In vivo 3D hip kinematics during racket swinging after total hip arthroplasty : evaluation of dynamic range of motion and liner-to-neck contact.

AIMS: Interest in sports participation after total hip arthroplasty (THA) has grown, and some patients return to playing racket sports after THA. However, dynamic hip kinematics and the risk of liner-to-neck contact during racket swinging remain unclear. This study aimed to evaluate in vivo, 3D dynamic hip kinematics during racket swinging after THA, and to identify factors associated with reduced liner-to-neck clearance, which may indicate an increased risk of liner-to-neck contact. METHODS: A total of 19 patients (24 hips) who were playing racket sports after primary THA were evaluated. Dyn

Comparison of cruciate retaining and PCL sacrificing TKA with respect to medial and lateral gap differences in varus knees after medial release.

We aimed to clarify whether the increase in medial gap after medial release is influenced by the retention or sacrifice of posterior cruciate ligament (PCL) during navigation-assisted total knee arthroplasty. After matched pairs were done according to the equality of preoperative varus deformity and medial collateral laxity, 54 knees of each type were available for this study. In the PCL sacrificing group, the mediolateral gap difference significantly increased in both flexion and extension as the preoperative mechanical axis angle increased whereas in the cruciate retaining group, the mediola

The effect of acetabular retroversion on ipsilateral injuries during traumatic hip dislocation.

BACKGROUND: Determine whether native acetabular anteversion angle increased the risk of ipsilateral limb injuries in patients with traumatic hip dislocations. METHODS: Retrospective clinical series completed at a large, tertiary health care system between February 2016-November 2021. Patients with a native traumatic hip dislocation requiring a closed reduction in the operating room or open reduction internal fixation (ORIF) of an associated fracture were included, identified using current provider terminology (CPT) codes 27,250 and 27,252. Standard acetabular version angles were measured on C

A modified S-ROM stem in primary total hip arthroplasty for developmental dysplasia of the hip.

This study examined the clinical outcome of 220 hips in 196 Asian patients who underwent primary total hip arthroplasty (THA) for treatment of developmental dysplasia of the hip (DDH) using a modified S-ROM modular (S-ROM-A) stem designed for Asians, after 2-5 years (mean, 3.3 years) of follow-up. The stem was placed so that the anteversion angle of the neck was decreased against the sleeve in 56% of the hips and increased in 18% of the hips. Bone ingrown fixation was achieved in 99.5% of the hips on X-ray at final follow-up. There were 2 (0.9%) dislocations postoperatively. In primary THA for

Orthopedics management of acromicric dysplasia: follow up of nine patients.

UNLABELLED: Acromicric dysplasia (AD) is an autosomal dominant disorder characterized by short stature, short extremities, stiff joint and skeleton features including brachymetacarpia, cone-shaped epiphyses, internal notch of the femoral head, and delayed bone age. Recently, we identified fibrillin 1 (FBN1) as the disease gene of AD. The aim of our study was to further describe the long-term follow up of AD patients with an emphasis on orthopedic management. Nine patients with FBN1 mutations were included in the study ranging in age from 5.5 to 64 years. For all, detailed clinical and radiolog

Treatment of hip instability after total hip replacement with pelvic osteotomy using a modified Stoppa approach.

INTRODUCTION AND IMPORTANCE: Hip dislocation remains a significant complication following total hip arthroplasty, even though its incidence has decreased. While closed reduction is typically performed for early dislocations, delayed or chronic dislocations often necessitate acetabular or femoral component revision. CASE PRESENTATION: This document describes the treatment of hip dislocation in a 56-year-old patient through pelvic osteotomy without component revision. An acetabular component malposition was identified, exhibiting an 80-degree inclination and 20-degree cup anteversion. Owing to

Accuracy of a novel, automated 2D-3D registration software for measuring cup position in total hip arthroplasty.

PURPOSE: This study evaluated the accuracy of an automated 2D-3D registration software for matching preoperative 3D models of the pelvis and acetabular component to intraoperative 2D fluoroscopy images in total hip arthroplasty (THA). METHODS: We developed a 2D-3D registration software that registers a 3D model of the pelvis from preoperative CT and a 3D model of the acetabular implant to intraoperative fluoroscopic imaging, thereby calculating the implant position relative to the 3D pelvic reference frame. A total of 145 datasets were used including 65 digitally reconstructed radiographs, 20

Computational mechanobiological model combining epiphyseal, apophyseal, and appositional growth and inner bone remodeling of the juvenile femur.

In silico models for simulating bone growth based on mechanical or non-mechanical epigenetic factors are widely used. In this study, a well-known mechanobiological model, which states that octahedral shear stress accelerates longitudinal bone growth and hydrostatic stress retards it, is applied to a finite element model of the femur of an 8-year-old boy. Proximal and distal epiphyseal plates as well as the growth plate of the greater trochanter, cartilaginous growth at the femoral isthmus, and appositional bone growth are included in the model. Furthermore, changes in the density of the cancel

[Rapid femur modeling method based on statistical shape model].

The geometric bone model of patients is an important basis for individualized biomechanical modeling and analysis, formulation of surgical planning, design of surgical guide plate, and customization of artificial joint. In this study, a rapid three-dimensional (3D) reconstruction method based on statistical shape model was proposed for femur. Combined with the patient plain X-ray film data, rapid 3D modeling of individualized patient femur geometry was realized. The average error of 3D reconstruction was 1.597-1.842 mm, and the root mean square error was 1.453-2.341 mm. The average errors of f

Effect of a modular neck hip prosthesis on anteversion and hip rotation in total hip arthroplasty for developmental dysplasia of the hip.

Total hip arthroplasty (THA) is often required to decrease the excessive anatomical femoral anteversion (AA) in developmental dysplasia of the hip. Studies have recommended decreasing the AA via the use of a retroverted modular neck. However, hip rotation after THA may strengthen or weaken the effect of changing the AA. Thus, the present study analyzed the effect of a retroverted neck on AA and hip rotation. Patients who underwent THA using a straight neck (ST group) or a 15° retroverted neck (RV group) in a version changeable dual modular system (Mainstay stem, Kyocera, Kyoto, Japan) were ret

Combined supracondylar femoral derotation osteotomy and patellofemoral ligament reconstruction for recurrent patellar dislocation and severe femoral anteversion syndrome: surgical technique and clinical outcome.

PURPOSE: Severly increased femoral anteversion is an important risk factor for patellofemoral instability. Recurrent dislocations cause a traumatic disruption of the medial patellofemoral ligament. Therefore a procedure that combines femoral derotation osteotomy and patellofemoral ligament reconstruction should be considered for patients with severely increased femoral anteversion. The aim of the study was to evaluate the subjective and objective outcomes after combined femoral derotation osteotomy and anatomical reconstruction of the MPFL. METHODS: 12 consecutive patients (12 knees) with pat

Wear Patterns in Knee Articular Surfaces in Varus Deformity.

This study was performed to study the relationship between the degree of varus deformity of osteoarthritic knees and the anatomic distribution of cartilage pathology. Bone wafers resected from the distal femur and proximal tibia were obtained from 107 patients (195 knees) with primary varus osteoarthritis. Severity and distribution of joint damage was scored for each articular surface and related to potential prognostic factors, including varus deformity, the ligamentous status of the knee, age, gender and BMI. There was highly significant association between varus angulation of the knee and t

Uncemented total hip arthroplasty with subtrochanteric derotational osteotomy for severe femoral anteversion.

Total hip joint arthroplasty (THA) in the presence of severe femoral anteversion (>50 degrees) is technically demanding. This problem is often encountered in patients with osteoarthritis secondary to hip joint dysplasia or congenital dislocation. We describe a method of THA in which an uncemented femoral prosthesis is used in conjunction with subtrochanteric derotational osteotomy. This technique allows the restoration of the normal proximal femoral anatomy, including the abductor muscle lever arm without resorting to greater trochanteric transfer. Correction of the excessive femoral anteversi

Femoral malrotation after surgical treatment of femoral shaft fractures in children: a retrospective CT-based analysis.

BACKGROUND: Rotational malalignment is an important and not always avoidable complication after surgical treatment of femoral shaft fractures. The purpose of this study was to determine the incidence of rotational malalignment in children after surgical treatment of femoral shaft fractures and to identify potential patient- and treatment-related risk factors based on data obtained from CT scans. METHODS: We conducted a retrospective analysis of all patients aged less than 15 years with femoral shaft fractures admitted to our level 1 trauma centre between January 2004 and July 2014. Patients h

A new technique of subtrochanteric shortening in total hip replacement for Crowe type 3 to 4 dysplasia of the hip.

A new technique of femoral fixation that uses a transverse osteotomy for subtrochanteric shortening and derotation in total hip arthroplasty for Crowe type 3 to 4 dysplasia of the hip is described. This series included 21 cases in 12 women and 2 men, with a mean age of 42.3 years. Follow-up averaged 41.2 months. Bone pegs prepared from the resected femoral segment were seated in the medullary canal around the stem for femoral fixation. Merle d'Aubigné scores for pain, motion, and walking improved from 2.9, 4.4, and 3.7 to 5.2, 5.4, and 5.5, respectively. Radiographic union was detected within

Mask Ventilation Failure During Induction of General Anesthesia in an Infant With Osteogenesis Imperfecta Type II.

Osteogenesis imperfecta (OI) is a congenital disease characterized by fractures and progressive bone deformities due to systemic bone fragility. A 12-month-old male infant diagnosed with OI type II, the most severe type, was scheduled for a tracheostomy. The patient presented with thoracic hypoplasia, which was treated with a high-flow nasal cannula, and a large skull owing to hydrocephalus with the head fixed in a left anteversion position. We encountered difficulties in mask ventilation during the rapid induction of general anesthesia. Oxygen saturation dropped temporarily, but the patient's

An articulated shape model to predict paediatric lower limb bone geometry using sparse landmarks.

Creating musculoskeletal models in a paediatric population currently involves either creating an image-based model from medical imaging data or a generic model using linear scaling. Image-based models provide a high level of accuracy but are time-consuming and costly to implement, on the other hand, linear scaling of an adult template musculoskeletal model is faster and common practice, but the output errors are significantly higher. An articulated shape model incorporates pose and shape to predict geometry for use in musculoskeletal models based on existing information from a population to pr

Revision Total Hip Arthroplasty with Retained Acetabular Component.

BACKGROUND: Aseptic loosening and osteolysis commonly limit the survivorship of total hip prostheses. Retention of a well-fixed acetabular component, rather than full acetabular revision, has multiple advantages, but questions have lingered regarding the clinical success and prosthetic survivorship following this procedure. We examined the impact of acetabular component position, polyethylene type, liner insertion technique, femoral head size, and simultaneous revision of the entire femoral component (as opposed to head and liner exchange) or bone-grafting on mid-term to long-term prosthetic s
