[Achondrogenesis. Apropos of a case].

The authors report a new case of achondrogenesis, the study of which is principally directed towards the histopathological examination of the conjugal cartilage. A brief review of the literature recalls the main clinical, radiological and histopathological characteristics of this fatal chondrodysplasia transmitted by an autosomal recessive means. In addition to other features, it reveals the difficulty which may be experienced in differentiating between the two types (I and II). Finally, the differential diagnosis is briefly assessed, the problem being the elimination of other non-transmissibl

The Interactivity between TGFβ and BMP Signaling in Organogenesis, Fibrosis, and Cancer.

The Transforming Growth Factor beta (TGFβ) and Bone Morphogenic Protein (BMP) pathways intersect at multiple signaling hubs and cooperatively or counteractively participate to bring about cellular processes which are critical not only for tissue morphogenesis and organogenesis during development, but also for adult tissue homeostasis. The proper functioning of the TGFβ/BMP pathway depends on its communication with other signaling pathways and any deregulation leads to developmental defects or diseases, including fibrosis and cancer. In this review we explore the cellular and physio-pathologica

Application of bone marrow-derived stem cells in experimental nephrology.

Recent advancement in developmental biology has led to the discovery of immature mesenchymal stem cells in bone marrow and several established organs. The therapeutic potentials of such stem cells for treating serious diseases constitute a major rationale for every research effort, and clinical trials for replacing some damaged tissues such as cartilage are currently under way. Although the feasibility of such stem cell therapies for renal diseases remains unknown, it is worthy of pursuing possibilities. As an experimental tool, we have made chimeric rats that carry the bone marrow of transgen

Novel Mutation in the DKC1 Gene: Neonatal Hoyeraal-Hreidarsson Syndrome As a Rare Differential Diagnosis in Pontocerebellar Hypoplasia, Primary Microcephaly, and Progressive Bone Marrow Failure.

Primary microcephaly and severe developmental delay are complex but unspecific signs pointing to various genetic or acquired diseases. A concomitant finding of hematological failure may lead to the differential diagnosis of rare genetic diseases such as chromosome breakage disorders or diseases associated with telomere dysfunction. X-linked Hoyeraal-Hreidarsson syndrome (HHS) is a rare heterogenic disorder characterized by severe neurological impairment and progressive bone marrow failure. The latter represents the main cause of mortality, usually in early childhood. We report on the clinical

Pathogenesis of membranous lipodystrophy. Case report and review of the literature.

Membranous lipodystrophy occurred in a 35-year-old woman and membrane-like material was found in her epiphyses and short bones. Electron microscopy allowed a study of the relationship between the pathologic tissue and normal fat, suggesting that the membrane-like material is derived from degenerated cells. Nucleation of hydroxyapatite crystals was also observed in the membranous material. Because basal ganglia calcifications were demonstrated with the use of computed tomography, brain and bone lesions may be assumed to share the same pathogenesis. Lipid and mucopolysaccharide metabolism was fo

Dysplasia Epiphysealis Hemimelica in the Lower Extremity.

BACKGROUND: Because of the rarity of dysplasia epiphysealis hemimelica (DEH), little is known about the relationship between disease classification and clinical symptoms or patient outcomes. This studies therefore aims to characterize DEH of the lower extremity and correlate radiographic classification to presenting symptomatology and need for surgical intervention. METHODS: A multi-center, retrospective review of all patients with DEH of the lower extremity over a 47-year period was conducted. Demographic data, presenting complaints, treatments, and symptoms at final follow-up were recorded.

Percutaneous hemiepiphysiodesis using transphyseal screws for adolescent tibia vara.

Hemiepiphysiodesis around the knee is becoming the mainstay procedure in adolescents for a wide range of aetiological deformities, when considering adolescent tibia vara (ATV), the published series have variable results. The purpose of this study was to review our experience with the percutaneous transphyseal screw (PETS) in these patients followed until bone maturity. We analysed the charts from 13 patients (20 knees) that underwent lateral tibial hemiepiphysiodesis using PETS. The radiographs were accessed before surgery, at implant removal, when occurred, and at the final follow-up. The cli

Pediatric aspects of skeletal dysplasia.

Skeletal dysplasia is a disorder of skeletal development characterized by abnormality in shape, length, a number and mineral density of the bone. Skeletal dysplasia is often associated with manifestation of other organs such as lung, brain and sensory systems. Skeletal dysplasias or dysostosis are classified with more than 400 different names. Enchondral bone formation is a coordinated event of chondrocyte proliferation, differentiation and exchange of terminally maturated chondrocyte with bone. Impaired enchondral bone formation will lead to skeletal dysplasia, especially associated with shor

The morbid anatomy of congenital deficiency of the tibia and its relevance to treatment.

Two specimens obtained from a 5-month-old boy with bilateral type 1a congenital tibial deficiency were dissected to describe their morbid anatomy. Examination revealed a rigid equinovarus deformity, absence of a tibial remnant, an abnormal saddle-shaped talus, and several tarsal coalitions. Observation of the arterial pattern during surgery supported the previously reported finding that persistence of an immature arterial structure is inherent in this condition. Knowledge of potential structural anomalies is essential during the planning of an amputation or of a knee or an ankle reconstruction

Radial lengthening for septic growth arrest.

A case is presented of a child who sustained septic growth arrest of the distal radial epiphysis following osteomyelitis at 5 weeks of age. At age 8 years and 8 months radial lengthening using the Wagner apparatus was performed. Two and one-half centimeters of total length (18% of original length) was gained. No significant complications were encountered and normal function has been maintained. Although follow-up is short and additional lengthening is anticipated, this report illustrates the technical considerations for performing this procedure.

[Cranio-vertebral junction anomalies and osseous dysplasias (author's transl)].

The cartilaginously preformed occipital region of the cranium is ontogenetically a part of the backbone. The cranio-vertebral junction is unique among the other junctional areas of the backbone: 1. It has not yet been proved that different combinations of cranial and caudal junction anomalies are not combined with cervical junction anomalies. 2. The assimilations of the atlas which are considered to be progressive and regressive variants and the so-called manifestations of the occipital vertebra, i.e. diametrically opposed members of a developmental line occur together with a frequency that is

Analyzing Patterns of Skeletal Indicators of Developmental Stress Through the Double Lens of Ontogeny and the Life Course Approach in a Contemporary Reference Sample.

OBJECTIVES: Skeletal indicators of developmental stress are commonly used to assess health, disease, and patterns of morbidity and mortality in past populations. Incorporating information about individual life history, such as adverse life events, allows for a more thorough understanding of their etiology. This paper adopts the double lens of ontogeny and the life course to analyze indicators of developmental stress in relation to known individual pathologies and developmental patterns of the cranium, vertebrae, and long bones. MATERIAL AND METHODS: Six skeletal indicators were collected on C

microRNA Regulation of Skeletal Development.

PURPOSE OF REVIEW: Osteogenesis is a complex process involving the specification of multiple progenitor cells and their maturation and differentiation into matrix-secreting osteoblasts. Osteogenesis occurs not only during embryogenesis but also during growth, after an injury, and in normal homeostatic maintenance. While much is known about osteogenesis-associated regulatory genes, the role of microRNAs (miRNAs), which are epigenetic regulators of protein expression, is just beginning to be explored. While miRNAs do not abrogate all protein expression, their purpose is to finely tune it, allowi

[Vertebral deformities and the underlying diseases].

The purpose of this review was to summarise the causes of vertebral deformity. Osteoporosis as the most frequent disease lies in the background of vertebral deformity which is the most common type of fractures. Vertebrae cannot resist mechanical loadings so they deform and/or break because of their decreased mineral density and changed trebecular microarchitecture. The diagnosis of vertebral deformity presents several problems in the practice. For example, patients with vertebral fractures do not report having symptoms even when present (asymptomatic deformity), moreover plain radiograph does

Spontaneous resolution of angular deformity of the distal femur in focal fibrocartilaginous dysplasia: a case report.

Focal fibrocartilaginous dysplasia (FFCD) is a rare cause of angular deformities around the knee in children. The FFCD lesion most commonly affects the proximal tibia and less commonly the distal femur. We report a case of distal femoral varus deformity secondary to FFCD. Spontaneous resolution of the deformity occurred during a 7-year period. Previous reported femoral cases showed progressive deformity and required treatment with soft-tissue releases with or without osteotomy. This is the first reported femoral case of spontaneous resolution of angular deformity secondary to FFCD. We recommen

Al-Gazali Skeletal Dysplasia Constitutes the Lethal End of ADAMTSL2-Related Disorders.

Lethal short-limb skeletal dysplasia Al-Gazali type (OMIM %601356), also called dysplastic cortical hyperostosis, Al-Gazali type, is an ultra-rare disorder previously reported in only three unrelated individuals. The genetic etiology for Al-Gazali skeletal dysplasia has up until now been unknown. Through international collaborative efforts involving seven clinical centers worldwide, a cohort of nine patients with clinical and radiographic features consistent with short-limb skeletal dysplasia Al-Gazali type was collected. The affected individuals presented with moderate intrauterine growth res

Guided growth using a tension-band plate in Blount's disease.

Our observational study's objective was to determine how effective guided growth with tension-band plates was to correct the deformity in Blount's disease. We reviewed the records of 14 children (18 limbs) with Blount's disease who were treated with tension-band plates as the only surgical intervention at a single institution over eight years. Five children (seven limbs) had infantile Blount's disease with Langenskiöld stage ≤2. Nine children (11 limbs) had late-onset Blount's disease. The mean age at operation was 7.2 years (SD, 3.1, range, 2.9-11.8). The tension-band plate effectively correc
