BACKGROUND: The most serious complication of patient-controlled analgesia (PCA) is respiratory depression (RD). The incidence of RD in the literature is derived from intermittent sampling of pulse oximetry (Spo(2)) and respiratory rate and defined as a deviation below an arbitrary threshold.
METHODS: We monitored postsurgical patients in a hospital ward receiving morphine or meperidine PCA with continuous oximetry and capnography. Nurses responding to audible monitor bedside alarms documented respiratory status and interventions.
RESULTS: A total of 178 patients were included in the analysis

During transcutaneous PCO2 (PtcCO2) monitoring of 15 freely ambulant patients suffering from panic attacks, all the patients experienced a typical attack. Seven were identified as hyperventilators, as PtcCO2 fell to abnormally low levels during the attack. This group could not be distinguished on the basis of either their usual symptoms of panic or the hyperventilation provocation test. There was no apparent association between absolute levels of PCO2 and the nature of symptoms. The anxiety ratings of hyperventilators were lower than those of non-hyperventilators. These data do not support the

PURPOSE OF REVIEW: Hemodynamic exploration is mandatory in patients with shock to identify the type of shock, to select the best therapeutic strategy, and to assess the efficacy of the selected therapy. In this review, we summarize the characteristics of the main available hemodynamic monitoring systems and emphasize on how to select the most appropriate ones in patients with circulatory shock.
RECENT FINDINGS: Over the past decade, hemodynamic monitoring techniques have progressively evolved from intermittent toward real-time measurements and from invasive toward less invasive approaches. No

The accuracy and precision of transcutaneous pressure measurements of oxygen (Ptc,O2) and carbon dioxide (Ptc,CO2) in the monitoring of nocturnal assisted ventilation in adult patients were evaluated. Transcutaneous measurements obtained with two analysers, Radiometer TINATCM3 (R) and Kontron MicroGas-7650 (K), were compared with arterial blood gases analysed in blood samples withdrawn simultaneously in 10 patients. Sensors were heated to 43 degrees C. Measurements of transcutaneous blood gases and arterial blood gases were collected six times at 1-h intervals. The data obtained with both inst

The first few months following injury to the spinal cord requires constant care of the subject if tissue breakdown is to be avoided. The management of acute traumatic cases involves complete bedrest in a supine position with appropriately positioned pillows to minimize trauma to the bone prominences. This study assesses the effectiveness of the management procedure in terms of the tissue response at the sacrum of 15 acute spinal cord injured subjects. The measurement of mean interface pressures during a representative period of recumbency was performed and these were related to changes in tran

To investigate the severity of oxygen desaturation following voluntary hyperventilation (VHV) in normal subjects and its possible relation to chemoresponsiveness, we examined respiration following VHV in 16 normal male subjects. Monitoring was performed according to the standard polysomnography protocol including measurements of arterial oxygen saturation (SaO2) and transcutaneous PCO2 (PtcCO2). The subjects hyperventilated voluntarily for 3 min, and were then observed for more than 15 min. They hyperventilated again for another 3 min, and were followed again for more than 15 min. Eleven subje

The alpha 2 agonist xylazine produced a dose-dependent decrease in mean arterial blood pressure in conscious rabbits when injected intrathecally (i.t.) through a cannula previously implanted under general anaesthesia. Intrathecal administration of 200 and 400 micrograms of xylazine produced a significant reduction in arterial blood pressure from control values (maximum depressions of 25% and 33%, respectively). There was little effect on cardiac output and arterial carbon-dioxide tension and no effect on respiratory rate, arterial oxygen tension and pulse rate. Intrathecal injection of 100 mic

BACKGROUND: Noninvasive evaluation of partial pressure of carbon dioxide (PCO2) is clinically important for screening and monitoring of hypercapnia, especially in patients with chronic obstructive pulmonary disease (COPD). However, the comparative accuracy of end-tidal PCO2 (PetCO2) and transcutaneous PCO2 (PtcCO2) monitoring in COPD remains uncertain. This study aimed to evaluate the agreement between PetCO2 obtained by using modified method of prolonged expiration with an integrated calculation algorithm (PetCO2-PA) and PtcCO2 with arterial PCO2 (PaCO2) in patients with COPD.

BACKGROUND: Widespread use of transcutaneous PCO2 (PtcCO2 ) monitoring is currently limited by concerns many practitioners have regarding accuracy. We compared the accuracy of PtcCO2 with that of PaCO2 measurements in critically ill children, and we investigated whether clinical conditions associated with low cardiac output or increased subcutaneous tissue affect this accuracy.
METHODS: We performed a single-center prospective study of critically ill children placed on transcutaneous monitoring.
RESULTS: There were 184 children enrolled with paired PaCO2 and PtcCO2 values. Subjects had a med

CO2 cerebrovascular reactivity has been recorded in 12 healthy volunteers and 10 patients with unilateral > 70% extracranial internal carotid artery (ICA) stenosis, using non invasive techniques. The relative changes of middle cerebral artery blood flow velocity (VMCA) and velocity waveform pulsatility (PIMCA) after that hypocapnia was induced by spontaneous hyperventilation were recorded. 35.5% average VMCA reduction and 63% PIMCA increment of basal values was produced in healthy subjects after hyperventilation. The percentage variation of CO2 Reactivity Index (RI), expressed in terms of VMCA

Transcutaneous oxygen tension is a flow related parameter. Detailed analysis of the physiology and physical chemistry of oxygen consumption and diffusion indicates that tcPO2 is not proportional to skin blood flow. Measurement of tcPO2 at 37 degrees C allows changes in skin blood flow to be clearly demonstrated and is of use in many areas of clinical medicine. Transcutaneous oxygen tension recorded using a polarographic oxygen electrode depends on skin blood flow, skin respiration, arterial oxygen concentration, temperature, skin and electrode permeability and the oxygen consumption of the ele

In moderate lower extremity artery disease (LEAD), when tissue ischaemia due to impaired inflow occurs at exercise but not during rest, exercise oximetry may be evaluated as a part of the diagnosis process. Initially used when assessing critical limb ischaemia at rest, transcutaneous oximetry (TcpO2) has also been used in the last two decades during exercise assessment as a non-invasive method to measure oxygen pressure at the skin's surface, offering insights into loco-regional oxygen delivery-requirement mismatch. The introduction of decrease from rest of oxygen pressure (DROP) analysis in t

BACKGROUND: Arterial blood gas analysis is the gold standard for monitoring of Pa CO2 and PaO2 during mechanical ventilation. However, continuous measurements would be preferred. Transcutaneous sensors continuously measure blood gases diffusing from the locally heated skin. These sensors have been validated in children mostly in intensive care settings. Accuracy in children during general anesthesia is largely unknown.
AIMS: We conducted a study in children undergoing general anesthesia to validate the use and to determine the accuracy of continuous transcutaneous measurements of the partial

OBJECTIVE: To assess the safety and efficacy of carbon dioxide (CO(2)) in colonoscopy examination.
METHODS: We randomized 349 patients to undergo colonoscopy with insufflation of air (n=175) or CO(2) (n=174). At colonoscopy, p (ET CO(2)) was observed at 4 time points: before the exam, arrived caecum, back rectum, and after the exam. Patient's experience of pain in the end and after the examination at 1, 3, 6, and 24 h was registered using a visual analog scale (VAS). Sedation was not used routinely.
RESULTS: The groups were similar in age, sex, inspection time, and caecal intubation rate (al

To investigate whether the noninvasive determination of transcutaneous oxygen tension (TcO2) can be used to evaluate the degree of hemiplegia, we measured TcO2 before and after a 2-mo rehabilitation course in 12 patients with hemiplegia caused by cerebrovascular diseases. All patients with no evidence of heart failure (ejection fraction evaluated by echocardiogram: 68.7 +/- 6.4%) began to receive conventional physical therapy a few days after the onset of stroke. The measurement of TcO2 was performed at 9:00 a.m. with an electrode placed on the skin surface of the flexion side of the forearm 5

Transcutaneous carbon dioxide monitoring has been used to investigate directly the frequency and role of hyperventilation during naturally occurring panic attacks in freely ambulant volunteers and patients. Illustrative preliminary data from healthy subjects and from four patients with panic attacks are presented. These confirm that some patients do hyperventilate during their attacks but indicate that some do not. The unreliability of traditional methods for identifying hyperventilators is demonstrated. These data indicate the potential of ambulant monitoring in research into hyperventilation

The level of amputation continues to present a challenge for surgeons. In view of this, 24 patients who required an amputation of their ischaemic leg were studied prospectively using Laser Doppler flowmetry (LDF), TcpO2 measurements and Doppler ultrasound to assess the best level for amputation. In all patients gangrene of the leg and rest pain were the indication for an amputation. Skin oxygen tension (TcpO2) and skin blood flow (LDF) measurements were obtained the day before surgery on the proposed anterior and posterior skin flaps for below knee amputation and the maximum Doppler systolic p
