IgE Nonresponders in Basophil Activation Tests: A Narrative Review From Mechanistic Insights to Clinical Implications and Future Directions.

It has been known for about 50 years that a proportion of human basophils does not release histamine or leukotrienes or upregulates activation markers in response to IgE-dependent stimuli. This group of so-called nonresponders with unresponsive basophils is found in up to 20% of tests conducted. There seems to be no relation to clinical symptoms. The nonresponder status is not defined in a standardized manner, but basophils are often considered nonresponsive if they release less than 5% to 10% of their histamine content or show activation of less than or equal to 10% CD63+ basophils after FcεR

Allergy to PEG (polyethylene glycol) - sensitivity of basophil activation test with COVID-19 mRNA-vaccine BNT162B2.

Basophil activation test (BAT) with COVID-19 mRNA vaccine seems particularly suitable for detecting sensitization to polyethylene glycol (PEG) in patients with PEG allergy. It was the aim of this study to determine the cutoffs for BAT using BNT162B2 (Comirnaty®) in a larger group of PEG allergic patients and controls. 10 PEG allergic patients and 10 controls were studied. BAT was performed using anti-CCR3 for basophil identification and anti-CD63 to assess basophil activation. Incubations with BNT162B2 at four different concentrations were performed. Basophil activation was significantly highe

Critical role of P1-Runx1 in mouse basophil development.

Runx1(P1N/P1N) mice are deficient in the transcription factor distal promoter-derived Runt-related transcription factor 1 (P1-Runx1) and have a > 90% reduction in the numbers of basophils in the BM, spleen, and blood. In contrast, Runx1(P1N/P1N) mice have normal numbers of the other granulocytes (neutrophils and eosinophils). Although basophils and mast cells share some common features, Runx1(P1N/P1N) mice have normal numbers of mast cells in multiple tissues. Runx1(P1N/P1N) mice fail to develop a basophil-dependent reaction, IgE-mediated chronic allergic inflammation of the skin, but respond

Advances in Food Allergy Diagnosis.

An accurate diagnosis of food allergy is extremely important to guide safe and yet not overly restrictive dietary management. The cornerstone of the diagnosis of food allergy is the clinical history; it allows appropriate selection of the allergens to be tested and interpretation of the results of allergy tests, namely Skin Prick Test (SPT), Specific IgE (sIgE) to allergen extracts and, more recently, specific IgE to allergen components and the Basophil Activation Test (BAT). SPT and sIgE to allergen extracts are very sensitive methods to detect IgE sensitization to a specific food and assess

Antibodies and superantibodies in patients with chronic rhinosinusitis with nasal polyps.

BACKGROUND: Chronic rhinosinusitis with nasal polyps is associated with local immunoglobulin hyperproduction and the presence of IgE antibodies against Staphylococcus aureus enterotoxins (SAEs). Aspirin-exacerbated respiratory disease is a severe form of chronic rhinosinusitis with nasal polyps in which nearly all patients express anti-SAEs. OBJECTIVES: We aimed to understand antibodies reactive to SAEs and determine whether they recognize SAEs through their complementarity-determining regions (CDRs) or framework regions. METHODS: Labeled staphylococcal enterotoxin (SE) A, SED, and SEE were

Basophil reactivity to BNT162b2 is mediated by PEGylated lipid nanoparticles in patients with PEG allergy.

BACKGROUND: The mechanisms underpinning allergic reactions to the BNT162b2 (Pfizer) COVID-19 vaccine remain unknown, with polyethylene glycol (PEG) contained in the lipid nanoparticle suspected as being the cause. OBJECTIVE: Our aim was to evaluate the performance of skin testing and basophil activation testing to PEG, polysorbate 80, and the BNT162b2 (Pfizer) and AZD1222 (AstraZeneca) COVID-19 vaccines in patients with a history of PEG allergy. METHODS: Three known individuals with PEG allergy and 3 healthy controls were recruited and evaluated for hypersensitivity to the BNT162b2 and AZD12

[Post-vaccination reactions and complications of pertussis, diphtheria and tetanus vaccines].

Clinical manifestation of postvaccinal reactions and complications and the presence of sensibilizating antibodies to certain components of the applied vaccines was analyzed in 80 children immunized by Di-Te-Per, Di-Te or Te vaccine. Most children showed a mild reaction; concerning statistical significance it was most frequently limited to the site of the vaccine application if the immunization was performed by Di-Te and Te vaccines, as compared to the immunization by Di-Te-Per vaccine. Indirect basophil degranulation test revealed that 50% of the analyzed reactions were of allergic origin. Sen

Improved Bin-Based Basophil Activation Test Facilitates Comparison of Wheat Allergy and Tolerance in Children and Adults.

Diagnosing wheat allergy remains challenging due to the use of oral wheat provocations, which implicate risks to patients and highlights the need for safer, non-invasive diagnostic methods. Here, we present the first direct comparison of a pediatric and adult cohort to study wheat allergy and wheat tolerance using an improved and validated basophil activation test (BAT). Blood samples from 24 children and 26 adults, clinically classified as facing oral food challenges, were analyzed using our bin-based BAT, enabling standardized data analysis and visualization. In children, the BAT showed sign

The role of basophil activation test and component-resolved diagnostics in the workup of egg allergy in children at low risk for severe allergic reactions: A real-life study.

BACKGROUND: The gold standard for diagnosing egg allergy in children is the oral food challenge (OFC). However, OFCs are time-consuming and risky procedures. Our study aimed to evaluate the utility of the basophil activation test (BAT) and component-resolved diagnostic in the diagnostic workup of children with egg allergy. METHODS: Overall, 86 children aged 6 months to 17 years, suspected of egg allergy, underwent OFC with boiled egg according to international standardized protocols. BAT and specific immunoglobulin E (sIgE) testing to component egg proteins (Gal d 1-4) were also performed. R

Venom immunotherapy: clinical efficacy, safety and contraindications.

Venom-specific immunotherapy (VIT) is considered for the treatment of patients with IgE-mediated systemic allergic reactions (SARs) after developing a Hymenoptera venom allergy. Tolerance is achieved in a majority of patients after only a few days or even hours of rush immunotherapy. After VIT discontinuation, the allergy returns in up to 15% of patients. During VIT, the majority of patients have local reactions at the site of venom injections. SARs to VIT are much more frequent in honeybee-treated patients than in wasp-treated patients. Increased baseline serum tryptase and increased allergen

Relevance of the basophil activation test in a cohort of 240 patients with chronic spontaneous urticaria.

BACKGROUND: The basophil activation test (BAT) is considered to be the best biomarker to predict autoimmune chronic spontaneous urticaria (aiCSU). To date, few studies have investigated the utility of BAT in real-world clinical practice, the role of aiCSU biomarkers in relation to omalizumab therapy and the degree of association between different aiCSU tests. OBJECTIVES: To analyse the clinical and laboratory features of a prospective cohort with chronic spontaneous urticaria (CSU) according to their BAT status, as well as to study omalizumab efficacy according to aiCSU biomarkers. METHODS:

Cutaneous basophil hypersensitivity. II. A light and electron microscopic description.

Delayed onset erythematous skin reactions elicited in guinea pigs early in the course of sensitization with azobenzenearsonate-protein conjugates or with protein antigens in incomplete Freund's adjuvant or in saline were found to have a characteristic morphology which sets them apart from delayed hypersensitivity and the classic antibody mediated reactions. The principle feature was massive dermal infiltration with basophilic leukocytes. Mononuclear cells of several types including activated and small lymphocytes, monocytes, macrophages, and blast cells were also present. Such reactions have i

High albumin sensitization drives uninterpretable basophil activation tests in cow's milk allergy.

OBJECTIVES: The basophil activation test (BAT) is increasingly used for diagnosing IgE-mediated food allergies, yet its performance in cow's milk allergy (CMA) remain insufficiently characterized in real-world pediatric practice. This study evaluates the diagnostic accuracy of BAT for CMA and identifies factors associated with non-interpretable results. METHODS: We carried out a single-center retrospective observational study in 59 children with a documented history of cow's milk reactions. Clinical status at the time of BAT was determined using oral food challenges (OFCs), skin prick tests,
