Predictors of future overdose among people who inject drugs in Baltimore, Maryland.

BACKGROUND: Longitudinal studies of future overdose risk among people who inject drugs (PWID) are needed to inform planning of targeted overdose preventions in the United States. METHODS: The Integrating Services to Improve Treatment and Engagement (INSITE) study followed 720 PWID between June 2018 and August 2019 to evaluate the delivery of mobilized healthcare services in Baltimore, Maryland. The present analyses used logistic regression to identify baseline characteristics predictive of non-fatal or fatal overdose during the 6-month follow-up among 507 participants with overdose informatio

Co-occurring Threats to Agency Among Female Sex Workers in Baltimore, Maryland.

Driven largely by the unequal distribution of power, female sex workers (FSW) globally bear a disproportionately high burden of HIV, sexually transmitted infections, and interpersonal violence. Prior literature has identified a number of multi-level factors that may serve to constrain FSWs' agency, or their ability to define and take action to realize goals. Among these are work-based violence and substance use, which are potentiated by the criminalization of sex work and structural vulnerability. Quantitative research related to U.S.-based FSWs' own sense of agency, as well as the barriers th

Enduring mental health in the Baltimore epidemiologic catchment area follow-up study.

PURPOSE: To estimate the prevalence of enduring mental health (EMH) and examine important correlates of EMH 23 years later in the Baltimore Epidemiologic Catchment Area Follow-Up study. METHODS: We estimated the prevalence of EMH among 964 adults with diagnostic data at all four study waves (1981-2004). Those with EMH were compared to those with any mental or behavioral disorder by demographic, psychosocial, and health characteristics. We used forward selection models to identify the most important predictors of EMH. RESULTS: Twenty-six percent of participants met criteria for enduring menta

HIV seroconversion among Baltimore City residents tested at a mobile van programme.

BACKGROUND: Many individuals with HIV in the USA are unaware of their diagnosis, and therefore cannot be engaged in treatment services, have worse clinical outcomes and are more likely to transmit HIV to others. Mobile van testing may increase HIV testing and diagnosis. Our objective was to characterise risk factors for HIV seroconversion among individuals using mobile van testing. METHODS: A case cohort study (n=543) was conducted within an HIV surveillance dataset of mobile van testing users with at least two HIV tests between September 2004 and August 2009 in Baltimore, Maryland. A subcoho

Review of accidents/injuries among emergency medical services workers in Baltimore, Maryland.

OBJECTIVES: To characterize the types of occupational exposures and injuries reported by emergency medical service (EMS) workers. METHODS: A blinded review of accidents/exposures among EMS workers employed by a Baltimore County fire department was conducted. Medical records for 1992 were reviewed. RESULTS: Two hundred and twenty-six reports were filed by EMS workers (n = 197) employed by a large, urban fire department in 1992. The most commonly reported injuries were sprains (23%), strains (20%), and exposure to blood and body fluids (15%). The body site most commonly injured was the back (2

HIV Screening Among Gonorrhea-Diagnosed Individuals; Baltimore, Maryland; April 2015 to April 2019.

BACKGROUND: Individuals diagnosed with gonorrhea are at elevated risk for HIV. Per US Centers for Disease Control and Prevention guideline, individuals being evaluated for gonorrhea should be screened for HIV concurrently. There is limited information on HIV screening among gonorrhea-diagnosed individuals across different health care settings. Our objective was to identify potential gaps in HIV screening among gonorrhea-diagnosed individuals in Baltimore City, Maryland. METHODS: We used Sexually Transmitted Disease Surveillance Network project data collected on a random sample of all gonorrhe

Exploring spatially varying demographic associations with gonorrhea incidence in Baltimore, Maryland, 2002-2005.

The ability to establish spatial links between gonorrhea risk and demographic features is an important step in disease awareness and more effective prevention techniques. Past spatial analyses focused on local variations in risk, but not on spatial variations in associations with demographics. We collected data from the Baltimore City Health Department from 2002 to 2005 and evaluated demographic features known to be associated with gonorrhea risk in Baltimore, by allowing spatial variation in associations using Poisson geographically weighted regression (PGWR). The PGWR maps revealed variation

Correlates of hepatitis C viral clustering among people who inject drugs in Baltimore.

This study examines correlates of hepatitis C virus (HCV) genetic clustering among community-recruited people who inject drugs enrolled in the AIDS Linked to the IntraVenous Experience cohort in Baltimore between 1988 and 1989. HCV RNA was extracted and the core/envelope-1 region was sequenced. Clusters were identified from maximum likelihood trees with 1000 bootstrap replicates using a 70% aLRT and a 4% genetic-distance threshold in Cluster Picker. Overall, 46% of participants were in a cluster, including 122 genotype-1a and 36 genotype-1b clusters with an average of 2-3 genetically linked HC

Structural vulnerabilities to HIV/STI risk among female exotic dancers in Baltimore, Maryland.

Female exotic dancers (FEDs) are an important, yet understudied group of women who may engage in drug- and sex-related HIV/STI risk behaviors through their work. The study objective was to identify co-occurring indicators of vulnerability (e.g., housing, income, incarceration) associated with HIV/STI risk behavior among FEDs in Baltimore, Maryland. Surveys administered during July 2008-February 2009 captured socio-demographic characteristics, drug use, and sexual practices among dancers (N = 101) aged ≥18 years. Multivariate logistic regression was used to assess the relationship between vulne

Antibodies to Bartonella species in inner-city intravenous drug users in Baltimore, Md.

BACKGROUND: Bartonella quintana has recently been associated with homeless alcoholic men. Both B quintana and Bartonella henselae have been shown to be opportunistic pathogens of people with acquired immunodeficiency syndrome. The reservoirs and modes of transmission of these infections are incompletely known. OBJECTIVES: To examine serum samples that were taken from inner-city intravenous (IV) drug users for antibodies to Bartonella organisms to determine whether there is an urban transmission cycle for Bartonella species and to examine the demographic and behavioral characteristics of IV dr

Epidemiologic observations on infection with Rochalimaea species among cats living in Baltimore, Md.

Cats from several sources in Baltimore, Md, were tested for seropositivity to Rochalimaea henselae and R quintana. Co-infection with Toxoplasma gondii or feline immunodeficiency virus was assessed as a risk factor for infection with Rochalimaea spp. Of 592 cats tested, 87 (14.7%) were seropositive for one or both Rochalimaea spp, although titers to R henselae were significantly higher than those to R quintana. Prevalence of seropositivity increased significantly with cat age and weight and was associated with seropositivity to T gondii but was not associated with gender. Prevalence of seroposi

The Baltimore pregnancy prevention program for urban teenagers. II. What did it cost?

An experimental pregnancy prevention program for junior and senior high school students consisted of classroom lectures, informal discussion groups and individual counseling in the schools and group education, individual counseling and reproductive health care in a nearby clinic. The structure of the program allowed for extensive individual counseling for students who desired it, and the reported costs are thus considered generous. School-based services utilized 40 percent of a total three-year budget of $409,250, and clinic services, 60 percent. The average cost per student served was $122, w

Black Americans' Diminished Return of Educational Attainment on Tobacco Use in Baltimore City.

BACKGROUND: Socioeconomic status (SES) indicators such as educational attainment are fundamental factors affecting health. One mechanism through which education affects health is by reducing the likelihood of engaging in high-risk behaviors such as smoking. However, according to the marginalization-related diminished returns (MDRs) theory, the association between education and health may be weaker for marginalized populations such as Black than White, primarily due to racism and discrimination. However, little is known about the racial variations in the differential associations between educat

Racial Disparities in Drug Arrest Before and After De Facto Decriminalization in Baltimore.

INTRODUCTION: To mitigate the harms of arrest and incarceration on health and racial equity, jurisdictions are increasingly enacting reforms to decriminalize drug possession through prosecutorial discretion (de facto). Impacts on health outcomes rely on whether this policy can reduce exposure to the carceral system among people who use drugs; however, data evaluating effects on arrest are lacking. This study explores the possible impacts of Baltimore City's enactment of de facto decriminalization on arrests by race. METHODS: Police and court records were used to explore the possible impacts o
