Gamma 3 U-Blade lag screws in patients with trochanteric femur fractures: are rotation control lag screws better than others?

BACKGROUND: Intramedullary hip nails may be classified as blades or screws depending on the type of lag screw used. Recently, a combination of lag screw types with a U-clip insertion has also been used. The purpose of this study was to evaluate the clinical and radiological outcomes of these new screw types. METHODS: A total of 185 patients with trochanteric femoral fractures (age ≥ 65 years) who underwent surgery with intramedullary nails were selected. Surgeries with InterTrochanteric/SubTrochanteric (ITST), Proximal Femoral Nail Antirotation (PFNA), and Gamma 3 U-Blade lag screws were perf

Use of an Angled Blade Plate for 31A3 Intertrochanteric Fractures.

BACKGROUND: A subgroup of pertrochanteric fractures-namely, the AO/OTA 31A3 fracture-continues to be a difficult problem to treat, even with cephalomedullary nails. We present the results for 26 patients with a 31A3 fracture treated with the angled blade plate. METHODS: The records of 26 consecutive patients with a 31A3 fracture that was treated operatively with the angled blade plate device between 2007 and 2012 at our center were reviewed, and the patients were contacted for follow-up. The functional outcome (traumatic hip rating score) and radiographic outcome (the neck-shaft angle at the

Subtrochanteric fixation stability depends on discrete fracture surface points.

Fixation of subtrochanteric femur fractures may present complications, including malunion, delayed union, or nonunion, and is thought to be related to early fracture stability. To examine the initial stability of subtrochanteric fracture fixation, we investigated construct stiffness, interfragmentary gaps, and overall and point-wise interfragmentary motion (ie, axial and shear displacements) in synthetic composite femurs fixed with a cephalomedullary nail or condylar blade plate. Simulated stable and unstable subtrochanteric femur fractures were created in composite femurs, anatomically reduce

Scalp-Split Technique: A Concept of Bone Splitting and Expansion for Thin Alveolar Ridges to Place Dental Implants.

Bone expansion is when the existing bone at the implant site is split and separated to create a gap to widen it. This article introduces a novel concept of bone expansion for the ultrathin alveolar ridges to place dental implants with zero wastage of existing natural bone in long-term edentulous arches. Instead of using twist drills and piezo surgery kits, the bone is split with a surgical scalpel blade to prevent natural bone wastage. The split helps to expand the bone horizontally by using expanders to accommodate the smaller diameter implants. This technique was applied to 12 individuals to

Radiographic subsidence after single-level anterior cervical discectomy and fusion; a comparison between blade-anchored zero-profile cages and stand-alone cages.

BACKGROUND: Cage subsidence is a common radiographic finding after anterior cervical discectomy and fusion (ACDF), particularly with stand-alone cages, and may compromise postoperative alignment and stability. Blade-anchored zero-profile cages have been developed to provide immediate fixation while avoiding plate-related complications; however, their influence on subsidence characteristics has not been fully elucidated. This study aimed to compare the magnitude and direction of radiographic subsidence between blade-anchored zero-profile cages and stand-alone cages after single-level ACDF. MET

The new proximal femoral nail antirotation (PFNA) in daily practice: results of a multicentre clinical study.

The treatment of unstable trochanteric femoral fractures is still challenging. The ideal implant should be easy to handle, allow for immediate full weight-bearing postoperatively and should have sufficient purchase in the femoral head/neck-fragment to limit cut-outs due to varus-deviation and rotation. The proximal femoral nail antirotation (PFNA), designed by AO, is an intramedullary device with a helical blade rather than a screw for better purchase in the femoral head and was tested in a clinical study. Consecutive patients with unstable trochanteric fractures (AO-classification 31.A.2 and

Use of temporary external fixation to generate a "customized" osteotomy of proximal femur in pediatric patients.

Proximal femoral osteotomies are performed commonly by pediatric orthopedic surgeons. Traditionally, utilization of a blade plate has been the standard method of realignment and fixation of the osteotomized fragments. In conjunction with use of temporary Schanz screws to generate the most appropriate realignment, newer implants with locking fixation capabilities can be used to stabilize essentially "customized" osteotomies of the proximal femur.

GBR assisted in situ Onlay bone grafting for the posterior mandible horizontal ridge augmentation: a case report and literature review.

The posterior mandible is the primary area for occlusal function. However, long-term tooth loss in the posterior mandible often leads to rapid absorption of both buccal and lingual trabecular bone plates and subsequent atrophy of the alveolar ridge. This ultimately results in horizontal bone deficiencies that complicate achieving an optimal three-dimensional placement for dental implants. Conventional techniques employed clinically for horizontal bone augmentation have limited efficacy, cause significant surgical trauma, and require extended treatment duration. Consequently, the selection of a

Combined Mechanical and Biological Salvage of Recalcitrant Subtrochanteric Non-union using Valgus Osteotomy, Blade Plate Fixation, and Endomedullary Fibular Graft.

INTRODUCTION: Subtrochanteric femoral non-union after failed fixation is a demanding reconstructive problem because of high mechanical loads on the proximal femur and impaired bone biology following repeated surgery. Persistent varus deformity, loss of medial support, and implant failure frequently lead to recurrent non-union. Valgus correction converts shear forces into compressive forces across the non-union site, and structural fibular grafting can provide additional biological and mechanical support in complex proximal femoral reconstruction. CASE REPORT: A 62-year-old male presented with

[Nonunions after intertrochanteric and subtrochanteric femoral fractures].

INTRODUCTION: The overall frequency of proximal femoral fractures means that we are repeatedly confronted with failed healing and implant failure, despite a relatively low nonunion rate especially in intertrochanteric fractures (< 5%). The aim of this paper is to present our approach to treating these nonunions of the proximal femur and discuss the treatment results. MATERIAL AND METHODS: Between 2009 and 2023, patients with nonunion of the proximal femur were retrospectively identified and analyzed. Age, gender, time to revision, the Weber-Cech classification of pseudarthrosis and radiograph

Analysis of risk factor for nail breakage in patients with mechanical failures after proximal femoral nail antirotation in intertrochanteric fractures.

Breakage of the intramedullary nail is a rare complication after proximal femoral nail antirotation (PFNA) in intertrochanteric fracture treatment. The purpose of this study was (1) to investigate the frequency of nail breakage among the patients who were treated for mechanical failure after PFNA for intertrochanteric/pertrochanteric fracture, and (2) to determine the risk factors for nail breakage in PFNA treatment of intertrochanteric fracture. To identify mechanical failure after internal fixation using PFNA, we retrospectively reviewed the data of 35 patients (35 hips) who required reoperat

Implantation of trabecular micro-bypass stent using a novel "landing strip" technique.

AIM: To describe a novel technique of creating a landing strip within the trabecular meshwork to guide trabecular micro-bypass stent (iStent) implantation in patients who underwent phacoemulsification. METHODS: Thirty-four eyes from 30 patients who underwent iStent implantation after phacoemulsification from May 2014 to February 2015 were included in our retrospective study. All iStents were implanted via the "landing strip" technique. A 25-gauge microvitreoretinal blade was used to bisect the trabecular meshwork to less than 1 clock-hour, effectively creating a landing strip. The iStent appl

Debridement of wear granulomata in revision total hip arthroplasty: use of a chondrotome shaver blade.

Osteolysis is a common complication following uncemented total hip arthroplasty and is usually secondary to a biological reaction to polyethylene wear debris. The resultant periprosthestic bone resorption and granulomata is frequently not associated with implant loosening. In such circumstances, an option is to leave the well-fixed component, debride the granulomata, and exchange the polyethylene liner. We describe a simple method of accessing and debriding periprosthestic ranulomata using a chondrotome shaver blade.
