On the Possibility of retrograde calculation of blood alcohol concentrations below 0.15‰.

It is widely recognised in forensic medicine that retrograde extrapolation of blood alcohol concentrations (BAC) is generally avoided once the ethanol concentration falls below 0.15‰. In Germany, retrograde calculation is commonly applied to estimate a minimum BAC in favour of the subject and a low elimination rate of 0.1‰/h is legally accepted; however, expert practice typically refrains from back-calculation when the measured BAC is below 0.15‰ ("Rückrechnungsverbot"). This pragmatic threshold is rooted in historical concerns about uncertainty near the end of the BAC curve and the increasing

The Drinking Dashboard for alcohol-induced blackout: A randomized pilot trial.

BACKGROUND: Alcohol-induced "blackouts," or memory loss for events that occur while drinking, are prevalent and problematic among young adults. They also increase motivation to change. This study developed and pilot-tested a theoretically informed digital health intervention ("Drinking Dashboard") for alcohol-induced blackouts. METHODS: Data were collected using qualitative (Study 1) and quantitative (Study 2) methods. Participants in both studies were young adults (ages 18-30 years) across the United States who reported alcohol-induced blackout(s) in the past month. Study 1 participants (N =

Determination of alcohol concentration in a single drop blood obtained via fingertip using gas chromatography/mass spectrometry coupled with solid-phase microextraction.

This study investigated headspace solid-phase microextraction (HS-SPME)-gas chromatography (GS)/mass spectrometry as a low-complexity method for accurate measurement of blood alcohol concentration (BAC) changes in humans over time following alcohol consumption. The aim was to develop an analytical method that would require as small blood samples as possible-smaller than that required for the conventional method-thereby reducing the burden on the subject. Polyethylene glycol (PEG) was used as the fiber material for SPME, and a DB-WAX capillary column was used for GC. For sample extraction via S

Argument for the pharmacokinetically-informed preclinical researcher: A commentary for Alcohol.

The purpose of this commentary is to advocate for the importance of carefully considering ethanol pharmacokinetics when conducting preclinical studies involving alcohol consumption. Researchers working in this field commonly use alcohol drinking as a principle or ancillary measurement. However, the amount of alcohol consumed cannot substitute for an understanding of dose, which is an essential referent in any study involving pharmacology, especially in studies using 24-h, two-bottle choice access to alcohol. Dose is the critical variable for understanding potency and efficacy, for translating

Drinker driver flyer diver.

Blood alcohol concentrations above defined levels are detrimental to cognitive performance. Empirical and published evidence suggest that nitrogen narcosis is analogous to alcohol intoxication with both impairing prefrontal cortex function. Nitrogen narcosis is also known to have been a factor in fatal accidents. To examine the effects of nitrogen narcosis, a recent publication used the Iowa Gambling Task tool, to simulate dynamic real-life risky decision-making behaviour. If the reported outcomes are corroborated in larger rigorously designed studies it is likely to provide further evidence t

Factors associated with higher alcohol concentrations in emergency department presentations: PACE study.

INTRODUCTION: The health impact from alcohol is of recognised concern, from acute intoxication as well as increased risk of chronic health issues over time. Identifying factors associated with higher alcohol consumption when presenting to the emergency department (ED) will inform public health policy and enable more targeted health care and appropriate referrals. METHODS: Secondary testing of blood samples collected during routine clinical care of 1160 ED patients presenting to the Royal Brisbane and Women's Hospital in Queensland, Australia, for 10 days between 22 January and 1 February 2021

Sex differences in alcohol dehydrogenase levels (ADH) and blood ethanol concentration (BEC) in Japanese quail.

Ethanol is one of the most widely used and abused drugs. Following ethanol consumption, ethanol enters the bloodstream from the small intestine where it gets distributed to peripheral tissues. In the bloodstream, ethanol is cleared from the system by the liver. The primary metabolism of ethanol uses alcohol dehydrogenase (ADH). In mammals, females appear to have higher ADH activity in liver samples than males. The purpose of the first experiment was to analyze sex differences in ADH levels following 12 d of ethanol administration (i.e., water or 2 g/kg) in male and female quail. Following the

Traumatic brain injury and alcohol intoxication: effects on injury patterns and short-term outcome.

PURPOSE: A significant number of patients with traumatic brain injuries (TBI) are diagnosed with elevated blood alcohol concentration (BAC). Recent literature suggests a neuroprotective effect of alcohol on TBI, possibly associated with less morbidity and mortality. Our goal is to analyze the association of different levels of BAC with TBI characteristics and outcome. METHODS: Adult patients with moderate to severe TBI (AIS ≥ 2) and measured BAC admitted to the Trauma Centre West (TCW), during the period 2010-2015, were retrospectively analyzed. Data included injury severity (AIS), length of

Analgesic Effect of Alcohol Mediates the Association between Alcohol Intoxication and Deliberate Self-Harm.

We examined whether the analgesic effect of alcohol mediates the association between alcohol and deliberate self-harm (DSH) using data from a larger study on alcohol effects. Men (n = 106) and women (n = 104) low-risk alcohol drinkers (ages M = 26.00, SD = 6.98) recruited from the community who had no suicide attempt or episode of deliberate self-harm within the past year were randomly assigned to either a placebo drink condition or a drink calibrated to reach approximately .050%, .075%, or .100% blood alcohol concentration. Notable within-condition BAC variability, as well as overlap between

Psychosocial sequelae of motor vehicle collisions: a follow-up study.

One-hundred-and-forty-nine motor vehicle collision trauma victims were interviewed one year after discharge from a Regional Trauma Unit. Follow-up data indicated major post trauma problems such depression, anxiety, family stress, financial problems and driving fears. Almost 40% reported drinking driving after the crash with a greater proportion of alcohol (blood alcohol content) positive drivers engaging in drinking driving than blood alcohol content negative drivers. Notably, almost 16% of the blood alcohol content positive and 13% of the blood alcohol content negative reported involvement in

Comparing levels of blood alcohol concentration and indicators of impairment in nightlife patrons.

INTRODUCTION AND AIMS: Breathalyser estimate of blood alcohol concentration (BAC) is widely used as an objective intoxication measure, but is not always practical in nightlife contexts. This study uses in situ data collected in nightlife environments to explore how four measures of intoxication are related so as to inform the development of a more practical and reliable method of differentiating intoxication for people working in the night-time economy. DESIGN AND METHODS: Nightlife patron interviews were conducted in five Australian cities. Participants completed demographic questions and we

Establishing national decision rules for forensic blood alcohol analysis in Italy: A dynamic framework incorporating measurement uncertainty and interlaboratory performance.

This article introduces a dynamic framework for reporting blood alcohol concentration (BAC) by integrating measurement uncertainty and interlaboratory variation. This approach is considered necessary to ensure more consistent decision-making rules when analytical results are used in a legal context. Although these rules have been developed within the Italian context, they are adaptable to other countries as well. Included in the framework are critical challenges stemming from variability in laboratory performance and method uncertainty. Data were collected through a national interlaboratory pr

Investigation of injuries sustained from falls down stairs.

Interpretation of injuries sustained from fatal falls involving stairs is a challenge encountered by death investigation teams regularly. The high incidence of this occurrence is because stairs are a common entity in society. Without a medical evaluation of an individual's injuries, it is difficult to determine whether a fall from stairs contributed to how the individual died. The purpose of this study is to characterize the injuries of individuals that were sustained from a confirmed fatal fall involving stairs within our jurisdiction (Ontario, Canada) during 2014-2020. In a retrospective cro

A fatal case of accidental asphyxia following nitrous oxide inhalation and alcohol consumption.

Nitrous oxide (N2O) abuse is becoming increasingly popular worldwide. Moreover, the use of N2O combined with other substances, such as alcohol, is also common. Accidental deaths associated with N2O abuse are rare in forensic practice, with most fatal cases involving continuous inhalation equipment or exposure in a confined space. In contrast, the inhalation of N2O using balloons is traditionally regarded as a relatively safe method. In this report, we present an unusual fatal case of a 16-year-old girl who died after drinking alcohol and inhaling N2O from balloons. The decedent was found in a

A ferret model of alcohol consumption during adolescence.

Binge drinking is a drinking pattern that results in a blood alcohol concentration (BAC) of 0.08 g/dL or higher in a 2-h period. Binge drinking rates are often highest in adolescents. Persistent adolescent binge drinking has unique consequences and can produce social, cognitive, and executive function deficits. Most of what we know about the mechanisms underlying the effects of adolescent alcohol exposure comes from rodent studies. We developed an adolescent alcohol consumption paradigm in ferrets termed Intermittent One-Bottle (IOB). Ferrets received a bottle of 5% alcohol for 24 h (no water)

A Novel and Simple Method for a Differentiation of Alcohol Types.

BACKGROUND: Alcohol poisoning is a significant global problem that has become an epidemic. The determination of the alcohol type is hereby essential as it may affect the course of the treatment; however, there is no routine laboratory diagnostic method for alcohol types other than for ethanol. In this study, we aimed to define a simple method for alcohol type differentiation by utilizing a combination of breathalyzer and spectrophotometrically measured serum ethanol results. METHODS: A breathalyzer and spectrophotometry were used to measure four different types of alcohol: ethanol, isopropano

What determines the success of states in reducing alcohol related crash fatalities? A longitudinal analysis of alcohol related crashes in the U.S. from 1985 to 2019.

In the United States, nearly 28 people die in alcohol-related motor vehicle crashes every day (1 fatality every 52 min). Over decades, states have enacted multiple laws to reduce such fatalities. From 1982 to 2019, the proportion of drivers in fatal crashes with a blood alcohol concentration (BAC) above 0.01 g/dl declined from 41% to 22%. States vary in terms of their success in reducing alcohol-related crash fatalities. The purpose of this study was to examine factors associated with changes in fatalities related to alcohol-impaired driving at the state level. We created a panel dataset of 50

Effects of alcohol intoxication on driving performance, confidence in driving ability, and psychomotor function: a randomized, double-blind, placebo-controlled study.

RATIONALE: Alcohol-induced driving impairment can occur with any departure from a zero-blood alcohol concentration (BAC). Because intoxication is characterised by impaired judgement, drivers under the influence of alcohol may overestimate their capacity to safely operate a vehicle. OBJECTIVES: This study examined the effects of alcohol on driving performance, four-choice reaction time (FCRT), and self-rated confidence in driving ability. It specifically focused on alcohol doses equal to commonly enforced legal BAC limits (i.e. 0.05% and 0.08%). METHODS: A randomized, double-blind, placebo-co
