Concordance between awake blood pressure on home and ambulatory blood pressure monitoring in older adults.

BACKGROUND: Determining the agreement between ambulatory blood pressure (BP) monitoring (ABPM) and home BP monitoring (HBPM) can inform their use. METHODS: Participants ≥65 years of age taking antihypertensive medication in the AMBROSIA-HOME study completed 24 hours of ABPM and 7 days of HBPM. Controlled office and awake BP was defined as mean systolic BP (SBP)/diastolic BP (DBP) <130/<80 mm Hg and uncontrolled office and awake BP as mean SBP or DBP ≥130/80 mm Hg. Masked uncontrolled hypertension was defined as controlled clinic BP with uncontrolled BP on HBPM or ABPM, and white coat effect a

Nocturnal Blood Pressure Dipping in Adults: A Comparison Between Cuffless and Ambulatory Blood Pressure Monitoring.

BACKGROUND: Cuffless blood pressure (BP) devices have demonstrated potential in estimating daytime BP but have not been able to predict nocturnal BP precisely, creating difficulties when the intended use is ambulatory BP monitoring (ABPM). We evaluated the ability of a photoplethysmography-based cuffless BP device, with the intended use 24-hour ABPM, to estimate nocturnal BP compared to a cuff-based oscillometric device. METHODS: Participants underwent 24-hour ABPM with the cuffless device placed on the non-dominant upper arm and a validated cuff-based oscillometric device on the dominant arm

Optimal timing for post-dialysis blood pressure measurement: relationship with home blood pressure monitoring.

In hemodialysis patients, blood pressure (BP) measured at the postdialysis period (POSBP) can be used to diagnose hypertension. However, the optimal time point for POSBP remains uncertain. This cross-sectional study evaluated 210 long-term dialysis patients (66.7% men; age = 56.9 ± 15.7 years), who underwent POSBP measurements at three time points [immediately after dialysis period ended (POSBP1); after blood return (POSBP2) and after fistula hemostasis or catheter sealing (POSBP3)] and home BP monitoring (HBPM) over one week. POSBP1, POSBP2, POSBP3 and HBPM values were 139.1 ± 22.4/73.2 ± 12.

Cutoffs for white-coat and masked blood pressure effects: an ambulatory blood pressure monitoring study.

The values used to define white-coat and masked blood pressure (BP) effects are usually arbitrary. This study aimed at investigating the accuracy of various cutoffs based on the differences (ΔBP) between office BP (OBP) and 24h-ambulatory BP monitoring (ABPM) to identify white-coat (WCH) and masked (MH) hypertension, which are phenotypes coupled with adverse prognosis. This cross-sectional study included 11,350 [Derivation cohort; 45% men, mean age = 55.1 ± 14.1 years, OBP = 132.1 ± 17.6/83.9 ± 12.5 mmHg, 24 h-ABPM = 121.6 ± 11.4/76.1 ± 9.6 mmHg, 25% using antihypertensive medications (AH)] an

Daily self-monitoring of blood pressure decreases systolic and diastolic blood pressure in hypertensive participants.

Hypertension is a major risk factor for cardiovascular diseases, and behavior modification has been shown to improve blood pressure (BP). We investigated whether daily self-monitoring of systemic BP and other factors related to cardiovascular events decreased BP in hypertensive participants. In this prospective, randomized, open, blinded-endpoint trial, we assigned 161 participants with hypertension to monitor their BP daily (BP-measurement group) or, in addition to BP, monitor their body fat, sleeping time, and daily step count (multiple-measurement group) or no self-monitoring (control group

Blood pressure variability and arterial stiffness parameters derived from ambulatory blood pressure monitoring.

Multiple blood pressure (BP) measurements allow an evaluation of BP variability and BP‑derived arterial stiffness indices. Periodic variations in BP are well known, from beat‑to‑beat BP readings in intra‑arterial measurement to seasonal variations in BP. Diurnal BP variation has been investigated in relation to its prognostic value. People with night‑to‑day BP ratio of 1 or higher, that is, those with a higher nocturnal than daytime BP, were older than those with normal dipping status at baseline and had a greater risk of cardiovascular mortality and morbidity. Short‑term BP variability was ev

Age and blood pressure measurement: experience with the TM2420 ambulatory blood pressure monitor and elderly people.

The accuracy of the TM-2420 ambulatory blood pressure monitor was assessed in elderly people. Ninety-four subjects (44 men and 50 women), aged 60-94 with systolic blood pressure (SBP) of 97-208 mmHg and diastolic blood pressure (DBP) of 45-109 mmHg, including 23 with isolated systolic hypertension, were studied in three centres. The monitor was compared simultaneously with pairs of observers using the Hawksley random zero sphygmomanometer. The standard deviation of the difference (SDD) between observers was 4.2 mmHg (SBP), 2.9 mmHg (DBP). The mean difference was 0.49 mmHg (SBP) and 0.27 mmHg (

Postpartum blood pressure trends and time in target range during remote blood pressure monitoring.

INTRODUCTION: Postpartum remote blood pressure monitoring (RBPM) programs have emerged as an important adjunct to the management of hypertensive disorders of pregnancy (HDP). These programs have demonstrated robust patient participation and satisfaction with reduced readmissions and severe maternal morbidity. Despite this, data describing blood pressure (BP) trends throughout program participation and across different antihypertensive regimens remain limited. We sought to evaluate BP trends and time in target range (TTR) during our 6-week RBPM program. METHODS: This is a retrospective cohort

Ambulatory blood pressure monitoring and peri-hemodialysis blood pressures in a southeast US hemodialysis unit.

AIMS: End-stage renal disease (ESRD) patients on dialysis are perceived to have difficult-to-control blood pressure (BP) and commonly treated with complex antihypertensive regimens. Our hypothesis was that peri-dialysis BP will overestimate the true burden of hypertension in these patients. SUBJECTS AND METHODS: We performed 44-h ambulatory blood pressure monitoring (ABPM) in 43 patients recruited from the University of Mississippi outpatient dialysis unit. Data collected included routine peri-dialysis blood systolic blood pressure (SBP), diastolic blood pressure (DBP), weight gain, and demog

Influence of quality of sleep on the nocturnal decline in blood pressure during ambulatory blood pressure monitoring.

OBJECTIVE: To assess the influence of the quality of sleep on the nocturnal physiological drop in blood pressure during ambulatory blood pressure monitoring. METHODS: We consecutively assessed ambulatory blood pressure monitoring, the degree of tolerance for the examination, and the quality of sleep in 168 patients with hypertension or with the suspected "white-coat" effect. Blood pressure fall during sleep associated with a specific questionnaire and an analogical visual scale of tolerance for ambulatory blood pressure monitoring were used to assess usual sleep and sleep on the day of examin

Systematic error in the determination of nocturnal blood pressure dipping status by ambulatory blood pressure monitoring.

BACKGROUND: Vertical displacement of the arm relative to the heart causes inverse changes in blood pressure of approximately 0.8 mmHg for every centimetre change in arm position. Therefore a potential confounding issue in assessing diurnal variation in blood pressure during ambulatory blood pressure monitoring (ABPM) is arm position during sleep. An increase in the number of patients with 'excessive' nocturnal dipping (> 20% decrease in night/day blood pressure) was observed following the creation of an instructional videotape in which patients were advised to muffle the noise of the monitor w

Differences in orthostatic blood pressure changes measured with an oscillometric blood pressure monitor and a mercury sphygmomanometer.

AIMS: To compare orthostatic blood pressure (BP) changes recorded with the SpaceLabs 90207 BP monitor (SL) and the standard mercury sphygmomanometer (HgS). METHODS: 85 hospital in-patients aged 60-90 years had nine BP measurements recorded by both instruments using the same arm sequential measurement technique in supine and standing position by two observers. Supine BP was taken as the final set of three supine measurements, ie, one made with the SL, and the mean of the two HgS readings immediately before and after the monitor reading. From the SL supine reading was subtracted the three stand

Home versus office monitoring of blood pressure: a European multicentre study of high blood pressure.

A prospective multicentre study has been set up to examine the following primary questions: (1) Is there a better correlation between blood pressure and mortality, morbidity or organ damage when blood pressure is measured by ambulatory recordings rather than by the usual office method? (2) Is there a difference in the rates of mortality, morbidity and organ damage when treatment is guided by ambulatory values instead of by office values? The aim is to study 2000 patients with primary hypertension, aged 35-70 years, for 5 years. Ambulatory recordings will be made every 6 months. Treatment will

A randomized controlled trial comparing remote blood pressure monitoring with office-based blood pressure monitoring for women at high risk of preeclampsia.

BACKGROUND: Women at a high risk of developing preeclampsia are recommended to have additional blood pressure surveillance, but frequent outpatient attendances are demanding for patients and the healthcare system. It is unclear whether remote blood pressure monitoring can replace conventional office-based monitoring and improve healthcare utilization without increasing the signal for harm. OBJECTIVE: To evaluate the effects of substituting clinic blood pressure monitoring with remote blood pressure monitoring on fetal and maternal outcomes, and healthcare utilization, in pregnant women at a h
