Bone conduction in otosclerosis--operated versus non-operated ears.

OBJECTIVE: To examine whether stapes surgery improves long-term bone conduction in patients with otosclerosis. SETTING: Tertiary university hospital. METHODS: Thirty patients (52 ears) who underwent stapedectomy between 1988 and 1994. Hearing tests were performed pre-operatively and 5 years postoperatively. RESULTS: The operated ears showed significant improvement in speech reception threshold and air conduction (250-4,000 Hz). Over the follow-up period, there were no significant differences in bone conduction thresholds between the operated and non-operated ears and no deterioration during

[The influence of tympanoplasty on bone conduction (author's transl)].

Seldom tympanoplasty damages the inner ear in a substantial amount: From 1814 operations upon chronic inflamed ears 4 patients (0.22%) became deaf, among the other patients bone conduction (b.c.) in the mean increased at 1000 cps for + 0.8 dB and decreased at 4000 cps for - 2.2 dB. In 9.8% b.c. showed an improvement for 10 dB or more at 1000 and 4000 cps, 9.7% showed an corresponding deterioration. For patients it was confirmed on a statistical base, that the influence on bone conduction demonstrated by animal experiments (Tonndorf), was mainly the result of changing the compliance of oval win

Transitions in auditory rehabilitation with bone conduction implants (BCI).

BACKGROUND: The bone conductive implants (BCI) are nowadays a reliable alternative for rehabilitation of specific forms of hearing loss, i.e. conductive, mixed or single sided deafness (SSD). Aims/Objective: To analyse the various factors in play when considering an auditory rehabilitation with a bone-conductive device (BCI). MATERIALS AND METHODS: The clinical charts of subjects who underwent BCI application at the same Implanting Center from 2005 to 2018 were retrieved analysing also the reason for eventual explantation and the alternative option (transition) for hearing rehabilitation. Advances in the Field of Bone Conduction Hearing Implants.

The number of marketed bone-conduction hearing implants (BCHIs) has been steadily growing, with multiple percutaneous devices and transcutaneous devices now available. However, studies assessing efficacy often have small sample sizes and employ different assessment methodologies. Thus, there is a paucity of evidence to guide clinicians to the most appropriate device for each patient. This paper outlines audiological guidelines for the latest devices, as well as research from the most up-to-date clinical trials. We also outline the evidence base for some potentially contentious issues in the fi

Model predictions for bone conduction perception in the human.

Five different pathways are often suggested as important for bone conducted (BC) sound: (1) sound pressure in the ear canal, (2) inertia of the middle ear ossicles, (3) inertia of the inner ear fluid, (4) compression of the inner ear space, and (5) pressure transmission from the skull interior. The relative importance of these pathways was investigated with an acoustic-impedance model of the inner ear. The model incorporated data of BC generated ear canal sound pressure, middle ear ossicle motion, cochlear promontory vibration, and intracranial sound pressure. With BC stimulation at the mastoi

[Acute otitis media associated with bone conduction hearing loss].

Eleven patients (12 ears) with acute otitis media associated with raised bone conduction were studied. The patients were from 19 to 70 years old and came to our hospital between November 1996 through May 1997. Pure tone audiometry revealed mixed hearing loss, but there was no bullous myringitis in all cases. They were treated by oral or intravenous antibiotics, steroids, and myringotomy. Bacteriological examination was done in seven cases, and revealed penicillin resistant Streptococcus pnumoniae in three cases. Complete recovery of hearing loss was obtained in 10 of 12 ears, but sensorineural

Auditory brainstem response. Sensory assessment by bone conduction masking.

Auditory brainstem response (ABR) thresholds to clicks were determined for 15 normal hearing adults and for four patients with conductive, sensorineural, or mixed hearing losses. Conductive losses were created in five of the normal hearing adult subjects for which threshold assessment was repeated. The response to a click at 5 dB above threshold was masked with a high-pass, bone-conducted noise. For both the patients and the normal subjects, subtracting 15 dB from the amount of noise required to mask this ABR response approximated sensory threshold. Tympanometry was performed on all patients,

Beyond the Air-Bone Gap: The Role of Bone Conduction Thresholds in Predicting Functional Outcomes and Guiding Surgical Decision-Making in Active Middle Ear and Bone Conduction Implants.

Introduction: In patients with conductive and mixed hearing loss, implantable hearing devices such as active middle ear implants (AMEIs) and bone conduction implants (BCIs) are established alternatives when conventional hearing aids fail. Although bone conduction (BC) thresholds are routinely used as eligibility criteria, their role as frequency-specific predictors of postoperative functional outcomes remains poorly defined. This study aimed to evaluate the influence of preoperative BC thresholds across the audiometric spectrum on postoperative speech recognition outcomes after implantation wi

Sound Localization and Lateralization by Bilateral Bone Conduction Devices, Middle Ear Implants, and Cartilage Conduction Hearing Aids.

Sound localization in daily life is one of the important functions of binaural hearing. Bilateral bone conduction devices (BCDs), middle ear implants, and cartilage conduction hearing aids have been often applied for patients with conductive hearing loss (CHL) or mixed hearing loss, for example, resulting from bilateral microtia and aural atresia. In this review, factors affecting the accuracy of sound localization with bilateral BCDs, middle ear implants, and cartilage conduction hearing aids were classified into four categories: (1) types of device, (2) experimental conditions, (3) participa
