Pituitary Gland Functional Connectivity and BMI.

The pituitary gland (PG) influences body weight through hormonal releases; however, the relation between body weight and PG's co-activities with other brain regions remains unclear. Here, we aimed to identify (1) the functional connectivity of the PG and (2) PG functional connectivity associated with body mass index by examining resting state functional magnetic resonance imaging data. Using enhanced Nathan Kline Institute-Rockland Sample, PG functional connectivity of 494 individuals was analyzed to assess in voxel-wise fashion. A negative association was found between BMI and PG functional c

Nutrition-First Support for GLP-1 and Dual Incretin Therapy in Obesity: A Practical Framework for Dietary Management, Symptom Tolerability, and Long-Term Weight Maintenance.

BACKGROUND: Glucagon-like peptide-1 receptor agonists (GLP-1RAs) and dual glucose-dependent insulinotropic polypeptide (GIP)/GLP-1 receptor agonists have transformed obesity treatment, producing substantial weight loss during active therapy. However, real-world effectiveness may be limited by gastrointestinal adverse events, reduced dietary intake, fat-free mass loss as part of total weight reduction, and weight regain after discontinuation. METHODS: This narrative review synthesizes current pharmacological, nutritional, gastrointestinal, body-composition, and implementation evidence to propo

Intravenous fluid administration and monitoring for adult ward patients in a teaching hospital.

Many acutely unwell, hospitalized patients receive continuous intravenous (IV) fluids. Complications of IV fluid administration include electrolyte abnormalities and fluid overload, which be detected by appropriate IV fluid administration and monitoring practices. This prospective clinical audit described the administration and monitoring of maintenance IV fluid in ward patients in an Australian tertiary teaching hospital. Of 332 inpatients (98%), 85 (26%) were receiving IV fluids. Serum electrolyte and renal function measurements were obtained in 85% versus 65% of patients receiving IV fluids

Imipramine and weight gain during the long-term treatment of recurrent depression.

The recently completed long-term maintenance trial of full-dose imipramine for recurrent unipolar disorder provided an opportunity to examine the extent to which such doses (200-300 mg daily) are associated with persistent and adverse side effects, particularly weight change. In 115 patients we monitored weight change during the three-year maintenance treatment phase to the point of trial completion, recurrence or termination. No differences were noted between individuals receiving active medication (average gain of 5.8 lbs. during an average treatment period of 725 days) versus those randomiz

Feeding behavior in monkeys with spontaneous diabetes mellitus.

In our laboratory, monkeys (Macaca mulatta) that attain middle age (10-14 years) and a body weight of over 15 kg often develop spontaneous diabetes mellitus. In some animals the development of the disease is accompanied by polyphagia and polydipsia. The polyphagia appears to be in response to loss of body weight. Gross estimates of energy balance in diabetic monkeys indicate that the daily maintenance cost (kcal) of diabetes is greater during the untreated phase of the disease than in the pre-diabetic state.

The PROMOTE study (High-protein and resistance-training combination in overweight and obesity) for short-term weight loss and long-term weight maintenance for Chinese people: a protocol for a pilot randomized controlled trial.

BACKGROUND: It is very important for clinicians and dieticians to explore reasonable weight management strategies for obese people that address both short-term weight loss and subsequent weight maintenance. We hypothesized that resistance training combined with a high-protein diet would result in similar short-term weight loss but better long-term weight maintenance than either a conventional low-fat diet control or a high-protein diet alone. METHODS/DESIGN: This is an 8-week randomized parallel controlled trial followed by a 24-week observational follow-up study. A 48-week supplementary foll

Percent Body Fat Measurement in the Medical Management of Children with Obesity.

Sustaining weight loss can be challenging, as physiological responses to weight loss, including metabolic and hormonal adaptations and decreased energy expenditure, promote weight regain. Paired with sustained dietary changes, physical activity can promote weight maintenance after successful weight loss, as physical activity can help maintain fat-free mass. We present several illustrative cases to highlight the potential use of body composition measurement using a bioelectrical impedance analysis (BIA) scale to augment obesity management counseling in a tertiary care pediatric weight-managemen

Hypogonadotropic hypogonadism in a homozygous MC4R mutation carrier and the effect of sibutramine treatment on body weight and obesity-related health risks.

OBJECTIVE: The first aim of our study was to define the hypogonadism manifested by low testosterone levels and incomplete male secondary sex characteristics in a 20-year-old male homozygous MC4R mutation carrier (G181D). The second aim of our study was to evaluate the effect of the anti-obesity drug sibutramine in this patient who failed to respond to an intensive lifestyle intervention and exhibited continuous weight gain. CASE REPORT: Anthropometric, biochemical, hormonal and psycho-behavioural parameters were investigated both at baseline and after a 1-year sibutramine treatment. To charac

Sustained weight loss following 12-month pramlintide treatment as an adjunct to lifestyle intervention in obesity.

OBJECTIVE: To assess long-term weight loss efficacy and safety of pramlintide used at different dosing regimens and in conjunction with lifestyle intervention (LSI). RESEARCH DESIGN AND METHODS: In a 4-month, double-blind, placebo-controlled, dose-ranging study, 411 obese subjects were randomized to receive pramlintide (six arms: 120, 240, and 360 microg b.i.d. and t.i.d.) or placebo in conjunction with a structured LSI program geared toward weight loss. Of the 4-month evaluable subjects (n = 270), 77% opted to continue preexisting treatment during an 8-month single-blind extension (LSI geare

Clinical pharmacokinetics of phenobarbital in neonates.

Demographic and clinical pharmacokinetic data collected from term and preterm neonates who were treated with intravenous phenobarbital have been analysed to evaluate the role of patient characteristics in pharmacokinetic parameters. Significant relationships between total body weight (TBW) or body surface area (BSA) and volume of distribution (Vd) and total body clearance (CL) were found. Coefficients of determination were: 0.55 and 0.59 for Vd, and 0.76 and 0.72 for CL against TBW and BSA, respectively. In addition, significant relationships between height of the infants and volume of distrib

The "obesity paradox" in dosing: a comprehensive analytical review of cockcroft-gault versus CKD-EPI for predicting clearance of narrow-therapeutic-index drugs in patients with BMI > 40 kg/m².

BACKGROUND: Class III obesity (body mass index [BMI] ≥ 40 kg/m²) now exceeds 9% prevalence in the United States and is projected to reach nearly one in four adults in several states by 2030. This demographic shift collides with a half-century of drug-dosing infrastructure built largely on Cockcroft-Gault (CG) creatinine clearance estimates that use total body weight and were derived in a small, mostly male, non-obese cohort. OBJECTIVE: To evaluate whether CG (with total body weight [TBW], ideal body weight [IBW], adjusted body weight with a 40% factor [AdjBW40], or lean body weight [LBW]) or

Very Low-Calorie Diet and 6 Months of Weight Stability in Type 2 Diabetes: Pathophysiological Changes in Responders and Nonresponders.

OBJECTIVE: Type 2 diabetes mellitus (T2DM) is generally regarded as an irreversible chronic condition. Because a very low-calorie diet (VLCD) can bring about acute return to normal glucose control in some people with T2DM, this study tested the potential durability of this normalization. The underlying mechanisms were defined. RESEARCH DESIGN AND METHODS: People with a T2DM duration of 0.5-23 years (n = 30) followed a VLCD for 8 weeks. All oral agents or insulins were stopped at baseline. Following a stepped return to isocaloric diet, a structured, individualized program of weight maintenance

Weight Stigma in the Development, Maintenance, and Treatment of Eating Disorders: A Case Series Informing Implications for Research and Practice.

Weight-centric health practices are based on the principle that excess weight predicts chronic disease, informing a growing sociopolitical movement to address an "obesity epidemic." This hyper-focus on preventing obesity may contribute to weight stigma (i.e., the devaluation and discrimination of individuals based on body size) and other iatrogenic outcomes for youth, including the development and maintenance of eating disorders (EDs). Current evidence-based treatments for EDs include language and practices that may reinforce fears of fatness, body shame, and unhealthy dietary restriction with

A three-year follow-up study of psoriasis patients treated with low dosages of etretinate orally and corticosteroids topically.

Etretinate 0.5 mg/kg body weight combined with 0.1% triamcinolone acetonide and 5% salicylic acid in an O/W cream gave more than 70% improvement in 62% of 75 patients. Of this satisfactorily improved group, at least 41% were still in the same condition after 3 years on a maintenance dose of, on average, 0.3 mg/kg body weight etretinate daily and 20 g weekly of a relatively strong corticosteroid cream. The side effects were acceptable and the convenience for the patients is great as compared with other treatment.

Weight maintenance through behaviour modification with a cooking course or neurolinguistic programming.

We compared the effect on weight regain of behaviour modification consisting of either a gourmet cooking course or neurolinguistic programming (NLP) therapy. Fifty-six overweight and obese subjects participated. The first step was a 12-week weight loss program. Participants achieving at least 8% weight loss were randomized to five months of either NLP therapy or a course in gourmet cooking. Follow-up occurred after two and three years. Forty-nine participants lost at least 8% of their initial body weight and were randomized to the next step. The NLP group lost an additional 1.8 kg and the cook
