Tin ear syndrome: rotational acceleration in pediatric head injuries.

An array of signs pathognomonic of child abuse is described for three young children. The eponym "tin ear syndrome" describes distinct unilateral ear bruising, radiographic evidence of ipisilateral cerebral edema with obliteration of the basilar cisterns, and hemorrhagic retinopathy. Rotational acceleration produced by blunt trauma to the ear is shown to be a necessary and sufficient cause for the production of this syndrome. An approximate value of 4,670 radians/s2 produced by a force of 309 to 347 N (70 to 80 lb) results in shear stress and tearing of the subdural veins, loss of cortical vas

[Clinical forensic examination findings and legal outcome in cases of suspected physical child abuse].

Clinical forensic examinations performed at the Institute of Legal Medicine of the Hanover Medical School between 1999 and 2008 in cases of suspected physical abuse of children were analyzed retrospectively with special emphasis on the legal consequences. Altogether, 192 children (85 girls, 107 boys) with a median age of 4.4 years were examined. In 47 cases (24.5 %), the clinical forensic examination findings were interpreted as accidental injuries, birth traumas or unspecific findings. 29 victims (20.0 %) had suffered a shaken baby syndrome. Only part of the presented cases ended with convict

Metaphyseal fractures in osteogenesis imperfecta.

Forty-one children with osteogenesis imperfecta have been reviewed. A minority (7/41) showed small metaphyseal fractures, resembling those seen in non-accidental injury, but in all of these there was obvious generalized bone disease so that confusion with non-accidental injury did not occur.

Patterns of reported child abuse and neglect.

Confirmed reports of abuse and neglect logged in a large state registry file from 1977 to 1984 are analyzed. Boys tend to sustain more frequent and more serious injuries. Victims of male perpetrators tend to sustain more serious injuries. A modest same-sex perpetrator/victim pattern is revealed for physical abuse; males are more likely to physically abuse boys while females are more likely to physically abuse girls. Among younger victims (aged 12 and younger) boys outnumber girls in all reporting categories except sexual abuse. Among adolescent victims, female victims greatly outnumber male vi

Suicide attempts and overdoses among adults entering addictions treatment: comparing correlates in a U.S. National Study.

BACKGROUND: Suicide attempts and non-fatal overdoses are both associated with substance use. The aim of the present study was to examine correlates of suicide attempts and non-fatal overdoses simultaneously among individuals seeking addictions treatment. METHODS: A large U.S. national sample of individuals entering addictions treatment participated in a cross-sectional survey (n=5892). Multinomial logistic regression modeling tested the adjusted associations of violence, injection drug use, specific substances, and depressive symptoms with a four-category outcome variable based on prior histo

Prevalence of childhood physical abuse in adult male veteran alcoholics.

Although past research has identified psychological and behavioral consequences for adults who were abused as children, few studies have examined the incidence and consequences of childhood physical abuse among adult alcoholics. The purpose of this study was to assess the prevalence of a childhood history of physical abuse in adult male alcoholics and to determine what differences may exist in the psychological profile and patterns of alcohol abuse in abused and nonabused alcoholics. The study sample was comprised of 100 male alcoholic inpatients from the alcoholism treatment unit at a metropo

Child abuse in Nigeria.

Although child abuse occurs in Nigeria, it has received little attention. This is probably due to the emphasis placed on the more prevalent childhood problems of malnutrition and infection. Another possible reason is the general assumption that in every African society the extended family system always provides love, care and protection to all children. Yet there are traditional child rearing practices which adversely affect some children, such as purposeful neglect or abandonment of severely handicapped children, and twins or triplets in some rural areas. With the alteration of society by rap

Epidemiologic differences between sexual and physical child abuse.

Sexual and physical child abuse are assumed to differ; however, these differences have not been well characterized epidemiologically. Furthermore, despite assumed differences, these types of abuse are often analyzed as one entity. This can have significant effects on assessment of risk and recommendations for intervention. We compared 735 cases of sexual abuse and 3,486 cases of nonsexual physical abuse confirmed by the Georgia Department of Protective Services. Sexual and physical child abuse cases differed in age, sex, and relationship of perpetrators and victims; demographic and socioeconom

Microcephaly following baby battering and shaking.

Three cases of microcephaly following the rough handling of babies are described. If detailed social and psychiatric information had not been available, these three children would have become part of the large proportion of individuals in subnormality hospitals who have unsatisfactory and incomplete diagnoses.

Retinal detachment in a battered infant.

A 12-week-old battered infant boy had bilateral retinal detachments. The retina in the left eye was considered inoperable, but retinal detachment surgery was performed in the right eye. This retina was reattached and limited vision was restored. The history, the location of the retinal pathology, and the absence of a lens coloboma helped differentiate this result of physical abuse from a congenital retinal detachment.

The shaken baby syndrome. A clinical, pathological, and biomechanical study.

Because a history of shaking is often lacking in the so-called "shaken baby syndrome," diagnosis is usually based on a constellation of clinical and radiographic findings. Forty-eight cases of infants and young children with this diagnosis seen between 1978 and 1985 at the Children's Hospital of Philadelphia were reviewed. All patients had a presenting history thought to be suspicious for child abuse, and either retinal hemorrhages with subdural or subarachnoid hemorrhages or a computerized tomography scan showing subdural or subarachnoid hemorrhages with interhemispheric blood. The physical e

[Diagnostic imaging in cases of suspected child abuse--focus craniocerebral trauma divergent guidelines and resulting problems].

For the clinical diagnosis of non-accidental craniocerebral trauma in children there are different recommendations and guidelines, which are not completely consistent. Depending on the examiner, the focus may be on therapeutic relevance, minimization of the exposure to radiation or potential conclusions as to the course of events. Under certain circumstances it may be difficult for the responsible institution to deal with all three perspectives, as is shown by the presented case. From the authors' point of view it would be desirable to work out a common guideline taking into account paediatric

Erythema nodosum in children: a study of 27 patients.

Erythema nodosum (EN) seems to occur in children more rarely than in adults. It still remains the most frequent acute panniculitis, for which the diagnosis is almost always clinical. In a retrospective study of 27 pediatric patients, we have attempted to clarify the clinical spectrum and prognosis of this disease and discuss the differential diagnosis of nodular eruptions on the lower limbs of children. In almost half the patients of our series, the cause of EN remained undetermined. Streptococcal infections (usually of the pharynx) were the most common cause of EN in children (22% of patients

Blunt abdominal trauma in childhood.

Blunt abdominal trauma in childhood is a common clinical occurrence. Accurate assessment requires an understanding of patterns of injury and the response of the child. Repetitive examinations in a comforting environment will yield superior clinical judgment. Renal trauma is frequent, requires thorough radiologic evaluation, and may be associated with an underlying anomaly. The goal of management is salvage of functioning nephrons. Definitive indications should exist prior to operative intervention. The goal of management of splenic injury should not be the justification of splenectomy. The ope

Circumferential growth plate fracture of the thoracolumbar spine from child abuse.

We report a unique fracture dislocation of the immature spine associated with child abuse and paraplegia. Because it occurs through growth centers, this fracture may be initially difficult to identify without special studies. The fracture patterns were isolated injuries in both children and were best shown with computer tomography. Mechanisms of injury are proposed and should suggest child abuse as a cause. Experience with these initial cases has suggested a treatment approach based on the mechanism of injury. There is no indirect means of reduction for this fracture and anterior open reductio

The pathology of fatal child abuse.

It is a sad indictment of human society that the abuse of children is such a prevalent and widespread problem. The acknowledgement that physical, emotional and sexual injury as well as intentional neglect can be inflicted upon the young by any person but especially by caregivers has been increasingly realised by the community. As a result, many professionals, especially in the medical sciences, are involved in the study and management of such cases with the ultimate goals of recognising children at risk, diagnosing those cases that have occurred, preventing initial or subsequent injury and bri

[Endocrine behavior of battered children in the acute stage of the aggression].

To date the cause of growth retardation of children who have suffered physical abuse and emotional deprivation is unknown. Hypophyseal disturbances in these patients have been proposed of the cause but there are still several concerns on the dynamics of growth hormone secretion in these children. In this study, eleven out of sixteen patients had a low height without important diminution of corporal weight. Growth hormone under basal conditions was found to be elevated in battered children compared with a control group (15.2 +/- 4.7 ng/mL vs 9.6 +/- 1.9 ng/mL, p = 0.025). Two weeks after hospit

Medical ethics and child abuse.

The practice of good medical ethics is of particular importance in cases of juvenile maltreatment. In a number of court cases concerning battered children doctors' statements have been presented to the effect that maltreatment has not taken place. In twelve cases reported during the period 1971-75 the doctors' statement was misleading. In each of twelve cases it should have been the doctor's duty to record in his statement that this was a case of child abuse and/or neglect. It appears that as far as the individual doctor was concerned, he often state "these things should be dealt with by the c

Ocular and cerebral trauma in non-accidental injury in infancy: underlying mechanisms and implications for paediatric practice.

AIMS: To determine the sites, mechanisms, and clinical significance of injuries to the eyes and brains of children with non-accidental injuries in relation to differing levels of trauma. METHODS: A forensic pathological study of injuries in the eyes and brains of 23 consecutive children dying of non-accidental injuries over a 4 year period (1988-92) under the jurisdiction of Yorkshire and Humberside coroners. RESULTS: Sixteen children died from cerebral injuries and seven died from non-cerebral causes. There were high incidences of retinal detachment (63%) and subhyaloid (75%), intraretinal
