial infections: the arthritis of leprosy.

Arthritis is a common feature of leprosy and contributes to disability. Direct invasion of joints and bones by mycobacteria may lead to a destructive arthritis in lepromatous disease. The infective process may involve few or many joints. Reactional states may occur spontaneously but usually after the initiation of anti-mycobacterial treatment. In both the type 1 reaction of borderline case and the type 2 reaction of the lepromatous disease, intense inflammation may occur at sites of infection. The immunology of the reactions is different but they share clinical features including a polyarthrit

Immunotherapy in gram-negative bacterial infections.

Endotoxins are responsible for initiation of septic shock which increases the number of fatalities in Gram-negative bacteremia among hospital patients. The mortality from septic shock is still high despite recent developments in antibiotic therapy because antibiotics are unable to decrease the level of free lipopolysaccharide in the blood stream. Another approach to the treatment and prevention of septicaemia involves stimulation of an immune response against LPS. It was found that immunization with the core structures of endotoxin conjugated with proteins protected animals against infections

Bacterial infections in HIV-infected patients.

Although the original opportunistic pathogens described in AIDS were protozoal and fungal organisms, bacterial infections are now recognized with increased prevalence and altered expression in patients with HIV infection. Especially since populations outside of North America and populations of i.v. drug abusers have been studied, bacterial infections have been shown to cause substantially increased morbidity and mortality both early and late in the course of HIV infection. Just as strategies have been developed for primary and secondary prophylaxis of classical HIV-related opportunistic infect

Bacterial enteric infections and vaccine development.

There is a great need for effective vaccines against the major bacterial enteropathogens. Bacterial enteric infections resulting in diarrhea, dysentery, or enteric fever constitute a huge public health problem, with more than a billion episodes of disease and several million deaths annually in developing countries. Diarrhea caused by a bacterial enteric infection is also the commonest illness experienced by international travellers. Studies of pathogenesis have established the importance of specific ligand-receptor interactions between the enteropathogens and the intestinal epithelium, resulti
