Intracholecystic papillary neoplasm (ICPN) of the gallbladder is a macroscopically visible premalignant lesion protruding into the gallbladder lumen, with infrequent association with invasive adenocarcinoma. Intraductal papillary neoplasm of the bile ducts (IPNB) is a non-invasive lesion characterized by intraductal papillary or villous architecture. Both ICPN and IPNB are rare findings in the gallbladder and biliary tract pathology. Diagnosis relies on clinical manifestations, imaging techniques, and comprehensive histological examination. Here, we present two cases: a 63-year-old male with m

INTRODUCTION AND IMPORTANCE: Intraductal papillary neoplasm of the bile duct (IPNB) is a rare neoplasm, mostly found in patients from far Eastern areas where hepatolithiasis and clonorchiasis are endemic. Very few cases are reported from India.
CASE PRESENTATION: We present a case of recurrent cholangitis in a 59-year-old male, initially suspected to have IPNB based on preoperative imaging. Confirmation occurred intraoperatively, with mucin-filled bile ducts and papillary lesions in the resected hepatic duct. Treatment included left hepatectomy, extrahepatic bile duct excision, and Roux-en-Y

BACKGROUND: Intraductal papillary neoplasm of the bile duct (IPNB) is a recently defined entity and its clinical characteristics and classifications have yet to be established. We aimed to clarify the clinical features of IPNB and determine the optimal morphological classification criteria.
METHODS: From 2003 to 2016, 112 patients with IPNB who underwent surgery were included in the analysis. After pathologic reexamination by a specialized biliary-pancreas pathologist, previously suggested morphological and anatomical classifications were compared using the clinicopathologic characteristics o

Current WHO terminology and recent publications have classified tumoral (grossly visible) intraductal pre-invasive neoplasms of bile duct (TIDN) into three categories: intraductal papillary neoplasm of bile duct (IPNB), intraductal papillary oncocytic neoplasm (IOPN), and intraductal tubulopapillary neoplasm (ITPN). A total of 227 cases of TIDN and related lesions ≥3 mm in height were examined by 10 biliary pathologists referring to these 3 categories and two pathologic gradings: two-tiered system (low- and high-grade dysplasia) and modified types 1 and 2 subclassification. Among them, IPNB wa

The College of American Pathologists recognizes "intraductal papillary neoplasm of the bile ducts (IPNB) with an associated invasive carcinoma" in its protocols for the pancreas and intrahepatic bile ducts, but not for the perihilar or distal bile ducts. We aimed to investigate the clinicopathologic and molecular features of 27 surgically resected perihilar IPNB cases. The cohort included 14 males and 13 females, with a median age of 70 years. Six IPNBs were noninvasive, while 21 had associated invasive carcinoma. The median overall tumor size was 2.8 cm, and the median invasive tumor size was

BACKGROUND: The precise clinical features and prognosis of IPNB remain challenging to define. More research is needed to enhance understanding of the disease.
METHODS: The study retrospectively analyzes the clinical data of 123 patients with IPNB from two medical centers during January 2013 and May 2024. The clinicopathological characteristics of IPNB were compared across subgroups. Risk factors for invasive IPNB and survival predictors of IPNB were identified by regression analysis. The prognosis of patients with IPNB was evaluated through survival and recurrence analysis.

A 74-year-old female was admitted for painless jaundice. Laboratory tests showed hyperbilirubinemia, cholestasis, normal coagulation, and Ca19-9:163U/L. The CT-scan reported dilation of the intrahepatic and extrahepatic bile ducts secondary to a 24mm tumor in the intrapancreatic common bile duct. The magnetic cholangioresonance showed multiple endoluminal polypoid lesions, suggestive of intraductal papillary neoplasm of the bile duct (IPNB). The endoscopic bile duct brushing was non-conclusive.

BACKGROUND: Intraductal papillary neoplasm of the bile duct (IPNB) is a premalignant neoplasm that can involve both the intrahepatic and extrahepatic bile ducts. Owing to the low incidence and confusing nomenclature, its clinicopathological features remain controversial. Additionally, only a few studies have reported on the long-term prognosis of IPNB to date. Therefore, the present study aimed to clarify the clinicopathological characteristics and prognosis of IPNB.
METHODS: Medical records of patients with IPNB treated at our hospital between August 2000 and October 2021 were retrospectivel

Intraductal papillary neoplasm of the bile duct (IPNB) is a rare bile duct tumor characterized by intraductal papillary or villous neoplasms covered by neoplastic epithelium with fine fibrovascular stalks in the dilated bile ducts (1). Its true etiology remains unknown. Herein, we report two cases of IPNB that underwent surgical resection. The first case was a 66-year-old male who complained of upper abdominal pain for three years. We found obstruction of the common bile duct and dilation of the intrahepatic and extrahepatic bile ducts after MRCP. Laparoscopic hepatic segmentectomy (S2, S3, S4

A 72-year-old man with obstructive jaundice, diagnosed with distal biliary carcinoma, underwent pylorus-preserving pancreaticoduodenectomy. The patient was histopathologically and immunohistochemically diagnosed with mixed neuroendocrine non-neuroendocrine neoplasm. Pathological examination revealed that the well-differentiated adenocarcinoma components occupied the superficial portion of the bile duct and the neuroendocrine carcinoma components were located in the deeper portion of the bile duct. Pathological examination showed that a portion of the biliary intraepithelial neoplasia arose fro

OBJECTIVE: To establish a classification system which differentiates cystic intraductal papillary neoplasm of the bile duct (cystic IPNB) from hepatic mucinous cystic tumors (MCN) based on their radiological difference.
METHODS: A total of 75 patients pathologically diagnosed as MCN and IPNB in two major hospitals from 2015 to 2024 were enrolled. Radiological features were recorded and compared between these two tumors. Variables with significant differences were included in multivariate logistic regression (LR) analysis. A decision model was built and simplified based on importance ranking o

Intraductal papillary mucinous neoplasm of the bile duct (IPMN-B) is a rare biliary tumor with marked heterogeneity. We report two cases of unresectable IPMN-B, both treated with gemcitabine plus oxaliplatin (Gemox)-based hepatic arterial infusion chemotherapy (HAIC) combined with tislelizumab and surufatinib. According to Response Evaluation Criteria in Solid Tumors version 1.1 (RECIST 1.1), calculated using the longest diameter of the target lesion, case 1 achieved radiographic stable disease (SD) and improvement in laboratory indices after two cycles of HAIC combined with targeted immunothe
